경추추간판탈출증을 동반한 후골간신경마비 환자에 대한 임상보고 (Clinical study on one case of patient of posterior interosseous nerve palsy accompanying HNP of C5-6,6-7)

  • After treating a patient suffering from metacarpophalangeal joint extension disturbance which is caused by posterior interosseous nerve palsy, some results are gained as follows. The symptom of posterior interosseous nerve palsy is simillar to the it of radial nerve palsy. But posterior interosseous nerve palsy isn't accompany with wrist drop. posterior interosseous nerve palsy is accompany with metacarpophalangeal joint extension disturbance. This symptom is caused by posterior interosseous nerve palsy. Posterior interosseous nerve palsy is correspond to MAMOKBULIN(麻木不仁), SUTONG(手痛), SUGI(手氣) in oriental medicine. The cause of this case on oriental medicine is Deficiency of qi and blood. Treatment which based on cause of oriental medicine-herb medication, acupuncture treatment- have a good effect to patient.

  • Posterior interosseous nerve(PIN) syndrome is an entrapment of the deep branch of the radial nerve just distal to the elbow joint. It is caused by acute trauma or masses compressing the nerve. We report an unusual case of PIN syndrome with wrist drop caused by compression of the nerve by anomalous vascular leash. The patient has recovered with the surgical decompression of the offending vessels and arcade of Frohse.

측와위로 시행한 견관절 관절경 후에 동시에 발생한 전방 및 후방 골간 신경 증후군 - 증례보고 - (Simultaneous Anterior and Posterior Interosseous Nerve Syndrome Following Shoulder Arthroscopy in the Lateral Decubitus Position - Case Report -)

  • 관절 관절경으로 수술적 치료를 시행한 후 전방 및 후방 골간 신경 증후군이 동시에 발생한 1예를 보고하고자 한다. 45세 남자 환자가 좌측 견관절의 불안정성으로 견관절 관절경 수술을 시행받았다. 전신 마취하 우측 측와위 자세에서 전완부를 신전한 상태에서 견인 장치를 부착한 상태로 2시간 동안 수술을 시행하였다. 1주일 후 환자는 좌측 전완부 동통, 수지의 신전력 및 무지와 인지의 심수지 굴근력의 저하로 외래를 방문하였다. 경과 관찰하였으나 증상이 지속되어 전방 골간 신경에 대해서는 신경 탐색술을 시행하였고 후방 골간 신경에서는 모래 시계 모양의 협착 소견이 발견되어 병변 절제 후 신경 외막 봉합술을 시행하였다. 수술 후 약 5개월간 외래에서 경과 관찰하였다. 신경 탐색술을 시행하고 3개월 후 총수지 신근의 기능이 회복하기 시작하였으며, 점차 회복하여 신경 탐색술 후 5개월째 증상이 완전히 회복되었다. 견관절 관절경 수술 후 전방 및 후방 골간 신경 증후군의 발생은 드물지만 견인 장치 및 수술 중 자세에 의해 유발될 수 있다. 따라서 견인 장치 부착 및 수술 자세에 대해 술자의 주의가 필요하다고 생각한다.

  • The upper trunk of the brachial plexus is the most common area affected by neuralgic amyotrophy (NA), and paresis of the shoulder girdle muscle is the most prevalent manifestation. Posterior interosseous nerve palsy is a rare presentation in patients with NA. It results in dropped finger on the affected side and may be misdiagnosed as entrapment syndrome or compressive neuropathy. We report an unusual case of NA manifested as PIN palsy and suggest that knowledge of clinical NA phenotypes is crucial for early diagnosis of peripheral nerve palsies.

  • 지방종은 가장 흔한 양성 종양이지만, 심부 조직에 발생하는 방골성 지방종은 매우 드문 질환이다. 저자들은 근위부 요골에 발생하여 후골간 신경마비를 일으킨 방골성 지방종 1례를 경험하였다. 53세 남자 환자가 3주전부터 발생하여 지속된 우측 수근 관절 및 수지의 신전 제한을 주소로 본원을 방문하였다. 검사 결과 방골성 지방종으로 확진되었고, 수술 6개월 후 증상 회복되어 정상적인 운동이 가능하였다.

Posterior Interosseous Nerve Palsy Caused by a Ganglion of the Arcade of Frohse

  • A 51-year-old male who is right-handed visited the outpatient for right fingers-drop. The patient's fingers, including thumb, were not extended on metacarpophalangeal joint. The active motion of the right wrist was available. The electromyography and nerve conduction velocity study were consistent with the posterior interosseous neuropathy. Further evaluation was done with the magnetic resonance imaging for finding the space-occupying lesion or any possible soft tissue lesion around the radial nerve pathway. On magnetic resonance imaging, the ganglion cyst, which was about 1.8 cm in diameter, was observed on the proximal part of the superficial layer of the supinator muscle (Arcade of Frohse). The surgical excision was done on the base of ganglion cyst at the base of stalk of cyst which looked to be connected with proximal radioulnar joint capsule. The palsy had completely resolved when the patient was observed on the outpatient department a month after the operation.

