Objectives : This paper serves to explore current trends of systems biology in Traditional Chinese Medicine (TCM) and examine how it may influence the Traditional Korean medicine. Methods : Literature review method was collectively used to classify Introduction to systems biology, diagnosis and syndrome classification of systems biology in TCM perspective, physiotherapy including acupuncture, herbs and formula functions, TCM systems biology, and directions of academic development. Results : The term 'Systems biology' is coined as a combination of systems science and biology. It is a field of study that tries to understand living organism by establishing a theory based on an ideal model that analyzes and predicts the desired output with understanding of interrelationships and dynamics between variables. Systems biology has an integrated and multi-dimensional nature that observes the interaction among the elements constructing the network. The current state of systems biology in TCM is categorized into 4 parts: diagnosis and syndrome, physical therapy, herbs and formulas and academic development of TCM systems biology and its technology. Diagnosis and syndrome field is focusing on developing TCM into personalized medicine by clarifying Kidney yin deficiency patterns and metabolic differences among five patterns of diabetes and analyzing plasma metabolism and biomarkers of coronary heart disease patients. In the field of physical therapy such as acupuncture and moxibustion, researchers discovered the effect of stimulating acupoint ST40 on gene expression and the effects of acupuncture on treating functional dyspepsia and acute ischemic stroke. Herbs and formulas were analyzed with TCM network pharmacology. The therapeutic mechanisms of Si Wu Tang and its series formulas are explained by identifying potential active substances, targets and mechanism of action, including metabolic pathways of amino acid and fatty acid. For the academic development of TCM systems biology and its technology, it is necessary to integrate massive database, integrate pharmacokinetics and pharmacodynamics, as well as systems biology. It is also essential to establish a platform to maximize herbal treatment through accumulation of research data and diseases-specific, or drug-specific network combined with clinical experiences, and identify functions and roles of molecules in herbs and conduct animal-based studies within TCM frame. So far, few literature reviews exist for systems biology in traditional Korean medicine and they merely re-examine known efficacies of simple substances, herbs and formulas. For the future, it is necessary to identify specific mechanisms of working agents and targets to maximize the effects of traditional medicine modalities. Conclusions : Systems biology is widely accepted and studied in TCM and already advanced into a field known as 'TCM systems biology', which calls for the study of incorporating TCM and systems biology. It is time for traditional Korean medicine to acknowledge the importance of systems biology and present scientific basis of traditional medicine and establish the principles of diagnosis, prevention and treatment of diseases. By doing so, traditional Korean medicine would be innovated and further developed into a personalized medicine.
Purpose: The aim of this study was to identify the experience of patients with liver cirrhosis. Method: This study was performed from march 2003 to June 2003. The participators were five men. Data collected through in-depth personal interviews, which were recorded and analysed according to the Colazzi's method. Result: Liver cirrhosis was classified into 20 themes, 70 formulated meaning, and 10 categories. The Result confirmed that the experience of liver cirrhosis patients were classified into 10 categories; unexpected change, limited daily living and role, difficult compliance with therapeutic regimen, unbelief so untreated with apathy, negative emotional change, lack of cause perception, self control, perceived family support, expectations for recovery and healing. Conclusion: We, cooperative researchers, realized that to reduce not only the days of hospital treatment and the economic loss, as well as the expenditure of insurance the importance of managing liver cirrhosis from early diagnosis and the physical, spiritual, social role in studying the patients who experience liver cirrhosis.
Objectives The study aimed to evaluate the therapeutic effect of Hyungtonghaeul-tang along with Korean medicine treatment for fibromyalgia syndrome. Methods A patient with fibromyalgia syndrome was treated with Hyungtonghaeul-tang as well as Korean medical treatment including acupuncture, electro-acupuncture, bee venom therapy, chuna, aromatherapy, and physical therapy. The counts of tenderness points, widespread pain index (WPI), symptom severity scale score (SS scale score), digital infrared thermal imaging (DITI), Ryodoraku diagnosis, visual analogue scale (VAS), fibromyalgia impact questionnaire (FIQ), and beck depression inventory (BDI) were measured. Results After treatment, the counts of tenderness point, WPI, SS scale score, VAS, FIQ, and BDI were decreased. The DITI and the Ryodoraku pattern recovered normally. Conclusions Hyungtonghaeul-tang along with Korean medicine treatment can be used in the treatment of fibromyalgia syndrome.
