The Effects of Fibular Repositioning Taping on Joint Angle and Joint Stiffness of the Lower Extremity in Sagittal Plane during a Drop Landing (낙하 착지 시 FRT가 하지의 관절의 시상각과 강직도에 미치는 효과)

  • Objective: To investigate effects of Fibular Repositioning Taping (FRT) on lower extremity joint stiffness and angle during drop-landing. Method: Twenty-eight participants (14 healthy, 14 with chronic ankle instability [CAI]) performed drop-landings from a 60 cm box; three were performed prior to tape application and three were performed post-FRT. Three-dimensional kinematic and kinetic data were collected using an infrared optical camera system (Vicon Motion Systems Ltd. Oxford, UK) and force-plate (AMTI, Watertown, MA). Joint stiffness and sagittal angle of the ankle, knee, and hip were analyzed. Results: The hip [Healthy: p<.05; M ± SD: 29.43 ± 11.27 (pre), 33.04 ± 12.03 (post); CAI: p<.05; M ± SD: 31.45 ± 9.70 (pre), 32.29 ± 9.85 (post)] and knee [Healthy: p<.05; M ± SD: 53.44 ± 8.09 (pre), 55.13 ± 8.36 (post); CAI: p<.05; M ± SD: 53.12 ± 8.35 (pre), 55.55 ± 9.81 (post)] joints demonstrated significant increases in sagittal angle after FRT. A significant decrease in joint angle was found at the ankle [Healthy: p<.05; M ± SD: 56.10 ± 3.71 (pre), 54.09 ± 4.31 (post); CAI: p<.05; M ± SD: 52.80 ± 6.04 (pre), 49.86 ± 10.08 (post)]. A significant decrease in hip [Healthy: p<.05; M ± SD: 1549.16 ± 517.53 (pre), 1272.48 ± 646.73 (post); CAI: p<.05; M ± SD: 1300.42 ± 595.55 (pre), 1158.27 ± 550.58 (post)] and knee [Healthy: p<.05; M ± SD: 270.12 ± 54.07 (pre), 239.13 ± 64.70 (post); CAI: p<.05; M ± SD: 241.58 ± 93.48 (pre), 214.63 ± 101.00 (post)] joint stiffness was found post-FRT application, while no difference was found at the ankle [Healthy: p>.05; M ± SD: 57.29 ± 17.04 (pre), 59.37 ± 18.30 (post); CAI: p>.05; M ± SD: 69.15 ± 17.63 (pre), 77.24 ± 35.05 (post)]. Conclusion FRT application decreased joint angle at the ankle without altering ankle joint stiffness. In contrast, decreased joint stiffness and increased joint angle was found at the hip and knee following FRT. Thus, participants utilize an altered shock absorption mechanism during drop-landings following FRT. When compared to previous research, the joint kinematics and stiffness of the lower extremity appear to be different following FRT versus traditional ankle taping.

Value of FDG PET/Contrast-Enhanced CT in Initial Staging of Colorectal Cancer - Comparison with Contrast-Enhanced CT

  • Background: FDG PET/CT is at an equivocal stage to recommend for staging of colorectal cancer as compared to contrast-enhanced CT (ceCT). This study was intended to evaluate the value of FDG PET/ceCT in colorectal cancer staging as compared to ceCT alone. Materials and Methods: PET/ceCT was performed for 61 colorectal cancer patients who were prospectively enrolled in the study. Three patients were excluded due to loss to follow-up. PET/ceCT findings and ceCT results alone were read separately. The treatment planning was then determined by tumor board consensus. The criteria for T staging were determined by the findings of ceCT. Nodal positive by PET/ceCT imaging was determined by visual analysis of FDG uptake greater than regional background blood pool activity. The diagnostic accuracy of T and N staging was determined only in patients who received surgery without any neoadjuvant treatment. Results: Of 58 patients, there were 40 with colon cancers including sigmoid cancers and 18 with rectal cancers. PET/ceCT in pre-operative staging detected bone metastasis and metastatic inguinal lymph nodes (M1a) that were undepicted on CT in 2 patients (3%), clearly defined 19 equivocal lesions on ceCT in 18 patients (31%) and excluded 6 metastatic lesions diagnosed by ceCT in 6 patients (10%). These resulted in alteration of management plan in 15 out of the 58 cases (26%) i.e. changing from chemotherapy to surgery (4), changing extent of surgery (9) and avoidance of futile surgery (2). Forty four patients underwent surgery within 45 days after PET/CT. The diagnostic accuracy for N staging with PET/ceCT and ceCT alone was 66% and 48% with false positive rates of 24% (6/25) and 76% (19/25) and false negative rates of 47% (9/19) and 21% (4/19), respectively. All of the false negative lymph nodes from PET/ceCT were less than a centimeter in size and located in peri-lesional regions. The diagnostic accuracy for T staging was 82%. The sensitivity of the peri-lesional fat stranding sign in determining T3 stage was 94% and the specificity was 54%. Conclusions: Our study suggested promising roles of PET/ceCT in initial staging of colorectal cancer with better diagnostic accuracy facilitating management planning.

