흉막 박피술후 폐기능회복에 관한 연구 (Pulmonary function improvement after decortication)

  • To study the recovery pattern of pulmonary function after decortication, the author performed serial pulmonary function tests using spirometry before and at lst., 3rd., 4th. week, lst., 3rd., 6th. month and 1st. year in 36 patients who underwent decortication from January 1989 to September 1991 at the Department of Thoracic and Cardiovascular Surgery, Pusan National University Hospital, Pusan, Korea. Patients were divided into 3 groups by the degree of compression of lung parenchyme. Group I was classified below 20%, Group II between 21 to 40%, Group III above 41%. Their serial changes of pulmonary function test were compared. The obtained results were as follows; 1. Maximal voluntary ventilation was recovered in 1st post perative week and even greater improvement was noted in group III in which ratio to 44 % of the preoperative value. 2. Vital capacity reached nearly to preoperative values in 3rd postoperative week and had increased much further to 26 % above the preoperative figure in group II. 3. Forced expiratory volume in 1 second returned rather slowly in 3rd-4th postoperative week and the mean VC was improved more higher in group II than the other groups following decorti cation. 4. There was an greatest improvement over all tests[MW, VC, FEV1] in 2nd decade which ratios to preoperative value were 34, 25 and 22 % respectively.

단일기관 소아환자에서 단일절개복강경하수술의 초기경험 (Early Experiences of Single Incision Laparoscopic Surgery in Pediatrics in a Single Center)

  • Laparoscopic surgery has become popular in the past few decades, owing to less postoperative pain, fast recovery, and better cosmetic outcomes. The laparoscopic approach has been employed in pediatric surgery for the same reasons. After the first attempts of single incision laparoscopic appendectomy in pediatrics in 1998, single incision laparoscopic surgery (SILS) has recently been proven to be safe and feasible for the pediatric population. However, limitations have been reported for SILS, such as the wide learning curve, compared to standard laparoscopic surgery, and the restricted number of hospitals with surgical training programs including SILS. In this study, we intend to present our initial experiences with SILS in children, and to describe the technique, instruments used, and outcomes. This is a retrospective study of 71 pediatric patients who underwent SILS, at a tertiary medical center, between September, 2012 and August, 2013. Electronic medical records were reviewed for demographics, type of procedure, operation time, use of additional ports, conversion to open surgery, complications and hospital stay. Additional ports were inserted in 4 cases, for the purpose of traction. Postoperative complications were noted in 13 cases, which were mostly related to wound inflammation or formation of granulation tissue. According to our analyses, patients with complications had significantly longer use and more frequent use of pain killers. Notwithstanding the small sample size, many of the procedures performed in pediatric patients seem to be possible with SILS.

심실중격결손의 개심교정에 있어서 수술 접근방법에 따른 차이 (Comparison of the Operative Approaches for Repair of Ventricular Septal Defect)

  • We compared postoperative results according to the different surgical approach in 180 cases of isolated ventricular septal defects operated at the department of Thoracic and Cardiovascular Surgery in Kyungpook University Hospital from January 1987 to December 1991. Of the 180 cases, 109 were males and 71 females, age ranging from 6 months to 15 years (mean: 5.6 years) and body weight ranging from 6 to 52㎏(mean : 20㎏). According to Soto's classification, perimembranous types were comprised of 119 cases (66%), doubly committed subarterial type 49 cases(27%), and muscular type 12 cases(7%). Patients were divided into three groups according to the incision methods: right atriotomy group (39%), right ventriculotomy group (47%), and pulmonary arteriotomy group (14%). The mean aortic cross clamp time was shorter in right atriotomy group (39 min.) than right ventriculotomy group (79min.) in the cases of large perimembranous VSD (P<0.001). Spontanous recovery rate of cardiac rhythm after VSD closure was higher in right atriotomy group (51%) than right ventriculotomy group (32%) in the cases of perimembranous VSD (P<0.05). The incidence of postoperative RBBB was 17.6% with no statistical differences between right atriotomy group(17.9%) and right ventriculotomy group(19.2%). Overall mortality rate was 5.6%(10 cases) with no significant differences according to surgical approach.