This study was designed to obtain basic data for development of evaluation tool which would be needed to measure the outcome of general quality nursing care of individual patient. The purpose of this study was to analyze and classify the outcome indicators of quality nursing care. The 29 articles of quality nursing care and outcome measures were selected coveniently, and analyzed to classify the outcome indicators of quality nursing care using open coding method. The results of this study were as follows: 1. Quality nursing care was defined as level of excellence of nursing care to achieve good patient outcome. 2. The 6 domains of which were health status, satisfaction, self care, patient progress and prognosis, and compliance were identified in outcome indicators of quality nursing care 3. Seven indicators of health status domain which were perceived health status, quality of life, well-being, daily activities, physical-physiological status, psychoemotional status, and social role functioning were identified. 4. Two indicators of satifaction domain which were patient satisfaction and family satisfaction were identified. 5. Three indicators of self care domain which were skill, knowledge, and home management were identified. 6. Seven indicators of patient progress and prognosis domain which were change of clinical status, resolution of nursing diagnosis and problem, days of stay, dicahrge state, recovery state, survival were identified. 7. compliance with therapeutic direction compliance was identified as an indicator of compliance domain. 8. It was sugested that studies for development of evaluation tools for outcomes of quality nursing the results of this study could be executed
The analysis on the therapic music of chinese five-sounds studied in order to the stand ardization of the five-sounds. The results were as follows; The central sound of Gung(宮) becomes Do. This music goes on slow speed from first to last and appears the bright, quiet and stable mood. The central sound of Sang(商) becomes Re. This music mainly uses the brass and the fist appears the heavy feeling but gradually becomes ascending mood. The central sound of Kang(角) becomes Mi. This music mainly uses the wood-wind imstrument and appears the most light and vivid feeling. The central sound of Chi(徵) becomes Sol. All instruments sound grandly in the first part, and subsequently sound gentle. The central sound of Woo(羽) becomes La. This music appears dark and tragic mood that is generally used in the minor key. It is played slowly on the whole and appears lyrical feeling in the process of controlling and quiet. In the future, the study on diagnosis and therapy through the standardization of the five-sounds will be continued.
Sepsis is conceptually defined as life-threatening organ dysfunction that is caused by a dysregulated host response to infection. Although there has been significant advancement in recent decades in defining and understanding sepsis pathology, clinical management of sepsis is challenging due to difficulties in diagnosis, a lack of reliable prognostic biomarkers, and treatment options that are largely limited to antibiotic therapy and fundamental supportive measures. The lack of reliable diagnostic and prognostic tests makes it difficult to triage patients who are in need of more urgent care. Furthermore, while the acute inpatient treatment of sepsis warrants ongoing attention and investigation, efforts must also be directed toward longer term survival and outcomes. Sepsis survivors experience incomplete recovery, with long-term health impairments that may require both cognitive and physical treatment and rehabilitation. This review summarizes recent advances in sepsis prognosis research and discusses progress made in elucidating the underlying causes of prolonged health deficits experienced by patients surviving the early phases of sepsis.
Objectives The aim of this study is to analyze the therapeutic direction and characteristics of gastrointestinal diseases by three societies of Korean medicine by comparing the frequent disease patterns and prescriptions of them. Methods The experts of the three societies of Sasang constitutional medicine, Hyungsang medicine and Sanghan-GeumGwe medicine discussed and confirmed a list of clinical data to make use for diagnosis. Then, clinical data on 200 patients suffering from gastrointestinal symptoms were collected. The each experts diagnosed appropriate disease patterns and decided prescriptions according to the method of their affiliated society. Results and Conclusions The three different societies' experts showed the similarity in the sense of categorizing disease patterns focusing on the specific physiology and pathology of the patients, while the each societies have different diagnosis systems. Treating chronic gastrointestinal patients, they treated the mental or psychological symptoms of the patients as well as their physical problems. Based on this study, it is necessary to develop a systematic and precise research model to objectively and statistically analyze the correlation of disease patterns and prescriptions by different societies.
