Serious Complications of the Percutaneous A1 Pulley Release: Case Reports and Literature Review

  • Percutaneous first annular pulley (A1 pulley) release, which has been increasingly used to treat trigger fingers, has been widely established as a safe and simple procedure. Multiple studies have reported positive results of percutaneous A1 pulley release. In this study, however, we report cases of patients who developed complications after undergoing percutaneous A1 pulley release at local clinics. A total of six patients visited our hospital for infectious complications after percutaneous A1 pulley release. Various sequelae such as damage to normal structures, insufficient procedure, and tissue necrosis were observed during the exploration. A retrospective study was conducted to identify the cause and trend of the observed complications by instruments (HAKI knife or needle). In the HAKI knife group, there was a tendency for damage to normal structures, while in the needle group, an insufficient release or serious soft tissue necrosis was observed. Based on these cases, our findings confirm the existence and characteristics of infectious complications following the percutaneous A1 pulley release. We further identify that the type of instrument used predicts the nature of complications. Thus, reliable and skilled performance of the procedure by experts is essential for safe treatment.

콘칼로리미터를 이용한 섬유강화플라스틱(FRP)의 연소특성 (Combustion Characteristics of Fiber Reinforced Plastic by Cone Calorimeter)

  • 본 연구는 작업장의 건축물이나 구조물의 구성요소로 사용되어지는 섬유강화플라스틱의 연소특성을 평가하였다. 섬유강화플라스틱의 연소특성은 ISO 5660에 따라 콘칼로리미터를 사용하여 수행하였다. 섬유 강화플라스틱의 착화시간과 열방출율은 복사열과 난연제의 함량에 따라 달랐다. 섬유강화플라스틱의 열방출율은 복사열의 증가에 따라 증가하였다. 섬유강화플라스틱의 착화시간과 최대 열방출율을 이용하여 플래쉬오버(Flashover)의 가능성을 Petrella가 제시한 분류방법에 따라 검토하였다.

The Influence of Corticosteroid Injections on Postoperative Outcomes of Carpal Tunnel Release: A Systematic Review

  • Background Carpal tunnel syndrome can be treated with corticosteroid injections (CIs) and surgery. In this systematic review, the influence of previous CI on different postoperative outcomes after carpal tunnel release is evaluated. Methods A systematic literature search using several databases was performed to include studies that examined patients diagnosed with carpal tunnel syndrome who received preoperative or intraoperative CIs. Results Of 2,459 articles, 9 were eligible for inclusion. Four papers reported outcomes of preoperative and four outcomes of intraoperative CIs. One study evaluated patients who received both intraoperative and preoperative corticosteroids. Conclusion Intraoperative CIs are associated with reduced postoperative pain after carpal tunnel release and support earlier recovery of the hand function that can be objectified in a faster median nerve conduction speed recovery and lower Boston Carpal Tunnel Questionnaire (BCTQ) scores. Using preoperative CIs did not lead to enhanced recovery after carpal tunnel release, and both preoperative and intraoperative CIs might be predisposing factors for infections.

콘칼로리미터를 이용한 플라스틱 단열재의 화재특성 (Fire Characteristics of Plastic Insulating Materials from Cone Calorimeter Test)

  • 본 연구는 산업현장에서 단열재로 .사용되고 있는 폴리스티렌 폼, 폴리우레탄 폼, 폴리에틸렌 폼과 같은 플라스틱 단열재의 화재특성을 구명하고자 하였다. 플라스틱 단열재의 화재특성은 ISO 5660에 따라 콘칼로리미터 시험을 수행하였다. 사용된 실험재료는 국내에서 생산되는 상업용 플라스틱 단열재를 사용하였고 그들의 조성은 제조자에 의해 밝혀지지 않았다. 연구 결과플라스틱 단열재의 열방출율은 재료의 밀도와 열플럭스의 증가에 따라 증가하였다. 플라스틱 단열재 중 폴리에틸렌 폼이 최대열방출율 및 평균열방출율이 가장 크게 나타났고, 열플럭스 및 밀도의 증가에 따른 최대열방출율의 증가율도 가장 큰 것으로 나타났다. 플라스틱 단열재에 의한 화재예방을 위해 제품의 종류 및 열플럭스의 크기에 따른 열방출 평가 기준을 제시하였다.

