중환자실 퇴원 환자의 불안, 우울, 외상 후 스트레스 장애 유병률 및 위험요인 (Prevalence and Risk Factors of Anxiety, Depression, and Post-Traumatic Stress Disorder in Critical Care Survivors)

  • Purpose : This study aimed to investigate the prevalence and risk factors of mental health problems in patients discharged from the intensive care unit (ICU). Methods : This was a secondary analysis study using data from a multicenter prospective cohort of post-ICU patients. We analyzed data of 311 patients enrolled in the primary cohort study who responded to the mental health questionnaire three months after the discharge. Anxiety and depression were measured on the Hospital Anxiety-Depression Scale, and post-traumatic stress disorder (PTSD) was measured on the Posttraumatic Diagnostic Scale. Results : The prevalence of anxiety, depression, and PTSD in patients at three months after ICU discharge were 25.7%, 17.4%, and 18.0%, respectively, and 7.7% of them experienced all three problems. Unemployment (OR=1.99, p=.033) and unplanned ICU admission (OR=2.28, p=.017) were risk factors for depression, while women gender (OR=2.34, p=.009), comorbid diseases (OR=2.88, p=.004), non-surgical ICUs (trauma ICU: OR=7.31, p=.002, medical ICU: OR=3.72, p=.007, neurological ICU: OR=2.95, p=.019) and delirium (OR=2.89, p=.009) were risk factors for PTSD. Conclusion : ICU nurses should proactively monitor risk factors for post-ICU mental health problems. In particular, guidelines on the detection and management of delirium in critically ill patients should be observed.

Factors on the Pathway from Trauma to Suicidal Ideation in Adolescents

  • Objectives: Suicide is the most frequent cause of death among Korean adolescents, and adolescents who have experienced trauma have an increased risk of post-traumatic stress disorder (PTSD) symptoms, depression, and suicide attempts. However, resilience and self-esteem are protective factors. We examined the effects of resilience and self-esteem on the relationship among traumatic experiences, PTSD symptoms, depression, and suicidal ideation. Methods: Middle-school students (n=403) completed questionnaires assessing traumatic experiences, PTSD symptoms, depression, suicidal ideation, resilience, and self-esteem. Path analysis was performed to investigate the mediating effects of PTSD symptoms, resilience, self-esteem, and depression on the relationship between trauma exposure and suicidal ideation. Results: Traumatic experience was positively correlated with PTSD symptoms, depression, and suicidal ideation. PTSD symptoms and depression were positively correlated with suicidal ideation. The relationship between traumatic experiences and suicidal ideation was mediated by PTSD symptoms, which had both direct and indirect effects on suicidal ideation; the indirect effect was mediated by resilience, self-esteem, and depression. Conclusion: Korean adolescents who had experienced trauma were more likely to develop PTSD symptoms, increasing their risk of depression and suicidal ideation. However, self-esteem and resilience may help protect against depression and suicidal ideation. Our findings could inform suicide prevention initiatives.

외상후 스트레스 장애에 대한 심리치료 효과 개관 : 재난 생존자를 중심으로 (Review of Psychological Treatment for Post-Traumatic Stress Disorder : Focus on Survivors of Disaster)

