고혈압노인의 식사시간 별 식후저혈압 발생실태 (Prevalence of Postprandial Hypotension in the Elderly People with Hypertension according to Mealtimes)

  • 본 연구의 목적은 고혈압노인의 식사시간 별 혈압변화와 식후저혈압(PPH: Postprandial hypotension) 발생을 파악하기 위해 아침, 점심, 저녁 세 끼 식사 전후의 혈압을 측정하였다. 노인 복지시설에 거주하는 고혈압 노인 187명으로부터 서면동의를 받은 후 시행되었다. 혈압측정 도구는 휴대용 혈압모니터(ABPM)를 사용하여 아침, 점심, 저녁의 식전 30분부터 식후 2시간까지 15분 간격으로 측정하였다. 수집된 자료는 SPSS Win 14.0 program으로 빈도와 백분율, 평균과 표준편차, ANOVA와 Scheffé 사후검정을 이용하여 분석하였으며, 연구결과는 다음과 같다. • 수축기혈압과 이완기혈압의 하강 정도는 아침 식후가 점심이나 저녁 식후 보다 유의하게 더 많았다. • PPH 발생률은 아침 식후에는 73.4%, 점심 식후에는 54.2%, 저녁에는 50.2%이었다. • PPH가 가장 많이 발생한 시점은 아침과 점심 저녁 모두 식사 시작 후 90분이었다. • 식후 혈압을 8회 측정한 것을 기준했을 때, 평균 PPH 발생 횟수는 아침 식후가 평균 3.0±2.7회로 점심 평균 2.1±2.5회와 저녁 평균 1.9±2.4회 보다 더 많았다. 고혈압노인의 간호에서는 점심이나 저녁 식사에서보다는 아침식사 후에 PPH 발생이 더 많다는 점을 고려하여야 할 것이다. 또한 식사 시작 후 1시간에서 2시간 동안은 PPH로 인한 건강문제가 발생하지 않도록 주의 관찰과 간호관리가 이루어져야 한다. 아울러 고혈압노인의 PPH를 조절할 수 있는 간호중재를 개발하기 위한 추후연구가 활성화되어야 한다.

인천지역 대학생의 변비유병률, 배변습관 및 영양소섭취 (Prevalence of Constipation, Bowel Habits and Nutrient Intakes of College Students in Incheon Area)

  • 대학생들의 변비유병률과 배변습관을 알아보고 영양소섭취 상태를 알아보고자 인천지역 대학교에서 건강관련 교양강의를 듣는 수강생들을 대상으로 (남자 166명, 여자 187명) 설문조사와 신체계측 및 3일간 식이섭취 조사를 실시한 결과는 다음과 같다. 조사 대상자의 평균 연령은 남학생 23.5세, 여학생 20.9세이었으며, 자택에서 거주하는 비율이 가장 많았으며 자취, 기숙사, 하숙이나 친척집에 거주하는 순이었다. 남학생의 63.9%, 여학생의 63.1%는 21~40만원의 용돈을 받고 있었고 지난 2개월 동안 남학생의 45.2%, 여학생의 47.1%는 체중을 유지하고 있었다. 로마기준 II를 이용하여 조사한 기능성 변비유병률은 남녀 각각 12.7%, 28.9%, 자의적 변비 유병률은 남녀 각각 12.0%, 36.9%로 여학생이 남학생에 비해 유의적으로 높았으며 일반사항에 따른 기능성 변비와 자의적 변비의 차이는 보이지 않았다. 자의적 변비 중 남학생의 35.0%, 여학생의 44.9%만이 기능성 변비의 진단기준에 부합하였고, 본인은 변비가 아니라고 응답했지만 기능성 변비의 진단기준에 부합한 경우는 11.4%이었다. 본인이 변비라고 인식하는 경우 남학생의 94.7%, 여학생의 91.0%는 의료기관을 방문한 경험이 없었고 남학생은 모두 변비약이나 건강기능식품을 이용하고 있지 않았으며 여학생은 변비 완화를 위해 10.3%가 변비약, 11.8%가 건강기능식품을 복용하고 있었다. 배변습관 조사 결과 남학생의 75.3%, 여학생의 40.1%는 주 5회 이상 배변을 하고 있었고, 변의 양은 2 cup 이상이라고 응답한 경우가 남학생은 93.4%, 여학생은 75.9%로 남학생이 많은 경향을 보였다. 남녀 모두 대상자의 70%정도가 배변 후 상쾌하다고 하였으며 남학생의 49.4%, 여학생의 24.6%만이 규칙적으로 배변을 하였다. 신체계측치와 영양소 섭취량 분석 결과 기능성 변비와 자의적 변비 유무에 따른 차이는 보이지 않았으나 단백질의 경우 성별이나 변비 유무와 상관없이 RI에서 UL 사이로 섭취하는 비율이 가장 높았고, 남학생은 여학생에 비해 EAR 보다 낮게 섭취하는 비율이 유의적으로 높았다. 비타민 C, Vit $B_2$, 엽산, 칼슘, 아연의 섭취는 EAR 이하로 섭취하는 비율이 모두 50%를 넘어 영양소의 섭취량이 매우 낮음을 알 수 있었고 특히 엽산은 남녀 모두 95% 이상이 EAR 이하로 섭취하고 있어 섭취 부족이 심각하였다. 변비와 관련이 있는 것으로 알려진 식이섬유 섭취량은 1,000 kcal당 섭취량으로 환산에 남학생은 8.5 g, 여학생은 8.9 g으로 한국인 영양섭취기준의 식이섬유 충분섭취량에 훨씬 못 미치는 수준이었다. 여학생의 경우 기능성 변비의 유병률은 28.9%, 자의적 변비의 유병률은 36.9%로 조사되었는데, 본인은 변비가 아니라고 응답했지만 기능성 변비의 진단기준에 부합한 경우도 13.6%이며 본인이 변비라고 응답한 경우의 대다수는 의료기관 방문이나 약물, 기능성 식품 섭취 같은 노력도 하지 않고 있어 만성적 변비로 인한 질병의 위험이 우려된다. 또한 식이섬유 섭취를 비롯한 각종 영양소들도 균형 있는 섭취를 하고 있지 않아 이후 다가올 임신기와 수유기의 영양문제로 이어질 것이므로 올바른 영양섭취를 비롯한 적절한 영양교육이 활성화 되어야겠다.

