수중 및 균형 운동이 파킨슨 유발 백서모델에서 중뇌 흑질의 Tyrosine Hydroxylase 발현과 기능 회복에 미치는 영향 (Influence of Aqua and Balance Exercise on Tyrosine Hydroxylase Expression in the Substantia Nigra and Functional Recovery in Hemiparkinsonian Rat Model)

  • 이 연구는 수중 운동, 균형 훈련과 복합 운동이 파킨슨 유발 흰쥐 모델에 미치는 영향을 관찰하기 위해 수산화도파민(6-hydroxydopamine)을 흰쥐의 좌측 내측전뇌다발에 주사하여 파킨슨 모델을 제작하였다. 실험동물은 수중 운동을 적용한 실험군 I, 균형 훈련을 적용한 실험군 II, 복합 운동(수중 운동+균형 훈련)을 적용한 실험군 III, 수술 후 표준사육장에서 운동을 적용하지 않은 대조군으로 배치하였다. 수술 후 7일, 14일, 21일에 실험동물의 중뇌 흑질의 도파민 세포감소를 관찰하기 위하여 TH(tyrosine hydroxylase)의 발현 양상을 관찰하였고, 행동학적 검사로 수술 후 1일, 7일, 14일,21일에 아포몰핀유도 회전 검사를 실시하였다. 실험 결과, 실험군들에서 대조군에 비하여 유의하게 높은 TH 발현과 아포몰핀유도 회전수를 보였다. 하지만 실험군들 사이에는 유의한 차이가 나타나지 않았다. 이상의 결과에서 파킨슨유발 흰쥐모델이 기능 회복에 수중 운동과 균형 훈련이 효과적임을 알 수 있었으며, 흑질 치밀부의 도파민성 신경섬유의 증감을 나타내는 TH의 발현에도 수중 운동과 균형 훈련이 효과적임을 알 수 있었다.

Pelvic floor muscle exercise with biofeedback helps regain urinary continence after robot-assisted radical prostatectomy

  • Background: To determine the benefit of pelvic floor muscle exercise (PFME) with visual biofeedback on promoting patient recovery from incontinence, we investigated variables associated with the early restoration of continence for patients who underwent robot-assisted radical prostatectomy (RARP). Methods: Of the 83 patients enrolled, 41 consecutive patients completed PFME (the exercise group), and the other 42 consecutive patients just before the PFME program commenced (the control group). The primary outcome was whether PFME engagement was associated with zero pad continence restoration within 3 months of surgery. Results: Continence restoration percentages (defined as zero pads used per day) at 1, 3, and 6 months after surgery were 49.4%, 77.1%, and 94.0%, respectively. The exercise group achieved significantly higher recovery rates at 1 month (p=0.037), 3 months (p<0.001), and 6 months (p=023). Cox regression analysis demonstrated that a lower Gleason score (<8; hazard ratio [HR], 2.167), lower prostate specific antigen (<20 ng/dL; HR, 2.909), and engagement in PFME (HR, 3.731) were independent predictors of early recovery from postprostatectomy incontinence. Stratification by age showed that those younger than 65 years did not benefit significantly from exercise (log-rank test, p=0.08), but that their elderly counterparts, aged 65-70 years (p=0.007) and >70 years old (p=0.002) benefited significantly. Conclusion: This study suggests that postoperative engagement in PFME with biofeedback speeds up the recovery of continence in elderly patients (≥65 years old) that undergo RARP.

회전근 개 파열의 수술적 치료시 초음파 연속 검사의 유용성 (Usefulness of Serial Ultrasonography of the Rotator Cuff Repair)