The Effects of Lumbar Sympathetic Ganglion Block in the Patients with Spinal Stenosis and the Skin Temperature Changes according to the Contrast Spread Patterns (요척주관 협착증 환자의 요부 교감신경 차단술의 효과 및 조영 양상에 따른 피부 체온의 변화)

  • Background: We hypothesized that if a fluoroscopic image of the lumbar sympathetic ganglion block (LSGB) showed the spread patterns of contrast at both the L2/3 and L4/5 disc areas, then this would demonstrate a more profound blockade effect because the spread patterns are close to sympathetic ganglia. In addition, we compared the effects of LSGB and transforaminal epidural steroid injection (TFESI) for the patients suffering with spinal stenosis. Methods: Eighty patients were divided into two groups (Group S: the patients treated with TFESI, Group L: the patients treated with LSGB). The patients of group L were classified into three groups (groups A, B and, C) according to their contrast spread pattern. The preblock and postblock temperature difference between the ipsilateral and contralateral great toe ($DT^{pre}$, $DT^{post}$, $^{\circ}C$), and the DTnet were calculated as follows. $DT^{net}$ = $DT^{post}$ - $DT^{pre}$. Results: Both group showed a significant reduction of the visual analogue score (VAS) and the Oswestry disability index (ODI) score. Only the patients of group L showed a significant increase of their walking distance (WD). Group A showed the most significant changes in the $DT^{post}$ ($6.1{\pm}1.2^{\circ}C$, P = 0.021), and the DTnet ($6.0{\pm}1.0^{\circ}C$, p = 0.023), as compared to group C. Conclusions: LSGB showed a similar effect on the VAS, and ODI, and a significant effect, on WD, compared with TFESI. Group A showed a significant sympatholytic effect, as compared to group C.

An Numerical Analysis of the Preloading Effect of IPS Retaining Wall through Earth Horizontal Displacement Measuring (IPS 흙막이 가시설의 수평 변위 계측을 통한 선행하중 효과 수치해석적 분석)

  • In this study, gathered measuring data at fields constructed by IPS(Innovative Prestressed Support) system controls the ground displacement and improves the constructability of earth work and structure work greatly, and compared with horizontal displacement calculated by Elasto-plastic analysis program(EXCAV/W). As the result, displacement of calculated by pre-loading data is reduced 13.2% average of general method, and measuring displacement is also reduced 26.7% average of general method. Therefore that IPS system is more safe than conventional strut method in contrast to displacement of underground wall. In addition, horizontal displacement is reduced through the pre-loading effect used by IPS system.

