A STUDY OF CLINICAL RESULTS ON STERI-OSS ENDOSSEOUS IMPLANTS (Steri-Oss 임플랜트의 임상 결과에 관한 연구)

  • This investigation evaluated patients who received Steri-Oss implants from the Dental Hospital of Chosun University during the period from March 1989 to August 1997. 346 fixtures of 127 patients were included in this study. The results were as follows ; 1.The follow-up period was defined as the period between the surgical placement of the implants and the last follow-up examination. The mean follow-up period was $2.17{\pm}1.21$ years. 2.The period between fixture installation and second surgery was $0.71{\pm}0.44$ years in the maxilla and $0.46{\pm}0.21$ years in the mandible. 3.The number of fixtures which were installed in the upper jaw(112) was less than that in the lower jaw(234) and in the posterior region(260) was more than in the anterior region(86). 4.The length of fixture which was most frequently used was 12 mm and least was 8mm. Screw implants were installed more than cylindrical implants. 3.8mm implant was the most common implans, followed by 4.5mm and 3.25mm. 5.The number of augmentation cases was more than that of non-augmentation cases and the rate of augmentation cases in the maxilla was more than that in the mandible. 6.Implant restorations for partial edentulos patients(94cases) were more than single- tooth implant restorations(33cases) or implant restorations for complete edentulos patients(10cases). 7.Free-standing prostheses for partially edentulous patients were more commom than any other type of connection between implants and natural teeth. 8.Plaque Index($0.95{\pm}0.74$) and Gingival Index($0.31{\pm}0.52$) were very similar around the natural teeth and reflected an acceptable level of plaque and gingivitis control. Mean value for keratinized mucosa index($1.93{\pm}1.20$) remained fairly constant around level 2(1-2 mm keratinized epithelium). 9.Patients were generally satisfied with implant in terms of comfort, function, speech and esthetics. 10.There was not a statistically significant differences in overall survial rate between implants placed in the maxilla (91.5%) and those placed in the mandible (93.8%). Fourteen implants lost before the prosthetic rehabilitation and eleven implants lost following variable periods in function after the prosthetic phase of the treatment. 11.Cause of implant failures was exfoliation or removal of fixture due to non-osseointegration before the prosthetic rehabilitation or due to fracture of fixture, masticatory pain after the prosthetic rehabilitation. 12.The survival rate of Steri-Oss implants using the Kaplan-Meier statistical analysis was 93.8% at 2 year and 86.6% at 5 year, In all cases, implant losses occured predominantly in the healing period. There was a steep decline in the rate of implant loss after the first year. 13.The survival rate of Steri-Oss implants in the anterior region was 94.8% at 2 year and 94.8% at 5 year and that in the posterior region was 92.8% at 2 year and 75.9% at 5 year. In conclusion, this study revealed a number of parameters and guidelines for achieving an optimal success rate in osseointegration.

A Collaborative Study on Korean Standard JE Vaccine for Potency Assay

  • The objective of this collaborative study was to establish a Korean standard of Japanese encephalitis (JE) vaccine (mouse brain-derived, formalin-inactivated) for potency assay. A candidate preparation proposed as a Korean standard was provided by GreenCross Vaccine, and six laboratories, including one national control laboratory and five manufacturers of JE vaccine, participated in the study. The potency of the candidate preparation and a reference standard obtained from Japan was estimated by mouse immunogenicity assay using the in vitro plaque reduction neutralization test (PRNT). The results of 72 assays conducted by the 6 laboratories showed that the overall mean potency estimate of the candidate preparation was 2.455 log median plaque reduction neutralization antibody titer per 0.5-ml dosage administered twice in mice at 7-day intervals, and that the mean potency ratio of the candidate preparation relative to the reference standard was 1.074. The potency estimates were quite variable among laboratories irrespective of the preparation. The variability of assays assessed by Z scores and coefficient of variation (CV) were in general within the level of acceptance (Z scores within $\pm3$ and $CV\;\leq\;15%$). Therefore, we concluded that the candidate preparation would be suitable as a national standard for testing the potency of JE vaccine (inactivated).

