Meningioma in a 20-Month-Old Boy

  • A 20-month-old boy presented with a intraparenchymal mass in the right frontoparietal area manifesting as complex partial seizure, secondary generalization and left hemiparesis. Magnetic resonance images (MRI) of the brain showed inhomogeneously enhancing mass in the right frontoparietal area which has irregular margin and perilesional edema. Based on the radiological findings, a preoperative diagnosis was an intraaxial tumor, such as pilocytic astrocytoma or dysembryoplastic neuroepithelial tumor. The patient underwent a surgery including frontal craniotomy. The tumor had a partially extreme adherence to the surrounding brain tissue but it showed no dural attachment. Gross-total resection of the tumor was achieved. Postoperative follow-up computed tomography scans showed no residual tumor. The pathological findings confirmed the tumor as a WHO grade I meningioma, transitional type. Nine months after the surgery, follow-up brain MRI showed no recurrence of the tumor, porencephaly in site where the tumor was resected; the patient's symptoms had fully recovered. We report the case of a meningioma in a 20-month-old boy.

Pseudoaneurysm of Anterior Tibial Artery after Ankle Arthroscopy (족관절 관절경 수술 후 발생한 전경골 동맥의 가성 동맥류)

  • Pseudoaneurysm is extremely rare complication after ankle arthroscopy with standard anteromedial and anterolateral portals. We report a case of a pseudoaneurysm of the anterior tibial artery detected at 3 months after ankle arthroscopy in a 16-year-old male. He had sustained painful swelling of his right ankle after the arthroscopic surgery, and referred to our hospital with an MRI checked postoperatively. We failed to make the diagnosis of pseudoaneurysm with the postoperative MRI, thus the patient underwent another arthroscopy which revealed massive hemarthrosis within the joint. The diagnosis was confirmed with an angiography, and the vascular lesion was ligated.

Isolated calcific tendinitis at the posterosuperior labrum: a rare case study

  • Calcific tendinitis of the shoulder joint, also known as chemical furuncle of the shoulder, causes intense shoulder pain and usually occurs within 1-2 cm from the insertion of the rotator cuff. We experienced a rare case of calcific tendinitis in the posterosuperior labrum of the shoulder joint in a 39-year-old male patient who presented with severe pain and weakness in the right shoulder. Radiographs and magnetic resonance imaging (MRI) findings showed calcific tendinitis in the posterosuperior labrum of the shoulder joint. A 1-week attempt at conservative treatment failed, so the calcified deposit in the posterosuperior labrum was arthroscopically removed. The patient's symptoms were completely relieved, and satisfactory clinical outcomes were achieved. Postoperative follow-up X-ray and MRI showed no recurrence of calcific tendinitis.

  • Encapsulated papillary carcinoma (EPC) is an uncommon breast malignancy that is known to be indolent and associated with an excellent prognosis. However, there is a rare possibility of locoregional relapse or metastasis. Here, we present a case of recurrent EPC in the ipsilateral internal mammary lymph node (IMLN) that was detected in the postoperative magnetic resonance imaging with abbreviated protocol (AB-MRI). AB-MRI could facilitate the early detection of recurrent disease in the IMLN and may provide prognostic gain for such patients.

  • We aim to monitor vascularization of early bone perfusion following rabbit lumbar intertransverse bone graft fusion surgery using magnetic resonance imaging assessment. Correlation with graft survival status was evaluated by histological method. Experimental animals were randomly divided into three groups and the model was established by operating bilateral lumbar intertransverse bone graft with different types of bone graft substitute material. The lumbar intertransverse area of three groups of rabbits was scanned via MRI. In addition, histological examinations were performed at the $6^{th}$ week after surgery and the quantitative analysis of the osteogenesis in different grafted area was carried out by an image analysis system. The MRI technique can be used for early postoperative evaluation of vascularized bone graft perfusion after transplantation of different bone materials, whereas histological examination allows direct visualization of the osteogenesis process.

