Neck Node Bolus Technique in the Treatment of Nasopharyngeal Carcinoma with Intensity-modulated Radiotherapy

  • Purpose: To study the effect of bolus versus no bolus in the coverage of the nodal tumour volume with intensity-modulated radiotherapy (IMRT) for the treatment of nasopharyngeal carcinoma (NPC). Methods and Materials: This retrospective study used data from 5 consecutive patients with NPC who were treated with bolus for large neck nodes using IMRT from November 2011-January 2012 in our institute. All these patients were treated radically with IMRT according to our institution's protocol. Re-planning with IMRT without bolus for these patients with exactly the same target volumes were done for comparison. Comparison of the plans was done by comparing the V70 of PTV70-N, V66.5 of PTV70-N, V65.1 of PTV70-N and the surface dose of the PTV70-N. Results: The mean size of the largest diameter of the enlarged lymph nodes for the 5 patients was 3.9 cm. The mean distance of the GTV-N to the skin surface was 0.6 cm. The mean V70 of PTV70-N for the 5 patients showed an absolute advantage of 10.8% (92.4% vs. 81.6%) for the plan with bolus while the V66.5 of PTV70-N had an advantage of 8.1% (97.0% vs. 88.9%). The mean V65.1 also had an advantage of 7.1% (97.6% vs. 90.5%). The mean surface dose for the PTV70-N was also much higher at 61.1 Gy for the plans with bolus compared to only 23.5 Gy for the plans without bolus. Conclusion: Neck node bolus technique should be strongly considered in the treatment of NPC with enlarged lymph nodes treated with IMRT. It yields a superior dosimetry compared t o non-bolus plans with acceptable skin toxicity.

유방암 환자의 3D-CRT, TOMO 방법에 따른 선량 분포 평가 (Dosimetric Comparison of Three Dimensional Conformal Radiation Radiotherapy and Helical Tomotherapy Partial Breast Cancer)

  • 목 적: 방사선치료계획에 있어서 정상조직과 치료부위의 선량 분포는 매우 중요하다. 이에 본원에서는 유방암 환자를 대상으로 Three-dimensional conformal radiation therapy (3D-CRT), Helical tomotherapy (TOMO)의 방법으로 방사선치료계획을 세웠으며 이에 선량분포를 분석하여 실제 임상에서의 적용여부를 알아보고자 한다. 대상 및 방법: 20명의(좌측: 10명, 우측: 10명) 유방보존절제술 환자를 대상으로 시행하였으며 방법으로는 같은 조건에서 3D-CRT는 Philips사의 Pinnacle을, TOMO는 TomoTherapy사의 TOMO Planning System을 이용해 치료계획을 세웠다. Dose-Volume Histogram (DVH)의 prescribed dose (PD)에 대한 PTV의 Homogeneity index (HI)와 Conformity index (CI)를 구하였고, 정상조직의 dose- volume 관계를 비교하였다. 결 과: Homogeneity index (HI)와 Conformity, index (CI)는 TOMO에서 우수한 결과를 나타났다. $V_{-50-IB-NPTV}$ (the percentage ipsilateral non-PTV breast volume that was delivered 50% of the prescribed dose)는 3D-CRT: 40.4%, TOMO: 18.3%, $V_{20-IL}$ (the average ipsilateral lung volume percentage receiving 20% of the prescribed dose)는 3D-CRT: 4.8%, TOMO: 14.2%, $V_{20-10H}$ (the average heart volume percentage delivered 20% and 10% of the prescribed dose in left breast cancer)는 3D-CRT: 1.6%, 3% TOMO: 9.7%, 26.3%의 결과를 보여준다. 결 론: 유방암 환자의 방사선치료계획 방법들은 PTV에서 원하는 선량분포를 보여줬다. 그러나 TOMO는 좋은 Homogeneity index (HI), Conformity index (CI)와 Breast를 보호하는 장점이 있는 반면에 Lung과 Heart에서는 많은 피폭선량이 있음을 알 수 있기에 TOMO의 방사선치료계획시 주의해야 할 점으로 사료된다.

