Effects of an Individualized Educational Program on Knowledge, Compliance and Physiologic Parameters in Non-Compliant Hemodialysis Patients (개별교육프로그램이 역할불이행 혈액투석환자의 투석관련 지식, 이행 및 생리적 지표에 미치는 효과)

  • Purpose: This study was done to provide effective nursing interventions using an individualized educational program designed to contribute to the knowledge, compliance and physiologic parameters (serum potassium, phosphorus and interdialytic weight gain) of non-compliant hemodialysis patients. Methods: There were 22 participants in the experimental group and 19 in the control group. Nurses with rapport with patients provided education to non-compliant hemodialysis patients for 20 minutes three times a week for six weeks. This education program consisted of individual consulting, telephones conversations regarding hemodialysis, and requests for patient support from patients' families and colleagues. Results: Hypothesis 1; "Knowledge about hemodialysis in the experimental group will be higher than the control. group" was not supported. Hypothesis 2; "Compliance will be higher in the experimental group than in the control group", and Hypothesis 3; "Physiologic parameters of experimental group will improve" were supported as serum potassium and interdialytic weight gain decreased, but results were not consistent for phosphorous. Conclusion: This program for non-compliant patients increased compliance and improved physiologic parameters. Therefore, this educational program should be effective as a nursing intervention.

Physiological Changes in Jeju Crossbred Riding Horses by Swim Training

  • The changes in physiologic parameters by swim exercise duration were examined in five female well-trained Jeju crossbred riding horses that had riding experience of more than three years without swim training experience. The horses were performed with swim exercise for 10 min (60.0 m/min) once a day for 14 days. Physiologic characteristics and haematic parameters were measured before swimming, immediately after swimming, and after a 10 min rest at first day ($D_0$), 7 days ($D_7$), and 14 days ($D_{14}$) of training. After 14 days of swim training, heart rate (p<0.05), blood glucose (p<0.05), lactate concentration (p<0.001), packed cell volume (p<0.01), and hemoglobin (p<0.01) measured immediately after swim and after 10 min rest showed significant lower values than those of $D_0$. The results illustrate the benefits of swim training for riding horses and the need for the establishment of swimming routines of appropriate duration and intensity to maximize the advantages of swim training.

Effects of Foot Reflexology Massage on Climacteric Symptom, Fatigue and Physiologic Parameters of Middle Aged Women (발반사마사지가 중년여성의 갱년기 증상, 피로 및 생리지수에 미치는 효과)

  • Purpose; This study was aimed to identify the effect of foot reflexology massage on climacteric symptom, fatigue and physiologic parameters of middle-aged women. Method; A non-equivalent pretest-posttest experimental design was used. Participants were recruited from the Community Health Center in Busan, Korea. Forty participants were assigned to either an experimental group(20) or a control group(20). Foot reflexology massage was administered twice a week for 6 weeks in the participant in experimental group. Results: There were statistically significant differences in climacteric symptom, fatigue, total cholesterol and cortisol level. However, there were no statistically significant differences in triglyceride, high density lipoprotein and low density lipoprotein. Conclusion; These results suggest that foot reflexology massage could be utilized as an effective nursing intervention to reduce climacteric symptom and fatigue in middle-aged women.

  • Background : The mortality from acute respiratory distress syndrome(ARDS) is >40-50%. Although some prospective trials have failed to demonstrate a survival benefit of steroids in the early stages of ARDS, there are some reports showing some success with steroids in the later stages. This study observed the changes in the physiologic parameters with time in late ARDS patients who were treated with steroids. Methods : The medical charts of 28 intensive care unit patients(male:female=24:4; mean age 64 years), who had been diagnosed with refractory late ARDS ($PaO_2/FIO_2$ <200) and were treated with corticosteroids from December 1999 to July 2002, were retrospectively reviewed. The patients were divided into two groups: the weaned group(n=14), which included the patients who had been successfully weaned from a ventilator after corticosteroid therapy, and the failed group(n=14), which included the patients who had failed weaning. The physiologic parameters included the $PaO_2/FIO_2$ ratio, the positive end-expiratory pressure(PEEP) level, the $PaCO_2$, compliance, the sequential organ failure assessment(SOFA) score, the acute physiologic and the chronic health evaluation(APACHE) II score, and the Murray Lung Injury Score(LIS) in the two groups were compared from the day of mechanical ventilation(Dmv) to 7 days after the corticosteroid therapy. Results : There was no significant difference in the clinical characteristics and the physiologic parameters between the two groups prior to the corticosteroid therapy except for the SOFA score at Dmv(weaned group : $6.6{\pm}2.5$ vs failed group : $8.8{\pm}2.9$, p=0.047). However, within 7 days after corticosteroid therapy, there was significant improvement in the $PaO_2/FIO_2$ ratio, the PEEP level, the $PaCO_2$, the SOFA score, the APACHE II score, and the LIS of the weaned group compared to the failed group. Conclusions : During corticosteroid therapy in late ARDS, the continuation of corticosteroid therapy should be determined carefully in patients who do not show improvement in their physiologic parameters by day 7.

