Pediatric Severe Traumatic Brain Injury : Updated Management

  • Traumatic brain injury (TBI) is the leading cause of death and disability in children. Survivors of severe TBI are more susceptible to functional deficits, resulting in disability, poor quality of life, cognitive decline, and mental health problems. Despite this, little is known about the pathophysiology of TBI in children and how to manage it most effectively. Internationally, efforts are being made to expand knowledge of pathophysiology and develop practical clinical treatment recommendations to improve outcomes. Here we discuss recently updated evidence and management of severe pediatric TBI.

Evaluation of the health status of preschool children stratified based on the weight-length index (WLI)

  • This study was conducted to prepare basic materials and offer advice regarding dietary habits to prevent and cure childhood obesity by comparing and analyzing dietary habit, nutritional status, blood factors, and mineral contents of hair. All subjects were stratified by their weight-length index (WLI). According to the standard WLI values, 64.9% of children were within the normal value, 13.5% of children were underweight, and 21.6% of children were overweight and obese (WLI ${\geq}$ 110%). Overall, the score assessed dietary habit for all children was $21.32{\pm}2.55$ point (921 subjects), with 5.1% of children having excellent dietary habits and 3.1% having poor dietary habits. Additionally, 37.9% of underweight children, 37.6% of normal weight children, and 43.2% of overweight and obese children consumed higher amounts of protein than underweight children did (meat, fish, eggs, and soy products) (P < 0.05). Overweight and obese children consumed more fried foods than underweight or normal weight children (P < 0.05). Moreover, 38.0% of the children had hemoglobin levels of 12 g/dl, while 7.6% were anemic (11.1 g/dl). When a hematocrit level of 33% was taken as the standard, 11.0% of children were anemic. The plasma transferrin content was $263.76{\pm}54.52$ mg/dl in overweight and obese children. The mean values of Fe, Cu, Ca, Cr, Mn, Se, Na, K, Li, V, Co, and Mo were within the reference values, but the Zn concentrations of underweight, normal weight, and overweight and obese children were $67.97{\pm}28.51$ ppm, $70.09{\pm}30.81$ ppm, and $73.99{\pm}30.36$ ppm, respectively. The Zn concentration of overweight and obese children ($73.99{\pm}30.36$ ppm) was lower than that of the standard value (180~220 ppm). Therefore, a nutritional education program and new guidance for dietary pattern should be developed to reduce the number of underweight and overweight and obese children.

EEG can Predict Neurologic Outcome in Children Resuscitated from Cardiac Arrest (심정지 후 회복된 소아 환자에서 뇌파를 통한 신경학적 예후 예측)

  • Purpose: Early prediction of prognosis of children resuscitated from cardiac arrest is a major challenge. We investigated the utility of electroencephalography (EEG) and laboratory studies for predicting of neurologic outcome in children resuscitated from cardiac arrest. Methods: We retrospectively analyzed medical records of patients who were resuscitated from cardiac arrest from 2006 to 2015 at the Gil Medical Center. Patients aged one month to 18 years were included. EEG analysis included background scoring, reactivity and seizure burden. EEG background was classified score 0 (normal/organized), score 1 (slow and disorganized), score 2 (discontinuous or burst suppression), and score 3 (suppressed and featureless). Neurologic outcome was evaluated by Pediatric Cerebral Performance Category (PCPC) at least 6 months after cardiac arrest. Results: Total 26 patients were evaluated. Nine patients showed good neurologic outcome (PCPC 1, 2, 3) and 17 patients showed poor neurologic outcome (PCPC 4, 5, 6). Patients of poor neurologic outcome group showed EEG background score 3 in 88.2%, whereas 44.4% in patients of good neurologic outcome group (P=0.028). Electrographic ictal discharges except non-convulsive status epilepticus were presented in 44.4% of good neurologic outcome group and 5.9% of poor neurologic outcome group (P=0.034). Ammonia and lactate levels were higher and pH levels were lower in poor outcome group than good neurologic outcome group. Conclusion: Suppressed and featureless EEG background is associated with poor neurologic outcome and electrographic seizures are associated with good neurologic outcome.

A Clinical study on the Treatment of Nasal disease using TonggyutangGamibang (통규탕가미방(通竅湯加味方)의 비질환(鼻疾患) 치료에 대한 임상고찰)

  • Objective: For last 20 years our living environment has been changed rapidly. Accordingly. there have been many changes in prevalence of diseases; In nasal disease of the pediatrics, the number of children suffering from chronic rhinorrhea and nasal obstruction has been increased. And respiratory symptoms suggesting rhinitis and sinusitis also were increased. Allergic disease is considered as an important cause of this phenomenon while importance of infection as pathogen is fading today. The aim of this study is to investigate the effect of TonggyutangGarnibang on the nasal disease of children. Methods: Children with nasal disease answer the questionnaires on their chief complaints and medical past history on their first visit to our clinic. They were treated 'With TonggyutangGarnibang for 4weeks. After treatment they answered the questionnaire on improvement of their symptoms. Results: Among treated patients, 70 were male(69%) and 32 were female(31%); sex ratio was 2.2.3:l(male:female). And children from 3-8 years old took majority of patients(64%). Major symptoms of children with nasal disease were as follows; nasal obstruction 9lchildren(62%), rhinorrhea 86children(84%), sneezing 63children(62%), cough 63children(62%), snoring 48children (47%), poor appetite 44children(43%), itching 39children(38%), headache 32children(31%), stomache 25children(25%), epistaxis 23children(23%), constipation 17children(17%), diarrhea 12children(12%). Past history and impression of the disease of the patients were as follows; allergic rhinitis(66%), atopic dermatitis(42%), sinusitis(40%), asthma(25%), urticaria(12%). Severity of symptom improved after TonggyutangGamibang treatment, it showed high treatment efficacy as follows; sneezing 70%, cough 70%, rhinorrhea 63%, nasal obstruction 59% Conclusion: This study shows TonggyutangGamibang has distinctive treatment effect on children with nasal disease.

