수술 후 폐 합병증 예방을 위한 심호흡 운동의 효과에 대한 체계적 고찰 (Effectiveness of Deep Breathing Exercise for Postoperative Pulmonary Complications Prevention: A Systematic Review)

  • Purpose: This study was done to evaluate effectiveness of deep breathing exercise as a postoperative intervention to prevent pulmonary complications. Methods: A search of databases from 1990 to 2012 was done including MEDLINE, EMBASE, CINAHL, Cochrane Library and eight Korean databases. Ten studies met eligibility criteria. Researchers trained in systematic review, independently assessed the methodological quality of selected studies using the Cochrane's risk of bias tool. Data were analyzed using RevMan 5.2 program. Results: Among ten RCTs in four studies, deep breathing exercise was compared with an instrument using interventions such as incentive spirometry, in the other four studies deep breathing exercise was compared with non-intervention, and in last two studies bundles of interventions including coughing and early ambulation were assessed. A significant difference was found between deep breathing exercise group and non-intervention group. The odds ratio (OR) of occurrence of pulmonary complications for deep breathing exercise versus non-intervention was 0.30. However, there was no significant difference between deep breathing exercise group and incentive spirometry group (OR=1.22). Conclusion: Deep breathing exercise is vital to improving cost-effectiveness and efficiency of patient care in preventing postoperative pulmonary complications. For evidence-based nursing, standardized guidelines for deep breathing in postoperative care should be further studied.

요부신전강화 운동프로그램이 단순추간판탈출증 수술환자의 요부근육 및 통증 그리고 사회복귀에 미치는 영향 (The effects of lumbar extensors strengthening program on low back muscle power and mass, pain, return to work of patients who took laser operation for herniated lumbar disc)

  • OBJECTIVE: The objective of this study was to know the effects of the postoperative lumbar extensor strengthening exercise program on back muscles strength and volume, pain, and the time of return to work. METHODS: A prospective controlled trial of lumbar extensor exercise program in patients who underwent microdiscectomy or percutaneous endoscopic discectomy for prolapsed lumbar intervertebral disc. Seventy-five patients were randomized into exercise group (20 male, 15 female) and non-exercise group (18 male, 22 female). Six weeks after surgery, patients in exercise group undertook a 12-week lumbar extension exercise (MedX) program. Assessment of spinal function was performed in all patients on postoperative 6 weeks, 18 weeks. The assessment included measures of lumbar extensor power, muscle mass of erector spinalis. All patients completed the visual analog scale (VAS) for evaluation of pain, and return to work. RESULTS: In muscle power, there were statistically significant improvements between pre and post test on muscle power in exercise group. But there were not statistically significant difference on muscle power in non-exercise group. In muscle mass, there were statistically significant difference between pre and post test on muscle mass in exercise group. But there were not statistically significant difference on muscle mass in non-exercise group. In the pain, there were statistically significant decrease between pre and post test on both group. But there were not statistically significant difference on fatty tissue and obesity in non-exercise group. The percentages of return to work in postoperative 4 months were significantly greater in the exercise group than in the non -exercise group. CONCLUSIONS: Postoperative lumbar extensor strengthening exercise program appears to be more beneficial to the patients who underwent operation for prolapsed lumbar intervertbral disc.

Controlled active exercise after open reduction and internal fixation of hand fractures

  • Background Hand fractures can be treated using various operative or nonoperative methods. When an operative technique utilizing fixation is performed, early postoperative mobilization has been advocated. We implemented a protocol involving controlled active exercise in the early postoperative period and analyzed the outcomes. Methods Patients who were diagnosed with proximal phalangeal or metacarpal fractures of the second to fifth digits were included (n=37). Minimally invasive open reduction and internal fixation procedures were performed. At 3 weeks postoperatively, controlled active exercise was initiated, with stress applied against the direction of axial loading. The exercise involved pain-free active traction in three positions (supination, neutral, and pronation) between 3 and 5 weeks postoperatively. Postoperative radiographs and range of motion (ROM) in the interphalangeal and metacarpophalangeal joints were analyzed. Results Significant improvements in ROM were found between 6 and 12 weeks for both proximal phalangeal and metacarpal fractures (P<0.05). At 12 weeks, 26 patients achieved a total ROM of more than 230° in the affected finger. Postoperative radiographic images demonstrated union of the affected proximal phalangeal and metacarpal bones at a 20-week postoperative follow-up. Conclusions Minimally invasive open reduction and internal fixation minimized periosteal and peritendinous dissection in hand fractures. Controlled active exercise utilizing pain-free active traction in three different positions resulted in early functional exercise with an acceptable ROM.

