The apnea-hypopnea index is widely regarded as a measure of the severity of obstructive sleep apnea (OSA), a condition characterized by recurrent episodes of apnea or hypopnea during sleep that induce airway collapse. OSA is a catastrophic problem due to the wide range of health issues it can cause, including cardiovascular disease and memory loss. This review was conducted to clarify the roles of various imaging modalities, particularly cone-beam computed tomography (CBCT), in the diagnosis of and preoperative planning for OSA. Unfortunately, 2-dimensional imaging techniques yield insufficient data for a comprehensive diagnosis, given the complex anatomy of the airway. Three-dimensional (3D) imaging is favored as it more accurately represents the patient's airway structure. Although computed tomography and magnetic resonance imaging can depict the actual 3D airway architecture, their use is limited by factors such as high radiation dose and noise associated with the scans. This review indicates that CBCT is a low-radiation imaging technique that can be used to incidentally identify patients with OSA, thereby facilitating early referral and ultimately enhancing the accuracy of surgical outcome predictions.

New modular factory-built methodologies implemented in the construction and industrial plant industries may bring down costs for modular reactors. A factory-built environment brings about benefits such as; improved equipment, tools, quality, shift patterns, training, continuous improvement learning, environmental control, standardisation, parallel working, the use of commercial off shelf equipment and much of the commissioning can be completed before leaving the factory. All these benefits combine to reduce build schedules, increase certainty, reduce risk and make financing easier and cheaper.Currently, the construction and industrial chemical plant industries have implemented successful modular design and construction techniques. Therefore, the objectives of this paper are to understand and analyse the state of the art research in these industries through a systematic literature review. The research can then be assessed and applied to modular reactors.The literature review highlighted analysis methods that may prove to be useful. These include; modularisation decision tools, stakeholder analysis, schedule, supply chain, logistics, module design tools and construction site planning. Applicable research was highlighted for further work exploration for designers to assess, develop and efficiently design their modular reactors.

The frozen elephant trunk (FET) technique can be applied to extensive aortic pathology, including lesions in the aortic arch and proximal descending thoracic aorta. FET is useful for tear-oriented surgery in dissections, managing malperfusion syndrome, and promoting positive aortic remodeling. Despite these benefits, complications such as distal stent-induced new entry and spinal cord ischemia can pose serious problems with the FET technique. To prevent these complications, careful sizing and planning of the FET are crucial. Additionally, since the FET technique involves total arch replacement, meticulous surgical skills are essential, particularly for young surgeons. In this article, we propose several techniques to simplify surgical procedures, which may lead to better outcomes for patients with extensive aortic pathology. In the era of precision medicine, the next-generation FET device could facilitate the treatment of complex aortic diseases through a patient-tailored approach.

Many failures in total or subtotal nasal reconstruction result from an underestimation of the amount of skin required for an adequate result, especially for sufficient lining. Such planning errors usually lead to poor results, with exposure of structural grafts, infection, scar retraction, airway obstruction, and finally loss of projection and shape of the reconstructed nose. Reconstruction options for cases in which previous attempts have failed are always limited, as well as in cases of trauma or burns affecting the soft tissues of the forehead and face. In such complex situations, one may employ free flaps or tissue expansion, but such resources may not be always available. We describe a technique indicated for salvage surgeries in patients whose previous nasal reconstructions have failed, allowing a generous amount of tissue transfer for the nasal region. The technique combines the use of supraclavicular and submental flaps, with simple execution, not requiring microsurgical skills or devices such as tissue expanders. Done in three stages, the described technique provides enough skin for a total nasal reconstruction. The final result is obtained after subsequent refinements, and the total number of procedures is equivalent to when more sophisticated techniques are employed, such as tissue expansion or microsurgery.

