The Effects of Education According to Constitution & Character of Patients Who have Stroke (뇌졸중 환자의 체질별 건강통제위 성격별 자가간호에 대한 교육의 효과)

  • This study was designed to explore the effectiveness of education to improve self-care of stroke patients with three types of charactor health locus of control and constitution. A total of 56 patients were selected by convenience sampling at one oriental hospital. The teaching of self-care was conducted by the researcher. The self-care performance was measured for 10 days. The data were analyzed using t-test, ANOVA and Scheffe method as post hoc test. The results of this study are as follows : 1. The self-care performance by patients' characteristics : Age and education had impact on the self-care performance. The level of self-care performance of young and high educated patients was high, gender, religion, occupation, monthly income, and the paralyzed side, however, didn't have influence on the self-care performance. 2. The self-care performance of patients with three types of constitution : There was no statistical significance on the self-care performance of patients group by constitution. However, average score of self-care performance of patients with So-Um was high. 3. The self-care performance of patients with three types of health locus of control : There was no statistical significance on self-care performance of patient group by health locus of control. 4. The correlation between the constitution and the health locus of control : There was no statistical significance on the introverted and chance health locus of control of patients with three types of constitution. However, there was a statistically significant relationship between the powerful others health locus of control and So-Yang. Even though there was no statistically significant self-care performance of patients with So-Um, there was a clinical significance on the high score of self-care performance of patients with So-Um as compared with the self-care performance of the patients. The health locus of control didn't have influence on the self-care performance of patients. There was a statistically significant relationship between the powerful others health locus of control and So-Yang.   • Background: A forward head posture (FHP) is one of the most common types of poor head posture in patients with neck disorder. A prolonged FHP might increase pressure on the posterior cranio-cervical structure and exhibit reduced performance on a cranio-cervical flexion test (CCFT). CCFT is included to activate deep cervical flexor muscles and inhibit excessive activation of superficial cervical flexor muscles. Therefore, the selective activation of deep cervical flexors is needed for effective exercise for FHP. Objects: The purpose of this study was to compare muscle thickness between longus colli (Lco) and sternocleidomastoid (SCM) using ultrasonography in subjects with FHP depending on head support. Methods: This was a cross-sectional, case-control research design study. The ultrasonographic images of Lco and SCM were taken in 17 subjects with FHP during the 5 phases of the CCFT with and without a head support. Towel was used for supporting head to make the neutral head position in supine. Changes in muscle thickness during the test were calculated to infer muscle activation. Data were analyzed using repeated measures of two-way analysis of variance with the significance level of .05. Results: When subjects performed the CCFT with head support, there was a significant difference in muscle thickness of Lco and SCM (p<.05). According to a post hoc paired t-test, change of thickness of Lco was greater at all phases, and change of thickness of SCM muscle was less at phase 4 and 5 in condition with head support (p<.01) compared to condition without head support (p<.01). Conclusion: The result of this study suggest that applying head support for neutral head position during CCFT could be a useful method for activating Lco muscle without excessive activation of SCM muscle.

The Effects of Joint Mobilization and Stretching on the Muscle Activity and Internal Rotation of Shoulder Joint in Patients With Impingement Syndrome With Posterior Shoulder Tightness (관절가동술과 스트레칭이 어깨후방뻣뻣함을 가진 충돌증후군의 어깨관절 근활성도와 안쪽돌림에 미치는 영향)

  • Background: Posterior shoulder tightness, which is a problem mainly seen in patients with shoulder impingement syndrome, disrupts the scapulohumeral rhythm between the humerus and scapulae. Objects: The aim of this study was to compare the effects of joint mobilization and stretching on shoulder muscle activity and internal rotation range of motion (ROM) of the glenohumeral joint in patients with impingement syndrome with posterior shoulder tightness. Methods: The research subjects included 22 in-patients with impingement syndrome with posterior shoulder tightness. They were randomly divided into two groups: one group (12 patients) was treated with joint mobilization and the other group (10 patients) was treated with stretching for the posterior shoulder tightness. Each treatment was performed five times a week for two weeks, and there were 15 sessions for each treatment. The ROM of the internal rotation and muscle activities of shoulder joint were evaluated pretest and posttest in each group. Electromyography data were collected from the upper, middle, and lower trapezius and serratus anterior during shoulder abduction of 90°, 120°, and 150°. Results: Both the joint mobilization and stretching groups showed significant decreases in muscle activity in the upper, middle, and lower trapezius on the posttest (p < 0.05). There was a significant difference in serratus anterior at 150° (p < 0.05), but there was no significant difference between group in post-hoc analysis (p > 0.025). The internal rotation ROM was significantly increased in the stretching group compared to that in the joint mobilization group (p < 0.025). Conclusion: This study found that both joint mobilization and stretching for posterior shoulder tightness were effective in muscle activity during arm abduction, also in order to increase internal rotation ROM of shoulder joint, stretching was effective in patients with impingement syndrome with posterior tightness.

