Angiomyolipoma of the Glabellar Region

  • Angiomyolipomas are hamartomatous lesions consisting of smooth muscle bundles, thick-walled blood vessels, and mature adipocytes. They are usually found in the kidneys of tuberous sclerosis patients and more rarely in other organs, such as the liver, the oral cavity, the sinonasal tract, the heart, the large intestines, the lungs, and the skin. Cutaneous angiomyolipoma has shown to be very rare and generally occurs at the ends of the digits, the elbows, the ears, and the nose. Herein, we report the first documented case of angiomyolipoma of the glabellar region.

  • An elastic-plastic solution for cavity expansion problem considering strength degradation, undrained condition and initial anisotropic in-situ stress is established based on the Tresca yield criterion and cavity expansion theory. Assumptions of large-strain for plastic region and small-strain for elastic region are adopted, respectively. The initial in-situ stress state of natural soil mass may be anisotropic caused by consolidation history, and the strength degradation of soil mass is caused by structural damage of soil mass in the process of loading analysis (cavity expansion process). Finally, the published solutions are conducted to verify the suitability of this elastic-plastic solution, and the parametric studies are investigated in order to the significance of this study for in-situ soil test.

The lateral intercostal artery perforator as an alternative donor vessel for free vascularized lymph node transplantation

  • Chronic lymphedema is caused by an impairment of the lymphatic system due to primary or secondary causes. Vascularized lymph node transplantation (VLNT) is currently the most promising and frequently used technique besides lymphaticovenous anastomosis. However, the vessel anatomy in the lateral thoracic region is sometimes quite variable. Based on our experiences with vascular anatomical inconstancy in the lateral thoracic region, we planned a lateral intercostal artery perforator flap for VLNT in a female patient with chronic stage II lymphedema of both legs after cervical cancer treatment. After surgery, the patient reported significant improvement in limb volume and the accompanying symptoms. The limb circumference was reduced by an average of 19.2% at 6 months postoperatively. Despite having a short pedicle and small vessel caliber, the lateral intercostal artery perforator flap can safely be used for VLNT in lymphedema patients with anatomical variants.

A Theoretical Comparison of Two Possible Shape Memory Processes in Shape Memory Alloy Reinforced Metal Matrix Composite

  • Two possible shape memory processes, austenite to detwinned martensite transformation and twinned martensite to detwinned martensite transformation of a shape memory alloy have been modeled and examined. Eshelby's equivalent inclusion method with Mori-Tanaka's mean field theory is used for modeling of the shape memory processes of TiNi shape memory alloy reinforced aluminum matrix composite. The shape memory amount of shape memory alloy, plastic strain and residual stress in the matrix are computed and compared for the two processes. It is shown that the shape memory amount shows differences in a small prestrain region, but the plastic strain and the residual stress in the matrix show differences in the whole prestrain region. The shape memory process with initially martensitic state of the shape memory alloy would be favorable to the increase in the yield stress of the composite owing to the large compressive residual stress and plastic strain in the matrix.

  • Neurofibromatosis, now termed neurofibromatosis type I, is known as a congenital and familial disease presenting abnormalities of the skin, nervous system, bones, and soft tissue. We experienced a case of extremely large neurofibromatosis which developed on the orbital and temporal region of a 24-year-old man. The tumor was widely excised including normal skin margin, outer table of cranium, a part of zygoma and maxilla. Bony defect was reconstructed by rib bone graft and secondary cosmetic correction of blepharoptosis was performed using supratarsal fixation in postoperative 6 months.

An assessment of non-linear elastic and elasto-plastic analyses with regards to tubular steel piles embedded in sands

  • This study examines two traditional approaches (non-linear elastic and elasto-plastic) in association with 2D and 3D FEM analyses of a box-section pile embedded in sand. A particular emphasis is placed on stress singularities concerning both reentrant corners of the pile section and the resulting tension zones. From the experience gained in this study, non-linear elastic soil models are less restrictive when one considers stress singularities and their possible effects on convergence of the solution. At least for monotonic loading, when compared with field tests, non-linear elastic models yield better results than the plasticity ones. On the other hand, although elasto-plastic models are not limited to monotonic loading, they are much more sensitive to stress singularities. For this reason, a spherical elastic region is necessary at the pile tip to ensure convergence. Without this region, one must artificially impose an apparent cohesion to limit the tension stresses within a sand medium.

  • Angiolymphoid hyperplasia with eosinophilia (ALHE) is a rare benign vascular tumor. The pathogenesis of ALHE is unknown; however, it may be linked to local trauma. ALHE predominantly occurs in areas of the preauricular region, forehead, and scalp; the masseter area is rarely involved. A 49-year-old man was referred for a mass in the right cheek region that was felt 2 months prior. Physical and imaging examination results suggested the presence of a benign tumor. Thus, surgical excision was performed. Pathologic findings confirmed an unexpected diagnosis of ALHE. This case was interesting, since the mass occurred at an unusual site with a misdiagnosis of an epidermal inclusion cyst.