  • Selective fascicular involvement of the median nerve trunk above the elbow leading to anterior interosseous nerve (AIN) syndrome is a rare form of peripheral neuropathy. This condition has recently garnered increased attention within the medical community owing to advancements in imaging techniques and a growing number of reported cases. In this article, we explore the topographical anatomy of the median nerve trunk and the clinical features associated with AIN palsy. Our focus extends to unique manifestations captured through MRI and ultrasonography (US) studies, highlighting noteworthy findings, such as nerve fascicle swelling, incomplete constrictions, hourglass-like constrictions, and torsions, particularly in the posterior/posteromedial region of the median nerve. Surgical observations have further enhanced the understanding of this complex neuropathic condition. High-resolution MRI not only reveals denervation changes in the AIN and median nerve territories but also illuminates these alterations without the presence of compressing structures. The pivotal roles of high-resolution MRI and US in diagnosing this condition and guiding the formulation of an optimal treatment strategy are emphasized.

  • This study was carried to identify the component of Large Intestine Meridian Muscle in human, dividing into outer, middle, and inner part. Brachium and antebrachium were opened widely to demonstrate muscles, nerve, blood vessels and the others, displaying the inner structure of Large Intestine Meridian Muscle. We obtained the results as follows; 1. Meridian Muscle is composed of the muscle, nerve and blood vessels. 2. In human anatomy, it is present the difference between a term of nerve or blood vessels which control the muscle of Meridian Muscle and those which pass near by Meridian Muscle. 3. 전완부 후골간 동맥 유리피판술을 이용한 안면부 조직 결손 재건 치험례 (Reconstruction of the Face Defects Using Posterior Interosseous Artery Forearm Free Flap)

  • With esthetic concern in the reconstruction of skin and soft tissue defects of face, the use of local flap has been the method of choice. However, when there is extensive tissue loss in the face, local flaps do not provide satisfactory results. The amazing development of microsurgical technique has decreased the percentage of free flap failure, thus making free flap use in reconstruction of facial soft tissue defects. Many free flaps has been applied for reconstruction of face defects. Especially, the radial forearm flap has numerous advantages with which facial reconstruction is made possible. But, its disadvantages are ; the sacrifice of one major artery supplying the hand and donor site complications. In order to circumvent these disadvantages, we employed posterior interosseous artery(PIA) forearm free flap for the reconstruction of the face defects. The posterior interosseous forearm island flap was first described by Zancolli and Angrigiani(1985). Currently, the PIA island flap and free flap have been used for hand reconstructions. The disadvantages of the PIA flap are ; the small caliber of the pedicle, different locations of the perforating branches, and the proximity of the motor branch of the radial nerve. But, its advantages lies in preserving the major artery of the hand, minimal donor site morbidity, and fairly well matched skin texture and color, and that the flap volume is sufficient, not too bulky with convenient handling. By using this flap, we performed 1 case of tumor resection and 1 case of traumatic defect. From our experiences we conclude that it is one of many useful methods in the reconstruction of the skin and soft tissue defects of the face. We also have discussed advantages and some limitations of various free flaps for reconstruction of the face.

후 골간 혈관경을 이용한 도상피판에 의한 손목 및 수부 연부조직 결손의 수복 (Coverage of the Wrist and Hand Soft Tissue Defects with the Posterior Interosseous Forearm Island Flap)

  • The traditionally useful coverage methods of the wrist and hand soft tissue defect are the chinese forearm flap, the ulnar forearm flap. But, this flaps are inevitably sacrifice major vessel to the hand. Advantages of the posterior interosseous artery island flap(PIA Flap) is no need to sacrifice blood supply to the hand and supply relatively large thin, good quality flap and more cosmetic than other forearm flaps. But, it is difficult to dissect and raise because of deep seat, close relation with the posterior interosseous nerve and anatomic variation. Authors evaluated 8 cases of 7 patients in the department of orthopaedic surgery, college of medicine, Hallym University from January, 1993 to December, 1995. The results are as follows: 1. The satisfactory coverage was achieved 7 cases and 1 case failed because of anatomic variation. 2 The pedicle length is average 9cm and the flap size is variable from 3cm by 4cm to 5cm by 8cm. 3. The donor site defect was repaired by direct closure in 5 cases, remained 3 cases combined with skin graft. From our experience we conclude that the PIA flap is one of the useful coverage methods of the wrist and hand soft tissue defect.