AN EVALUATION OF THE PRIMARY IMPLANT STABILITY AND THE IMMEDIATE LOAD-BEARING CAPACITY ACCORDING TO THE CHANGE OF CORTICAL BONE THICKNESS (치밀골 두께 변화에 따른 임플랜트 1차안정성과 즉시하중부담능 평가)

  • Statement of problem. Cortical bone plays an important role in the primary implant stability, which is essential to immediate/early loading. However, immediate load-bearing capacity and primary implant stability according to the change of the cortical bone thickness have not been reported. Purpose. The objectives of this study were (1) to measure the immediate load-bearing capacity of implant and primary implant stability according to the change of cortical bone thickness, and (2) to evaluate the correlation between them. Material and methods.48, screw-shaped implants (3.75 mm$\times$7 mm) were placed into bovine rib bone blocks with different upper cortical bone thickness (0-2.5 mm) and resonance frequency (RF) values were measured subsequently. After fastening of healing abutment. implants were subjected to a compressive load until tolerated micromotion threshold known for the osseointegration and load values at threshold were recorded. Thereafter, RF measurement after loading, CT taking and image analysis were performed serially to evaluate the cortical bone quality and quantity. Immediate load-bearing capacity and RF values were analyzed statistically with ANOVA and post-hoc method at 95% confidence level (P<0.05). Regression analysis and correlation test were also performed. Results. Existence and increase of cortical bone thickness increased the immediate load-bearing capacity and RF value (P<0.05) With the result of regression analysis, all parameter's of cortical bone thickness to immediate load-bearing capacity and resonance frequency showed significant positive values (P<0.0001). A significant high correlation was observed between the cortical bone thickness and immediate load-beating capacity (r=0.706, P<0.0001), between the cortical bone thickness and resonance frequency (r=0.753, P<0.0001) and between the immediate load-bearing capacity and resonance frequency (r=0.755, P<0.0001). Conclusion. In summary, cortical bone thickness change affected the immediate load-baring capacity and the RF value. Although RF analysis (RFA) is based on the measurement of implant/bone interfacial stiffness, when the implant is inserted stably, RFA is also considered to reflect implant/bone interfacial strength of immediately after placement from high correlation with the immediate load-baring capacity. RFA and measuring the cortical bone thickness with X-ray before and during surgery could be an effective diagnosis tool for the success of immediate loading of implant.

EFFECT OF SURFACE TREATMENT METHODS ON THE SHEAR BOND STRENGTH OF RESIN CEMENT TO ZIRCONIA CERAMIC

  • Statement of problem. The aims of the study were to evaluate the effect of current surface conditioning methods on the bond strength of a resin composite luting cement bonded to ceramic surfaces and to identify the optimum cement type. Material and methods. The sixty zirconia ceramic specimens(10 per group) with EVEREST milling machine and 60 tooth block were made. The zirconia ceramic surface was divided into two groups according to surface treatment: (1) airborne abrasion with $110{\mu}m$ aluminum oxide particles; (2) Rocatec system, tribochemical silica coating. The zirconia ceramic specimens were cemented to tooth block using resin cements. The tested resin cements were Rely X ARC, Panavia F and Superbond C&B. Each specimen was mount in a jig of the universal testing machine for shear strength. The results were subjected to 2-way ANOVA and Post hoc tests was performed using Tukey, Scheffe, and Bonferroni test. Results. The mean value of shear bond strength(MPa) were as follows: $$RelyXARC(+Al_2O_3),5.35{\pm}1.69$$; $$RelyXARC(+Rocatec),8.50{\pm}2.13$$; $$PanaviaF(+Al_2O_3),9.58{\pm}1.13$$; $$PanaviaF(+Rocatec),12.98{\pm}1.71$$; $$SuperbondC&B(+Al_2O_3)8.27{\pm}2.04$$; $$SuperbondC&B(+Rocatec),14.46{\pm}2.39$$. There was a significant increase in the shear bond strength when the ceramic surface was subjected to the tribochemical treatment(Rocatec 3M) in all cement groups(P<0.05). Bonding strengths of cements applied to samples treated with $Al_2O_3$ were compared; Rely X ARC showed the lowest values, whereas Panavia F cement showed higher value than that of Superbond C&B group with no statistical significance. When the bond strength of cements with of Rocatec treatment was compared, Rely X ARC showed lowest values. Overall, it was apparent that tribochemical treated Super-Bond possessed higher mean bond strength (14.46MPa; P<0.05) than that of Panavia F cement group with no significance. Conclusions. Silica coating followed silanization(Rocatec treatment) increase the bond strength between resin cement and zirconia ceramic. Panavia F containing phosphate monomer and Superbond C&B comprised of 4-META tend to bond chemically with zirconia ceramic, thus demonstrating higher bond strength compared to BisGMA resin cement. Superbond C&B has shown to have highest value of bonding strength to zirconia ceramic after Rocatec treatment compared to other cement.