Effect of Lumbar Stabilizing Taping on The Electromyographic Activation of Trunk and Gluteal Muscles During One-Leg Standing (한발 서기 동작 시 요부 안정화 테이핑이 체간부와 고관절 주위근의 근활성도에 미치는영향)

  • Purpose: The purpose of this study was to investigate trunk and gluteal muscle activation during one-leg standing or two-leg standing with lumbar stabilizing taping using non-elastic tape. Method: The subjects of this study were twenty subjects(man=11, women=9) who be in good physical health and have not problem to back muscle and one leg standing. The surface electromyographic(EMG) data were recorded on external oblique(EO), gluteus medius(GMed), gluteus maximus(GMax), quadratus lumborum(QL) while pre- and post-lumbar stabilizing taping in two-leg standing and one-leg standing. The analysis of data was performed using the paired samples t-test to compare the difference of EMG activity of pre and post lumbar stabilizing taping. Result: Contrast of pre-lumbar stabilizing taping the muscle activity of QL in post-lumbar stabilizing taping is significant decrease on two-leg standing posture(p<.05), and the muscle activity of GMed is significant increase on one-leg standing posture(p<.05). Thus, we suggest that lumbar stabilizing taping using by functional tape will be able to affect on lumbar stability and gluteal muscle retraining.

Influence of River Discharge Fluctuation and Tributary Mixing on Water Quality of Geum River, Korea (유량변화와 지류유입에 따른 금강의 수질 변화)

  • To study the influence of changes in river discharge on water quality of the main stem of the Geum River, we investigated variation of inflow load from tributaries with river discharge. We also studied the mixing behavior of pollutants during mixing of waters of the main stem and Gap Stream. For this study, we collected water quality data such as suspended solids (SS), biochemical oxygen demand (BOD), chemical oxygen demand (COD), total organic carbon (TOC), total nitrogen (TN) and total phosphorus (TP) representing pre-monsoon, monsoon, and post-monsoon events of 2013 from a website of Water Information System. Based on inflow load, the Gap and Miho streams may be ones of tributaries which may largely influence water quality of main stem in upper river region. The Suksung and Nonsan Streams seemed to further affect water quality downstream. Results of modified EMMA indicated SS and TP may have another source(besides Gap Stream) at pre-monsoon, monsoon, and post-monsoon period. In contrast, TN and organic matter (BOD, COD, TOC) were conservative at pre-monsoon and post-monsoon. However, when river discharge increased, these pollutants may also came from unspecified non-point sources. Therefore, we need to attempt to find non-point sources for the pollutants in the main channel of upper Geum River region.

Sewage Sludge Treatment with Internal Recirculation and Diverse Pre-treatment Methods Using Combined Digestion Process (혼합 소화공정에서 내부반송과 다양한 전처리를 통한 하수 슬러지 처리)

  • In this study, various influent sludge pre-treatment methods and the internal recirculation of thickened sludge from effluents using a liquid/solid separation unit were adopted to investigate their effects on the sludge digestion and methane production in a combined mesophilic anaerobic and thermophilic aerobic sludge digestion process. A lab-scale combined sludge digestion process was operated during 5 phases using different feed sludge pre-treatment strategies. In phase 1, the feed sludge was pre-treated with a thermal-alkaline method. In contrast, in phases 2, 3 and 4, the internal recirculation of thickened sludge from the effluent and thermal-alkaline, thermal, and alkaline pre-treatment (7 days) were applied to the combined process. In phase 5, the raw sludge without any pre-treatment was used to the combined process. With the feed sludge pre-treatment and internal recirculation, the experimental results indicated that the volatile suspended solid (VSS) removal was drastically increased from phases 1 to 4. Also, the methane production rate with the thermal-alkaline pre-treatment and internal recirculation was significantly improved, showing an increment to 285 mL/L/day in phase 2. Meanwhile, the VSS removal and methane production in phase 5 were greatly decreased when the raw sludge without any pre-treatment was applied to the combined process. Considering all together, it was concluded that the combined process with the thickened sludge recirculation and thermal-alkaline pre-treatment can be successfully employed for the highly efficient sewage sludge reduction and methane gas production.