Purpose: This systematic review aims to determine whether robot-assisted training is more effective in gait training for persons with subacute hemiparetic stroke. Methods: This study adopted a systematic review study design focused on subacute hemiparetic stroke, and four core academic databases were searched until June 11, 2021, for relevant studies, including PubMed, Embase, the Cochrane Library, and ProQuest Central. The review included randomized controlled trials (RCTs) evaluating the effects of robotic-assisted training on gait performance in persons with a diagnosis of subacute hemiparetic stroke. The selected RCT studies were qualitatively synthesized based on the population, intervention, comparison, outcome, settings, and study design (PICOS-SD). Results: The study selected five RCTs involving 253 subacute hemiparetic stroke patients and performing robotic-assisted gait training using the following devices: the Lokomat, Morning Walk, Walkbot, ProStep Plus, or Gait Trainer II. Five RCTs were eligible for the meta-analysis after quantitative synthesis, and the results showed that the robot-assisted gait training group had a greater gait performance than the control group based on the 10-meter walk test, Berg balance scale, Rivermed mobility index, functional ambulation category, and modified Barthel index. Conclusion: The results of this study showed that the gait performance of subacute hemiparetic stroke patients changes throughout robot-assisted gait training, but there were no indications that any of the clinically relevant effects of robot-assisted training are greater than those of conventional gait training. Further, the small sample size and different therapeutic intensities indicate that definitive conclusions could not be made.
We reviewed 30 cases of superior vena cava syndrome in adult patients who were seen at the Yeungnam University Hospital from January 1985 to June 1990. The results were as follows : 1. The male-to-female ratio was 6.5:1, and the most patients were in the age group between the sixth and seventh decades. 2. The most common symptoms were dyspnea (87%) and followed by cough (63%), facial swelling (63%) and chest pain (44%) and the physical signs were dilated neck vein (97%), facial edema (93%) and facial flushing (45%) in order of frequency. 3. The simple chest x-ray findings were superior mediastinal widening (90%), right hilar mass (77%) and pleural effusion (31%). 4. Diagnosis was made by history and physical examination (100%), chest C-T scan (100%), simple chest x-ray (97%), bronchoscopy with biopsy (40%) and so on. 5. 21 cases of patients were confirmed by histology : 14 cases (46%) of bronchogenic ca. 4 cases (14%) of lymphoma, 3 cases (10%) of metastic lung ca. Of bronchogenic ca. small cell ca was 7 cases (23%), squamous cell ca, 5 cases (17%), and unclassified ca was 2 cases (6%). 6. In response of treatment, the clinical improvement was achieved in 18 cases with radiotherapy alone. 1 case with chemotherapy only, and 6 cases with radio-chemotherapy.
외상(trauma)이란 신체 외부에서 작용한 힘에 의한 여러 해부학적, 병태생리학적 변화를 수반하는 신체 손상을 의미한다. 외상 환자는 우리 사회가 고도로 발달해 감에 따라 그 수도 따라 증가하고 있다. 치료 및 진단기술의 발전과 보급의 증가로 외상의학의 중요성은 점점 커지고 있으며 그 수요 역시 증가하고 있다. 그중 특히 흉부 및 흉부의 대혈관의 손상은 환자의 생명을 위협하고 그 후유증 역시 심각한 경우가 많아 진단 및 치료 방법의 중요성이 점차 증가하고 있는 추세이다. 흉부 및 흉부의 대혈관의 외상이 발생하였을 경우 그 사고 수상 기전 및 관련된 해부학적 손상 기전에 따라서 다양한 정도의 신체 손상이 발생한다. 주요한 손상으로는 흉부의 동맥에서 발생하는 출혈이 있으며, 이는 혈역학적 불안정과 응고장애 등을 동반하여 환자의 생명을 위협하게 된다. 이러한 손상은 즉각적인 진단과 빠른 치료적 접근이 예후를 증진하는데 도움을 주는 경우가 많다. 이러한 환자의 치료로는 환자의 상태에 따라서 수술적 접근 방법과 중재적 시술로 접근하는 방법이 있다. 이 중 중재적 시술은 그 편리성과 신속성 및 높은 치료 효과로 인해 점차 각광받고 있으며, 전 세계적으로 보다 많은 외상의료기관에서 점차 시행되고 있다. 대표적인 흉부 외상 환자의 인터벤션 치료로는 비대동맥성 손상의 경우 색전술(embolization)이 있으며, 대동맥의 경우 thoracic endovascular aortic repair (TEVAR)가 있다. 이러한 시술들은 수술적 치료 방법에 비하여 내과적 혹은 외과적 부작용이 적고, 수술적 방법보다 신속하게 시행할 수 있는 점 등 보다 많은 장점을 가지고 있으며 외상 환자의 치료성적 향상에 기여하고 있다.
