정상인과 통증이 있는 족부의 내측 종아치의 높이와 부피 (The Height and Volume of Medial Longitudinal Arch in Normal and Painful Feet)

  • Purpose: The purpose of this study was to investigate the differences in simple radiographic parameters and results of 3-D scan among normal and patient groups. Materials and Methods: Seventy subjects in each group were studied. Control group consisted of subjects without plantar foot pain (normal group), and two patient groups were one with plantar forefoot pain (metatarsalgia group), the other with plantar heel pain (heel pain group). Simple radiographic parameters were obtained and 3-D scan was done with foot scanner (Nexscan, K&I, Korea) and The height and volumn of the space under the medial longitudinal arch was analyzed (Enfoot, K&I, Korea). These parameters were compared and correlation between radiological parameters and results of the 3-D scan were studied. Results: The results of all parameters istributed normally. There was no signigicant differences among the groups in radiological parameters (talo-first metatarsal angle, calcaneal pitch angle and height of the talar head in standing lateral radiograph) and arch height and arch volumn on 3-D scan. There were statistically significant correlations between radiological and 3-D scan results. Conclusion: This study revealed that there is no significant differences in medial longitudinal arch height and volumn among normal and different patient groups and there are variety of arch height in patients with similar symptoms.

다제내성 결핵의 치료 중 역설적 반응으로 나타난 종골 골수염 (Calcaneal Osteomyelitis Presenting as a Paradoxical Reaction during Treatment of Multidrug-Resistant Tuberculosis)

  • Tuberculosis in the foot progresses gradually; thus, diagnosis is usually delayed, and early treatment is rarely provided. If osteomyelitis occurs due to delayed diagnosis and treatment, surgical treatment should be considered. We report the case of a 46-year-old man with osteomyelitis of the calcaneus who was diagnosed with multidrug-resistant pulmonary tuberculosis and he was treated with anti-tuberculosis drugs. Bilateral adrenal masses, abscess of both testes and a small wound in the left plantar heel were observed. Both adrenal masses and abscess were regarded as paradoxical reaction of anti-tuberculosis treatment. After 1 month, he developed a pain in the left plantar heel that was compatible with calcaneal osteomyelitis in radiological features. He underwent right orchiectomy for right scrotal abscess aggravation and surgical treatment for left calcaneal osteomyelitis. Mycobacterium tuberculosis was confirmed by polymerase chain reaction. The patient was immobilized by cast for 8 weeks and the heel pain gradually improved.

  • Plantar fasciitis is most common cause of heel pain which starts anterior tubercle of calcaneus. It is chronic inflamation of plantar fascia, reduces collagen and water content of heel pain which incur the degenerative changes with elastic fiber weakness. We treated one patient after extracorporeal shock wave treatment. We diagnosed him with X-ray and treated her conservative maneuver as to oriental medical method. We measured Vas(Visual analogue scale) and thermographic picture of both leg. Visual analogue scale is from 10 to 4 and thermographic picture of both leg were improved in cases.

  • Plantar fasciitis is a common pathological condition of the foot and often be a challenge for clinicans to successfully treat. The purpose of this article is to present and discuss selected literature on the function and anatomy, causes, symptoms and clinical treating methods of plantar fasciitis. A majority of patients with plantar fasciitis present with either a pronated or a cave foot. Pain is usually localized to the plantar medial heel at the attachment of the plantar fascia to the calcaneus. Surgical and nonsurgical techniques have been used in the treatment of plantar fasciitis. Nonsurgical management for the treatment of the symptoms and discomfort associated with plantar fasciitis can be classified into brod categories: reducing pain and inflammation. reducing tissue stress to a tolerable level, and restoring muscle strength and flexibility of involved tissue.

