Polyethylene Glycol Plus Electrolytes with Stimulant Laxative in Paediatric Faecal Disimpaction: A Randomised Controlled Study

  • Functional constipation is common in the paediatric population all over the world. Effective disimpaction to evacuate the impacted faecal matter forms an essential initial step in the management of constipation. Though different regimens of polyethylene glycol 3350 with electrolytes (PEG+E) are accepted as the prime medication for disimpaction, response is not always satisfactory. A randomised prospective study was undertaken, in a tertiary paediatric Gastroenterology centre to find out the outcome of a 2-day disimpaction when a stimulant laxative sodium picosulphate was added to PEG3350+E (PEG+E+PS group) and comparing it with the outcome using PEG3350+E (PEG+E group) alone. Hundred and one children were randomised into two groups to receive PEG+E+PS and PEG+E. Results revealed that PEG+E+PS group proved significantly superior to PEG+E group in most of the efficacy-parameters in terms of disimpaction as well as long-term management of constipation. Though stimulant laxatives are being used for disimpaction, comparative data are lacking. This was the 1st such comparative study looking at the efficacy of these two processes of disimpaction along with long term effect on treatment.

  • 목 적 : 만성 기능성 변비 환아의 치료에서 PEG 4000과 락툴로스의 임상적 효용성에 대해 비교 분석 하였다. 방 법 : 2004년 9월부터 2007년 2월까지 노원 을지병원 소아과에 만성 반복성 복통이나 배변 장애를 주소로 내원하여 만성 기능성 변비로 치료받고 1년간 외래 추적이 가능했던 환아 56명을 대상으로 하였다. 전해질이 함유되지 않은 PEG 4000을 투여한 24명의 환아와 락툴로스를 투여한 32명의 환아들을 대상으로 치료 시작시와 치료 1개월, 3개월, 6개월, 1년 후에 각 약물의 효과와 부작용에 대해 비교 분석하였다. 결 과 : 두 군 모두 배변 횟수가 치료전에 비해 유의하게 증가하였고(P<0.05), 복통 또한 치료 전에 비해 유의한 감소가 있었다(P<0.05). 치료를 시작한 시점에서 12개월이 되었을 때 변비와 유분증으로부터의 회복은 PEG 투여군에서는 12례(60%)에서, 락툴로스 투여군에서는 15례(57.7%)에서 있었으며 두 군 간에 유의한 차이는 없었다. 결 론 : 유소아 만성 변비에서 PEG 4000과 락툴로스는 장기적 유지 요법으로 안전하고 효과적인 약물이며 변비와 유분증으로부터의 회복에 두 군간에 유의한 차이는 없었다.

  • 목 적 : 전해질이 함유된 PEG 용액은 삼투성 제재로 소아의 대장내시경 및 수술 전 저치를 위한 장세척에 주로 사용되어 왔으나 다른 삼투성 제재에 비하여 만성 기능성 변비 치료의 분변박힘 제거에 대한 연구는 극히 미흡하다. 이에 저자들은 만성 기능성 변비로 입원한 환아들을 대상으로 분변 박힘 제거에 있어서 전해질이 함유된 PEG 용액의 효과 및 안전성에 대하여 알아보고자 하였다. 방 법: 2000년 5월부터 2003년 7월까지 을지병원 소아과에 입원한 만성 기능성 변비 환아 26명(11명은 외래에서 일반적 방법으로 분변박힘 제거에 실패하였다)을 대상으로 전해질이 함유된 PEG 용액을 경구 요법(9명, 34.6%), 관장 요법(10명, 38.4%), 또는 병행 요법(7명, 14.3%)을 시행하여 분변 박힘 제거의 효과와 배변횟수의 변화, 부작용의 발생을 확인하였다. PEG 용액은 경구 요법의 경우 1회 60-80 mL/kg(PEG로 3.9-5.3 g/kg)를 3시간 이내에 음용하였으며, 하루에 1-2회 시행하였다. 관장 요법의 경우 1회 15-25 mL/kg(PEG로 0.975-1.625 g/kg)로 하루에 1-3회 관장을 시행하였다. 결 과 : 모든 환아에서 PEG 용액 요법 시행 전에 단순 복부 방사선 사진에서 분변 박힘이 확인되었으며 시행 후에는 분변 박힘이 호전되었다. 배변횟수는 관장 요법을 시행한 환아 중에서 1명을 제외한 모든 환아에서 유의한 증가를 보였다. 부작용은 하루 6회 이상의 수양성 변이 3명(11.5%)에서 발생하였고, 두통이 1명(3.8%)에서 발생하였으나 별다른 치료없이 호전되었다. 이외의 부작용의 증상은 발생하지 않았다. 치료 후 혈청 전해질 농도 검사는 16명의 환아에서 시행되었으며 두통을 호소했던 환아에서 경미한 고나트륨혈증(146 mmol/L)이 있었으며 이외의 환아에서는 정상 범위 내로 나타났다. 치료 후 혈청 삼투질 농도 검사는 11명의 환아에서 시행됐으며 모두 정상 범위 내로 나타났다. 순응도는 경구 복용군의 환아들이 짠맛에 대한 부담감을 호소하였으나 복용에는 지장이 없었다. 결 론 : 일반적인 요법으로 분변 박힘 제거에 실패한 환아를 비롯한 만성 기능성 변비 환아에서 전해질이 포함된 PEG 용액은 분변 박힘 제거에 효과가 있었으며 심각한 부작용은 없었으며 순응도도 좋은 것으로 나타났다. 그러나 연구 대상의 수가적고, 모든 환아에서 치료 후의 혈청 전해질 농도 및 삼투질 농도의 확인이 이루어지지 않은 문제점이 있기 때문에 향후 보다 많은 소아를 대상으로 연구가 시행되어져야 할 것으로 생각된다.