An Ecological Study on the Environment of the Children in the Deprived Areas in Seoul (빈민지역 아동의 생활환경에 관한 생태학적 연구)

  • The present study was intended to investigate ecological characteristics of poverty areas in Seoul from the perspectives of children's development. More specifically, the objectives of the study are to examine environmental ecology of the poverty areas and it's potential psycho-social development of children. Three poverty areas and 150 children currently living in the are as were sampled ; 70 children from middle class families and 88 from lower class respectively. Focal ecological aspects surveyed family environment, outdoor play ground and facilities, and village housing structures. Psycho-social dimension's development dealt with in the study included children's perceptions of parental child rearing practices and role taking, and children's future perception and perspectives. Data were collected through interviews and observations using structured formats. Following results were obtained from the data : 1. Children from the poverty families showed a higher tendency to have working mothers than those from middle class. 2. The rate of having single room is higher for children from middle class by two times than from lower class. 3. Children from lower class tended to have less favorable perceptions of attitudes tote and their own families than those from middle class. 4. The likelihood of showing discrepancy in vocational choice for children between parents and children is higher for children from lower class and those from middle class. 5. Children from lower class tended to allot more time in playing with peers than those from middle class. 6. The size of outdoor play ground are inadequately poor both for middle and lower class children. The inadequateness is particularly severe for the lower class children, showing that the size and facilities of playing ground available for the child from lower class is less than half compared with those of the children from middle class.

  • Background: Epilepsy is a disorder of the central nervous system characterized by loss of consciousness and convulsions. Epilepsy, a typical chronic disease, may cause high level of psychosocial difficulties for all family members, including stigmatization, stress, martial problems, poor self esteem and restriction of social activities. Objective & Method: To demonstrate the therapeutic effect of herbal medicine (Kamigoakhyangjeonggisan) on patients with epilepsy. We treated two cases of epileptic children, who had frequent seizures. Results: Two children improved in the frequency of the seizure, the combined disabilities. Conclusion: We report that we had good effects of herbal medicine treatment on two cases of epileptic children.

Kindergartners' Reading of Words in Hangul : Effects of Phonological Awareness and Processing (음운론적 인식과 처리능력이 4-6세 유아의 한글 단어 읽기에 미치는 영향)

  • Causal relationships of kindergarteners' phonological awareness and processing to their ability to read words was investigated with the participation of 289 4- to 6-year-old children attending three kindergartens in Busan. Results showed gradual growth in reading ability with age. Children performed best in reading words and poorest in reading low frequency letters. They showed continuous development in skills of syllable deletion, phoneme substitution, phoneme insertion, phonological memory and naming. Discontinuous development was found in counting syllables. Longer syllables were difficult to count, and middle syllables of 3 syllable words were hard to delete. Children had poor perception of final consonants of Consonant-Vowel-Consonant syllables. Children's phonological awareness and processing were latent variables strongly related to ability to read words written in Hangul.

Behavioral insomnia in infants and young children

  • In infants and young children, bedtime problems and night waking are common and the main presentations of insomnia. Poor sleep may critically impact the daytime functioning and mood of the child and their caregivers. A comprehensive sleep history, a sleep diary/log, and the BEARS (Bedtime problems, Excessive daytime sleepiness, Awakenings during the night, Regularity and duration of sleep, and Sleep-disordered breathing) sleep screen are useful for diagnosing sleep problems in young children. Behavioral therapies for this type of insomnia include extinction, bedtime fading with positive routines, and scheduled awakening. Previous studies of behavioral interventions for young children showed significant improvements in sleep-onset latency, night waking frequency, and night waking duration. Parent education about their child's sleep, bedtime routines, and sleep hygiene is essential for treatment.

  • Recently, the number of children who are required to take special care are increasing because of the fast-changing society and the environment factor. Among them, most children in poor family are leading to Children with borderline intelligence, so urgent action are needed to prevent this situation. This study is conducted to establish the reason why children with borderline intelligence are taking place. In addition, this study embodies serious game as a solution which is able to prevent and cure this children with borderline intelligence problem. This game was made to improve weak memory, concentration and judgment of children with borderline intelligence and was verified in effectiveness by thirty people and experts.

Speech processing strategy and executive function: Korean children's stop perception

  • The current study explored how Korean-speaking children processed the multiple acoustic cues (VOT and f0) for the stop laryngeal contrast (/t'/, /t/, and /$t^h$/) and examined whether individual perceptual strategies could be related to a general cognitive ability performing executive functions (EF). 15 children (aged from 7 to 8) participated in the speech perception task identifying the three Korean laryngeal stops (3AFC) on listening to the auditory stimuli of C-/a/ with synthetically varying VOT and f0. They completed a series of EF tasks to measure working memory, inhibition, and cognitive shifting ability. The findings showed that children used the two cues in a highly correlated manner. While children utilized VOT consistently for the three laryngeal categories, their use of f0 was either reduced or enhanced depending on the phonetic categories. Importantly, the children's processing strategies of a f0 suppression for a tense-aspirated contrast were meaningfully associated with children's better cognitive abilities such as working memory, inhibition, and attentional shifting. As a preliminary experimental investigation, the current research demonstrated that listeners with inefficient processing strategies were poor at the EF skills, suggesting that cognitive skills might be responsible for developmental variations of processing sub-phonemic information for the linguistic contrast.