내측 족저 피판을 이용한 족부의 재건 (The Reconstruction of Foot using Medial Plantar Flap)

  • Purpose : Plantar surfaces, calcaneal area, and region of Achilles insertion, which are extremely related with weight-bearing area and shoes application, must be reconstructed with glabrous and strong fibrous skin. Numerous methods of reconstructing defects of these regions have been advocated, but the transfer of similar local tissue as a cutaneous flap with preservation of sensory potential would best serve the functional needs of the weight-bearing and non-weight-bearing surfaces of this region. Therefore it is recommended to use the limited skin of medial surface of foot that is similar to plantar region and non-weight-bearing area. In this paper we performed the medial plantar flap transfered as a fasciocutaneous island as one alterative for moderate-sized defects of the plantar forefoot, plantar heel, and area around the ankle in 25 cases and report the result, availability and problem of medial plantar flap. Materials and methods : We performed proximally based medial plantar flap in 22 cases and reverse flow island flap in 3 cases. Average age was $36.5(4{\sim}70)$ years and female was 3 cases. The causes of soft tissue defect were crushing injury on foot 4 cases, small bony exposure at lower leg 1 case, posterior heel defect with exposure of calcaneus 8 cases, severe sore at heel 2 cases, skin necrosis after trauma on posterior foot 4 cases, and defect on insertion area of Achilles tendon 6cases. Average follow up duration was 1.8(7 months-9.5 years) years. Results: Medial plantar flaps was successful in 22 patients. 18 patients preserved cutaneous branches of medial plantar nerve had sensation on transfered flap but diminished sensation or dysesthesia. At the follow up, we found there were no skin ulceration, recurrence of defect or skin breakdown in all 18 patients. But there was one case which occurred skin ulceration postoperatively among another 4 cases not contained medial plantar nerve. At the last follow up, all patients complained diminished sensation and paresthesia at medial plantar area distally to donor site, expecially with 4 patients having severe pain and discomfort during long-time walking. Conclusion : Medial plantar island flap based on medial plantar neurovascualr pedicle have low failure rate with strong fibrous skin and preserve sensibility of flap, so that it is useful method to reconstruct the skin and soft tissue defect of foot. But it should be emphasized that there are some complications such like pain and paresthesia by neuropraxia or injury of medial plantar nerve at more distal area than donor site. We may consider that medial plantar flap have limited flap size and small arc of rotation, and require skin graft closure of the donor defect and must chose this flap deliberately.

Two Cases of Plantar Fasciitis Treated with Magnetic Acupuncture

  • Objectives : The aim of the study was to ascertain the effectiveness of magnetic acupuncture in patients with plantar fasciitis. Methods : The patients were treated with magnetic acupuncture using a magnetic therapy stimulator (Whata 153), as well as with moxibustion therapy. Acupuncture treatment was conducted on the acupoints of Ashi-as well as on 太谿 (KI3), 崑崙 (BL60), and 委中 (BL40)-for 15 minutes. The intensity of pain was evaluated using the visual analogue scale (VAS), and the tenderness threshold (TT) was measured at each visit using an algometer. Results : After treatment, evaluation using the VAS had improved, as had the TT; furthermore, plantar fasciitis pain was reduced. Conclusion : Magnetic acupuncture relieved pain in plantar fasciitis. In further clinical study, more research on magnetic acupuncture is required.

종골부 동통 증후군의 원인분석 (Bone Spur and Over Weight in Painful Heel Syndrome and Tenderness, Underlying Cause Analysis)

  • Chiefly, painful heel syndrome is observed at old age. Many possible reasons were bursitis, plantar fasciitis, calcaneal periostitis around the calcaneus and achilles tendon. But the exact cause was not clearly identified due to complexity of subcalcaneal pain mechanism. The incidence of calcaneal spur and overweight were significant in painful heel syndrome. Our purpose of this study was analysis of underlyng cause and correlation about bony spur and overweight in painful heel syndrome. The author used incidence of heel spur on painful heel syndrome and body mass index to evaluate overweight. The material is 55 cases of painful heel syndrome patients and 60 cases of control group. Bony spur was one of the cause of painful heel syndrome. Body weight and calcaneal spur was developing factors on painful heel syndrome. And the result was as follows. 1. Spur formation incidence is 35 cases (63.6 %) in painful heel syndrome, 8 cases (13.3%) in normal control group. So, patient's group is significantly high (p<0.01). 2. Body mass index is 26.48 in painful heel syndromes, 21.75 in normal control groups. Overweight above index 27 is 22 cases (40%) in painful heel syndromes, 3 cases (5%) m normal control groups. So, patient's group is significantly high (p<0.01). 3. In painful heel syndrome, tenderness site is 46 cases (83.6%) in medial calcaneal tuberosity, 4 cases (7.3%) in central calcaneal tuberosity, 1 cases (1.8%) in both site. 4. Underlying causes of painful heel syndrome is 19 cases (34.5%) in plantar fasciitis, 16 cases (29.1%) in calcaneal periostitis, 11 cases (20%) in bursitis, 4 cases (7.3%) in tendinitis, 2 cases (3.6%) in entrapment neuropathy.