  • We have investigated the effect of the number of ethylene oxide (EO) units inside poly(ethylene glycol)dimethacrylate (PEGDMA) on the ionic conductivity of its gelled polymer electrolyte, whose content ranges from 50 to 80 wt%. PEGDMA gelled polym er electrolytes, a crosslinked structure, were prepared using simple photo-induced radical polymerization by ultraviolet light. The effect of the number of EO on the ionic conductivity was clearly shown in samples of lower liquid electrolyte content. We have concluded that the ionic conductivity increased in proportion to both the number of EO units and the plasticizer content. We have also studied the electrochemical properties of 13PEGDMA (number of EO units is 13) gelled polymer electrolyte.

  • The widely used polyethylene glycol (PEG)-based solutions have been proven effective for bowel preparation when 4 L of the solution is administered before colonoscopy. However, large volumes of the solutions are generally poorly tolerated. A new PEG-based solution consisting of 2 L of PEG and a high dose of ascorbic acid has recently become available. Electrolyte abnormalities caused by PEG-based solutions have rarely been reported. We report on a case of acute severe hyponatremia with associated generalized tonic-clonic seizures after bowel preparation with a low-volume PEG plus ascorbic acid solution in a 74-year-old woman with no history of seizures. She took a beta blocker, an angiotensin-converting enzyme inhibitor, and glimepiride for hypertension and diabetes mellitus. She showed general weakness, nausea, agitation, muscle cramping, and seizures after ingestion of the PEG plus ascorbic acid solution. Her serum sodium level was 112 mEq/L. Her symptoms improved after intravenous administration of hypertonic saline. Physicians should pay attention to screening for electrolytes and development of neurological symptoms during bowel preparation.

Effect of Additives on the Cloud Point of Polyethylene Glycols

  • Polyethylene glycol 20, 000 and 6, 000 were found to have an upper consolute temperature, called "cloud point", and the effects of various additives on the polythylene glycols were investigated in this study. Electrolytes lowered the cloud point in proportion to their concentrations through dehydration and electrostriction. It was found that anions played a more important role than cations and the effects of both the cations and the anions clearly followed the classical Hofmeister series. However, the Schultze Hardy rule holds for the effect of anions, and fails for the effect of cations. Salts of large polarizable anions such as iodide and thiocynate rather raised the cloud point, and their effects were ascribed to the fact that they break the water structure and weaken hydrophobic bonding of the polyxyethylene moiety. Nitrates of polyvalent cations also raised the cloud point. This was ascribed to the complex formation between the polyvalent cations and ether oxygens of the polyoxyethylenes. This explained the failure 'of the Schultz-Hardy rule for cations. Uncharged aromatic compounds drastically lowered the clound point, while aliphatic alcohols slightly lowered the cloud point, This result suggests that there might be some interaction between ether oxygens and aromatic nucleus.c nucleus.

Efficacy and Safety of Combined Oral and Enema Therapy Using Polyethylene Glycol 3350-Electrolyte for Disimpaction in Pediatric Constipation