복강 내압 항진증이 내과계 중환자의 예후에 미치는 영향 (The Effects of Intra-Abdominal Hypertension on the Prognosis of Critically Ill Patients in the Intensive Care Unit (ICU))

  • 연구배경: IAP의 증가는 혈류량을 감소시켜 여러 장기에 영향을 준다. IAH는 IAP가 12 mmHg 이상 또는 APP가 60 mmHg 이하로, ACS는 IAP가 20 mmHg 이상이면서 동시에 하나 이상의 장기 손상이 있는 경우로 각각 정의한다. 저자들은 중환자에서 IAH 및 ACS의 유병률과 이들이 환자의 예후에 미치는 영향을 고찰하고자 하였다. 방 법: 고려대학교 의료원 중환자실에 입원하는 내과계 환자를 대상으로 하여, APACHE II 및 III 점수, SAPS II 점수를 기록하였다. IAP는 삼중관 요도관을 통하여, 입원 7일째까지 매일 3회 측정하였고, 환자가 사망하거나 일반 병실로 전실 또는 요도관을 제거할 때까지 계속하였다. 환자의 예후는 28일을 기준으로 판단하였다. 결 과: 총 111명 대상 환자들의 입원 1일 IAH 및 ACS의 유병률은 각각 47.7%, 15.3%였다. 사망군과 생존군 사이에 IAH의 유병률은 유의한 차이가 없었으나, ACS는 사망군에서 더 높았다 (p=0.004). 사망군에서 IAP는 더 높았고 (p=0.012), APP는 유의하게 낮았다 (p<0.001). 결 론: 중환자에서 입원 1일에 측정한 IAP의 증가, APP의 감소 및 ACS 발생은 환자의 사망과 연관이 있을 것으로 판단된다.

치과기공사의 근무 환경 및 업무특성 평가에 관한 연구 (A Study on Working conditions and Characteristics of Dental Technicians)

  • Purpose: This study was conducted to improve the working conditions of dental technicians, through survey for working conditions related to perform one's duty, the extent of personal exposure to substances hazardous and complaint rate and factor of job stress and subjective symptom on musculoskeletal disorders. Methods: Using four types of structured questionnaires: social and demographic factor; the actual conditions of working space; working conditions; and characteristics of dental technicians, respondents filled in the questionnaires and the results were analyzed statistically. Results: The evaluation of comfort in working place found that satisfaction level on indoor noise was the lowest with 2.6 on a five-point scale and privacy space followed 2.8 point. In reasons for dissatisfaction with job, over workload ranked the highest by 25.4% and role overload had the highest by 39.5% in job stress factors. Based on the results, we could draw conclusion that dental technicians were unsatisfied with role overload and heavy workload a day. In reasons for musculoskeletal symptom prevalence, long-patient work was the highest by 24.1%. It was thought to cause dental technicians need deep procession and long-patient work due to the nature of their job. Conclusion: In conclusion, noise reduction and privacy space are required to improve job efficiency of dental technicians. In addition; we need to think of ways to increase the job satisfaction with adequate rest while deep procession and long-patient working.