  • Background: The purpose of this study is to examine the correlation between high heels and body imbalance among female college students in their twenties who mainly wear high heels and prevent associated problems. Methods: The subjects included 89 female college students in their twenties. They were measured in plantar pressure with a gait analyzer. Their legs were measured in length with a tape measure. Their pain intensity and pain frequency were measured in visual analog scale and in pain rating score. Results: There were statistically positive correlations between right leg length and low back pain frequency (p<.05) and negative correlations between the left hindfoot and low back pain frequency (p<.05). There were statistically positive correlations between right leg length and knee pain frequency (p<.05) and positive correlations between the ankle pain intensity and right leg length (p<.05). Conclusion: The stronger the pain was in the ankle, the stronger and more frequent their lumber pain was. When the pressure of the left heel was lower, the frequency of lumbar pain increased.

요족형 족저근막염 환자에 대한 도침요법과 근막 추나요법 병행 치료의 임상적 효과: 증례 보고 (A Case Report of Talipes Cavus-Type Plantar Fasciitis Treated with Acupotomy and Fascia Chuna Therapy)

  • Objectives This case study aimed to investigate the effect of acupotomy and fascia chuna therapy on talipes cavus-type plantar fasciitis. Methods We classified the foot arch type using a podoscope and estimated the outcome by evaluating the numeric rating scale, pain disability index, and EuroQol-5-dimension measurement. Results After treatment, heel pain decreased and the quality-of-life score improved. Conclusions This study suggests that treatment with acupotomy and muscle energy techniques based on the foot arch type may be effective for plantar fasciitis. A limitation of this study is the small number of cases. Further clinical studies are required.

평지와 오르막경사의 트레드밀 걷기와 달리기 동안 발보장구가 발바닥근막염과 관련된 발아치 스트레인에 미치는 영향 (Effects of Foot Orthotics on the Foot Arch Strain related to Plantar Fasciitis During Treadmill Level and Uphill Walking and Running)

  • There is some evidence that one of major factors to produce plantar fasciitis depends on the magnitude of the foot arch strain. The orthotics that can reduce the foot arch strain during locomotion may be effective to prevent or treat plantar fasciitis. Therefore, the purpose of this study was to investigate the effect of control condition and three types of foot orthotics on 3-dimensional foot arch strain that can produce plantar fasciitis during treadmill level and uphill walking and running. Sixteen male subjects are recruited and the arch length and height strain according to three types of foot orthotics with respect to control condition were measured by using two digital video cameras. The first hypothesis which the comfort of foot orthotics would be increased from arch pad, half length orthotics to full length orthotics was mostly accepted. It suggested that the types of the foot orthotics could be properly prescribed according foot regions that is pain or abnormal. The second hypothesis which the foot arch strain can be reduced by foot orthotics during level heel-toe walking and running and the third hypothesis which the foot arch strain can be reduced by foot orthotics during uphill heel-toe walking and running were rejected. The foot arch length and height strain during walking and running showed small and subject-specific characteristics and could not be optimal biomechanical variable to prove the overall comfort. The forth hypothesis which the foot arch strain cannot be reduced by foot orthotics during uphill toe walking and running was accepted. With the foot arch length and height strain during uphill toe walking and running the windlass mechanism suggested by Hicks can be explained successfully and excessive uphill toe walking and running can be one of cause of plantar fasciitis. The dynamic investigation on the foot arch such as walking and running should be carefully observed with integrated insights considering ligaments and foot bones as well as plantar fascia, extrinsic muscles and tendons, and intrinsic muscles and tendons.