Feasibility and Effects of a Postoperative Recovery Exercise Program Developed Specifically for Gastric Cancer Patients (PREP-GC) Undergoing Minimally Invasive Gastrectomy

  • Purpose: Exercise intervention after surgery has been found to improve physical fitness and quality of life (QOL). The purpose of this study was to investigate the feasibility and effects of a postoperative recovery exercise program developed specifically for gastric cancer patients (PREP-GC) undergoing minimally invasive gastrectomy. Materials and Methods: Twenty-four patients treated surgically for early gastric cancer were enrolled in the PREP-GC. The exercise program comprised sessions of In-hospital Exercise (1 week), Home Exercise (1 week), and Fitness Improvement Exercise (8 weeks). Adherence and compliance to PREP-GC were evaluated. In addition, body composition, physical fitness, and QOL were assessed during the preoperative period, after the postoperative recovery (2 weeks after surgery), and upon completing the PREP-GC (10 weeks after surgery). Results: Of the 24 enrolled patients, 20 completed the study without any adverse events related to the PREP-GC. Adherence and compliance rates to the Fitness Improvement Exercise were 79.4% and 99.4%, respectively. Upon completing the PREP-GC, patients also exhibited restored cardiopulmonary function and muscular strength, with improved muscular endurance and flexibility (P<0.05). Compared to those in the preoperative period, no differences were found in symptom scale scores measured using the European Organization for Research and Treatment of Cancer (EORTC) Core Quality of Life Questionnaire (QLQ-C30) and Quality of Life Questionnaire-Stomach Cancer-Specific Module (QLQ-STO22); however, higher scores for global health status and emotional functioning were observed after completing the PREP-GC (P<0.05). Conclusions: In gastric cancer patients undergoing minimally invasive gastrectomy, PREP-GC was found to be feasible and safe, with high adherence and compliance. Although randomized studies evaluating the benefits of exercise intervention during postoperative recovery are needed, surgeons should encourage patients to participate in systematic exercise intervention programs in the early postoperative period (Registered at the ClinicalTrials.gov, NCT01751880).

요추수술 후 운동요법이 통증, 운동 자기효능감과 일상생활활동 장애에 미치는 효과 (The Effects of Muscle Strengthening Exercise for Postoperative Lumbar Spinal Surgery on Pain, Exercise Self-Efficacy, Activities of Daily Living)

  • Purpose: The purpose of this study was to examine the effects of exercise on pain, exercise self-efficacy, activities of daily living for postoperative lumbar spinal surgery. Method: The study used a nonequivalent control group pre- and post-test design in quasi-experimental research. The data were collected from April, 2010 to July, 2011. The subjects were 40 lumbar spine surgery patients who had been hospitalized in neurosurgery of G university hospital, 20 assigned for the experimental and 20 for the control group. Exercise of this study reformed to be adapted to lumbar spine surgery patients on the basis of William's flexion exercise and exercise for HIVD surgery patients developed by Kim(1995) and was practiced three times a day for 3 weeks. The data were analysed with ${\chi}^2$-test, t- test, Repeated measures ANOVA using the SPSS program. Result: There was a significant decrease in pain in the experimental group, but there was no significant increase in exercise self-efficacy and activities of daily living between the groups. Conclusion: Muscle strengthening exercise after surgery was effective in improvement of pain. Postoperative muscle strengthening exercise in lumbar spinal patients may be a useful intervention to reduce pain.

