Tumor resection from retromolar trigone, posterolateral maxilla, and anterior mandibular ramus using lower cheek flap approach: a case report and review of literature

  • A surgical approach involving the retromolar trigone, posterolateral maxilla, and pterygoid region is the most challenging in the field of maxillofacial surgery. The upper cheek flap (Weber-Ferguson incision) with subciliary extension and the maxillary swing approach have been considered as alternatives; however, neither approach provides sufficient exposure of the pterygoid region and the anterior portion of the mandibular ramus. In this report, we describe two cases in which a lower cheek flap approach was used for complete tumor resection in the retromolar trigone and the anterior mandibular ramus. This approach allows full exposure of the posterolateral maxilla and the pterygoid region as well as the retromolar trigone without causing major sensory disturbances to the lower lip. A mental nerve anastomosis after tumor resection was performed in one patient and resulted in approximately 90% sensory recovery in the lower lip. The lower cheek flap approach provides adequate exposure of the posterolateral maxilla, including the pterygoid, retromolar trigone, and mandibular ramus areas. If the mental nerve can be anastomosed during flap approximation, postoperative sensory disturbances to the lower lip can be minimized.

IOIO (Infraorbital-Intraoral) 절개선을 이용한 상악골 절제술 (HEMIMAXILLECTOMY VIA INFRAORBITAL INTRAORAL-INCISION)

  • Surgical resection of tumors in the maxillofacial region sometimes results in extended defects of soft and hard tissue that frequently causes aesthetic, functional and especially mental damages. It is essential for patients with such facial defects to reduce the scar and maxillofacial asymmetry. To attain esthetic facial appearance after hemimaxillectomy, we devise a new design, so called 'IOIO Incision' (InfraOrbital-IntraOral incision). The new approach is established on infraorbital region to expose maxillofacial skeleton in aspect of face. And the other incision is designed on intraoral region. The IOIO incision provide excellent aesthetic result after hemimaxillectomy, because of reduced minimal facial scar contraction. Maxillofacial surgeons are used to designing Weber-Fergusson incision in resection of maxillofacial tumors, but disadvantages of the incision were large scar and asymmetry of face. To improve theses problem, we attempted IOIO Incision.. For correct osteotomy of posterolateral wall of maxillary sinus, 1. Fenestra formation on zygomatic body for easily access of reciprocating saw to posterolateral wall of maxillary sinus. 2. To achieve better visual field in posterolateral aspect of maxilla, fat tissue is removed from infratemporal fossa. This new, versatile procedure can be used for benign and malignant lesions of the maxillary area. We introduce cases with review of literatures.

First Zoea of Pagurus joponicus (Crustacea: Decapoda: Anomura: Paguridae) Reared in the Laboratory

  • The first zoeal stage of Pagurus japonicus is described and illustrated. Its morphological characteristics are compared with of other species of the genus Pagurus. The first zoea of P. japonicus was very similar to that of P. similis. The zoeas of the two species could be distinguished from other pagurid zoeas by morphological characteristics of the carapace without posterolateral spine. two setae on the endopod of the antenna, and 3+3 setae on the endopod of the maxilla. They show affinities more to the species of the family Diogenidae than to the species of the genus Pagurus in respect of zoeal morphology.

상악골에 발생한 화골성 섬유종의 증례보고 및 문헌고찰 (A CASE REPORT AND RIVIEW OF LITERATURE ON OSSIFYING FIBROMA IN MAXILLA)

  • 화골섬유종은 비교적 천천히 증식하는 종양으로 발견전 몇년간은 나타나지 않는다. 악골에서 화골섬유종의 잘 경계되어진 성질은 완전한 제거가 용이하여 이로인한 치료결과도 양호하다. 그러나 때때로 유년기 하악에서 이 종양의 골격적 양상을 볼 수 있다. 그러한 경우 종양의 침윤적인 성장 때문에 건강한 조직을 포함한 절제가 적용된다. 본 증례의 환자는 34세의 여성으로 약 두달전 우측 안모의 점진적인 팽윤을 인지하였으며 상악우측 구치부의 간헐적인 동통을 호소하였고 종물은 상악결절부위 및 우측 상악동의 전측방벽까지 팽윤되어 있었다. 방사선적 조사에서 우측 상악동의 후측방벽과 전방벽 그리고 우측 치조돌기와 경구개 까지의 파괴와 함께 다수의 치밀한 구형의 석회화물질을 함유한 종물이 관찰되었다. 종물은 최근의 빠른 침윤성 성장을 고려하여 부분적 상악골 절제술로 제거되었으며 6개월간 술후검사에서 환자는 특이한 술후 이상을 보이지 않았다.

구개골에 발생한 악성 Peripheral Nerve Sheath Tumor의 증례보고 (MALIGNANT PERIPHERAL NERVE SHEATH TUMOR ON PALATE: A CASE REPORT)

  • Summary: The malignant peripheral nerve sheath tumor(MPNST) is an aggressive neoplasm and can either arise independently or result from malignant change in preexisting neurofibromatosis (von Recklinghausen's disease). Its histologic characteristics remain controversial, but currently it is believed that the schwann cell is the origin of the peripheral nerve sheath tumors. MPNST is an uncommon neoplasm of the head and neck region, and its presentation in the oral cavity is quite rare. In this study, we report a patient with a rare case of a MPNST involving the maxilla. A case report: A 29-year-old female presented with a chief complaint of painless swelling with bleeding tendency on the left maxillary tuberosity area 2 months ago. Clinical examination showed a $5.0{\times}3.0cm^2$ sized, indurative swelling on the site. Conventional radiographs showed a relatively well-defined soft tissue mass involving the left maxillary sinus, and destruction of the anterior, posterolateral walls of the left maxillary sinus. Subtotal maxillectomy and split-thickness skin graft from thigh were undertaken. In histochemical and immunohistochemical studies, the specimen revealed positive reactivities to Vimentin and S-100 protein. Final diagnosis was made as MPNST.