조기유방암환자의 이차원치료계획과 삼차원치료계획의 방사선조사범위의 차이 (Difference in the Set-up Margin between 2D Conventional and 3D CT Based Planning in Patients with Early Breast Cancer)

  • 목 적: 유방암 치료를 위한 방사선치료계획에 전산화단층촬영을 이용한 모의치료가 널리 이용되고 있다. 이는 환자 개개인의 해부학적 특징에 기반하여 종양 표적과 정상 조직을 정의할 수 있다. 본 연구는 일반적으로 유방 크기가 작은 한국인 유방암 환자에서 적용하는 방사선치료계획 방법에 따라 치료용적 및 치료범위에 차이가 있는지를 알아보기 위해 시행하였다. 대상 및 방법: 2008년 11월부터 2009년 1월까지 조기유방암으로 유방보존술을 시행 받은 25명의 환자를 대상으로 통상적인 접면조사의 이차원치료 계획과 전산화단층촬영을 기반으로 한 삼차원치료계획을 시행하였다. 이차원 치료계획에서 방사선조사영역의 경계는 유방실질을 촉지하여 결정하였고 위쪽으로는 쇄골의 바로 아래, 내측은 몸의 정중선, 외측은 중심 액와선 그리고 아래쪽은 유방밑주름에서 2 cm 아래에 표시하였다. 삼차원치료계획에서 임상표적체적은 모든 유선 조직을 포함하였고 계획용 표적체적은 임상표적체적에서 피부 쪽을 제외한 모든 방향으로 1 cm의 여유를 두어 결정하였다. 이차원과 삼차원치료계획 간의 조사영역의 경계 차이 및 동등조사영역의 차이를 확인하고 신체질량지수, 폐경 여부 및 유방크기의 관련성을 평가하였다. 그리고 선량분포로 지정한 유방조사량과 삼차원 선량분포에 기반하여 측정된 조사된 폐용적과 심장용적을 평가하였다. 결 과: 2명의 환자를 제외하고 계획용표적체적(PTV)은 삼차원치료계획이 이차원치료계획보다 모든 방향에서 표시한 방사선조사영역을 감소시켰고 위쪽 경계에서 그 차이값이 가장 컸다. 동등조사영역크기는 한 명을 제외하고 삼차원치료계획에서 이차원치료계획에서보다 작았으며(평균 0.9 cm), 신체질량지수, 폐경 여부 및 유방크기(브래지어 사이즈)와의 관련성은 보이지 않았다. 조사된 폐용적은 삼차원치료계획에서 유의하게 감소하였고, 심장용적 또한 감소하였지만 통계적으로 유의한 차이는 보이지 않았다. 결 론: 통상적인 치료계획과 비교하여 전산화단층촬영을 기반으로 한 삼차원치료계획이 작은 유방을 가진 환자에서 방사선조사영역 및 손상위험장기의 조사량을 줄이면서 수술 후 변화를 포함하여 더 정확한 종양치료가 가능할 수 있음을 보여주었다. 그러나 앞으로 임상표적체적의 정의에 대한 추가적인 연구 및 일반적인 합의가 필요하다.

Benefit of volumetric-modulated arc therapy over three-dimensional conformal radiotherapy for stage I-II extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue in the stomach: a dosimetric comparison

  • Purpose: To retrospectively analyze dosimetric parameters of volumetric-modulated arc therapy (VMAT) and three-dimensional conformal radiotherapy (3D-CRT) delivered to extranodal marginal zone B-cell lymphomas of mucosa-associated lymphoid tissue in the stomach (gastric MALT lymphoma) to find out advantages of VMAT and conditions for definite benefits of VMAT. Materials and Methods: Fifty patients with stage I-II gastric MALT lymphoma received VMAT (n = 14) or 3D-CRT (n = 36) between December 2005 and April 2018. Twenty-seven patients were categorized according to whether the planning target volume (PTV) overlaps kidney(s). Dosimetric parameters were analyzed by dose-volume histogram. Results: Radiation dose to the liver was definitely lower with VMAT in terms of mean dose (p = 0.026) and V15 (p = 0.008). The V15 of the left kidney was lower with VMAT (p = 0.065). For those with PTV overlapping kidney(s), the left kidney V15 was significantly lower with VMAT. Furthermore, the closer the distance between the PTV and kidneys, the less the left kidney V15 with VMAT (p = 0.037). Delineation of kidney(s) by integrating all respiratory phases had no additional benefit. Conclusions: VMAT significantly increased monitor units, reduced treatment time and radiation dose to the liver and kidneys. The benefit of VMAT was definite in reducing the left kidney V15, especially in geometrically challenging conditions of overlap or close separation between PTV and kidney(s).

