교감신경계, Renin-Angiotensin계, Vasopressin계의 차단이 혈압 및 Norepinephrine, Angiotensin II 및 Vasopressin의 승압효과에 미치는 영향 (Influence of Blockade of Sympathetic Nervous System, Renin-Angiotensin System, and Vasopressin System on Basal Blood Pressure Levels and on Pressor Response to Norepinephrine, Angiotensin II, and Vasopressin)

  • 마취가토에서 혈압유지에 중요한 역할을 하고 있는 교감신경계, renin-angiotensin계, vasopressin계를 차단하였을때의 혈압자체의 변동과 norepinephrine (NE), angiotensin II (AII) 및 vasopressin (VP)의 승압효과의 변동을 조사하였다. 교감신경계와 renin-angiotensin계의 차단에는 각각 교감신경절 차단약인 chlorisondamine (CS)과 pirenzepine (PZ), angiotensin 변환효소억제약인 enalapril (ENAL)를 사용하였다. VP계의 차단에는 혈장 VP농도를 하강시킴이 알려져 있는 kappa opioid 수용체의 작용약인 bremazocine (BREM)을 사용하였다. CS (0.4mg/kg), ENAL (2mg/kg), BREM (0.25mg/kg)은 각각 비슷한 정도의 저혈압상태를 일으켰다. BREM에 의한 저혈압은 VP와 같은 효과를 가진 합성약인 desmopressin으로 유의하게 길항되었으며 BREM에 의한 저혈압이 적어도 일부 혈장 VP농도의 하강과 관계있음을 시사하였다. CS는 ENAL 또는 BREM으로 하강된 혈압을, ENAL은 CS 또는 BREM으로 하강된 혈압을, BREM은 CS 또는 ENAL로 하강된 혈압을, 더욱 하강시켰다. CS, PZ 그리고 ENAL 또는 CS, PZ 그리고 BREM에 의한 저혈압은 CS이외의 세약물에 의한 저혈압보다 심하였다. CS는 NE에 의한 승압효과 뿐만아니라 AII와 VP의 승압효과도 강화시켰다. AII의 승압효과는 또 ENAL과 BREM으로도 증대되었다. VP의 승압효과는 BREM으로도 강화되었다. ${\alpha}$-수용체의 길항약인 phentolamine과 phenoxybenzamine은 AII와 VP승압효과를 강화시켰다. 3승압계 차단이 혈압자체에 미치는 실험결과는 3계가 모두 혈압조절에 관여하고 그 중에서도 교감신경계가 가장 큰 역할을 하고 있음을 가리키고 있다. 한 승압계의 차단하에서, 그 계의 승압 hormone 뿐만아니라 다른 계의 승압 hormone의 승압효과도 증대됨은 이 3승압계가 긴밀한 상호작용을 하고 있는 증거이다.

오미자 추출액이 알루미늄 투여에 의한 흰쥐의 알루미늄 축적률에 미치는 영향 (Biochemical Study on the Effects of the Omija(Schizandra chinensis Baillon) Extracts in Rat Kidney Toxicity Induced by Aluminum)

  • To investigate the protective effect of omija(Schizandra chinensis Baillon) tea extracts against the aluminum toxicity, Sprague-Dawley rats($100{\pm}10\;g$) were divided into 6 groups consisting of a control group, a 3% Schizandra chinensis Baillon tea plus extract group, a 1,000 and 2,000 ppm aluminum group, and a 1,000 and 2,000 ppm aluminum plus 3% Schizandra chinensis Baillon tea plus extract group. The rats administered aluminum were given 1,000 and 2,000 ppm of $Al_2(SO_4)_3$ dissolved in distilled water. The aluminum content in tissues from rats administered aluminum was lower than in the tissues of rats of administered aluminum plus 3% Schizandra chinensis Baillon tea in the water extract. Aspartate aminotransferase (AST) and alanine aminotransferase(ALT) levels were increased in the aluminum group and lower in the group receiving a 3% extract of Schizandra chinensis Baillon tea. Lactate dehydrogenase(LDHase) was lower in the group receiving a 3% extract of Schizandra chinensis Baillon tea plus aluminum than in the aluminum group. Cholinesterase(ChEase) was higher in the 3% Schizandra chinensis Baillon tea-aluminum group than in the aluminum group. Plasma levels of renin were increased in the aluminum group, compared to the group receiving 3% Schizandra chinensis Baillon tea plus aluminum. Plasma levels of aldosterone were increased in the aluminum group compared with the 3% Schizandra chinensis Baillon tea plus aluminum group. These results suggest that the group receiving Schizandra chinensis Baillon tea in water extract had a lowered level of aluminum accumulation, and it is believed that the Schizandra chinensis Baillon tea plus water had some protective effects against aluminum toxicity when administered in rats, but the mechanism of these effects remains obscure.