Observations of Infection Structures after Inoculation with Colletotrichum orbiculare on the Leaves of Cucumber Plants Pre-inoculated with Two Bacterial Strains Pseudomonas putida or Micrococcus luteus

  • Infection structures were observed at the penetration sites on the leaves of cucumber plants inoculated with Colletotrichum orbiculare using a fluorescence microscope. The cucumber plants were previously drenched with suspension of bacterial strains Pseudomonas putida or Micrococcus luteus. The plants pre-inoculated with both bacterial strains were resistant against anthracnose after inoculation with C. orbiculare. To investigate the resistance mechanism by both bacterial strains, the surface of infected leaves was observed at the different time after challenge inoculation. At 3 days after inoculation there were no differences in the germination and appressorium formation of conidia of C. orbiculare as well as in the callose formation of the plants between both bacteria pre-inoculated and non-treated. At 5 days, the germination and appressorium formation of the fungal conidia were, however, significantly decreased on the leaves of plants pre-inoculated with M. luteus at the concentration with $1.0{\times}10^7\;cfu/ml$. Furthermore, callose formation of plants cells at the penetration sites was apparently increased. In contrast, there were no defense reactions of the plants at the concentration with $1.0{\times}10^6\;cfu/ml$ of M. luteus. Similarly, inoculation P. putida caused no plant resistance at the low concentration, whereas increase of callose formation was observed at the higher concentration. The results of this study suggest that the resistant mechanisms might be differently expressed by the concentration of pre-treatment with bacterial suspension.

Effects of Different Levels of Dietary Energy and Nutrient Density during the Pre-Peak and Peak Periods on Egg Quality in Hy-Line Brown Laying Hens

  • In this study, we investigated the effects of feeding diets with different levels of energy and nutrient density on the egg quality of laying hens during the pre-peak and peak periods. A total of 192 (Hy-line brown) laying hens were used in a 15-week trial. The hens were randomly allotted to one of four treatments, each with four replicates (12 hens per replication). We assessed the effects of four level of dietary energy (2,710, 2,850, 2,870 and 2,890 kcal/kg) and three levels of nutrient density (Methionine + Cysteine: 0.56%, 0.85%, 0.80% and Crude Protein: 14.5%, 19%, 18%). Differences in the energy and nutrient density contents of diets showed no significant effect (P>0.05) on the average daily gain, average daily feed intake, feed conversion ratio, egg weight, or egg production of hens during the pre-peak and peak periods. However, hens subjected to 2,890 kcal/kg during the pre-peak period were found to lay eggs with significantly thicker shells, and yolk color was found to be significant enhanced in hens fed this diet during the pre-and peak periods. In contrast, we detected no significant effects of dietary energy or nutrient density on the Haugh unit or eggshell strength. In summary, increasing the energy level of diets from 2,710 to 2,890 kcal/kg was found to have positive effects on the shell thickness and yolk color of eggs produced by laying hens.

Comparison of Multilevel with Single Level Injection during Lumbar Sympathetic Ganglion Block: Efficacy of Sympatholysis and Incidence of Psoas Muscle Injection

  • Background: We prospectively evaluated the incidence and possible factors causing intramuscular injection during lumbar sympathetic ganglion block and compared the multiple needle technique to the single technique to obtain a profound and complete block effect. Methods: Among 83 patients, 58 patients (group A, n = 27, multiple needle technique and group B, n = 31, single needle technique) were reevaluated for the changes of skin temperature (Ts) and mean segment of longitudinal contrast spread. After injecting the contrast agent, the incidence of psoas muscle injection and the change of Ts was compared between two groups. Results: The incidence of psoas muscle injection was 21.3% (46/216) and it was associated with the level of injection (L2) significantly (${\chi}_2$ = 14.773, P = 0.001). $DT^{post}$ (postblock temperature difference between ipsilateral and contralateral great toe, $4.6{\pm}2.8^{\circ}C$, $1.8{\pm}1.6^{\circ}C$, P < 0.001 for group A and B) and $DT^{net}$ ($DT^{post}$ - $DT^{pre}$, $3.9{\pm}2.7^{\circ}C$, $1.5{\pm}1.5^{\circ}C$, P < 0.001 for group A and B) was significantly higher in group A. The mean segment of longitudinal contrast spread was $8.1{\pm}0.9$ for group A and $3.2{\pm}1.6$ for group B (P < 0.001). Conclusions: The LSGB at the L2 level showed the lowest incidence of psoas muscle injection of contrast. Multiple needle approach showed more significant increase of $DT^{net}$ and $DT^{post}$.