  • Objective : Disaster causes psychological distress to a large number of people in a short period of time, by both direct and indirect exposure to traumatic events embedded in various realms of disaster experience. Optimal, well-planned treatment interventions should follow from the early acute period to recovery phase, extending up to several months later. In this context, there is an increasing need for systemic review to gain comprehensive insights for disaster interventions. These need to be added to public policy, and for the prevention and treatment of disaster-related psychopathology. Here, we review the published studies on psychological interventions for disaster-related posttraumatic stress disorder. Methods : Specific psychological interventions regarded as effective treatments for have been selected for this review, such as CBT (Cognitive-Behavior Therapy), Exposure Therapy, EMDR (Eye Movement Desensitization & Reprocessing), SIT (Stress Inoculation Therapy) and Psychoeducation. In addition, natural disasters, industrial disasters, and accidents involving aircraft and ships were also categorized as disasters, along with war and combat trauma. Results : Cognitive behavior therapy and exposure therapy had the strongest research support for effectiveness, and could be considered as the first-choice treatment for disaster-related PTSD. The second line of treatment is EMDR, although this treatment modality has the advantage of reaching certain treatment improvements in fewer sessions. However, the effects of SIT and psychoeducation to the survivors of disasters, remains unclear at this point. Additionally, we propose the possibilities of using virtual reality component and imagery rescripting as modified forms of traditional cognitive behavior therapy and exposure therapy. Conclusion : Cognitive behavior therapy and exposure therapy, deemed effective treatments for various trauma, are considered to be effective for survivors from disasters. However, the efficacy of other interventions has not yet been examined methodologically in well-designed studies, such as randomized controlled trials. In particular, future empirical studies are needed, since it is difficult to conclude that psychological interventions have similar effects on different types of disasters.

북한 이탈 어머니들의 자녀양육행동과 양육효능감, 적응스트레스 및 외상 후 스트레스 증상 (Relationships between Parenting Behavior, Parenting Efficacy, Adaptation Stress and Post Traumatic Stress Disorder among Mothers who Defected from North Korean)

  • Purpose: The purpose of this study was to investigate the relationships between parenting behavior, parenting efficacy, adaptation stress and PTSD (Post Traumatic Stress Disorder) among mothers who have defected from North Korean. Methods: A descriptive study was conducted to illustrate the parenting behavior, parenting efficacy, adaptation stress and PTSD and report the relationships among them. Data were collected between August and November of 2009 in Hanawon and 105 mothers who met eligibility criteria participated in the study. Results: Ignorant/controlling parenting behavior of parenting showed the highest score in our study. The level of parenting efficacy was $2.72{\pm}0.36$ and adaptation stress level was $3.35{\pm}0.47$ showing perceived hostility the highest and homesickness the lowest. Participants with moderate to severe PTSD accounted for 79.5% of the mothers, and 75% of them reported symptoms for 3 months or more. Authoritative parenting behavior showed negative relationships with parenting efficacy, cultural crisis and fear, adaptation stress and PTSD. Conclusion: Mothers who have defected from showed negative parenting behaviors and high adaptation stress level and PTSD while parenting efficacy was high. These findings indicate that parenting intervention programs which can guide these mothers towards positive parenting behaviors need to be developed.

Social Network Effects on Post-Traumatic Stress Disorder (PTSD) in Female North Korean Immigrants

  • Objectives: The goal of this paper is to examine the social network effects on post-traumatic sdress disorder (PTSD) in female North Korean immigrants who entered South Korea in 2007. Specifically, it attempts to verify if the density and composition of networks make a difference after controlling for the network size. Methods: A multivariate logistic regression is used to probe the effects of social networks using the North Korean Immigrant Panel data set. Because the data set had only completed its initial survey when this paper was written, the analysis was cross-sectional. Results: The size of the support networks was systematically related to PTSD. Female North Korean immigrants with more supporting ties were less likely to develop PTSD, even after controlling for other risk factors (odds-ratio for one more tie was 0.8). However, once we control for the size of the network, neither the density nor the composition of the networks remains statistically significant. Conclusions: The prevalence of the PTSD among female North Korean immigrants is alarmingly high, and regardless of the characteristics of supporting network members, the size of the supporting networks provides substantial protection. This implies that a simple strategy that focuses on increasing the number of supporting ties will be effective among North Korean immigrants who entered South Korea in recent years.

