Poor People and Poor Health: Examining the Mediating Effect of Unmet Healthcare Needs in Korea

  • Objectives: The purpose of this study was to estimate the mediating effect of subjective unmet healthcare needs on poor health. The mediating effect of unmet needs on health outcomes was estimated. Methods: Cross-sectional research method was used to analyze Korea Health Panel data from 2011 to 2015, investigating the mediating effect for each annual dataset and lagged dependent variables. Results: The magnitude of the effect of low income on poor health and the mediating effect of unmet needs were estimated using age, sex, education level, employment status, healthcare insurance status, disability, and chronic disease as control variables and self-rated health as the dependent variable. The mediating effect of unmet needs due to financial reasons was between 14.7% to 32.9% of the total marginal effect, and 7.2% to 18.7% in lagged model. Conclusions: The fixed-effect logit model demonstrated that the existence of unmet needs raised the likelihood of poor self-rated health. However, only a small proportion of the effects of low income on health was mediated by unmet needs, and the results varied annually. Further studies are necessary to search for ways to explain the varying results in the Korea Health Panel data, as well as to consider a time series analysis of the mediating effect. The results of this study present the clear implication that even though it is crucial to address the unmet needs, but it is not enough to tackle the income related health inequalities.

소득이전의 빈곤완화 및 빈곤이행 효과에 관한 연구 (The Effect of Income Transfer on Poverty Rate)

  • The primary purpose of this study is to analyze the decreasing effect of public and private income transfers on poverty rate. Two year data of Korean Labor and Income Panel Study (KLIPS, 1998, 1999) are used for the analysis, and 1/2 of median income and 1/3 of mean income are adopted to measure poverty rate. Although private income transfer contributes more to reduce the rate than the public transfer, the main effect for decreasing poverty rate is forced by the wage. Statistically significant variables that affect to the exit of poverty based on the logistic regression analysis are number of family members(-), wage(+), property income(+), social insurance benefit(+), and the transfer income(+). Therefore, the future policy should be more related with the active labor market policy for developing better human resources among the poor family.

Social Distancing, Labor Supply, and Income Distribution

  • The effects of social distancing measures on income distributions and aggregate variables are examined with an off-the-shelf heterogeneous-agent incomplete-market model. The model shows that social distancing measures, which limit households' labor supply, can decrease the labor supply of low-income households who hold insufficient assets and need income the most given their borrowing constraints. Social distancing measures can therefore exacerbate income inequality by lowering the incomes of the poor. An equilibrium interest rate can fall when the social distancing shock is expected to be persistent because households save more to prepare for rising consumption volatility given the possibility of binding to the labor supply constraint over time. When the shock is expected to be transitory, in contrast, the interest rate can rise upon the arrival of the shock because constrained households choose to borrow more to smooth consumption given the expectation that the shock will fade away. The model also shows that social distancing shocks, which diminish households' consumption demand, can decrease households' incomes evenly for every income quantile, having a limited impact on income inequality.

Has Income-related Inequity in Health Care Utilization and Expenditures Been Improved? Evidence From the Korean National Health and Nutrition Examination Survey of 2005 and 2010

  • Objectives: The purpose of this study is to examine and explain the extent of income-related inequity in health care utilization and expenditures to compare the extent in 2005 and 2010 in Korea. Methods: We employed the concentration indices and the horizontal inequity index proposed by Wagstaff and van Doorslaer based on one- and two-part models. This study was conducted using data from the 2005 and 2010 Korean National Health and Nutrition Examination Survey. We examined health care utilization and expenditures for different types of health care providers, including health centers, physician clinics, hospitals, general hospitals, dental care, and licensed traditional medical practitioners. Results: The results show the equitable distribution of overall health care utilization with pro-poor tendencies and modest pro-rich inequity in the amount of medical expenditures in 2010. For the decomposition analysis, non-need variables such as income, education, private insurance, and occupational status have contributed considerably to pro-rich inequality in health care over the period between 2005 and 2010. Conclusions: We found that health care utilization in Korea in 2010 was fairly equitable, but the poor still have some barriers to accessing primary care and continuing to receive medical care.

Factors contributing to participation in food assistance programs in the elderly population

  • BACKGROUND/OBJECTIVES: The study objectives were to examine the participation rate in food assistance programs and explore the factors that contribute to such participation among the Korean elderly population. SUBJECTS/METHODS: The study sample comprised 3,932 respondents aged 65 years or older who were selected from a secondary data set, the fourth Korean Welfare Panel Study (KoWePS). The factors, related to participation in programs were examined based on the predisposing, enabling and need factors of the help-seeking behavior model. Multiple logistic regression analysis was used to select the best contributors among the factors related to program participation. RESULTS: The predisposing rate in food assistance programs was 8.5% (7.1% for men and 10.4% for women). When all variables were included in the model, living without spouse, no formal education, low income, having social security benefits and food insecurity in elderly men, and age, low income, having social security benefits and feeling poor in elderly women were significantly related to a higher tendency to program participation. CONCLUSIONS: The predisposing and need factors, such as living without spouse, low education level, food insecurity and feeling poor were important for program participation, as well as enabling factors, such as household income and social security benefits. A comprehensive approach considering these factors to identify the target population for food assistance programs is needed to increase the effectiveness and target population penetration of these programs.