Clinical Result of Conservative Treatment and Operative Treatment for Interdigital Neuroma (지간 신경종에서 보존적 치료 및 수술적 치료의 임상적 결과)

  • Purpose: To evaluate the usefulness of ultrasound and MRI in diagnosing Morton's Neuroma, and the effect and significance of stepwise treatment. Materials and Methods: Out of all patients suspected of Morton's neuroma through history taking and physical examination, 77 patients (84 feet, 95 lesions) in whom Morton's neuroma was confirmed by ultrasound or MR imaging study or was clinically suspected with negative imaging studies, and followed up for over 3 months were included. In all cases, history taking and imaging study were done, and by comparison with operative findings of the patients, the sensitivity of ultrasound and MRI was checked. Postoperative evaluation was done using the AOFAS scale. The patient's satisfaction was also examined. Results: Morton's neuroma occurred most frequently at the $3^{rd}$ web space of the foot (56%), followed by the $2^{nd}$ web space (44%). Out of 15 cases suspected of morton's neuroma through ultrasound study, 13 were pathologically positive showing a sensitivity of 85.7%. Out of 16 cases suspected of morton's neuroma through MRI, 14 were pathologically positive showing a sensitivity of 83.3%. There was no significant difference in sensitivity between the two imaging modalities. Conclusion: In diagnosing Morton's neuroma, ultrasound examination had a similar sensitivity with MRI, therefore can be used as a screening study.

Preoperative Magnetic Resonance Imaging Features Associated with Positive Resection Margins in Patients with Invasive Lobular Carcinoma

  • Objective: To investigate preoperative magnetic resonance imaging (MRI) findings associated with resection margin status in patients with invasive lobular carcinoma (ILC) who underwent breast-conserving surgery. Materials and Methods: One hundred and one patients with ILC who underwent preoperative MRI were included. MRI (tumor size, multifocality, type of enhancing lesion, distribution of non-mass enhancement [NME], and degree of background parenchymal enhancement) and clinicopathological features (age, pathologic tumor size, presence of ductal carcinoma in situ [DCIS] or lobular carcinoma in situ, presence of lymph node metastases, and estrogen receptor/progesterone receptor/human epidermal growth factor receptor type 2 status) were analyzed. A positive resection margin was defined as the presence of invasive cancer or DCIS at the inked surface. Logistic regression analysis was performed to determine pre- and postoperative variables associated with positive resection margins. Results: Among the 101 patients, 21 (20.8%) showed positive resection margins. In the univariable analysis, NME, multifocality, axillary lymph node metastasis, and pathologic tumor size were associated with positive resection margins. With respect to preoperative MRI findings, multifocality (odds ratio [OR] = 3.977, p = 0.009) and NME (OR = 2.741, p = 0.063) were associated with positive resection margins in the multivariable analysis, although NME showed borderline significance. Conclusion: In patients with ILC, multifocality and the presence of NME on preoperative breast MRI were associated with positive resection margins.

Postanesthetic Cerebral Infarction Following Neck Dissection -A case report - (경부청소술 마취 후 발생한 뇌경색 -증례 보고-)

  • Postoperative stroke is uncommon even in elderly patients, who have a higher incidence of all types of postoperative complications. The mechanism of postoperative stroke is not certain, but can be explained by intravascular clottings originated from thrombus or embolus or by intracranial hemorrhage. In a 66-year-old male patient with current hypertension medication, who underwent both neck dissection for malignancy metastasis under general anesthesia, the left hemiparesis and delayed emergency were found postoperatively. After transferred to intensive care unit, he got the thrombolytic therapy and then the therapies to decrease the swelling of the brain on the diagnosis of cerebral infarction in the vascular distribution of the middle cerebral artery. A brain MRI definitely showed the midline deviation to the left of the right brain hemisphere due to the progressing edematous changes. As he got worse, the emergency neurosurgical operation was proposed but rejected by his family. He died at postoperative 3 days. In this hypertensive patient. perioperative stroke could be originated from the surgical stimuli on major vessels, which were inevitable in neck dissection during the operation. We report this case of the postoperative stroke, which could be highly possible to be associated with extensive head and neck surgery.

Dumbbell Neurogenic Tumor (Dumbbell 신경종양 -1례 보고-)

  • Dumbbell neurogenic tumor is a rare disease, arising about 10 % of mediastinal neurogenic tumor. We report a 31 year old man who was suffered from paresthesia and weakness of lower extremities for about 8 months. Chest x-rays revealed a dumbbell shaped mass shadow in left lower posterior mediastinum. MRI demonstrated a well defined dumbbell shaped mass in the mediastinum of T7 level, including spinal cord compression by the extended tumor into the spinal canal and enlarged intervertebral foramen. The patient underwent one-stage combined resection of the tumor through the thoracotomy and laminectomy, simultaneously. The tumor was confirmed as neurilemmoma. The postoperative course was smooth and uneventful.