Studies on Antitumor Components of the Cultured Mycelia of Interspecific Protoplast Fusant F-2 of Ganoderma lucidum and Ganoderma applanatum (영지와 잔나비걸상버섯의 원형질체 융합균주의 항암 성분에 관한 연구)

  • On the five interspecific protoplast fusants of Ganoderma lucidum and G. applanatum was the antitumor test performed. The fusant F-2 was selected, to examine the cultured mycelia (protein bound polysaccharide) as antitumor components. When a dose of 20 mg/kg/day of each components purifed from F-2 fusant was, i.p., injected into ICR mice, the inhibition ratio of Fr. II against the solid form of sarcoma 180 increased to 1.5 times as compared with that of their parents. When Fr. II was examined for immunopotentiation activity, it increased the amount of the superoxide anion in activated macrophages to 1.2 times and the count of hemolytic plaque forming cells in the spleen to 4.3 times as compared with that of each control group. Its chemical analysis showed 85.2% polysaccharide which consisted of glucose, galactose, mannose, fucose and xylose, and 0.39% protein of 15 amino acids. The content of hexosamine was 0.39% and the molecular weight of Fr. V was $5.6{\times}10^4$ dalton.

THE DENTAL CARIES, CARIOGENIC MICROORGANISMS AND LEVELS OF SALIVARY IMMUNOGLOBULIN IN SUBJECTS WITH DOWN'S SYNDROME (다운증후군 환자의 치아우식증과 치아우식 원인균 및 타액내 면역항체의 연관성)

  • This study investigated the relationship between dental caries and various oral factors in subjects with Down's syndrome. We compared 25 subjects with Down's syndrome with 63 healthy control. The dental caries index and plaque index were examined, and the total salivary immunoglobulin A and S. mutans specific salivary immunoglobulin A concentration were measured using ELISA. The S. mutans counts, Lactobacillus counts and buffer capacity were measured with Dentocult test medium. The decayed and filled surface index of deciduous teeth in subjects with Down's syndrome was lower than in controls(p<0.001). The plaque index and total salivary immunoglobulin A concentration showed no difference, S. mutans specific salivary immunoglobulin A concentration and buffer capacity in subjects with Down's syndrome were lower than in controls(p<0.001). There was no significant difference between two groups in the S. mutans counts and Lactobacillus counts. In 9-11 year age group, S. mutans counts in subjects with Down's syndrome was lower than in controls(p<0.001) and S. mutans specific salivary immunoglobulin A concentration was lower(p<0.05). There was a high correlation among deciduous dental caries index and buffering capacity and S. mutans counts.

  • The purpose of this study is to determine if a relationship exists among osteoporosis, alveolar bone density and periodontal disease in postmenopausal osteoporotic women and postmenopausal healthy women. Twenty-two women were evaluated for this study. They were attending the postmenopausal clinic, Seoul National University Hospital and generally healthy except osteoporosis. They had experienced menopause not less than one year when we began to examine them. Bone densities of lumbar area(L2-L4) was determined by DEXA(LUNAR-expert Co,. U.S.A). We diagnosed osteoporosis when T-score was below -2.5 and healthy state when T-score was over -1. Osteoporotic(10 female), not hormone-treated group and healthy control group(12 female) were asked for their age, menopausal age, menopausal period and the number of remaining teeth and examined clinically for plaque index(PI), gingival index(GI), clinical attachment loss(CAL) on their 6 Ramfjord index teeth. Intraoral radiographs were taken in maxillary anterior zone. All films were equally exposed and developed. Each films was digitized and analysed using image processing software, Scion image. Alveolar bone regions of interest were selected and Intensity of each pixel was quantized in the array ranging from 0(white) to 255(black). The two groups were comparable with respect age, menopausal age, menopausal period and number of remaining teeth. The osteoporotic women had significantly lower alveolar bone density than controls in maxilla. But no significant difference was found with respect clinical attachment loss, plaque index and gingival index. Supported by the Ministry of Public Health and Welfare, Korea (HMP-00-CH-10-0009).