Mechanism of resistance acquisition and treatment of macrolide-resistant Mycoplasma pneumoniae pneumonia in children

  • Mycoplasma pneumoniae pneumonia (MPP) is one of the most common forms of community-acquired pneumonia in children and adolescents. Outbreaks of MPP occur in 3- to 7-year cycles worldwide; recent epidemics in Korea occurred in 2006-2007, 2011, and 2015-2016. Although MPP is known to be a mild, self-limiting disease with a good response to macrolides, it can also progress into a severe and fulminant disease. Notably, since 2000, the prevalence of macrolide-resistant MPP has rapidly increased, especially in Asian countries, recently reaching up to 80%-90%. Macrolide-resistant Mycoplasma pneumoniae (MRMP) harbors a point mutation in domain V of 23S rRNA with substitutions mainly detected at positions 2063 and 2064 of the sequence. The excessive use of macrolides may contribute to these mutations. MRMP can lead to clinically refractory pneumonia, showing no clinical or radiological response to macrolides, and can progress to severe and complicated pneumonia. Refractory MPP is characterized by an excessive immune response against the pathogen as well as direct injury caused by an increasing bacterial load. A change of antibiotics is recommended to reduce the bacterial load. Tetracyclines or quinolones can be alternatives for treating MRMP. Otherwise, corticosteroid or intravenous immunoglobulin can be added to the treatment regimen as immunomodulators to downregulate an excessive host immune reaction and alleviate immune-mediated pulmonary injury. However, the exact starting time point, dose, or duration of immunomodulators has not been established. This review focuses on the mechanism of resistance acquisition and treatment options for MRMP pneumonia.

Tobacco Use Among Students Aged 13-15 Years in South Korea: The 2013 Global Youth Tobacco Survey

  • Objectives: We examined the prevalence of tobacco use and exposure to secondhand smoke among middle-school students in Korea using the Global Youth Tobacco Survey (GYTS) in 2013. Methods: The GYTS in Korea was conducted between July and August 2013 by the Korea Centers for Disease Control and Prevention. Data were collected using a self-administered anonymous questionnaire from a nationally representative sample of middle-school students aged 13-15 years in sampled classrooms. Results: The GYTS in Korea was completed by 4235 students aged 13-15 years in 43 middle schools. Approximately one in five of the students (17.8%) reported that they had tried cigarettes in the past, while 5.2% reported currently being cigarette smokers. Current cigarette smoking was higher in boys (7.5%) than in girls (2.6%). Of the students, 29.7% had been exposed to secondhand smoke at home, 47.4% inside enclosed public places, and 53.9% in outdoor public places. Of the current cigarette smokers, 25.7% bought their cigarettes from a store despite a law prohibiting this. Additionally, 58.0% of students noticed point-of-sale tobacco advertisements or promotions, 66.8% of current cigarette smokers wanted to stop smoking, and 70.9% of students had been taught about the dangers of tobacco use in school. Conclusions: These findings provide an opportunity to develop, implement, and evaluate a comprehensive tobacco control policy. The results suggest that youth have relatively easy access to cigarettes and are regularly exposed to secondhand smoke in public places, as well as to point-of-sale tobacco advertisements and promotions. Strict enforcement of the ban on tobacco sales to youth, expanding smoke-free areas, and advertising bans are needed to reduce tobacco use among youth.

노인의 건강상태 및 간호 요구도 조사 (A Study on Physical Health Status and Nursing Needs of the Elderly)

  • This study was conducted to collect the baseline data of physical health status and nursing needs of the elderly for developing an educational program for nursing professionals. The data was collected from 287 people who were 65 years old or older belonging to a selected 8 out of the 39 welfare centers in Pusan during the period from February 25 to November 30, 1996. Socio-demographic characteristics, frequency of hospitalization, and the prevalence of chronic illnesses were measured to assess the physical health status of the center's elderly admitted to the hospital during the period of data collection. The 5-point Likert-type scale composed of 20 items was used. The data was analyzed with the Statistical Package for the Social Sciences Program (SPSS PC+). The results were summarized as follows : 1. The proportion of elderly subjects with chronic illnesses was 73.6%. Among 6 types of chronic illnesses, hypertension was the most prevalent(23.7%), cardiovascular disease was the second(18.1%), gastrointestinal disease the third(17.1%), musculosskeletal disease the fourth(12.9%), liver and biliary disease the fifth(12.5%), and diabetes the sixth. 2. The average score of the subjects' health status was 3.649 on the 5-point Likert scale. Among the 6 factors related to physical health, personal hygiene was the strongest factor(4.612) ; elimination was the second(4.491) ; sensation the third(4.369), outside activity the fourth(3.675), digestion the fifth(3.331), and sexual life the sixth. 3. Gender, marital status, religion, educational level, family structure, living arrangements, and perceived health status were significantly related to the elderly's physical health status. 4. Among the nursing needs of the elderly, emotional support and care was the strongest need(74.2%) ; physical therapy was the second(73.2%) ; education about diease control the third(64.8%) ; medication the fourth(63.8%) ; and the range of motion exercise the fourth(61.0%).