측두근 운동이 개두술 후 저작에 미치는 효과 (Effects of Temporal Muscle Exercise on Mastication after Craniotomy)

  • Purpose: The purpose of this study was to verify the effects of gum chewing temporalis muscle exercise on masticatory discomforts after craniotomy. Methods: Data collection was performed from July 10 to October 24, 2008. Forty participants who were treated with elective craniotomy were enrolled in this study. Twenty participants in the exercise group were given a gum chewing exercise protocol for five days and twenty participants in the control group received usual postoperative care only. All the participants were examined on the first, third and fifth postoperative day regarding the masticatory pain score, mouth opening range, and satisfaction with mastication. Collected data were analyzed using t-test, $x^2$-test and repeated measures ANOVA. Result: Masticatory pain of experimental group was significantly lower than the control group ($p$ <.001). Mouth opening range and satisfaction with mastication of experimental group were significantly improved in experimental group compared with the control group ($p$ <.001). Conclusion: The gum chewing temporalis muscle exercise after craniotomy is a useful intervention to reduce masticatory pain and to improve mouth opening range, recovery rate of mouth opening range and satisfaction with mastication.

Classic Floating Elbow in Adults: A Case Series

  • Background: The aim of this study was to assess demographics, clinical outcomes, and complications of classic floating elbow in adults. Methods: Six patients with ipsilateral diaphyseal fractures of the humerus, radius, and ulna were reviewed retrospectively. All patients were treated operatively and available for follow-up at a minimum of 1 year after surgery. The average age of the patients was 45.2 years (22-64 years) and the average follow-up period was 37.0 months (14-103 months). They were evaluated with postoperative outcome measures, including a visual analog scale (VAS) for pain, Mayo elbow performance score (MEPS), and American Shoulder and Elbow Surgeons (ASES) shoulder score. Residual complications were also evaluated. Results: Five patients (83.3%) had open fracture, and 4 patients (66.7%) presented with associated nerve injury. All fractures were united within postoperative 4 months, except 1 delayed union. The average VAS pain score, MEPS, and ASES shoulder score at the final follow-up examination was 2.5, 79.8, and 67.5 respectively. Three patients including 2 cases of joint stiffness with incomplete recovery from nerve injury and 1 case of complex regional pain syndrome had poor clinical outcome. Conclusions: Although the classic floating elbow is rare, these injuries potentially have associated problems such as open fracture or nerve injury. The presence of residual neurological symptoms predispose to poorer clinical outcomes.

개흉술후 벽측흉막외 신경차단법에 의한 진통효과 (Post-thoracotomy pain control with paravertebral intercostal nerve block)

  • Effrctive analgesia after elective thoracotomy can be provided by continuous extrapleural intercostal nerve block.This study was designed to prove the effectiveness of continuous extrapleural intercostal nerve block. Twenty patients undergoing elective thoracotomy were randomized into two groups. Group I received lumbar epidural block[N=10] and group II received continuous extrapleural intercostal nerve block[N=10]. Postoperative pain relief was assessed on Numeric Rating Scale[NRS] and recovery of pulmonary function was assessed by coparison of preoperatrive and postoperative FVC[Forced Vital Capacity], FEV1[Forced expiratory Volume in 1 second], VC[Vital Capacity]. Arterial blood gas analysis[ABGA], vital signs and amount of additive analgesics were compared also. No significant difference was observed between the groups concerning these parameters mentioned above. Systemic complications, such as urinary retention[2/10] and weakness of lower extremity[2/10], occurred in group I but no complication occurred in group II. We conclude that continuous extrapleural intercostal nerve block is as effective as epidural block in pain relief and restoration of pulmonary mechanics with fewer comlications. Also because of it`s ease and safetiness, this must be considered as a substitute of epidural block in routine use for thoracotomy pain relief.