인칭검사를 이용하여 부분 절개를 시행한 수근관 증후군 환자의 추적관찰 (Follow Up Study of Carpal Tunnel Syndrome Patients Underwent Partial Release of Transverse Carpal Ligament Using Inching Test)

  • Purpose: Complete release of the transverse carpal ligament (TCL) is accepted as the standard treatment for carpal tunnel syndrome (CTS). However, loss of grip and pinch power are reported in some patients after complete release of the TCL. This study was designed to evaluate the effectiveness of complete versus partial carpal tunnel release by using the inching technique. Methods: Nineteen patients (a total of 27 hands) who each had a confirmed diagnosis of CTS were selected from September 2002 to February 2003. The cases were divided into three groups(mild, moderate and severe) based on preoperative electrodiagnostic studies. The patients with partial carpal tunnel syndrome were classified into the mild or moderate groups, while patients with complete carpal tunnel syndrome were classified into the moderate or severe groups. Patient oriented data (functional and symptomatic) were collected and electrophysiologic studies were undertaken preoperatively and postoperatively(on the 2nd week, 1st month, 3rd month and 6th month after surgery).Results: In this study, the mild and moderate groups showed both good functional and symptomatic results and improvements in electrophysiologic studies.Conclusion: Carpal tunnel syndrome patients classified into mild or moderate groups based on nerve conduction studies, and whose precise compression sites were pinpointed using the inching technique, can be treated by partial carpal tunnel release.

  • In previous study, the decrease and recovery of total stiffness in welded structure was discussed on the basis of experimental examination through tensile loading and unloading test of welded specimen. The recovery of structure stiffness was caused by the release of welding residual stress through mechanical loading. In this study, analysis model that is indispensable for the effective application of MSR method was established on the basis of test and measurement result. Thermal elasto-plastic analysis for welding process was performed by non-coupled analysis. Analysis results of welding process were transfer to elasto-plastic model for tensile loading & unloading by restart technique. In elasto-plastic analysis model for mechanical loading & unloading, hardening appearance of weld metal was considered by rezoning technique and tying technique was used for JIG condition of test machine.

Does Aging Matter? The Efficacy of Carpal Tunnel Release in the Elderly

  • Open release remains the gold standard in the treatment of carpal tunnel syndrome in cases where conservative treatment fails. However, the efficacy of carpal tunnel release in the elderly has been debated in the literature throughout the years. This review aims to review the current evidence pertaining to the efficacy of carpal tunnel release in the elderly. Based on the current evidence, the outcome of carpal tunnel release is unpredictable in the elderly. Elderly patients are also less satisfied with the operation compared to younger patients. The authors recommend that these messages be conveyed to elderly patients before surgery. Moreover, open carpal tunnel release should be offered in the early stages of treatment whenever operative management is indicated.

Open carpal release using local anesthesia without a tourniquet: Does bleeding tendency affect the outcome?

  • Background The aim of this study was to analyze the clinical results of minimal single palmar-incision carpal tunnel release without a tourniquet. Methods We reviewed the medical records of 75 patients (90 cases of carpal tunnel syndrome) who underwent minimal single-palmar incision carpal tunnel release without a tourniquet from June 2010 to January 2018. Ten patients had a bleeding tendency. We compared the preoperative and postoperative Boston Carpal Tunnel Syndrome Questionnaire (BCTQ) scores. We also analyzed outcomes and complications according to the presence of a bleeding tendency. Results In all cases, there was a complete disappearance or marked improvement in symptoms within 6 months, with no recurrence. The postoperative BCTQ score showed a significant improvement compared to the preoperative score, and no statistically significant difference in BCTQ scores was detected according to the presence of a bleeding tendency. Conclusions Carpal tunnel release without a tourniquet using a minimal single palmar incision is effective and reliable. This technique prevents unnecessary pain associated with the tourniquet and is especially helpful in patients with a bleeding tendency or those treated with hemodialysis.