L-Tetrahydropalmatine Ameliorates Development of Anxiety and Depression-Related Symptoms Induced by Single Prolonged Stress in Rats

  • Abnormal adaptation of the stress-response system following traumatic stress can lead to alterations in the hypothalamic-pituitaryadrenal (HPA) axis that may contribute to the development of post-traumatic stress disorder (PTSD). The present study used several behavioral tests to investigate the anxiolytic-like and antidepressant activity of L-tetrahydropalmatine (L-THP) in an experimental rat model of anxiety and depression induced by single prolonged stress (SPS), an animal model of PTSD. Male rats were treated intraperitoneally (i.p.) with vehicle or varied doses of THP 30 min prior to SPS for 8 consecutive days. Daily THP (50 mg/kg) administration significantly increased the number and duration of open arm visits in the elevated plus maze (EPM) test, reduced the anxiety index, increased the risk assessment, and increased the number of head dips over the borders of the open arms after SPS. THP was also associated with increased time spent at the center of the open field, reduced grooming behaviors in the EPM test, and reduced time spent immobile in the forced swimming test (FST). It also blocked the decrease in neuropeptide Y (NPY) and the increase in corticotrophin-releasing factor (CRF) expression in the hypothalamus. This is the first study to determine that THP exerts pronounced anxiolytic-like and antidepressant effects on the development of the behavioral and biochemical symptoms associated with PTSD, indicating its prophylactic potential. Thus, THP reversed several behavioral impairments triggered by the traumatic stress of SPS and is a potential non-invasive therapeutic intervention for PTSD.

한의치료와 '마음의 방 그리기'를 활용한 공황장애 치험 1례 보고 (A Case Report on Panic Disorder Patient Using the 'MRM (Mentalizing the Rooms of Mind)' and Korean Medicine Therapy)

  • Objectives: The purpose of this study was to introduce the progress of treatment and improve clinical use after conducting Mentalizing the Rooms of Mind, the main technique of Mindfulness & Loving Beingness psychotherapy, for a patient with panic disorder. Methods: We conducted a Mentalizing the Rooms of Mind for 10 sessions on an age 23 female diagnosed with panic disorder based on the DSM-5 diagnostic criteria. After receiving consent from the subject, through chart review, the progress of treatment was observed focusing on the MMPI-2 and CSEI-s (The Core Seven Emotions Inventory-short form) conducted pre- and post-treatment. This study was approved by the Institutional Review Board of Wonkwang University Sanbon Hospital (WMCSB202007-55). Results: 1. The MMPI-2 clinical scales of an age 23 female with panic disorder showed a 7 (Pt)-1 (Hs)-3 (Hy) profile pre-treatment, but for post-treatment, the scale showed 1 (Hs)-3 (Hy) profile, and the 7 (Pt) scale showed significant decline. In the MMPI-2 reconstructed clinical scales, RC7 (Dysfunctional Negative Emotions) and RC8 (Aberrant Experiences) showed significant decline. 2. In the pre- and post-treatment MMPI-2 content scales, Anxiety, Fears, Obsessiveness, Social Discomfort, and Work Interference scores decreased, showing overall positive stability. On the MMPI-2 supplementary scales, the Anxiety and Post-Traumatic Stress Disorder scores decreased, and the Ego Strength increased, resulting in improved overall psychological adaptation. 3. Pre- and post-treatment of an age 23 female with panic disorder, CSEI-s showed significant decline of 恐, 驚, 悲, and 思. So it seems that the emotions caused by Chiljeongsang (七情傷) were more stable than before treatment. Conclusions: As shown above, the treatment of panic disorder through Mentalizing the Rooms of Mind, a major technique of Mindfulness & Loving Beingness psychotherapy, showed positive changes in MMPI-2 as well as improvement of the subjective symptoms. Thus, Mentalizing the Rooms of Mind has high clinical use, and it seems that it is necessary to create a manual for this in the future.

