Clinical Course of Suspected Diagnosis of Pulmonary Tumor Thrombotic Microangiopathy: A 10-Year Experience of Rapid Progressive Right Ventricular Failure Syndrome in Advanced Cancer Patients

  • Background and Objectives: Several cases involving severe right ventricular (RV) failure in advanced cancer patients have been found to be pulmonary tumor thrombotic microangiopathies (PTTMs). This study aimed to discover the nature of rapid RV failure syndrome with a suspected diagnosis of PTTM for better diagnosis, treatment, and prognosis prediction in clinical practice. Methods: From 2011 to 2021, all patients with clinically suspected PTTM were derived from the one tertiary cancer hospital with more than 2000 in-hospital bed. Results: A total of 28 cases of clinically suspected PTTM with one biopsy confirmed case were included. The most common cancer types were breast (9/28, 32%) and the most common tissue type was adenocarcinoma (22/26, 85%). The time interval from dyspnea New York Heart Association (NYHA) Grade 2, 3, 4 to death, thrombocytopenia to death, desaturation to death, admission to death, RV failure to death, cardiogenic shock to death were 33.5 days, 14.5 days, 7.4 days, 6.4 days, 6.1 days, 6.0 days, 3.8 days and 1.2 days, respectively. The NYHA Grade 4 to death time was 7 days longer in those who received chemotherapy (7.1 days vs. 13.8 days, p value=0.030). However, anticoagulation, vasopressors or intensive care could not change clinical course. Conclusions: Rapid RV failure syndrome with a suspected diagnosis of PTTM showed a rapid progressive course from symptom onset to death. Although chemotherapy was effective, increased life survival was negligible, and treatments other than chemotherapy did not help to improve the patient's prognosis.

  • In this study, the progressive failure behavior of the composite fan blade dovetail element under tensile loading is numerically investigated through finite element(FE) simulation. The accuracy of prediction by FE simulation is verified through tensile testing. The dovetail element is one of the joints for coupling the fan blade with the disk in a turbofan engine. The dovetail element is usually made of a metal material such as titanium, but the application of composite material is being studied for weight reduction reasons. However, manufacturing defects such as drop-off ply and resin pocket inevitably occur in realizing complex shapes of the fan blade made by composite materials. To investigate the effect of these manufacturing defects on the composite fan blade dovetail element, we performed numerical simulation with FE model to compare the prediction of the FE model and the tensile test results. At this time, the cohesive zone model is used to simulate the delamination behavior. Finally, we found that FE simulation results agree with test results when considering thermal residual stress and through-thickness compression enhancement effect.

  • The electric propulsion development is underway to minimize pollutants and greenhous gas emissions during the operation of ships / offshore installation vessels. The importance of the use and efficient management of batteries, which is an ESS system in ships / offshore installation vessels, is increasing. Generally, in ESS where battery is applied, cell balancing and life span are monitored in real time by BMS. Ships / offshore installation vessel are equipped with several ESS rooms, and ESS rooms with ESS systems of the same specification are being constructed due to the recent demand for electric propulsion development. In this paper, we propose an algorithm to additionally predict and diagnose battery pack and cell balancing failures by comparing BMS data for each rooms. The proposed algorithm compares the BMS data of each ESS Room according to the environmental change of the ship / offshore installation vessels, measures accurate status information, and reliably monitors it to prevent accidents in advance.

  • This paper presents an adaptive active tracking system with sector based scanning for a single PTZ camera. Dividing sectors on an image reduces the search space to shorten selection time so that the system can cover many targets. Upon the selection of a target, the system estimates the target trajectory to predict the zooming location with a finite amount of time for camera movement. Advanced estimation techniques using probabilistic reason suffer from the unknown object dynamics and the inaccurate estimation compromises the zooming level to prevent tracking failure. The proposed system uses the simple piecewise estimation with a few frames to cope with fast moving objects and/or slow camera movements. The target is tracked in multiple steps and the zooming time for each step is determined by maximizing the zooming level within the expected variation of object velocity and detection. The number of zooming steps is adaptively determined according to target speed. In addition, the iterative estimation of a zooming location with camera movement time compensates for the target prediction error due to the difference between speeds of a target and a camera. The effectiveness of the proposed method is validated by simulations and real time experiments.

  • In recent years, most of flood damage is associated with the levee failure. The objective of this study is to predict flow depths, flood area, flooding time and flood damage through flood inundation analysis considering the overflow of levee and the characteristics of levee failure. The hydrological parameters were extracted from GIS data such as DEM, land cover and soil map to estimate levee failure discharge. In addition, the characteristics of flood wave propagation could be accurately predicted as flood inundation analysis was accomplished considering the affection of structure within protected lowland and hourly prediction of flooded areas and estimation of flood strength will be utilized as basic data for the flood defence and establishment of measure to reduce flood damage.