The project for rehabilitation and revegetation of the abandoned coal-mine lands is a very important national environmental restoration project in the view point of rehabilitation and revegetation of denuded forest-lands caused by coal mining as well as restoration of disturbed natural environment and control of the variable pollutions. In Korea, because a large number of coal mines had been developed in order to fill up abundantly consumption of coal as a major energy source in the developing period, a lot of denuded forest-lands caused by coal mining had distributed in the whole country. And, due to the absence of effective rehabilitation and revegetation works on the denuded forestlands caused by coal-mining, most of them had been remained with being damaged. In 1990, area of the abandoned coal-mine lands, requiring the rehabilitation and revegetation works, was about 1,437.1 ha. For the past ten years ('90~'99), about 1,081.8 ha out of them had been rehabilitated and revegetated, and the rehabilitation planning area was about 33.0 ha in 2000. So, remaining area out of abandoned coal-mine lands will be about 322.3 ha after 2000. In principle, after abandoning coal-mine, mine owners must carry out the rehabilitation and revegetation works on the abandoned mine lands by themselves. But, most of mine owners were in financial difficulty after abandoning coal-mine, so that principle couldn't have obtained the desired effects. To solve this problem, from 1995, Coal Industry Promotion Board (CIPB) have carried out the rehabilitation and revegetation works on the abandoned coal-mine lands at government budgets, and they have obtained good results in the construction area. However, due to application of the "conventional erosion control measures and techniques" to the rehabilitation and revegetation measures on the abandoned coal-mine lands, the results and effects of the works excuted have not been successful. Therefore, unique measures and techniques for rehabilitation and revegetation of the abandoned coal-mine lands will have to be developed, especially including development of new techniques on the soil-dressing and soil-covering, seed spray and hydro-seeding measures with seed-fertilizer-soil materials as the mechanized measures, and using of new materials for the tree planting and seedling measures.
Up-front irradiation technique as 3-dimensional conformation, or intensity modulation has kept large proportion of brain tumors from being complicated with acute radiation reactions in the normal tissue during or shortly after radiotherapy. For years, we've cannot help but counting on 2-D vertex beam technique to reduce acute reactions in the brain tumor patients because we're not equipped with 3-dimensional planning system. We analyzed its advantages and limitations in the clinical application. From 1998 to 2001, vertex or oblique vertex beams were applied to 35 patients with primary brain tumor and 25 among them were eligible for this analysis. Vertex(V) plans were optimized on the reconstructed coronal planes. As the control, we took the bilateral opposed techniques(BL) otherwise being applied. We compared the volumes included in 105% to 50% isodose lines of each plan. We also measured the radiation dose at various extracranial sites with TLD. With vertex techniques, we reduced the irradiated volumes of contralateral hemisphere and prevented middle ear effusion at contralateral side. But the low dose volume increased outside 100%; the ratio of V to BL in irradiated volume included in 100%, 80%, 50% was 0.55+/-0.10, 0.61+/-0.10, and 1.22+/-0.21, respectively. The hot area within 100% isodose line almost disappeared with vertex plan; the ratio of V to BL in irradiated volume included in 103%, 105%, 108% was 0.14+/-0.14, 0.05./-0.17, 0.00, respectively. The dose distribution within 100% isodose line became more homogeneous; the ratio of volume included in 103% and 105% to 100% was 0.62+/-0.14 and 0.26+/-0.16 in BL whereas was 0.16+/-0.16 and 0.02+/-0.04 in V. With the vertex techniques, extracranial dose increased up to $1{\sim}3%$ of maximum dose in the head and neck region except submandibular area where dose ranged 1 to 21%. From this data, vertex beam technique was quite effective in reduction of unnecessary irradiation to the contralateral hemispheres, integral dose, obtaining dose homogeneity in the clinical target. But it was associated with volume increment of low dose area in the brain and irradiation toward the head and neck region otherwise being not irradiated at all. Thus, this 2-D vertex technique can be a useful quasi-conformal method before getting 3-D apparatus.