  • Purpose We designed this study for the purpose of determining the relationship between periodontal disease activity and PLBW, using the evaluation of probing pocket depth, loss of attachment, gingival index, gingival crevicular fluid amount and subgingival microflora. Methods A total of 100 volunteer mothers(mean age 30.44) at the Department of Obstetrics and Gynecology Seoul National University Hospital were selected for this study.Pregnancy outcomes were categorized into cases and controls in two ways. our definition was based on the following; Group 1 : Any PLBW cases Vs. All NBW controls Group 2 : PLBW cases Vs. NBW controls A periodontal exam was performed on the Ramfjord( #16, 21, 24, 36, 41, 44) teeth and Clinical evaluation consisted of probing pocket depth, loss of attachment, gingival index and gingival crevicular fluid amount. Subgingival plaque samples were collected by three sterile #35 paper points. The total number of anaerobic colonies and aerobic bacteria were enumerated after incubation. Antisera to P. gingivalis, P. intermedia, A. actinomycetemcomitans were produced in white rabbits with live whole cells suspensions. The specific fluorescent bacteria obtained by immunofluorescence and total cell counts obtained by dark-field microscopy were counted on four fields. The percent of each specific microorganism in the total cell count was determined. Results Any PLBW and PLBW cases showed significantly greater probing depth and attachment loss than all NBW and NBW controls. Cases group had significantly increased anaerobic bacterial counts compared with control group and no differences in the other microbes. This study confirmed that periodontal disease is a statistically significant risk factor for PLBW by investigating clinical parameters and subgingival plaque analysis.

Antitumor and Immunologic Activity of Chitosan Extracted from Shell of Shrimp (새우 껍질에서 추출한 키토산의 항암 및 면역활성)

  • This study was designed to investigate the antitumor effect and immunological activities of chitosan extracted from Solenocera prominetis toward on mire bearing sarcoma-180. The growth inhibition ratio of the chitosan toward sarcoma-180 showed at the highest level of 63.84% when chitosan were administrated at the concentration of 40mg/kg. The direct cytotoxic effect of chitosan was not observed in the mice bearing sar-roma-180 in vitro. In the effect of immunological activities, dose-dependent responses indicated by the increase of leucocyte, peritoneal exudate cell than that of control group when chitosan administered to the mice in the concentation of 30mg/kg and 40mg/kg. Also dose-dependent responses showed also by the increase of immunoorgans weights such as body weight, liver, spleen or thymus in the same concentration of 30mg/kg and 40mg/kg. Food pad swelling having the relationship with arthus reaction of antibody-mediated hypersensitivity and delayed type hypersensitivity was recovered the almost normal level. In the efforts of macrophge on phagocytes, there were not substantial differences in phagorytic and corrected phagocytic index. In the number of plaque forming cell(PFC), PFC on the 10$^{7}$ spleen cells were increased the levels of 18.88% and 31.83% when chitosan were adminstersd at the concentration of 30mg/kg and 40mg/kg.

  • This study was undertaken to evaluate the role of peritoneal exudate cells in the transfer of immunity against the liver cuke, Clonorchis sinensis in the inbred BALB/c mice. Ten donor mice were divided into 2 groups. One group consisted of 5 mice was infected orally with 20 metacercariae of C. siitensis, and the other group was injected intraperitoneally with 20 excysted larvae. Thirty days after immunization, the peritoneal exudate cells tore obtained from the donor mice. Twenty recipient mice were divided into 4 equal groups for the purpose of primary immunization. The mice of Group I were injected intraperitoneally with $2{\times}10^6$ peritoneal exudate cells of the donor mice infected orally, those of Group II were injected intraperitoneally with $2{\times}10^6$ peritoneal exudate cells of the donor mice injected intraperitoneally. Those of Group III were injected orally with 20 metacercariae of C. sinensis. The group IV mice served as controls. Four days after the primary iMmunization all recipient mice were challenged orally with 20 metacercariae of C. sinensis, and then killed 30 days after the challenging infection. When the peritoneal exudate cells were injected into the recipient mice, pronounced reduction in eggs per gram of the feces was found in the mice o( Group I and Group II, but no reduction in those of Group III. In the worm burdens of C. sinensis, the number of flukes found in the mice of Group II was only significantly less than those in the control group (IV). In addition the number of plaque forming cells per spleen in the mice of Group II was found larger than those in Group I. It is likely that donor peritoneal exudate cells transferred to the recipients might result in the production of relative immunity.