Influence of Intravenous Contrast Medium on Dose Calculation Using CT in Treatment Planning for Oesophageal Cancer

  • Objective: To evaluate the effect of intravenous contrast on dose calculation in radiation treatment planning for oesophageal cancer. Methods: A total of 22 intravein-contrasted patients with oesophageal cancer were included. The Hounsfield unit (HU) value of the enhanced blood stream in thoracic great vessels and heart was overridden with 45 HU to simulate the non-contrast CT image, and 145 HU, 245 HU, 345 HU, and 445 HU to model the different contrast-enhanced scenarios. 1000 HU and -1000 HU were used to evaluate two non-physiologic extreme scenarios. Variation in dose distribution of the different scenarios was calculated to quantify the effect of contrast enhancement. Results: In the contrast-enhanced scenarios, the mean variation in dose for planning target volume (PTV) was less than 1.0%, and those for the total lung and spinal cord were less than 0.5%. When the HU value of the blood stream exceeded 245 the average variation exceeded 1.0% for the heart V40. In the non-physiologic extreme scenarios, the dose variation of PTV was less than 1.0%, while the dose calculations of the organs at risk were greater than 2.0%. Conclusions: The use of contrast agent does not significantly influence dose calculation of PTV, lung and spinal cord. However, it does have influence on dose accuracy for heart.

  • Radiation therapy (RT) has improved patient outcomes, but treatment-related complication rates remain high. In the conventional 2-dimensional and 3-dimensional conformal RT (3D-CRT) era, there was little room for toxicity reduction because of the need to balance the estimated toxicity to organs at risk (OARs), derived from dose-volume histogram data for organs including the lung, heart, spinal cord, and liver, with the planning target volume (PTV) dose. Intensity-modulated RT (IMRT) is an advanced form of conformal RT that utilizes computer-controlled linear accelerators to deliver precise radiation doses to the PTV. The dosimetric advantages of IMRT enable better sparing of normal tissues and OARs than is possible with 3D-CRT. A major breakthrough in the treatment of esophageal cancer (EC), whether early or locally advanced, is the use of proton beam therapy (PBT). Protons deposit their highest dose of radiation at the tumor, while leaving none behind; the resulting effective dose reduction to healthy tissues and OARs considerably reduces acute and delayed RT-related toxicity. In recent studies, PBT has been found to alleviate severe lymphopenia resulting from combined chemo-radiation, opening up the possibility of reducing immune suppression, which might be associated with a poor prognosis in cases of locally advanced EC.

Planning and Dosimetric Study of Volumetric Modulated Arc Based Hypofractionated Stereotactic Radiotherapy for Acoustic Schwannoma - 6MV Flattening Filter Free Photon Beam

  • Background: The purpose of this study was to assess the dosimetric and clinical feasibility of volumetric modulated arc based hypofractionated stereotactic radiotherapy (RapidArc) treatment for large acoustic schwannoma (AS >10cc). Materials and Methods: Ten AS patients were immobilized using BrainLab mask. They were subject to multimodality imaging (magnetic resonance and computed tomography) to contour target and organs at risk (brainstem and cochlea). Volumetric modulated arc therapy (VMAT) based stereotactic plans were optimized in Eclipse (V11) treatment planning system (TPS) using progressive resolution optimizer-III and final dose calculations were performed using analytical anisotropic algorithm with 1.5 mm grid resolution. All AS presented in this study were treated with VMAT based HSRT to a total dose of 25Gy in 5 fractions (5fractions/week). VMAT plan contains 2-4 non-coplanar arcs. Treatment planning was performed to achieve at least 99% of PTV volume (D99) receives 100% of prescription dose (25Gy), while dose to OAR's were kept below the tolerance limits. Dose-volume histograms (DVH) were analyzed to assess plan quality. Treatments were delivered using upgraded 6 MV un-flattened photon beam (FFF) from Clinac-iX machine. Extensive pretreatment quality assurance measurements were carried out to report on quality of delivery. Point dosimetry was performed using three different detectors, which includes CC13 ion-chamber, Exradin A14 ion-chamber and Exradin W1 plastic scintillator detector (PSD) which have measuring volume of $0.13cm^3$, $0.009cm^3$ and $0.002cm^3$ respectively. Results: Average PTV volume of AS was 11.3cc (${\pm}4.8$), and located in eloquent areas. VMAT plans provided complete PTV coverage with average conformity index of 1.06 (${\pm}0.05$). OAR's dose were kept below tolerance limit recommend by American Association of Physicist in Medicine task group-101(brainstem $V_{0.5cc}$ < 23Gy, cochlea maximum < 25Gy and Optic pathway <25Gy). PSD resulted in superior dosimetric accuracy compared with other two detectors (p=0.021 for PSD.