폐관류 검사를 이용한 폐절제술 후 심폐운동기능의 예측 (Prediction of Post-operative Cardiopulmonary Function By Perfusion Scan)

  • 연구배경 : 폐 절제술이 술후 운동능력의 변화에 어떤 영향을 주는지 잘 알려져 있지 않으나 폐 관류 검사를 이용하여 술후 운동능력을 예측하고자 하는 몇몇 연구들이 있었다. 그러나 이들 연구들은 검사시점 및 대상환자의 선정 등에 문제가 있음이 지적되고 있다. 본 연구에서 폐암 환자에서 술후 운동능력을 예측하는데 술전 폐관류 검사의 유용성 및 술전 후의 폐기능의 변화가 운동능력의 변화와 상관관계가 있는지를 알아보고자 하였다. 방 법 : 술전 1주 이내에 폐기능 검사, 심폐 운동부하 검사, technetium 99m labelled macroaggregated albumin (99mTc-MAA)을 이용한 폐 관류 겸사 및 체중, 체질량 지수를 측정하였다. 폐관류 검사를 이용한 술후 검사의 예측치는 preoperative values measured$\pm$[% perfusion remained/(% perfusion resected+% perfusion remained)]를 이용하여 구하였다. 폐관류 검사를 이용한 술후 검사치의 예측도를 알기 위하여 술후 실측된 검사치와 폐관류 검사로 예측한 검사치의 비[postoperative value measured/postoperative value predicted)$\pm$100%]를 구하였다. 또한 환자의 술전 후 각각의 실측된 검사치 간의 차이를 비교하기 위하여[(preoperative value measured-postoperative value measured)/preoperative value measured]$\pm$100%를 측정하였다. 결 과 : 대상군에서 술전 및 술후 6개월에 측정한 체중 및 체질량 지수의 차이가 없었다. 술후 실측된 $VO_{2max}$는 폐관류 검사에 의하여 예측된 $VO_{2max}$의 112%이었으며, $WR_{max}$는 119% 이었다. 그러나 술후 실측된 심폐 운동부하 검사치는 폐관류 검사에 의하여 예측된 심폐 운동부하 검사치와 각각 $r_s$, 값이 0.794와 0.932로 유의한 상관관계를 보였다. 심폐 운동부하 검사치에서는 전폐절제술 환7-r군에서 술후 실측된 $VO_{2max}$$WR_{max}$는 예측된 각각의 검사치의 121.05%, 136.51%이었으며, 엽절제술 환자군에서는 각각 107.94%와 111.59%이었다. 심폐 운동부하 검사치의 감소는 엽절제술 환자군에서 $VO_{2max}$$WR_{max}$가 각각 7.69%와 3.73%이었고 전폐절제술 환자군에서는 15.71%와 7.14% 었다. 술전 후 $FEV_1$, FVC 및 TLC의 변화는 술전 후 $VO_{2max}$$WR_{max}$의 변화와 유의한 상관관계를 보였다. 결 론 : 술전 폐관류 검사는 술후 운동능력을 예측하는데 유용하다고 판단된다. 그러나 술전 폐관류 검사는 술후 운동능력을 낮게 평가하는 경향을 보였다.

아킬레스건 파열 이후의 능동적인 관절 범위 운동이 없는 간단한 재활 운동 방법 (Simple Postoperative Exercise of Acute Achilles Tendon Rupture without Active Range of Motion Exercise)

  • Purpose: Postoperative exercise for acute Achilles tendon rupture is important for a patient's return to daily life and sports. On the other hand, the protocol requires considerable effort to educate patients and continuous checking. This study evaluated the outcome of a new simple and delayed rehabilitation protocol after Achilles tendon rupture repair. Materials and Methods: From July 2014 to November 2020, one hundred eighty-three patients were operated on by one surgeon. The exercise protocol was classified into two methods. One group (immediate protocol, control group) started immediate full weight bearing with a 20° plantar flexion range of motion from two days postoperatively. Ankle dorsiflexion was restricted to 0°. The other group (delayed protocol, case group) started full weight bearing with a controlled ankle motion boot from two weeks postoperatively. No range of motion exercise was allowed until six weeks postoperatively. Age, sex, body mass index, ankle range of motion, muscle power, time to return to previous physical activity, functional score, and complication rate were evaluated. The results of the two groups were compared using a Mann-Whitney test. Statistical significance was set as p<0.05. Results: The range of motion, double heel rising, and one-leg standing were achieved faster in the control group (p<0.05). However, single-heel rising, repeated single-heel rising, return to previous activity (work, run, and sport), and functional scores showed no statistical difference (p>0.05). Conclusion: Simple and delayed postoperative rehabilitation of acute Achilles tendon rupture without active range of motion exercises showed satisfactory functional results and a low complication rate.