부신증식으로 인한 원발성 Aldosterone증 1예 (1 Case of Primary Aldosteronism Caused by Adrenal Hyperplasia)

  • 본 환자는 전신쇠약감 피로감, 고혈압과 혈청 potassium치가 저하되어 있고 혈청 Renin활성도가 감소되어 있으면서 Aldosterone억제 시험에도 억제가 되지 않았음을 볼 때 원발성 Aldosterone증을 의심할 수 있었다. 또 자세변화 시험에서 혈청 Renin활성도가 떨어져 있었고 자세변화에 따라 혈청 Aldosterone이 증가했으며 컴퓨터 단층촬영에서 별다른 병변이 뚜렷이 보이지 않음을 볼때 부신증식성 Aldosterone증으로 의심된다. 따라서 본 환자에게 spironolactone을 투여했고 이후 외래를 통해 추적검사 중이며 혈압 조절이 양호할 뿐 아니라, 불편을 느끼던 증상들도 호전이 되었음을 안 수 있었다. 끝으로 원발성 Aldosterone증에 대한 관심을 더 기울인다면 더 많은 증례가 발견될 것으로 사료되고, 본 저자들은 보다 많은 증례를 찾아 추후 증례보고및 추적검사할 예정이다.

대돈(大敦)${\cdot}$용천(湧泉) 염전(捻轉) 및 영수(迎隨) 보사수기법(補瀉手技法)이 2K1C에 의하여 유발(誘發)된 고혈압(高血壓) 백서(白鼠)에 미치는 영향(影響) (The Effects of Acupuncture by Needle Manipulation at $LR1{\cdot}KI1$ on the Blood Pressure in Hypertensive Rat induced by Two Kidney One Clip (2K1C).)

  • Objectives : The purpose of this study was to examine effects of acupuncture by needle manipulation at $LR1{\cdot}Kl1$ on the blood pressure, plasma levels of ANP(atrial natriuretic peptide), renin and cardiomegalic index in hypertensive rat Induced by two kidney one clip(2K1C). Material and methods : The groups divided into 7 groups; Control, no treatment. Acu-1,acupuncture at $LR1{\cdot}Kl1$ bilaterally and the needle was twirled and rotated forward with the thumb of the right hand 6 times. Acu-2, acupuncture at $LR1{\cdot}Kl1$ bilaterally and the needle was twirled and rotated forward with the forefinger of the right hand 6 times. Acu-3, acupuncture at $LR1{\cdot}Kl1$ bilaterally and the needle was inserted in the opposite direction(body direction) as the channel runs, Acu-4, acupuncture at $LR1{\cdot}Kl1$ bilaterally, the needle was inserted in the opposite direction(body direction) as the channel runs and the needle was twirled and rotated forward with the forefinger of the right hand 6 times. The acupuncture executed 5times for 2 weeks. Results : Systolic blood pressure was decreased significantly in Acu-2, Acu-3 and Acu-4. Plasma renin was decreased significantly in Acu-2 and Acu-4. Plasma ANP was increased significantly in Acu-2, however was decreased significantly in Acu-4. Cardiac hypertrophy index was decreased significantly in Acu-1, Acu-2, Acu-3 and Acu-4. Conclusions : These results suggest that acupuncture by needle manipulation at $LR1{\cdot}Kl1$ is effective in hypertension by renal blood problem.

고혈압 청소년에서의 혈청 adiponectin치와 adiponectin I164T 유전자다형성 (Serum level of the adiponectin and adiponectin I164T polymorphism in hypertensive adolescents)

  • 목 적 : Adiponectin은 대사 증후군에서 중요한 역할을 하는 물질이며, 비만, 2형 당뇨, 관동맥 질환 등에서 그 수치가 감소하는 것으로 알려져 있다. 고혈압과 혈청 adiponectin치의 관계는 여러 저자에 의하여 발표되었으나 아직 논란의 여지가 남아있는 상태이다. 우리는 낮은 adiponectin치가 청소년 고혈압과 어떤 관계가 있는지, 또 영향을 끼치는 기전이 유전적인지 혹은 생물학적인 것인지 알고자 유전자 다형성 분석을 하였다. 방 법 : 16에서 17세 사이의 40명의 고혈압 청소년과 20명의 대조군 청소년을 대상으로 각 군의 혈청 adiponectin, insulin, renin, aldosterone, ACE를 측정하여 비교하였다. cIMT와 PWV도 측정하였다. PCR법으로 adiponectin I164T 유전자 다형성을 조사하였다. 결 과 : 고혈압 청소년군에서 cIMT와 PWV가 의미있게 높았다. 혈청 insulin, renin, aldosterone치도 고혈압군에서 의미있게 높았다. 두 군간에 adiponectin치는 의미있는 차이를 보이지 않았다. 본 연구에 참여한 청소년에서는 TC genotype은 발견되지 않았고 모두 TT genotype을 보였다. 결 론 : 결론적으로 adiponectin치는 고혈압 청소년 군에서나 정상군에서나 차이가 없었다. 두 군에서 유전적인 다형성도 보이지 않았다. 더 큰 규모의 연구가 필요할 것으로 생각된다.