  • Purpose: We evaluated the efficacy and safety of combined oral and enema therapy using polyethylene glycol (PEG) 3350 with electrolyte solution for disimpaction in hospitalized children. Methods: We retrospectively studied 28 children having functional constipation who received inpatient treatment between 2008 and 2016. The amount of oral PEG 3350 electrolyte solution administered was 50-70 mL/kg/d (PEG 3350, 3-4.1 g/kg/d), and an enema solution was administered 1-2 times a day as a single dose of 15-25 mL/kg (PEG 3350, 0.975-1.625 g/kg/d). A colon transit time (CTT) test based on the Metcalf protocol was performed in some patients. Results: Administration of oral and enema doses of PEG 3350 electrolyte solution showed $2.1{\pm}0.3$ times and $2.9{\pm}0.4$ times, respectively. After disimpaction, the frequency of defecation increased from $2.2{\pm}0.3$ per week to once a day ($1.1{\pm}0.1$ per day). The number of patients who complained of abdominal pain was reduced from 15 (53.6%) to 4 (14.3%). Before hospitalization, nine patients underwent a CTT test, and 5 of 9 patients (55.6%) were classified as belonging to a group showing abnormalities. And in some patients, mild adverse effects were noted. We examined electrolytes and osmolality before and after disimpaction in 16 of 28 patients, and no abnormalities were noted. Conclusion: In terms of therapeutic efficacy and safety, combined oral and enema therapy using high-dose PEG 3350 with electrolytes is considered superior to conventional oral monotherapy or combined oral and enema therapy on an outpatient basis.

유연한 전기변색 소자를 위한 세공충진 고분자 전해질 멤브레인의 개발 (Development of Pore-filled Polymer Electrolyte Membranes for Flexible Electrochromic Devices)

  • 유연한 전기변색 소자(electrochromic device, ECD)는 스마트 윈도우 등 다양한 분야에서 응용이 기대되는 유망한 기술이다. 고분자 전해질은 유연한 ECD의 탈-변색 성능 및 물리적 안정성을 결정하는 중요한 구성요소이다. 본 연구에서는 효과적인 유연한 ECD 제조 및 내구성 향상을 위해 치수안정성이 우수한 세공충진 고분자 전해질 멤브레인(PFPEM)을 개발하였다. 저렴하며 물리적 및 화학적 안정성이 우수한 폴리에틸렌 재질의 다공성 지지체의 세공에 접착력이 우수한 polyvinyl acetate와 이온전도도를 향상시킬 수 있는 polyethylene glycol을 사용하여 제조한 고분자 전해질을 충진하였다. 제조된 PFPEM의 최적 리튬 염(LiTFSI) 함량은 약 27 wt%에서 결정되었으며 우수한 치수안정성와 접착 강도 그리고 종래의 고분자 전해질에 근접하는 이온전도 특성을 가지고 있음을 확인하였다. 다공성 지지체의 사용으로 가시광 투과율이 저하되었으나 변색 상태에서는 오히려 장점으로 작용할 것으로 전망되었다.

  • Constipation is one of the most common symptoms that a child visits pediatrician. The general approach to the child with functional constipation includes the following steps; disimpaction, maintenance and withdrawal of medication. There are many drugs which can be applied to children; osmotic agents (lactulose, sorbitol, magnesium hydroxide/citrate, polyethylene glycol with/without electrolytes, sodium phosphate, glycerin), stimulants (senna, bisacodyl, caster oil), lubricant (mineral oil), bulking agent (psyllium, cellulose, glucomannan). At each stage of treatment, one or some of these drug can be applied to the purpose. The author tries to summarize recent studies on drugs for constipation in child, and finally introduces new dugs for constipation which is under investigation.

Methyl Methacrylate의 Emulsion Polymerization에 關한 硏究 (A Study on the Emulsion Polymerization of Methyl Methacrylate)

  • With the selected emulsifiers for the emulsion polymerization of methyl methacrylate, the HLB of the emulsifier in the reaction system has been studied on the effect of the ratio of tetra sodium-N-(1,2-dicarboxy ethyl)-N-octadecyl sulfosuccinamate(Aerosol 22) to polyethylene glycol nonyl phenyl ether (Noigen EA 160), and also sodium lauryl sulfate(Quolac EX-UB), Disodium-N-octadecyl sulfosuccinamate (Aerosol 18) and Aerosol 22 as emulsifiers having various hydrophilic groups in the molecules have been studied. Results are as follows; 1) The viscosity of the emulsions and the molecular weight of the polymers have maximum values at a constant HLB value of emulsifiers, but their stabilities show minimum point at the value with the titration with the three kinds of mono, bi, tri-valent electrolytes. These results are agreed on the theory of Greth & Wilson in which the properties of polymer emulsions depend upon the HLB system of emulsifiers. 2) The viscosity of the emulsions and the molecular weights of the produced polymers increase more in the case of blending of Aerosol 22 to Noigen EA-160 than of the separate using. 3) The coagulation effects of the divalent electrolytes($ex,\;Ca^{++},\;Zn^{++}$) are contrast to the effects of monovalent($ex,\;Na^+$) and trivalent($ex,\;Al^{+++}$) in the emulsions with Aerosol 18 or Aerosol 22 which have more than two hydrophilic groups. It seems that the stability of the O/W emulsions by electrolytes is directly related to the parameters of surface physical chemistry such as surface geometry of surface chemical constitution of polymer particles.