Pain Relieving Effect of Intraoral Sucrose Replacement in Neonates (신생아의 통증완화를 위한 자당(sucrose) 경구투여 효과)

  • The ability of neonates to perceive and react to pain, has been acknowledged recently. Recent researches have been shown that even short term pain can have lasting negative effects. We know that most of the anatomical pathway and neurotrans-mitter function necessary for pain perception are fully or nearly fully developed in the neonatal period. Many people are still reluctant to believe that pain felt by neonates may be as severe as that felt by older children or adults yet. The objective of the study is to assess and compare the analgesic effects of orally administered sucrose and pacifiers. And to determine the synergistic analgesic effects of sucrose and pacifiers. The tools for this study is Lawrence's NIPS (neonatal infant pain scale) with behavioural pain responses and index for pain physiological reponses as heart rate, respiratory rate and degree of SaO₂. The participants are 96 healthy infants of neonatal age 1-7days and randomised to receive heel prick before 2minutes to blood sampling and physiological test in Nursery of K Medical University Hospital on May-July, 2000. The experimental group assigned to one of three treatment groups : no treatment; a pacifier; placebo(2ml 25% sucrose). Collected data were analyzed with the SAS program using X²-test, ANOVA and Duncan's multiple range test as post hoc. The results were as follows. 1) Pain behaviour responses: The pain score of placebo group(2ml 25% sucrose) is lower than no treatment group and pacifier group significantly(P=.000). Placebo group is different from no treatment and pacifier group with Duncan's multiple range test. 2) Pain physiologic responses (1) heart rate: The heart rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group significantly(P=.000). The heart rate change of placebo group is less than the other groups. (2) respiratory rate: The respiratory rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.2340). But, the respiratory rate of placebo group is lower than the other groups. (3) SaO₂: The SaO₂ score of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.3265). But, the change of SaO₂ score of placebo group is less than the other groups. In conclusion, the sucrose placebo showed pain relief effect in behavior responses and less physiological responses. Accordingly, the sucrose placebo should be applied nursing intervention for simple pain management as heel prick.

Evaluation of the Radiopacity of Contemporary Luting Cements by Digital Radiography (디지털방사선촬영술을 이용한 합착용 시멘트의 방사선불투과성 평가)

  • This study examined the radiopacity of eight contemporary luting cements by direct digital radiography. Five disc-shaped specimens ($5mm{\times}1mm$) were prepared for each material tested (BisCem, Clearfil SA Luting, Duolink, Maxcem Elite, Multilink Speed, Panavia F 2.0, RelyX Unicem Clicker, V-link). The specimens were radiographed using a Kodak CS 7600 image plate (Carestream Health, Inc., Rochester, NY, USA) and an aluminum step wedge with a range of thicknesses (1.5 to 16.5 mm in 1.5 mm increments) and a 1 mm tooth used as a reference. A dental X-ray machine Kodak 2200 Intraoral X-ray System (Carestream Health, Inc., Rochester, NY, USA), operating at 70 kVp, 4 mA, 0.156 s and a source-to-sample distance of 30 cm, was used. According to international standards, the radiopacity of the specimens was compared with that of an aluminum step wedge using NIH ImageJ software (available at http://rsb.info.nih.gov/ij/).The data was analyzed by ANOVA and a Tukey's post hoc test. Maxcem Elite (5.66) showed the highest radiopacity of all materials, followed in order by Multilink Speed (3.87) and V-link (2.83). The radiopacity of Clearfil SA Luting (1.35), BisCem (1.33), Panavia F 2.0 (1.29) and Duolink (1.10) were between enamel (1.79) and dentin (0.19). RelyX Unicem Clicker (0.71) showed the lowest radiopacity, which was higher than that of dentin. All materials showed a radiopacity above the minimum recommended by the International Organization for Standardization and the American National Standards/American Dental Association with the exception of RelyX Unicem Clicker.