숙지황 전탕액이 백서의 신장내 cGMP 생성과 ANP 수용체에 미치는 영향 (Effects of Shudihuang Water Extracts on the cGMP Production and Receptors for Atrial Natriuretic Peptide in the Kidney in Rats)

  • The purpose of this study is to investigate the effect of Shudihuang water extracts on the cGMP production of medulla and cortical membranes, receptors for atrial natriuretic peptide in the kidney by in vitro autoradiography in rats for 8, 16 weeks. The cGMP production of medullary and cortical membranes of the kidney decreased after the administration of Shudihuang water extracts. The density of receptors for atrial natriuretic peptide in the kidney decreased after the administration of Shudihuang water extracts only for 16 weeks. These results suggest that the long term administration of Shudihuang water extracts has decreased plasma renin activity and plasma levels of aldosterone modulated cGMP production of medullary and cortical membranes, density of receptors for atrial natriuretic peptide in the kidney.

좌귀음과 육미지황탕 전탕액 투여가 가토신기능 및 혈장 Aldosterone농도에 미치는 영향 (Effect of Choakwiyeum and Yukmijihwang-tang Water Extract on the Renal Aldosterone Concentration in Rabbits)

  • To evaluate the effect of Choakwiyeum(CKY) and Yukmijihwang-tang(YJT) water extract on the renal function and the levels of plasma aldosterone, this experiment was performed in the rabbbits. These result indicate that increase in urine volume(UV) after administration of CKY and YJT water extract is related to increase in glomerular filtration rate(GFR), free water clearance(FWC) and renal plasma f1ow(RPF) and also to decrease in plasma aldosterone concentration(PAC), which seems to be affected by renin-angiotensin-aldosterone system, and that reduced blood pressure seems to be caused by reduced peripheral resistance.

  • The aim of this experiments was to investigate the effects of Chinese magnolia vine water extract on the renal function, plasma renin activity, plasma levels of aldosterone and atrial natriuretic peptide in rats. The results of this study were as follows; 1. Water balance decreased significantly after administration with Chinese magnolia vine extracts (0.2 ml/200 g of body weight). 2. Urine volume increased significantly after administration with Chinese magnolia vine extracts. 3. Urinary excretion of sodium increased significantly after administration with Chinese magnolia vine extracts. 4. Urinary excretion of creatinine increased significantly after administration with Chinese magnolia vine extracts (0.2 ml/200 g of body weight). 5. Plasma levels of atrial natriuretic peptide decreased significantly after administration with Chinese magnolia vine extracts (0.1 ml/200 g of body weight). 6. Plasma levels of aldosterone decreased significantly after administration with Chinese magnolia vine extracts (0.2 ml/200 g of body weight).

Enhanced Expression of Angiotensinogen mRNA in Rat Central and Peripheral Tissues Following Hemorrhage

  • The renin-angiotensin system plays an important role in the regulation of blood pressure and in body fluid homeostasis. There is increasing evidence for generation of endogenous angiotensin II in many organs and for its role in paracrine functions. Studies were designed to investigate whether hemorrhage produces rapid changes in the gene expression of angiotensinogen in peripheral and brain tissues. Wistar rats received saline drinking water for 7 days, were bled at a rate of $3\;ml\;kg^{-1}\;min^{-1}$ for 7 min, and then decapitated 0, 2, 4, 8, or 24 hr after hemorrhage. Hemorrhage produced a produced hypotension with tachycardia at $2{\pm}8\;hr$, but blood pressure and heart rate had not fully recovered to the basal level at 24 hr. Plasma renin concentration was significantly increased at 2, 4, and 8 hr (maximum sixfold increase at 4 hr) and had returned to the basal level at 24 hr. Renal renin content was significantly increased only at 4 hr after hemorrhage. Angiotensinogen mRNA in both the kidney and liver were stimulated at 2 to 8 hrs, but recovered to the basal level at 24 hr. On the other hand, angiotensinogen mRNA levels il the hypothalamus and brainstem were continuously increased from 2 to 24 hrs. The present study demonstrates the presence of angiotensinogen mRNA in both hepatic and extrahepatic tissues, and more importantly, their up-regulation after hemorrhage. These results suggest that the angiotensinogen-generating systems in the liver, kideny and brain are, at least in part, under independent control and play a local physiological role.