An Experimental Study on the Effects of Houttuynia cordata Thunb and sanggukeum on Immune Function (어성초(魚腥草) 및 상국음(桑菊飮)이 면역기능(免疫機能)에 미치는 영향(影響))

  • In order to investigate the effect of Houttuynia cordata Thunb and Sanggukeum on immune function, the author performed this experimental study. Delayed type hypersensitivity (DTH) and rosette forming cells(RFC) for cell-mediated immune response, hemagglutinin (HA) titers, hemolysin (HL) titers and plaque forming cells (PFC) for humoral immune response, immunoglogbulin (Ig G) titer, splenic natural Killer cell activity (NKCA) carbon clearance for phagocytic function of MPS(mononuclear phagocyte system) and change of weight were measured in ICR mice. The results were summarized as follows ; 1. DTH was increased with statistical significance in all of the treated group as compared with the control group. 2. RFC was increased with statistical significance in case of Houttuynia cordata Thunb but in case of sanggukeum and gamisanggukeum valuable increase of RFC was not recognized as compared with the control group. 3. HA titers were increased with statistical significance in case of Houttuynia cordata Thunb but in cases of Sanggukeum and Gamisanggukeum HA titers were not recognized as compared with the control group. 4. HL titers were increased with statistical significance in case of Houttuynia cordata Thunb but in cases of Sanggukeum and Gamisanggukeum valuable increase of HL titer was not recognized as compared with the control group. 5. PFC was increased in all of the treated group but valuable increase of PFC was not recognized as compared with the controal group. 6. Ig G titers were increased in all of the treated group but valuable increase of Ig G titer was not recognized as compared with the control group. 7. NKCA was increased with statistical significance in case of Houttuynia cordate Thunb but in case of Sanggukeum and Gamisanggukeum valuable increase of NKCA was not recognized as compared with the control group. 8. Carbon clearance was increased with statistical significance in case of Sanggukeum but in case of Houttuynia cordata Thunb and Gamisanggukeum valuable increase of carbon clearance was not recognized as compared with the control group. 9. Change of weight was increased with statistical significance in all of the treated group. Through in vivo experimental study in ICR mice, Houttuynia cordata Thunb enhances the cell-mediated immune responce, the humoral immune responce and natural killer cell activity. And Houttuynia cordata Thunb enhances immune responce as compared with that plused Sanggukeum. Sanggukeum enhances carbon clearance and enhances a little cell-mediated immune responce, the humoral immune response and natural killer cell activity. According to the above results it seems Houttuynia cordata Thunb and Sanggukeum was able to use Infection, Inflammation and Tumor.

Scanning electron microscopic study on characteristics of tooth surface when using gracey curet and ultrasonic scaler (Gracey curet과 Ultrasonic scaler 사용 시의 치면 특성의 주사현미경적 연구)

  • Objectives: The purpose of this study is to assess the characteristics of tooth surface after using Gracy curet and Ultrasonic scaler Methods: In this study, 12 teeth extracted were used. 12 specimens were divided into three groups with the same numbers, which were classified into Control group, Gracy curet use group, and Ultrasonic scaler use group, and after performing instrument operation, we measured the roughness and the loss degree of tooth surface by using SEM. Results: In groups using Gracy curet and Ultrasonic scaler, the roughness and the loss of tooth surface increased significantly(p<0.05). In the roughness of groups using Gracy curet and Ultrasonic scaler, Ultrasonic scaler group was higher in crown, but Gracy curet group was higher in root. As a result of observation through SEM, the roughness and the loss degree increased in order of Control group, Ultrasonic scaler use group, and Gracy curet use group. Conclusions: Taken together above results, both hand instrument and ultrasound equipment create roughness and loss in crown and in root, and hand instruments makes rougher than ultrasonic instruments in root, so it is thought to require thorough and accurate technical application not to damage tooth surface when removing plaque.
