The influence of nutrition during early life on physical growth as well as mental development has been thoroughly discussed in the literature. The physical dimensions of the body are greatly influenced by nutrition, particularly during the period of rapid growth in early childhood. Nutritional status affects every pediatric patient's response toillness. Good nutrition is important for achieving normal growth and development. It is indicated that permanent impairment of the central nervous system may result from dietary restriction of imbalance during certain periods of life. If children under 3 years of age show a good nutritional status, it may be assumed that they are well nourished. Several common diseases of children such as iron deficiency, chronic constipation and atopic dermatitis are known food related diseases. Patients with chronic illness and those at risk of malnutrition should have detailed nutritional assessments done. Components of a complete nutritional assessment include a medical history, nutritional history including dietary intake, physical examination, anthropometrics (weight, length or stature, head circumference, midarm circumference, and triceps skinfold thickness), pubertal staging, skeletal maturity staging, and biochemical tests of nutritional status. The use of age, gender, and disease-specific growth charts is essential in assessing nutritional status and monitoring nutrition interventions. Nutrition assessment and dietary counseling is helpful for the cure of disease, and moreover, the prevention of illness.
PURPOSE: This study examined the effects of both lower extremities proprioceptive neuromuscular facilitation training with functional electrical stimulation on the balance and gait of stroke patients. METHODS: Ten patients with stroke were divided randomly into two groups of five patients each who met the selection criteria. The training was conducted five times a week, for 60 minutes. The experimental group received both lower extremities proprioceptive neuromuscular facilitation training with functional electrical stimulation while the control group received general physical therapy with functional electrical stimulation. RESULTS: The timed Up-and-Go (TUG) test result was statistically significant after the intervention in the experimental group (p<.041). Berg Balance Scale (BBS) assessment was statistically significant after the intervention in the experimental group (p<.047) and between the experimental and control groups (p<.012). The cadence assessment was statistically significant after intervention in the experimental group (p<.031) and between the experimental and control groups (p<.015). The stride length assessment was not statistically significant after intervention in the experimental group and between the experimental and control groups. Gait velocity assessment was statistically significant after the intervention in the experimental group (p<.031) and between the experimental and control groups (p<.015). CONCLUSION: Both lower extremities proprioceptive neuromuscular facilitation training with functional electrical stimulation had positive effects on the balance and gait of stroke patients.
Purpose: The intention of this study was to investigate the effects of modified constraint-induced movement therapy (mCIMT) with bilateral arm training (BAT) on the motor performance and daily activity performance of individuals with chronic hemiparetic stroke. Methods: Sixteen subjects one year after stroke participated in this study with a control group; the pretest-posttest method was used. The subjects were randomly allocated into two groups: combination of bilateral arm training and modified constraint-induced movement therapy (n=8), and modified constraint-induced movement therapy (n=8). The mCIMT group received therapy for 90 minutes in 3 sessions per week over a period of 4 weeks. The patients receiving a combination of mCIMT and BAT were treated for the same period and frequency. The results were evaluated using the Fugl-Meyer Assessment, Action Research Arm Test (ARAT), and Motor Activity Log-Amount of Use, and Quality of Movement (MAL-AOU, QOM) assessment tools. Results: The Fugl-Meyer Assessment showed that hand and wrist performance improved significantly more in the mCIMT group than in the Combination group (p<0.05). Result from the ARAT assessment showed greater scores for gross movement in the combined group than in the mCIMT group (p<0.05). The MAL-AOU showed that there was greater improvement in the combined group than in the mCIMT group (p<0.05). Conclusion: The forced use of the more affected side can be important for the enhancement of upper extremity performance for chronic hemiparetic stroke patients during their daily activities.
Pain is a protective mechanism for the body in that it occurs when tissues are damaged. Patients who need physical therapy intervention has various pain. Thus, pain is one of aim for physical therpay. The purposes of this review are to give information and basic data for pain control which is related physical therapy. We discuss about pain pathways, pain theory, type of pain, variance which is affected pain, assessment tools, and managing pain.
The main purposes of this study were to find the correlation between walking ability assessment tools using the Modified Barthel Index (MBI), Functional Independence Measure (FIM), Spinal Cord Injury Measurement II (SCIM II), Walking Index for Spinal Cord Injury (WISCI), walking velocity, and walking endurance. The study population consisted of 56 patients with spinal cord injury referred to the department of Rehabilitative Medicine in the National Rehabilitation Hospital. All subjects were ambulatory with or without an assistive device. All participants were assessed by MBI, FIM, SCIM II, WISCI, walking velocity, and walking endurance. The data were analyzed using Pearson correlation analysis and X2. There was significant correlation between the MBI, FIM, SCIM II, WISCI, walking velocity, and walking endurance (p<.01). In particular, WISCI has a significant correlation with SCIM II(p<.001). Therefore the WISCI scale is an appropriate assessment tool to predict the gait ability of patients with spinal cord injury. Further study about MBI, FIM, SCIM II, WISCI, walking velocity, and walking endurance is needed using a longitudinal study design.
