Effects of Chromium Picolinate Supplementation on Growth Hormone Secretion and Pituitary mRNA Expression in Finishing Pigs

  • The purpose of the present study was to investigate the effects of chromium picolinate (CrPic) on growth hormone (GH) secretion and pituitary GH mRNA expression in finishing pigs. Forty eight crossbred pigs with an initial body weight of 65.57 kg (SD = 1.05) were blocked by body weight and randomly assigned to two treatments with three replicates. Each group was fed the diet supplemented with or without $200{\mu}g/kg$ chromium from CrPic for 40 days. The results showed that average daily gain of pigs was increased by 9.84% (p<0.05), and longissimus muscle area was increased by 17.29% (p<0.05) with the supplementation of CrPic. The results of GH dynamic secretion showed that supplemental CrPic increased the mean level and peak value of GH by 36.58% (p<0.05) and 26.60% (p<0.05), respectively, while there was no significant effect on basal value, peak amplitude and peak duration. Pituitary mRNA expression of GH was not significantly influenced by supplemental CrPic. These results indicated that CrPic increased pigs GH secretion without change of pituitary GH mRNA expression.

Molecular cloning, tissue distribution and quantitative analysis of two proopiomelanocortin mRNAs in Japanese flounder (Paralichthys olivaceus)

  • Proopiomelanocortin (POMC) plays an essential role in the stress response of the hypothalamic-pituitary-adrenal axis, and is the precursor of biologically active peptides such as adrenocorticotropin (ACTH), $\alpha$-melanocyte-stimulating hormone ($\alpha$-MSH), $\beta$-melanocyte-stimulation hormone ($\beta$-MSH) and $\beta$-endorphin. We have synthesized two different forms of POMC cDNA clones, POMC-I and POMC-II, from a pituitary cDNA library for Paralichthys olivaceus, or Japanese flounder. jfPOMC-I cDNA consists of 954bp and encodes a polypeptide of 216 amino acid residues, whereas jfPOMC-II consists of 971bp which encode a polypeptide of 194 amino acid residues. The high levels of jfPOMC-I and -II mRNAs detected in the pituitary tissue and moderate levels detected in the brain tissue plus our quantitative RT-PCR analysis, which showed there to be no significant difference between the levels of jfPOMC-I and -II mRNAs, indicate that there may be no functional separation between these two mRNAs in the flounder.

  • A sixty-three year old woman presented with sudden onset of dystonic twitching of the extremities and repeatedly downgrading of consciousness in the midnight. She had no history of medical illness in the past but had been suffered from fatigue recently before the onset of the attack. At admission she was neurologically free including neuro-ophthalmologic examination. MRI showed a high signal T1WI mass in the sella protruding into the suprasellar cistern. Hormonal study revealed low value of T3, T4, LH and FSH. Under the impression of pituitary apoplexy she underwent operation via transcranial route. When the protruding diaphragma sellae was opened, milkish pus spilled out. The thick capsule was biopsied and pus was sent for the study. The results of Gram staining and culture were both negative but pathologic report of the specimen was high infiltration of lymphocytes with few polymorphs.

Neurocysticercosis Involving the Pituitary Stalk : Case Report and Literature Review

  • Neurocysticercosis (NCC) is the most common parasitic infestation of the central nervous system. Most cases of NCC are to related and/or associated with inflammation within the cerebral parenchyma. A 71-year-old woman presented with a 4-year history of visual disturbance. This symptom had become aggravated 4 weeks earlier. Her visual acuity gradually decreased and superior hemianopsia was noted. Magnetic resonance imaging (MRI) revealed an enhanced and thickened pituitary stalk accompanying a suspicious mass. The provisional diagnoses were lymphoma, glioma, or other inflammatory conditions. Laboratory studies, including blood and hormonal studies, showed normal findings. Surgical resection was performed. In the pathological examination, degenerated parasitic wall structure was seen and its contents were composed of completely degenerated focal globular structures suggesting the scolex of cysticercus. We report an unusual case of NCC involving the pituitary stalk which was presented with a juxtasellar tumor. The possible underlying mechanisms are discussed with a review of pertinent literature.

Molecular Cloning of Estrogen Receptor $\alpha$ in the Masu Salmon, Oncorhynchus masou

  • A cDNA encoding the masu salmon, Oncorhynchus masou, estrogen receptor $\alpha$ (msER$\alpha$) was cloned from the pituitary gland by polymerase chain reaction (PCR). This cDNA contains an open reading frame encoding 513 amino acid residues, and the calculated molecular weight of this protein is about 56,430 Dalton. The amino acid sequences of the DNA binding and ligand binding domains of msER$\alpha$ showed high homology to those of other fish species (84-100%). Reverse transcription PCR analysis showed that the mRNA level of msER$\alpha$ in the pituitary was slightly higher in estradiol-17$\beta$(E2) injected masu salmon than that of control fish. To test the biological activity of msER$\alpha$, the cDNA was ligated to a mammalian expression vector and transfected into a gonadotrope-derived cell line, L$\beta$T2, with a reporter plasmid including estrogen responsive element. Expression of the reporter protein, luciferase, was E2 and msER$\alpha$-dependent. The masu salmon ER$\alpha$ is structurally conserved among teleost species and functions as a transcriptional activator in the pituitary cells.