Comparative study of analgesia with bupivacaine 0.25% versus 0.5% for third molar removal under general anesthesia

  • Background: The aim of this study was to compare the effectiveness and duration of action of two concentrations of bupivacaine with adrenaline for postoperative pain in patients undergoing surgical removal of four third molars under general anesthesia. Methods: Sixty patients undergoing surgical removal of four wisdom teeth received bupivacaine 0.5% (n = 30) or 0.25% (n = 30). The severity of pain in the immediate recovery period and at 2 and 24 h after surgery was recorded using the visual analogue scale. Differences were assessed by box and whisper plot and the Student's t-test. Results: The analgesic effects of the 0.25% and 0.5% doses were significantly different (P = 0.022) at 30 min after surgery but not after 2 and 24 h. The difference of mean of 0.25% and 0.5% was much higher after 0.5 h but less after 2 and 24 h. Conclusions: Bupivacaine 0.5% was statistically better for pain control during the immediate postoperative period, but there was no significant difference in pain control between the two dose strengths at 2 and 24 h after surgery.

Single-incision Laparoscopic Gastrectomy for Gastric Cancer

  • The implementation of national cancer screening has increased the detection rates of early gastric cancer (EGC) in Korea. Since the successful introduction of laparoscopic gastrectomy for gastric cancer in the early 1990s, this technique has demonstrated improved short-term outcomes without compromising long-term oncologic results. It is associated with reduced pain, shorter hospitalization, reduced morbidity rates, better cosmetic outcomes, and equivalent mortality rates as those for open surgery. Laparoscopic gastrectomy improves patients' quality of life (QOL) and provides favorable prognosis. Single-incision laparoscopic gastrectomy (SILG) is one extremely minimally invasive method, theoretically offering improved cosmetic results, less postoperative pain, and earlier recovery after surgery than conventional multiport laparoscopic gastrectomy. In this context, SILG is thought to be an optimal method to promote and maximize patients' QOL in the acute postoperative phase. However, the technical difficulties of this procedure have limited its use. Since the first report describing single-incision distal gastrectomy in 2011, only 16 studies to date have evaluated SILG. Most of these studies have focused on the technical feasibility and safety of SILG because its long-term outcomes have not been reported. This article reviews the advantages and limitations of SILG.

Improvement of Infraorbital Rim contour Using Medpor

  • Background: Asymmetry of the infraorbital rim can be caused by trauma, congenital or acquired disease, or insufficient reduction during a previous operation. Such asymmetry needs to be corrected because the shape of the infraorbital rim or midfacial skeleton defines the overall midfacial contour. Methods: The study included 5 cases of retruded infraorbital rim. All of the patient underwent restoration of the deficient volume using polyethylene implants between June 2005 and June 2011. The infraorbital rim was accessed through a subciliary approach, and the implants were placed in subperiosteal space. Surgical outcomes were evaluated using preoperative and postoperative computed tomography studies. Results: Implant based augmentation was associated with a mean projection of 4.6 mm enhancement. No postoperative complications were noted during the 30-month follow-up period. Conclusion: Because of the safeness, short recovery time, effectiveness, reliability, and potential application to a wide range of facial disproportion problems, this surgical technique can be applied to midfacial retrusion from a variety of etiologies, such as fracture involving infraorbital rim, congenital midfacial hypoplasia, lid malposition after blepharoplasty, and skeletal changes due to aging.

환상췌장 수술 후 합병된 신생아 패혈증 증례보고 (A Case of Neonatal Sepsis after Operation of Annular Pancreas in Newborn)

  • Annular pancreas is a rare congenital anomaly with the descending duodenum encircled by a ring of pancreatic tissue, which may cause partial or complete obstruction of the duodenum. In newborn, the symptoms can be those of duodenal stasis resulting from partial intestinal obstruction secondary to some degree of duodenal stenosis. A male newborn weighing 2.0 Kg was born by C-section delivery at 37 weeks' gestation to a 27-year-old mother who had a hydramnios. He was in relatively good condition at birth except regurgitation of saliva and intermittent apnea. A plain film of the abdomen showed the double-bubble of gas filled stomach and proximal duodenum, and upper gastrointestinal series showed a dilated proximal duodenum, with a complete obstruction of the descending duodenum. Intraoperative finding revealed encirclement of the duodenal second portion by pancreatic tissue. Duodenojejunostomy was performed. After the operation, he had developed two serious complications, neonatal septicemia by Enterobacter cloacae on postoperative day 12 and systemic candidiasis on postoperative day 19, and been managed with ventilatory support, antibiotics, and antifungal agents with recovery.