  • 목적: 회전근 개 파열 환자에서 수술 전의 자기 공명 영상 검사와 초음파 검사에 따른 관절경 소견을 비교하고, 수술 후 2주, 6주 및 3개월의 회전근 개의 건재 상태를 초음파로 확인하는데 있다. 대상 및 방법: 2008년 2월부터 5월까지 본원 정형외과에 내원하여 회전근 개 파열로 관절경적 치료를 시행한 환자 29명을 대상으로 초음파 검사와 자기 공명 영상 검사를 시행한 후 관절경적 시술을 이용하여 비교하였으며, 수술 후 2주, 6주 및 3 개월에 회전근 개의 건재 상태를 초음파를 이용하여 관찰하였다. 결과: 회전근 개 파열을 확인할 수 있는 초음파 검사와 자기 공명 영상 검사의 예민도는 각각 100%였다. 전층 회전근 개 파열에서 초음파 검사와 자기 공명 영상 검사의 예민도는 각각 95%, 82%였으며, 부분층 회전근 개 파열에서 초음파 검사는 50%, 자기 공명 영상 검사는 33%의 예민도를 나타내었다. 전체 환자에 대한 초음파 검사의 정확도는 86%, 자기 공명 영상 검사의 정확도는 69%이었다. 전층 회전근 개 파열로 복원술을 시행한 22예 중 수술 후 2주에 시행한 추시 관찰 초음파 검사상 11예(50%, 소 및 중 파열 2예, 대 및 광범위 파열 9예)에서 회전근 개의 봉합 부위에서 수술 소견과 일치하는 틈새로 진단되는 저에코를 확인할 수 있었으며, 수동적 신장 운동을 시행한 후 수술 6주의 추시 관찰 초음파 검사상 5예(23%, 소 및 중 파열 1예, 대 및 광범위 파열 4예)에서 재파열 소견을, 능동적 운동을 시행한 후 수술 3개월의 추시 관찰 초음파 검사상 7예(32%, 소 및 중파열 2예, 대 및 광범위 파열 5예)에서 재파열 소견을 보였다. 결론: 회전근 개 파열에 대한 관절경적 치료 후 초음파 연속 검사는 술 후 틈새와 술 후 재파열을 구분하는데 도움을 줄 수 있었으며, 수술 후 6주에 23%, 3개월에 32%의 재파열 소견을 보인 바 재활 치료시 추가 치료의 방침을 세우는데 도움을 줄 수 있을 것으로 생각되었다.

회전근 개 간격의 가교 반흔 유착에 의한 외상성 견관절 강직증의 관절경적 치료 - 증례 보고 - (Arthroscopic Treatment of Post-traumatic Stiff Shoulder by Rotator Interval Bridging Scar Adhesion - Case Report -)

  • In adhesive capsulitis of the shoulder of no response to nonoperative treatment, an arthroscopic capsular release and manipulation improves range of motion and pain relief. We performed an arthroscopic examination in the stiff shoulder, of which she had no response to nonoperative treatment, after the conservative treatment of a clavicular shaft fracture by motorcycle-driver traffic accident. We found the intra-articular 'rotator interval bridging scar adhesion' between subscapularis tendon and antero-superior glenoid fossa under the rotator interval which was no adhesion and contracture itself. We performed the scar adhesion removal and synovectomy, maintaining the rotator interval. We recommended nonsteroidal anti-inflammatory drug for postoperative pain relief and continuous active and passive range of motion (ROM) exercise to gain motions. Preoperatively, active and passive range of motion were 70° for forward elevation, 60° for abduction and especially 0° for external rotation. After postoperative 2 months, active ROM were 150° for forward elevation, 130° for abduction and 80° for external rotation. After postoperative 6 months, passive and active ROM were full. UCLA score improved from preoperative 9 points to postoperative 29 points.

One Year Experience of the Hand Allotransplantation First Performed after Korea Organ Transplantation Act (KOTA) Amendment

  • The revision of the Korea Organ Transplantation Act (KOTA) in 2018 included hand/arm among the organs that can be transplanted. The first hand transplantation since the revision of KOTA took place in January 2021. A 62-year-old male patient experienced hand amputation on July 13, 2018, by a catapult injury. The patient first visited our institute 3 months after the injury. After serial interviews and an overall evaluation, the patient was registered on the hand transplantation waiting list in January 2020. On January 9, 2021, the patient underwent hand transplantation at the right distal forearm level. The total operation time was 17 hours 15 minutes, and the cold ischemic time was 4 hours 9 minutes. Postoperative immunosuppression was administered based on the protocol used for kidney transplantation. Two acute rejection episodes occurred, on postoperative days 33 and 41. Both rejection episodes were reversible with rescue therapy of a higher tacrolimus trough level, steroid pulse therapy, and topical immunosuppressants. Controlled passive range of motion exercise was started on postoperative day 10. Dynamic splint was applied on postoperative day 18. At 1 year, graft maintenance and functional improvement were satisfactory, and the patient showed a Disabilities of Arm, Shoulder and Hand score of 25.8. We successfully performed the first hand transplantation surgery under the KOTA amendment. It came from the organic and effective cooperation of plastic, orthopaedic, and transplantation departments and we believe it will guarantee the future ongoing success.