An analysis of emotional English utterances using the prosodic distance between emotional and neutral utterances (영어 감정발화와 중립발화 간의 운율거리를 이용한 감정발화 분석)

  • An analysis of emotional English utterances with 7 emotions (calm, happy, sad, angry, fearful, disgust, surprised) was conducted using the measurement of prosodic distance between 672 emotional and 48 neutral utterances. Applying the technique proposed in the automatic evaluation model of English pronunciation to the present study on emotional utterances, Euclidean distance measurement of 3 prosodic elements such as F0, intensity and duration extracted from emotional and neutral utterances was utilized. This paper, furthermore, extended the analytical methods to include Euclidean distance normalization, z-score and z-score normalization resulting in 4 groups of measurement schemes (sqrF0, sqrINT, sqrDUR; norsqrF0, norsqrINT, norsqrDUR; sqrzF0, sqrzINT, sqrzDUR; norsqrzF0, norsqrzINT, norsqrzDUR). All of the results from perceptual analysis and acoustical analysis of emotional utteances consistently indicated the greater effectiveness of norsqrF0, norsqrINT and norsqrDUR, among 4 groups of measurement schemes, which normalized the Euclidean measurement. The greatest acoustical change of prosodic information influenced by emotion was shown in the values of F0 followed by duration and intensity in descending order according to the effect size based on the estimation of distance between emotional utterances and neutral counterparts.   • The Purpose of this study was to investigate the influences of the teacher-directed and the learner-directed ICT used instruction on learning achievement of home economics and resource management behavior according to the learning ability of students and resource management behavior of a mother. The subjects of this study were middle school students, who was divided into three groups traditional instruction group, teacher-directed ICT used instruction group, and learner-directed ICT used instruction group. This study was carried out the experimental instruction. After experimental instruction, the learning achievement and resource management behavior was estimated by test sheets. The data were processed with two-way ANCOVA and Scheffe test for post-hoc analysis The results of study were as follows : First, there was no significant difference in the influences of the ICT used instruction on learning achievement of home economics according to the learning ability. Second, there was significant difference in the influences of the ICT used instruction on learning achievement of home economics according to the resource management behavior of a mother. Third, there was no significant difference in the influences of the ICT used instruction on the resource management behavior according to the learning ability. Fourth, there was no significant difference in the influences of the ICT used instruction on the resource management behavior of a student according to the resource management behavior of a mother.

A Qualitative Analysis on Paraspinal Muscles in Patients with Acute Low Back Pain and Chronic Low Back Pain (급성 요통환자와 만성 요통환자에서 척추주변근육의 정량적 분석)

  • This study was conducted to measure and analyze the changes in paraspinal muscles of acute and chronic low back pain patients using MRI, and to provide clinical basic data for diagnosis and treatment for low back pain. For this purpose, 20 patients with acute low back pain frome August 2012 to January 2013 which occurred within 12 weeks, and 20 patients with chronic low back pain that progressed over 12 weeks, were chosen as subjects, and their MRI measurements were compared with one another. As a result, in relation to in the fatty degeneration ratio of the left spine and right spine, there were significant differences in erector spinae and multifidus(p<.001), and in relation to the Fat Infiltration ratio between all the groups, there were significant differences in psoas major, erector spinae and multifidus between the acute low back pain patient group and the chronic low back pain patient group(p<.001). In the post-hoc test, multifidus and erector spinae in the acute low back pain group and chronic low back pain group showed the highest Fat Infiltration ratio. The serious Fat Infiltration of multifidus and erector spinae in the chronic low back pain group led to weakened strength of muscles that stabilize the spine. In conclusion, it is considered that this study would present important data and basis in making acute and chronic low back pain patients pay more attention to multifidus and psoas major during rehabilitation exercise, and selecting a rehabilitation exercise program.