인도네시아 정당체계의 연속성과 정치균열의 문제: 도시부문 설문조사를 바탕으로 (Islam, Authoritarian Legacy, and Income in the Continuity of Indonesia's Party System: Based on Urban Survey Data)

  • This study addresses the issue of the continuity in Indonesia's party system with respect to Islam, authoritarian legacy and income, using the urban-based individual face-to-face survey data. The existing studies focus on the historical continuity of the Indonesian party system between the 1955 and 1999 democratic elections. Yet, this study deals with the continuity and discontinuity between the pre-transitional 1997 election and the transitional 1999 election. It finds that the effect of Islam is largely independent of the democratic transition: Islam-oriented voters under the authoritarian rule tend to remain in the Islamic camp even during and after the democratic transition, while most of the secular voters prior to the democratic transition continue to shun any Islamic parties during the democratization. The effect of authoritarian legacy is also found to be meaningful, even if not as sticky as Islam. Finally, the variable of income is significant. Contrary to the popular belief, the PDIP is not a party of the urban poor but the PPP is the urban poor's favorite choice. This implies the linkage between poverty and political Islam.

Tax Incidence of Philippine Tax Reform: Poverty and Distributional Effect

  • The purpose of the study is to determine the poverty and distributional effects of the implementation of Tax Reform for Acceleration and Inclusion Law. The Computable General Equilibrium-Top Down Behavioral Microsimulation was used to obtain the effects of the tax reform on macroeconomic and microeconomic levels. Moreover, the Poverty Gap Index, Squared Poverty Gap Index, Foster, Greer, and Thorbecke Measures of Poverty, and Sen-Shorrocks-Thon Index were used to measure the poverty effect of the tax reform. Meanwhile, the Gini Coefficient and SST Gini Coefficient Index were used to measure the distributional effect of the tax reform. The results show that the implementation of the tax reform has resulted in a significant increase in household income and disposable income. Region IV has the highest estimated increase in household income. Meanwhile, Region IV remained to have the lowest household income. Further, the findings of this study suggest that the tax reform resulted in a significant decrease in the magnitude of poor and the number of poor in the Philippines. However, the result of the study also suggests that the effect of tax reform manifests no differences in terms of the poverty gap measured through the Foster, Greer, and Thorbecke poverty index due.

저소득 한부모가정, 사례에 비추어 본 지원방안 연구 : 건강가정지원센터 활용을 중심으로 (A Study on How to Provide Support to Poor Single Families based on Case Studies)

  • The purpose of this study is to identify the characteristics and the limits of, as well as, Propose an improvement of, the government based policies that support poor single families. For this purpose, this study has analyzed the government based supporting policies for poor single families into four different aspects; income support, dwelling support, medical support, and child-caring support. Also, in order to analyze the situation of the poor single family as well as the limits of the government based supporting policies, an in-depth interview has been conducted with 8 personnel (including 7 single parents and 1 social worker). In the final analysis, a total of 5 case studies have been used to identify the characteristics of the government based supporting policies for various poor single families. As a result, it turned out that the economic situation of the poor single families were extremely unfavorable, and the quality of life was extremely low in the aspects of dwelling, nutrition, health child nurturing and education. Therefore, we are proposing the following supporting policies for the improvement of these families' living conditions: increasing income levels, providing job opportunities, securing dwelling places, providing medical support, and implementing child care benefit policies. Furthermore, we are proposing an expansion of the human services provided by the healthy family support center to these poor single families.

소득불평등 개념과 실태 (The Concept and Actual Conditions of Inequality)

  • 본고는 (1) 소득불평등도에 대한 개념 인식을 설문조사를 통해 외국의 경우와 비교분석하고 (2) 서로 다른 개념의 소득불평등 지수가 실증적으로 어떠한 차이가 있는지를 확인하고 (3) 그를 통하여 향후 소득분배 정책방향을 모색하기 위하여 작성되었다. 설문조사의 결과는 경제학에서 일반적으로 사용하고 있는 상대적 소득불평등도의 기본 가정에 대하여 응답자의 다수가 동의하고 있지 않다는 다소 놀라운 결과를 포함하고 있다. 이는 외국의 경우와 유사한 결과이나, 외국의 경우와 차이점은 한국인은 현 시점에서 상대적 소득불평등도의 증가보다는 절대적 소득불평등도, 즉 절대적 소득격차의 변화에 좀더 민감하게 반응한다는 것이다. 한편 양극화 지수는 지니계수와 이론적으로는 다른 기반을 가지고 있으나 한국에 있어 통계학적으로 다른 지수라 보기 어렵다는 점이 확인되었다. 소득불평등도의 변화는 그 변화의 양상이 절대적이냐 상대적이냐에 따라 성장과 빈곤의 변화에 서로 다른 영향을 미친다는 것이 그동안 학계에 축적된 연구 결과이기 때문에, 국가발전전략의 설정에 있어 소득분배정책의 방향은 빈곤 감소적 성장을 추구할 필요가 있다는 것이 본고가 제시하는 시사점이다.

The Impact of Capital Account Openness on Income Inequality: Empirical Evidence from Asia

  • The relationship between income inequality and capital account openness is empirically investigated in this study, where macroeconomic variables have opposing effects. Panel data used in the study from the KAOPEN Index and World Bank consists of 28 Asian countries and has been examined; it contains annual observations from 1970 to 2018. The data is examined using a random-effect model based on GMM estimates. Income inequality and capital account openness are positively and significantly related, according to our findings. Overall, the findings imply that increasing income gaps reduced capital investment in nations with large discrepancies. The growing economic discrepancy is being caused by the rich's increasing income share at the expense of the poor. In Asia, inward capital account openness exacerbates income inequality, while outward capital account openness exacerbates it. As a result, income inequality slows economic growth, leading to inflation, unemployment, and increased government spending in several Asian countries. Our control factors, GDP, and other secondary school enrolments, all had a statistically significant negative relationship with income inequality. Income disparity has a positive and statistically significant association with government spending, inflation, population, trade openness, and unemployment. Income disparity has a negative association with capital account openness, gross domestic product, and secondary school enrollment.