  • Risk-based inspection (RBI) is a well-known method that is used to optimize inspection activities based on risk analysis in order to identify the high-risk components of major facilities such as power plants. RBI, when implemented and maintained properly, improves plant reliability and safety while reducing unplanned outages and repair costs. Risk is given by the product of the probability of failure (POF) and the consequence of failure (COF). A semi-quantitative method is generally used for risk assessment. Semi-quantitative risk assessment complements the low accuracy of qualitative risk assessment and the high expense and long calculation time of quantitative risk assessment. The first step of RBI is to identify important failure modes and causes in the equipment. Once these are defined, the POF and COF can be assessed for each failure. During POF and COF assessment, an effective inspection method and range can be easily found. In this paper, the calculation of the POF is improved for accurate risk assessment. A modified semi-quantitative risk assessment was carried out for boiler facilities of thermal power plants, and the next maintenance schedules for the equipment were decided.

Integral Method of Stability Analysis and Maintenance of Slope (비탈면 안정해석과 유지관리의 통합해석기법)

  • Even if the various data analyzing methods were suggested to examine the measured slope behaviors, it is difficult to find methods or procedures for connecting the analyzed results of slope stability and measured slope data. This research suggests the analyzing methods combing the stability analysis and measured data based on progressive failure of slope. Slope failure analysis by time degradation were calculated by strength parameters composed of strength reduction coefficients, also which were compared to the measured data according to the variations of safety factor and displacement of slopes. The accumulated displacement curve were shown as 3rd degree polynomials by suggested procedures, which was the same as before researches. The reverse displacement velocity curves were shown as linear function for prediction of brittle slope failures, also they were shown as 3rd degree polynomials for ductile slope failures, which were the same as the suggested equation by Fukuzono (1985) and they were very similar behaviors to the in-situ failure cases.

Reliability Design Analysis for Underwater Buriend PBA Based on PoF (고장물리 기반 수중 매설형 PBA에 대한 신뢰성 설계 연구)

  • Purpose: PBA buried in underwater requires high reliability because of its mission critical characteristic and harsh operational environment during its life cycle. Therefore, various reliability improvement activities are necessary. The defect on PBA manufacturing process have been studied, as a result, many activities and standards have been presented. However, there are less studies regarding failure pattern on physical features based on design. In this paper, we studied a possible failure patten based on physical features that is related with manufacturing process of PBA. And reliability improvement design based on PoF (Physical of Failure) were intruduced in this paper. Methods: A reliability prediction simulation were performed on the components A and B of the H system using Sherlock Software which is a PoF commercial tool from DFR solution. Solder fatigue and PTH fatigue analysis based on thermal cycling profiles and random vibration was analyzed on three earthquake response spectrum. Result: It was validated that life time and reliability improvement design through solder fatigue and PTH fatigue analysis in case of component. For compoenet B, random vibration fatigue was additionally analyzed and validated reliability for earthquakes profile. Conclusion: In design stage prior to manufacturing, PoF can be analyzed, and it is possible to make a reliability improvement/validated design using design data. This study can be applied in every design step and contribute to make more stable development product.

A Study on Performance Reliability Analysis Device of Primary Battery (1차 전지의 성능 신뢰도 분석 장치에 관한 연구)

  • In industrial situation, electronic and electro-mechanical systems have been using different type of batteries in rapidly increasing numbers. These systems commonly require high reliability for long periods of time. Wider application of battery for low-power design as a prime power source requires us knowledge of failure mechanism and reliability of batteries in terms of load condition, environment condition and other explanatory variables. Battery life is an important factor that affects the reliability of such systems. There is need for us to understand the mechanism leading to the failure state of battery with performance characteristic and develop a method to predict the life of such battery. The purpose of this paper is to develope the methodology of monitoring the health of battery and determining the condition or fate of such systems through the performance reliability to predict the remaining useful life of primary battery with load condition, operating condition, environment change in light of battery life variation. In order to evaluate on-going performance of systems and subsystems adopting primary batteries as energy source, The primitive prototype for performance reliability analysis device was developed and related framework explained.

A Study on the Reliability Prediction and Lifetime of the Electrolytic Condenser for EMU Inverter (전동차 인버터 구동용 전해콘덴서의 신뢰도예측과 수명 연구)

  • Inverter module, which feeds the converted power to the traction motor for EMU. Consists of the power semiconductors with their gate drive unit(GDU)s and the control computer for driving, voltage, current and speed controls. Electrolytic condenser, connected to the gate drive unit and a core component to drive the power semiconductor, has problems such as reduction in lifetime and malfunction caused by electrical and mechanical characteristic changes from heat generation during high speed switching for generation of stable power. In this study, To check the service life of electrolytic condenser, the test was carried out in two ways. First, In the case of accelerated life testing of condenser, the Arrhenius model is a way of life testing. Another way is to analyze the reliability of the failure data by the method of parametric data analysis. Eventually, life time by accelerated life test than a method of failure data analysis(Weibull distribution) was found to be slightly larger output.