개방형 현미경적 요추간판 제거술 후 동적 요부 안정화 운동에 따른 체간 안정성의 개선과 유지 (The Effect of Dynamic Lumbar Stabilization Exercise to Be Affected with Improvement and Maintenance of Trunk Stability after Opened Microscopic Laser Discectomy)

  • Objects: This study is designed to analysis improvement and maintaince of trunk stability targeting patients who need post operative rehabilitation exercise after undergoing opened microscopic laser discectomy(OMLD) due to HNP. Method: Between March 2004 and February 2005 a total sixty patients who underwent OMLD due to $L_4/L_5$, $L_5/S_1$ HNP and who agreed to the experiment were subject for this study. Experimental group consisted of 18 subjects, and they underwent 45 minutes dynamic lumbar stabilization exercise And control group consisted of 18 subjects who conducted conservative physical therapy based on the use of physical modality for 45 minutes except to exercise. Results: Experimental group that was lumbar extensor's isometric peak torque, weight distribution of both leg, trunk muscle balance and Oswestry LBP disability index increased during 12 weeks in a statistically significant manner compared to before exercise (p<.05). When re-test was tried, isometric peak torque (p>.05) and Oswestry LBP disability index(p<.05) maintained 12th week level or improved. Weight distribution rate of both leg and trunk muscle balance maintained the level of 8th week of exercise execution(p<.05). Control group that was lumbar extensor's isometric peak torque, weight distribution rate of both leg and trunk muscle balance aggravated during 12 weeks compared to pre-test(p>.05) But, Oswestry LBP disability index increased in a statistically significant level compared to pre-test(p<.05). When re-test was tried, isometric peak torque increased slightly compared to pre-test, but decreased when at least $60^{\circ}$ flexion angle(p>.05). Weight distribution rate of both leg and trunk muscle balance also aggravated compared to pre-test(p>.05), and Oswestry LBP disability index was similar to the 4th week of physical therapy execution(p>.05). Conclusion: Successive postoperative Especially, Application of dynamic lumbar stabilization exercise program is important than traditional lumbar strengthening exercise program in the maintaince of spinal stability.

후외상성 주관절 강직에서의 변연 관절 성형술 (Debridement Arthroplasty for Post-Traumatic Stiff Elbow)

  • Stiffness of the elbow joint is relatively common after trauma, ectopic ossification, bum, postoperative scar, and etc. Mild flexion deformity can be reduced by use of active or passive motion exercise, dynamic sling, hinged distractor device, or turnbuckle orthosis. But these methods have disadvantages of difficulty in gaining acceptable range of motion only with stretching exercise, re-contracture after conservative managements and poor results that flexion contracture remained. The common described operative exposures for treatment of the stiff elbow are anterior, lateral, posterior, and medial approach. Through Anterior, lateral and medial approach each has not access to all compartments of the elbow. But, posterior approach has benefits that access to posterior, medial and lateral aspects of the elbow and as needed, fenestration to the olecranon fossa that produces a communication between the anterior and posterior compartments of the elbow are possible. From June 1991 through April 1997, 11 patients who had posttraumatic stiff elbow, were treated with debridement arthroplasty through the posterior approach. The purpose of this study are to introduce technique of the debridement arthroplasty and to evaluate final outcomes. With regarding to preoperative pain degree, mild degree matches to 3 cases, moderate to 3 cases, and severe to 2 cases. In preoperative motion, flexion was average 85° and extension was 30°. Postoperatively nine patients had got the complete relief of pain and two patients continued to have mild pain intermittentely. Postoperative flexion improved to 127° and extension to 2°, so that elbow flexion had improved by an average of 42° and elbow extension by 28°. On the objective scale all patients had good or excellent results and they all felt that they were improved by operation. Debridement arthroplasty is one of excellent procedures for the intractable stiff elbow if it is not unstable or it has not incongrous. But it need a meticulous operative technique and a well-programmed rehabilitation.