Study for the Liquid Metals Enabled Stretchable Electronics (액체금속을 활용한 신축성 전자소재 개발 동향)

  • Stretchable and flexible electronics that comply with dynamic movements and micromotion of the human tissues can enable real-time monitoring of physiologic signals onto the human skin and in the brain, respectively. Especially, gallium based liquid metal stretchable electronics can offer human-interactive biosensors to monitor various physiologic parameters. However, the liquid-like nature, surface oxidation and contamination by organic materials, and low biostability of the liquid metals have still limited the long-term use as bioelectronics. Here we introduced electrochemical deposition without oxidation pathways to overcome these practical challenges in liquid metal bioelectronics. CNT/PDDA composite with reduction way and PEDOT:BF4 with oxidation way under organic solvent are suggested as rationally designed material engineering approaches. We confirmed that the structures with the soft, flexible, and stretchable liquid metal platform can successfully detect dopamine with a high sensitivity and selectivity, record neural signals including action potentials without scar formation, and monitor physiologic signals such as EMG and ECG.

Effects of Individualized Education on Knowledge, Compliance, and Physiologic Parameters in Hemodialysis Patients (개별교육이 혈액투석환자의 투석관련 지식, 환자역할행위 이행 및 생리적 지수에 미치는 효과)

  • Purpose: This study was designed to explore the effects of providing individualized education for hemodialysis patients on their knowledge of hemodialysis, compliance of patient role behavior, and physiologic parameters. Method: A quasi experimental design with a non-equivalent control group and a non-synchronized design was used. The experiment was conducted with a total of 40 hemodialysis patients (20 in the experimental group and 20 in the control group) at the artificial kidney center, C University Hospital. The experimental group was provided with individualized education, 30 minutes per session, three times per week, for two weeks. Results: The experimental group had significantly higher knowledge of hemodialysis than the control group after the education. The compliance of patient role behavior was more enhanced in the experimental group than the control group. The experimental group showed significantly higher values of blood urea nitrogen, but not the values of blood creatinine, albumin, kalium, and phosphorus. Conclusion: The individualized education was found to be an effective intervention for improving patients' knowledge of hemodialysis and compliance of patient role behavior.

Effects of Empowerment Education Program for Hemodialysis Patients on Self-efficacy, Self-care Agency, Self-care Activities and Physiologic Parameters (능력증강 교육프로그램이 혈액투석환자의 자기효능감, 자가간호역량, 자가간호행위 및 생리적 지표에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of empowerment education program on self-efficacy, self-care agency, self-care activities and physiologic parameters among hemodialysis patients. Method: Nonequivalent control group pretest-posttest design was used. There were 36 subjects in the experimental group and 35 subjects in the control group. The empowerment education program was administered for 4 weeks (1 time per week). Data were collected pre-treatment and post treatment. The $X^2$-test or Fisher's exact test, ANCOVA, t-test and paired t-test were performed to determine the group differences, using SPSS 14.0 version. Results: Self-efficacy, self-care agency, self-care activities and weight variation significantly improved after the empowerment education program in hemodialysis patients. Conclusion: The empowerment education program for hemodialysis patients was effective to improve the degree of self-efficacy, self-care activities and self-care agency and to decrease interdialytic weight variation.

Hemodynamically Isolated Intracranial Aneurysms on DSA and CEMRA: Clinical and Experimental Data

  • Purpose: To evaluate the physiologic background of poorly visualized aneurysms during DSA a contrast-enhanced MRA(CEMRA) due to hemodynamical isolation on clinical and experiment data. Method: Two cases of intracranial aneurysm which were poorly visualized on DSA a CEMRA and one case of intracranial aneurysm which had poor turnover of contrast mediu during DSA were selected for this clinical study. We evaluated the turnover of blood in t terminal aneurysm of handmade elastic silicon phantoms for comparative experiment. Flo experiments with DSA and contrast enhanced MRA were performed in elastic phantoms aneurysm with 3 different diameters (2, 5 and 10 mm) of neck mimicking basilar ti aneurysm, attached to pulsatile pump similar to that of human physiologic parameters. W compared the results with those of computational flow dynamics(CFD).

  • Mechanical factors in the human body are considered to play a dominant role in low back problems. Various spinal structures. including muscles, act in unison to resist the external load. An estimation of the muscle forces in this structure requires a knowledge of the orientation, location and area of cross-section of the muscles to complete the formulation of a truly three-dimensional mathematical model of the spine. The geometric parameters which are calculated were the line of action, the centroid and physiologic area of cross-section of each muscle as a function of the spinal level. This geometric data were obtained from CT scans of 11 subjects participating in this study.