혈장 세로토닌과 외상후 스트레스 장애 : 월남전 참전 재향군인을 대상으로 (Plasma Serotonin Level of Vietnam War Veterans with Post-Traumatic Stress Disorder and Symptom Severity)

  • Objective : The objective of this study was to examine the relationship between plasma serotonin concentration and posttraumatic stress disorder (PTSD) symptoms in chronic PTSD patients who have been taking medication. Methods : Plasma serotonin level of 14 PTSD patients and a control group of 28 Vietnam War veterans was measured by HPLC (high performance liquid chromatography). The Combat Exposure Scale (CES), Mississippi Scale for Combat-Related Posttraumatic Stress Disorder (M-PTSD), Clinician Administered PTSD Scale (CAPS), Hamilton Rating Scale for Depression (HRSD), and Hamiltion Anxiety Scale (HAS) were used to evaluate PTSD symptom severity. Results : Serotonin level was significantly higher in the PTSD group than in the control group (p=0.036, p=0.006, respectively). M-PTSD (p<0.001), CAPS (p<0.001), HRSD (p<0.001), and HAS (p<0.001) scale scores were significantly higher in the PTSD group than in the control group; however, the CES score failed to show a significant improvement (p=0.964). There were no significant differences between plasma serotonin and PTSD symptoms. Conclusion : In chronic PTSD patients who have been taking medications, we can not predict treatment effect and symptom severity by measuring only plasma serotonin levels. PTSD is a complicated disorder which may likely be related to a variety of neurotransmitter systems. Therefore, further research which investigate relationships with norepinephrine, dopamine, and other neurotransmitters as well as serotonin is needed to improve the treatment of PTSD.

Effects of an Online Imagery-Based Treatment Program in Patients with Workplace-Related Posttraumatic Stress Disorder: A Pilot Study

  • Objective We developed easily accessible imagery-based treatment program for patients with post-traumatic stress disorder (PTSD) related to workplace accidents and investigated the effects of the program on various PTSD related symptoms. Methods The program was based on an online platform and consisted of eight 15-min sessions that included script-guided imagery and supportive music. Thirty-five patients with workplace-related PTSD participated in this program 4 days per week for 4 weeks. Its effects were examined using self-report questionnaires before and after the take-home online treatment sessions. Results After completing the 4-week treatment program, patients showed significant improvements in depressed mood (t=3.642, p=0.001) based on the Patient Health Questionnaire-9 (PHQ-9), anxiety (t=3.198, p=0.003) based on the Generalized Anxiety Disorder seven-item (GAD-7) scale, and PTSD symptoms (t=5.363, p<0.001) based on the Posttraumatic Stress Disorder Check List (PCL). In particular, patients with adverse childhood experiences exhibited a greater degree of relief related to anxiety and PTSD symptoms than those without adverse childhood experiences. Conclusion The present results demonstrated that the relatively short online imagery-based treatment program developed for this study had beneficial effects for patients with workplace-related PTSD.

산림치유프로그램이 소방공무원의 외상 후 스트레스 및 기분상태 변화에 미치는 효과 (Effects of Forest Therapy Program on Stress levels and Mood State in Fire Fighters)

  • 본 연구는 국립산림치유원에서 실시한 4박 5일형 산림치유프로그램이 소방공무원의 외상 후 스트레스 및 기분상태 변화에 미치는 효과를 알아보고자 수행되었다. 소방공무원 293명을 대상으로 설문조사를 실시하였으며, SPSS 24.0을 활용하여 빈도분석, 대응 t-검정 및 공분산분석을 실시하였다. 산림치유프로그램 참여 전과 후의 외상 후 스트레스, 기분상태의 차이 및 인구학적 특성에 따른 차이를 분석하였다. 외상 후 스트레스 결과, 참여 전 평균 11.38 ± 12.58점에서 참여 후 6.91 ± 10.50점으로 감소하여 통계적으로 유의한 영향을 미치는 것으로 나타났다. 기분상태검사 결과, 긍정적 요인은 증가하였고, 부정적 요인은 감소하였다. 전체 기분상태 또한 프로그램 참여 전 8.58 ± 18.47점에서 참여 후 -0.63 ± 15.83점으로 통계적으로 유의한 영향을 미치는 것으로 나타났다. 수면시간에 따른 스트레스 저감 효과 차이 분석 결과, 외상 후 스트레스는 수면시간이 6~8시간일 때 효과가 높게 나타났다. 본 연구결과는 소방공무원의 외상 후 스트레스 관리와 해소를 위한 방법이 될 수 있는 근거자료로 활용되기를 기대한다.