  • Therapeutic management of brain tumors is based on accurate knowledge of their size, location and histologic type. Stereotaxic cytology under CT guidance has been applied to the investigation of brain tumors, especially in the sellar turcica, third ventricle, and pineal regions. In the present case, the tumor protruded into the nasal cavity, so we were able to get cytologic material via fine needle aspiration. Although pituitary adenoma is not an unsual tumor, there have been few reports about its cytologic features. We present the cyotologic and histologic features of a case of pituitary adenoma with immunohistochemical study.

Treatment of Cushing's disease with macroadenoma through transsphenoidal surgery (나비굴경유 수술을 통해 완치된 뇌하수체 거대선종에 동반된 쿠싱병 1예)

  • Cushing's disease (CD) is a rare illness characterized by chronic hypercortisolism secondary to the overproduction of adrenocorticotropic hormone by a pituitary adenoma, which is associated with a high risk of developing serious complications, such as diabetes mellitus, cardiovascular disease, and emotional disorders. Endoscopic transsphenoidal surgery is performed for the treatment of CD, and was initially preferred over other types of treatments. However, the recurrence after pituitary surgery for CD is a common problem after an initial successful surgery. In microadenomas, the remission rates were higher than those of macroadenoma. This patient had a giant tumor that was greater than 4 cm in length on sella magnetic resonance imaging, and panhypopituitarism was detected using a combined pituitary stimulation test. After transsphenoidal surgery, the patient required temporary hormone replacement for a short period of time. After 1 year, he showed a normal cortisol response on the overnight dexamethasone suppression test and low morning cortisol levels. Therefore, we indicated that the patient was cured of giant macroadenoma with panhypopituitarism before surgery, and thus, reported this case.

A Clinical Study on 2 Cases of Secondary Amenorrhea diagnosed as hypothalamic-pituitary failure (시상하부-뇌하수체 기능 부전으로 진단받은 속발성(續發性) 무월경(無月經) 환자 2례에 대한 임상적 고찰)

  • Purpose: To report the effect of oriental medicine on 2 patients with secondary amenorrhea diagnosed as hypothalamic-pituitary failure. Methods: The patients who had secondary amenorrhea were treated with the oriental medical treatments such as herbal medication, herbal acupuncture and moxibustion therapy, and so forth. Results: After the oriental medical treatments, the patients could have their periods again, their menstrual condition was improved. Conclusion: This case report shows that the oriental medical therapy is effective for treating secondary amenorrhea diagnosed as hypothalamic-pituitary failure.

The Surgical Result of Pituitary Adenoma by Transsphenoidal Approach (경접형골동으로 수술한 뇌하수체 선종의 치료성적)

  • Objective : Transsphenoidal approach(TSA) has been used as useful operative method for pituitary tumor but is still controversal in case of cavernous sinus invasion or severe suprasellar extension. This study was performed to evaluate the surgical result, recurrence, effect of adjuvant treatment, especially in cases of suprasellar extension or cavernous sinus invasion. Material and Methods : We studied 56 cases of surgically treated pituitary adenoma that we were able to follow up, treated by TSA from 1993 to 1998. There were 24 cases of suprasellar extension and 11 cases of cavernous sinus invasion. The medical records and radiological findings were reviewed. Surgical results including hormonal function and recurrence rates were analyzed according to extent of tumor invasion. Mean follow-up period was 19.1 months. Results : Tumors with suprasellar extension were removed totally in 54%, whereas total tumor removal was possible only in 38% with cavernous sinus invasion. Overall of recurrence rate was 14% and recurrence rate was 25% in suprasellar extension and 9% in cavernous sinus invasion. In cases of both suprasellar extension and cavernous sinus invasion, tumors that were treated by TSA and radiation showed recurrence rate of 7%, whereas those treated by surgery alone showed 28% of recurrence. Conclusion : Transsphenoidal approach is safe and useful operative method for pituitary adenoma and adjuvant therapy including radiation therapy is effective means to decrease the recurrence in cases of suprasellar extension or cavernous sinus invasion.

Outcome of Endoscopic Transsphenoidal Surgery in Combination with Somatostatin Analogues in Patients with Growth Hormone Producing Pituitary Adenoma

  • Objective : To determine the efficacy of endoscopic surgery in combination with long-acting somatostatin analogues (SSAs) in treating patients with growth hormone (GH)-secreting pituitary tumor. Methods : We performed retrospective analysis of 133 patients with GH producing pituitary adenoma who underwent pure endoscopic transsphenoidal surgery in our center from January 2007 to July 2012. Patients were followed up for a range of 3-48 months. The radiological remission, biochemical remission and complication were evaluated. Results : A total of 110 (82.7%) patients achieved radiological complete resection, 11 (8.2%) subtotal resection, and 12 (9.0%) partial resection. Eighty-eight (66.2%) patients showed nadir GH level less than 1 ng/mL after oral glucose administration. No mortality or severe disability was observed during follow up. Preoperative long-acting SSA successfully improved left ventricle ejection fraction (LVEF) and blood glucose in three patients who subsequently underwent success operation. Long-acting SSA (20 mg every 30 days) achieved biochemical remission in 19 out 23 (82.6%) patients who showed persistent high GH level after surgery. Conclusion : Endoscopic transsphenoidal surgery can biochemically cure the majority of GH producing pituitary adenoma. Post-operative use of SSA can improve biochemical remission.