척추수술을 받는 노인에게 시행한 수술 전 팬플룻 호흡운동 훈련의 효과 (Effect of Breathing Exercise Using Panflutes on the Postoperative Compliance, Pulmonary Infections and Life Satisfaction in Elderly Patients Undergoing Spinal Surgery)

  • Purpose: The purpose of this study was to examine the effects of breathing exercises performed using panflutes in elderly patients undergoing spinal surgery. Methods: The study design was a nonequivalent control group non-synchronized pre-post test. The study included 24 patients in both the experimental group and the control group. The experimental group completed a daily breathing exercise regimen using panflutes for 30minutes after meals, whereas the control group was provided standard preoperative education, including breathing exercises using incentive spirometers. After the exercise regimen, breathing exercise compliance, pulmonary infections, and life satisfaction were measured in both groups, and the data were analyzed using the SPSS/WIN program. Results: The compliance rate of breathing exercises was significantly higher in the experimental group. The experimental group presented no pulmonary infections in the later period, whereas the control group presented higher pulmonary infection rates in the same period. In addition, the life satisfaction score in the experimental group significantly increased. Conclusion: The breathing exercise program using panflutes for elderly patients undergoing spinal surgery enhanced their breathing exercise compliance and their daily life satisfaction in addition to reducing their pulmonary infection rates.

측두근 운동이 개두술 후 저작에 미치는 효과 (Effects of Temporal Muscle Exercise on Mastication after Craniotomy)

  • Purpose: The purpose of this study was to verify the effects of gum chewing temporalis muscle exercise on masticatory discomforts after craniotomy. Methods: Data collection was performed from July 10 to October 24, 2008. Forty participants who were treated with elective craniotomy were enrolled in this study. Twenty participants in the exercise group were given a gum chewing exercise protocol for five days and twenty participants in the control group received usual postoperative care only. All the participants were examined on the first, third and fifth postoperative day regarding the masticatory pain score, mouth opening range, and satisfaction with mastication. Collected data were analyzed using t-test, $x^2$-test and repeated measures ANOVA. Result: Masticatory pain of experimental group was significantly lower than the control group ($p$ <.001). Mouth opening range and satisfaction with mastication of experimental group were significantly improved in experimental group compared with the control group ($p$ <.001). Conclusion: The gum chewing temporalis muscle exercise after craniotomy is a useful intervention to reduce masticatory pain and to improve mouth opening range, recovery rate of mouth opening range and satisfaction with mastication.

전방 십자인대 재건술 후 추나치료와 운동요법을 병행한 한방재활치료의 치료효과 증례 2 (Korean Medical Rehabilitation with CHUNA Manual Therapy and Exercise Therapy on Postoperative of Anterior Cruciate Ligament Rupture of Knee. Two Case Reports.)

  • Objective : The objective of this study is to report the effect of Korean Rehabilitation Treatment with CHUNA Manual Therapy and Exercise Therapy on Postoperative Anterior Cruciate Ligament rupture of knee. Methods : Two patients were treated by Korean Rehabilitation with CHUNA Manual Therapy and Exercise Therapy at knee. We evaluated the effectiveness by Visual Analog Scale(VAS) and Western Ontario and McMaster Universities Arthritis Index(WOMAC) and Range of motion(ROM) Results : Both patients had significant improvement on Knee ROM, VAS score and WOMAC index. Also physical examination of Knee was improved. Conclusions : This study showed that Korean Rehabilitation Treatment with CHUNA Manual Therapy and Exercise Therapy has meaningful effect on Postoperative of anterior cruciate ligament rupture of knee. But there is a limit on this study due to sufficient number of cases. Further studies will be needed.