Recent advances in technology of medical image have made surgical simulation that is helpful to diagnosis, operation plan, or education. Improving and enhancing the medical imaging have led to the availability of high definition images and three-dimensional (3D) visualization, it allows a better understanding in the surgical and educational field. The Real human field of view is stereoscopic. Therefore, with just 2D images, stereoscopic reconstruction process through the surgeon's head, is necessary. To reduce these process, 3D images have been used. 3D images enhanced 3D visualization, it provides significantly shorter time for surgeon for judgment in complex situations. Based on 3D image data set, virtual medical simulations, such as virtual endoscopy, surgical planning, and real-time interaction, have become possible. This article describes principles and recent applications of newer imaging techniques and special attention is directed towards medical 3D reconstruction techniques. Recent advances in technology of CT, MR and other imaging modalities has resulted in exciting new solutions and possibilities of shoulder imaging. Especially, three-dimensional (3D) images derived from medical devices provides advanced information. This presentation describes the principles and potential applications of 3D imaging techniques, simulation and printing in shoulder and elbow practice.
This paper attempts to present a review about simulation of different greenhouse parameters and energy supplying techniques by using building energy simulation, to find out the optimal solution for keeping greenhouse microclimate favorable for the crop production. The objectives of conducting this study were, to describe the various energy systems and techniques used for the greenhouse energy management and efficiency analysis of these technologies by using building energy simulation. We describe different models to understand the behavior of the energy saving technologies with respect to the resources available and different outside climatic conditions. We identified main features of the building energy simulation software, that enable users, to simulate hybrid agricultural building projects by using user defined parameters. At the end of the paper we draw some important concluding remarks on the basis of reviewing all the investigators contributions for the developments of simulation model of agricultural greenhouse energy management, using a building energy simulation software specifically TRNSYS. In conclusion, this paper provides information that TRNSYS have great potential for agricultural buildings energy simulation along with the renewable energy resources and energy saving techniques. This review paper provides aid to greenhouse researcher and energy planner for the future studies of greenhouses energy planning.
Purpose: This study was designed to investigate the dosimetric difference between intensity-modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) in head and neck cancer (HNC). The study primarily focuses on low-dose spillage evaluation between these two techniques. Methods: This retrospective study involved 45 patients with HNC. The treatment plans were generated using the IMRT and VMAT techniques for all patients. Dosimetric comparisons were performed in terms of target coverage, organ-at-risk (OAR) sparing, and various parameters, including conformity index, uniformity index, homogeneity index, conformation number, low-dose volumes, and normal tissue integral dose (NTID). Results: No significant (P>0.05) difference in planning target volume coverage (D95%) was observed between IMRT and VMAT plans for supraglottic larynx, hard palate, and tongue cancers. A decrease in dose volumes ranging from 1 Gy to 30 Gy was observed for VMAT plans compared with those for IMRT plans, except for V1Gy and V30Gy for supraglottic larynx cancer and V1Gy for tongue cancer. Moreover, decreases (P<0.05) in NTID were observed for VMAT plans compared with that for IMRT plans in supraglottic larynx (4.50%), hard palate (12.80%), and tongue (7.76%) cancers. In contrast, a slight increase in monitor units for VMAT compared with those for IMRT in supraglottic larynx (0.46%), hard palate (2.54%), and tongue (7.56%) cancers. Conclusions: For advanced-stage HNC, both IMRT and VMAT offer satisfactory clinical plans. VMAT offers a conformal and homogeneous dose distribution with comparable OAR sparing and higher dose falloff outside the target volume than IMRT, which provides an edge to reduce the risk of secondary malignancies for HNC over IMRT.

As the frequency of extreme rainfall events increase due to climate change, climate change adaptation measures have been proposed by the central and local governments. In order to reduce flood damage in urban areas, various flood response policies, such as low impact development techniques and enhancement of the capacity of rainwater drainage networks, have been proposed. When these policies are established, regional characteristics and policy-effectiveness from the cost-benefit perspective must be considered for the flood mitigation measures. In this study, capacity enhancement of rainwater pipe networks and low impact development techniques including green roof and permeable pavement techniques are selected. And the flood reduction effect of the target watershed, Gwanak campus of Seoul National University, was analyzed using SWMM model which is an urban runoff simulation model. In addition, along with the quantified urban flooding reduction outputs, construction and operation costs for various policy scenarios were calculated so that cost-benefit analyses were conducted to analyze the effectiveness of the applied policy scenarios. As a result of cost-benefit analysis, a policy that adopts both permeable pavement and rainwater pipe expansion was selected as the best cost-effective scenario for flood mitigation. The research methodology, proposed in this study, is expected to be utilized for decision-making in the planning stage for flood mitigation measures for each region.