Five-year investigation of a large orthodontic patient population at a dental hospital in South Korea

  • Objective: The purpose of this study was to investigate the characteristics of orthodontic patients at Yonsei Dental Hospital from 2008 to 2012. Methods: We evaluated Angle's classification from molar relationships, classification of skeletal malocclusion from the A point-nasion-B point angle, facial asymmetry, and temporomandibular joint disorders (TMDs) from the records of 7,476 patients who received an orthodontic diagnosis. The orthognathic surgery rate, extraction rate, and extraction sites were determined from the records of 4,861 treated patients. Results: The patient number increased until 2010 and gradually decreased thereafter. Most patients were aged 19-39 years, with a gradual increase in patients aged ${\geq}40years$. Angle's Class I, Class II divisions 1 and 2, and Class III malocclusions were observed in 27.7%, 25.6%, 10.6%, and 36.1% patients, respectively, with a gradual decrease in the frequency of Class I malocclusion. The proportion of patients with skeletal Class I, Class II, and Class III malocclusions was 34.3%, 34.3%, and 31.4%, respectively, while the prevalence of facial asymmetry and TMDs was 11.0% and 24.9%, respectively. The orthognathic surgery rate was 18.5%, with 70% surgical patients exhibiting skeletal Class III malocclusion. The overall extraction rate among nonsurgical patients was 35.4%, and the maxillary and mandibular first premolars were the most commonly extracted teeth. Conclusions: The most noticeable changes over time included a decrease in the patient number after 2010, an increase in the average patient age, and a decrease in the frequency of Angle's Class I malocclusion. Our results suggest that periodic characterization is necessary to meet the changing demands of orthodontic patients.

Screening of 185DelAG, 1014DelGT and 3889DelAG BRCA1 Mutations in Breast Cancer Patients from North-East India

  • Around 1.35 million people of worldwide suffer from breast cancer each year, whereas in India, 1 in every 17 women develops the disease. Mutations of the Breast Cancer 1 (BRCA1) gene account for the majority of breast/ovarian cancer families. The purpose of study was to provide a prevalence of BRCA1 germline mutations in the North-East Indian population. In relation to the personal and family history with the breast cancer, we found mutations in 6.25% and 12.5% respectively. Three mutations, 185DelAG, 1014DelGT and 3889DelAG, were observed in our North-East Indian patients in exons 2 and 11, resulting in truncation of the BRCA1 protein by forming stop codons individually at amino acid positions 39, 303 and 1265. Our results point to a necessity for an extensive mutation screening study of high risk breast cancer cases in our North-East Indian population, which will provide better decisive medical and surgical preventive options.

영유아 식품알레르기의 임상적 조망 (Clinical Perspectives of Food Allergy in Infants and Young Children)

  • Food allergies affect 7~8% of infants and young children, and their prevalence appears to have increased in recent years. Food allergy refers to an abnormal immunological reaction to a specific food. These reactions can be recurrent each time the food is ingested. Food allergy manifests itself with a wide spectrum of clinical characteristics including IgE-mediated diseases as immediate reactions, non-IgE-mediated disorders as delayed reactions, and mixed hypersensitivities. As a consequence, the clinical picture of a food allergy is pleomorphic. A well-designed oral food challenge is the most reliable diagnostic test for infants and young children whose clinical history and physical examination point towards a specific food allergy. Food specific IgE antibody tests (RAST, MAST, skin prick test, Uni-CAP, etc) are an alternative tool to determine oral food challenge for IgE-mediated disorders, but not for non-IgE-mediated allergies. Moreover, parents often impose their children on unnecessary diets without adequate medical supervision. These inappropriate dietary restrictions may cause nutritional deficiencies. This review aims to introduce clinical perspectives of food allergy in infants and young children and to orient clinicians towards different strains of diagnostic approaches, dietary management, and follow-up assessment of tolerance development.