A Case of Myxoid Liposarcoma of the Cheek (협부에 발생한 점액양 지방육종의 치험례)

  • Purpose: Liposarcoma is the most common soft tissue sarcoma, and usually occurs on the thigh or in the retroperitoneal space, but rarely in the oral region. This report presents a case of liposarcoma of the cheek and includes a review of the literatures. Methods: A 21-year-old woman was admitted with a palpable mass in her cheek of about two years duration, which increased in size gradually initially, but had increased rapidly over the three months. There was no particular pain or tenderness. MRI showed a well-enhanced, well-defined mass, which suspected to be hemangioma. Results: The spherical, well-encapsulated mass was surgically excised. Biopsy results revealed myxoid liposarcoma. FDG PET-CT on the seventh postoperative day, revealed a minimal to mild FDG-uptake soft tissue lesion around the mass defect area without evidence of distant metastasis. The patient is being observed and undergoing radiation therapy. Conclusion: Liposarcoma in the head and neck region is a rare disease, and can be overlooked as a benign tumor without a pathologic diagnosis. Therefore, proper treatment and follow-up are required based on an understanding of this disease.

Characteristics of Fat Tissue According to the Anatomical Regions of the Body: Computed Tomographic and Histological Findings (신체 부위에 따른 지방조직의 특성: 컴퓨터단층촬영 및 조직학적 소견)

  • Purpose: The subcutaneous fat tissue is separated into 2 layers by the subcutaneous fascia: the superficial and deep fat layers. The two fat layers have different structures according to the body regions. The purpose of this study is to evaluate the distribution and pattern of the two fat layers in the human body by computed tomography (CT) and histological analysis according to age, sex, anatomical region, and body mass index (BMI). Methods: This study included 200 males and 200 females who underwent 64-channel dynamic CT in our hospital. The patients were divided into 5 groups according to 10 years of their ages separately in either male or female gender. The thickness of the superficial and deep fat layers was measured in the abdominal, pelvic, and femoral regions, and we analyzed the values. Statistical analyses were performed using SPSS software. The $3{\times}3$-cm whole fat layers were harvested from the same sites of 3 cadavers for histological examination, and one cadaver was dissected for gross evaluation. Results: The total thickness of subcutaneous fat tissue was greater in females than in males, and the ratio of the superficial fat layer to the whole fat layer was higher in females. The superficial fat layer became thinner with increasing age in males. As BMI increased, the total fat layer became thicker, and the superficial fat layer became thicker than the deep fat layer. On histological examination, the superficial fat layer had small adipose lobules and showed a densely distributed pattern in the abdominal region, whereas in the femoral region, it had large adipose lobules and showed a sparsely distributed pattern. There were no significant differences in the histological findings of the deep fat layer between the 3 body regions. Conclusion: Significant differences in histological findings of the two fat layers were found in relation to age, sex, anatomical region, and BMI. The superficial fat layer became thinner with increasing age in males, but it was constant in females. As BMI increased, the total thickness of subcutaneous fat tissue became greater, and the superficial fat layer became thicker than the deep fat layer. Our measurements can be used to understand the characteristics of the fat layers in relation to age, sex, anatomical region, and BMI.

A Study on the Knee Point of Low-cycle Fatigue Life in High Formability Titanium Alloy SP-700 (티탄계 초소성합금 SP-700의 저사이클 피로수명곡선의 절곡현상에 대하여)

  • Previous studies has shown that the curve of low-cycle fatigue life was not expressed with the single line subjected to Manson-Coffin's law type and bent to short life in low ${\Delta}{\varepsilon}_p$ region. The main cause of this phenomenon has been considered that the localization of plastic strain in the crack initiation process fosters the crack initiation. In this study, the low-cycle fatigue life was investigated for each specimens omitted crack initiation process and it was found that fatigue life curve in log(${\Delta}{\varepsilon}_p$)-log($N_f$)was bent in low ${\Delta}{\varepsilon}_p$ region as ever. Therefore, the main cause of appearance of knee point in fatigue life curve is not found in the crack initiation process but in the crack propagation process. In the crack propagation process, the localization of the plastic strain in the vicinity of crack tip and the influence of test environment on the crack propagation rate were observed and these inclinations were more remarkable in low ${\Delta}{\varepsilon}_p$ region. Hence, it was concluded that these two phenomena in the crack propagation process were proved to the main cause which accelerates the crack propagation in low ${\Delta}{\varepsilon}_p$ region and bent the fatigue life curve in result.