어깨뼈 다이나믹 테이핑이 어깨 수술 환자의 통증과 기능장애 수준, 상지 자세와 관절가동범위에 미치는 영향 (The Effect of Scapular Dynamic Taping on Pain, Disability, Upper Body Posture and Range of Motion in the Postoperative Shoulder)

  • PURPOSE: The aim of this study was to investigate the effect of scapular dynamic taping on pain, disability, upper body posture, and range of motion (ROM) in the postoperative shoulder. METHODS: Twenty-two patients who underwent acromioplasty and rotator cuff repair surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, n=11) and a control group (CG, n=11). For the EG, exercise therapy, manual therapy, and scapular dynamic taping were applied for 6 weeks; for the CG, only exercise therapy and manual therapy were applied for 6 weeks. Shoulder pain, disability, upper body posture, and ROM were evaluated at baseline, after 3 weeks of intervention, and after 6 weeks of intervention. Assessment tools included quadruple visual analog scale (QVAS) for level of pain; shoulder pain and disability index (SPADI) for functional disability level; forward head angle (FHA), forward shoulder angle (FSA), and pectoralis minor index (PMI) for upper body posture; and ROM testing. RESULTS: Significant differences were observed between the EG and CG in SPADI total scores; internal rotation and external rotation ROM of the glenohumeral joint ; FSA ; and PMI. All groups showed statistically significant improvement in QVAS; SPADI; flexion; abduction; external rotation and internal rotation ROM of the glenohumeral joint; FSA; and PMI. CONCLUSION: These results suggest that, for patients who have undergone acromioplasty and rotator cuff repair surgery, the addition of scapular dynamic taping during therapy is effective for improvement of shoulder disability level, ROM, and upper body posture.

구조화된 운동 프로그램이 인공고관절 전치환술 환자의 고관절 통증, 신체기능 및 삶의 질에 미치는 효과 (Effects of Structured Exercise Program on Hip Pain, Physical Function and Quality of Life in Patients with Total Hip Arthroplasty)

  • Purpose: The purpose of this study was to develop a 12-week structured exercise program (12-week-SEP) and evaluate its effects on pain, physical function and quality of life in patients with total hip arthroplasty (THA). Methods: A nonequivalent control group non-synchronized design was utilized to examine the effects of the 12-week-SEP on patients' outcomes after THA. A total of 46 adult patients (experimental group=25, control group=21) who had THA were recruited for the study. The 12-week-SEP consisted of education, exercise (muscle strengthening and progressive walking), and feedback. Data were analyzed by SPSS/WIN 21.0 using the $x^2$ test and repeated measures ANOVA. Results: The mean age of the participants was 60.9 years and 25 men (54.5%) and 21 women (45.7%) were included in the study. There were significant intervention effects on pain reduction and increased physical function but no effects on quality of life. Conclusion: The 12-week-SEP provided pain relief and enhanced physical function in patients underwent THA. Clinical application of the 12-week-SEP can improve postoperative nursing practices for patients with THA. Future research with larger sample is of necessity to obtain greater generalizability of the empirical evidences of the 12-week-SEP.

판막상부 대동맥 협착증의 수술요법 (Surgical Treatment of the Supravalvular Aortic Stenosis)

  • Supravavular aortic stenosis is a congenital narrowing of the ascending aorta just distal to the level of the origins of the coronary arteries, that may be localized or diffuse. Five patients with supravalvular aortic stenosis were operated upon between July, 1986 arid June, 1990. Four of these patients were William`s syndrome [mental retardation, elfin face], and one was isolated supravalvular aortic stenosis. Preoperative diagnosis of the supravalvular aortic stenosis was made by left side cardiac catheterization and angiocardiography. There are three types of supravalvular aortic stenosis such as membranous, hourglass and hypoplastic. Four of our patients were of hourglass type, and one was hypoplastic type. Patch aortoplasty was performed in all cases. Preoperative systolic gradients ranged from 45 to 1SO mmHg [average 102.6 mmHg]: postoperative gradients ranged from 0 to 75 mmHg [average 39 mmHg]. The patient of hypoplastic type has been suffered from mild exercise intolerance even after the operation, and the postoperative echocardiography revealed the systolic gradient of 100 mmHg [preoperative 180 mmHg]. The results of surgery for hourglass type were excellent. But the patient with hypoplastic form would be benefited from some modifications of the operation.