유방암 방사선치료에서 변형영상정합기법을 이용한 선량비교 (Dose Comparison Using Deformed Image Registration Method on Breast Cancer Radiotherapy)

  • 본 연구는 거대유방암 환자대상으로 환자의 움직임과 모양의 변화에 따른 선량변화를 콘빔단층촬영영상(CBCT)과 변형영상정합기법(DIR)을 적용하여 적응형방사선치료계획을 재구성하여 기존에 사용된 쐐기접선조사기법(TWF)과 종속조사면 병합치료방법(FIF) 그리고 세기조절방사선치료계획(IMRT)을 이용하여 선량비교평가를 시행하고자 하였다. CT와 CBCT를 MIM6 사용하여 변형영상정합기법영상(DIRCT)을 만들어 각각의 치료계획을 시행하였다. 계획표적체적(PTV)은 왼쪽 유방이고 균질성지표(HI), 적합성지표(CI), 조사범위지표(CVI)의 값을 확인하였다. 관심장기(OAR)는 반대쪽 유방, 폐 그리고 심장이며 평균선량, 최대선량과 장기 특성에 따른 기타 지표들을 비교 분석하였다. PTV인 유방의 HI 값은 DIRCT를 이용한 모든 치료계획방법에서 증가하였으며, CVI, CI의 결과에서는 DIRCT를 이용한 치료계획방법에서 모두 감소하였다. 폐에서의 평균선량과 최대선량은 DIRCT 이용한 IMRT 치료계획방법이 다른 치료계획보다 증가함을 보였으며, 심장의 평균선량과 최대선량은 DIRCT를 이용한 TWF와 FIF는 감소했으며, 오히려 IMRT의 결과는 증가하였다. 유방암 방사선치료 시 환자의 움직임과 셋업 오차로 인해 유방모양의 변경을 감암했을 때 TWF와 IMRT보다 FIF 방법을 이용했을 때 반대편 유방과 심장, 폐의 피해선량을 효과적으로 감소시킬 수 있을 것으로 사료된다.

Feasibility Study of Deep Inspiration Breath-Hold Based Volumetric Modulated Arc Therapy for Locally Advanced Left Sided Breast Cancer Patients

  • Background: The purpose of this study was to assess the feasibility of deep inspiration breath-hold (DIBH) based volumetric modulated arc therapy (VMAT) for locally advanced left sided breast cancer patients undergoing radical mastectomy. DIBH immobilizes the tumor bed providing dosimetric benefits over free breathing (FB). Materials and Methods: Ten left sided post mastectomy patients were immobilized in a supine position with both the arms lifted above the head on a hemi-body vaclock. Two thermoplastic masks were prepared for each patient, one for normal free breathing and a second made with breath-hold to maintain reproducibility. DIBH CT scans were performed in the prospective mode of the Varian real time position management (RPM) system. The planning target volume (PTV) included the left chest wall and supraclavicular nodes and PTV prescription dose was 5000cGy in 25 fractions. DIBH-3DCRT planning was performed with the single iso-centre technique using a 6MV photon beam and the field-in-field technique. VMAT plans for FB and DIBH contained two partial arcs ($179^{\circ}-300^{\circ}CCW/CW$). Dose volume histograms of PTV and OAR's were analyzed for DIBH-VMAT, FB-VMAT and DIBH-3DCRT. In DIBH mode daily orthogonal ($0^{\circ}$ and $90^{\circ}$) KV images were taken to determine the setup variability and weekly twice CBCT to verify gating threshold level reproducibility. Results: DIBH-VMAT reduced the lung and heart dose compared to FB-VMAT, while maintaining similar PTV coverage. The mean heart $V_{30Gy}$ was $2.3%{\pm}2.7$, $5.1%{\pm}3.2$ and $3.3%{\pm}7.2$ and for left lung $V_{20Gy}$ was $18.57%{\pm}2.9$, $21.7%{\pm}3.9$ and $23.5%{\pm}5.1$ for DIBH-VMAT, FB-VMAT and DIBH-3DCRT respectively. Conclusions: DIBH-VMAT significantly reduced the heart and lung dose for left side chest wall patients compared to FB-VMAT. PTV conformity index, homogeneity index, ipsilateral lung dose and heart dose were better for DIBH-VMAT compared to DIBH-3DCRT. However, contralateral lung and breast volumes exposed to low doses were increased with DIBH-VMAT.

Intensity Modulated Radiation Therapy of Brain Tumor

  • As intensity modulated radiation therapy compared with conventional radiation therapy, tumor target dose increased and normal tissues and critical organs dose reduced. In brain tumor, treatment planning of intensity modulated radiation therapy was practiced in 4MV, 6MV, 15MV X-ray energy. In these X-ray energy, was considered the dose distribution and dose volume histogram. As 4MV X-ray compared with 6MV and 15MV, maximum dose of right optic-nerve increased 10.1 %, 8.4%. Right eye increased 5.2%, 2.7%. And left optic-nerve, left eye, optic chiasm and brainstem incrased 1.7% - 5.2%. Even though maximum dose of PTV and these critical organs show different from 1.7% - 10.1% according to X-ray energies, these are a piont dose. Therefore in brain tumor, treatment planning of intensity modulated radiation therapy in 9 treatment field showed no relation with energy dependency.