PURPOSE: This study compared cognitive task-directed functional motor control ability for reaching and kicking movements in younger and older adults. METHODS: Subjects were divided into two groups of younger and older adults, with 13 subjects in each group. Subjects were required to perform a dual task combining a functional movement and cognitive component. The task consisted of reaching and kicking movements. Participants performed indicated movements when a target appeared on a monitor. The target randomly appeared on the monitor every 10 seconds. The total performance time (TPT), joint angular velocity (JAV), and muscle activation time were used to evaluate motor control ability. RESULTS: There were significant differences in all evaluation factors in a comparison of younger and older adults (p<.05). TPT was significantly shorter in older adults, and JAV and muscle activation time were significantly slower than that in the younger adult group. Although the results for older adults were within the normal range for functional assessment, their motor control abilities were significantly worse for cognitive tasks compared with those of younger adults. CONCLUSION: The results of this study indicated that a motor control assessment tool using a cognitive task would be helpful in assessment of motor control ability in healthy older adults.
Purpose : The purpose of this study is to assess the effect for gait, balance, and depression for stroke patients by group task-related program training based motor learning theory. Methods : The subjects of this study were administrated to the 11 stroke patients (9 male, 2 female) by 5 weeks, 3 times per week, 15 times. The group task-related program training were performed gait, balance, treadmill, muscle strengthening, and game program. Each program took 7~10 minutes and total time took 60 minutes including moving time. The difference of program training were compared using the paired t-test. Results : The results of this study revealed that Fugl-Meyer motor assessment, Chedoke-McMaster Stroke assessment of lower extremity and Berg balance scale were significantly correlated. However, impairment item of Chedoke-McMaster Stroke assessment, spatio-temporal gait parameters, Timed up and go test, and depression item of Minnesota Multiphasic Personality Inventory were not significantly correlated. Conclusion : These results support that group task-related program could be a useful treatment to improve the balance skills and motor function of lower extremity for the chronic stroke patients.
The aim of this study was performed to determine the inter-rater reliability of the Stroke Rehabilitation Assessment of Movement (STREAM) translated in Korean. This was a new clinical measurement tool for evaluating the recovery of voluntary movement and basic mobility following stroke. A direct-observation reliability study was conducted on 20 patients who had strokes and were in a rehabilitation setting. Subjects were assessed by two physical therapists. The reliability of the STREAM scores was demonstrated by weighted kappa statistics for inter-rater agreement on scores for individual items ranged from .83 to 1.0, intraclass correlation coefficients for total score was .99, and for subscale scores was ranged from .96 to .99. The internal consistency of the STREAM scores was demonstrated by Cronbach alphas of greater than .99 on the subscales and overall. These high levels of reliability support the use of the STREAM translated in Korean instrument for the measurement of motor recovery following stroke.
The purpose of this study was to find which spatiotemporal gait parameters gained from stroke patients could be predictive factors for the gait part of Tinetti Performance-Oriented Mobility Assessment (POMA-G). Two hundred forty-six stroke patients were recruited for this study. They participated in two assessments, the POMA-G and computerized spatiotemporal gait analysis. To analyze the relationship between the POMA-G and spatiotemporal parameters, we used Pearson's correlation coefficients. In addition, multiple linear regression analyses (stepwise method) were used to predict the spatiotemporal gait parameters that correlated most with the POMA-G. The results show that the gait velocity (r=.67, p<.01), cadence (r=.66, p<.01), step length of the affected side (r=.49, p<.01), step length of the non-affected side (r=.53, p<.01), swing percentage of the non-affected side (r=.47, p<.01), and single support percentage of the affected side (r=.53, p<.01) as well as the double support percentage of the non-affected side (r=-.42, p<.01) and the step-length asymmetry (r=-.64, p<.01) correlated with POMA-G. The gait velocity, step-length asymmetry, cadence, and single support percentage of the affected side explained 67%, 2%, 2%, and 1% of the variance in the POMA-G, respectively. In conclusion, gait velocity would be the most predictive factor for the POMA-G.
Background: At present time, smartphones have become very popular and powerful devices, and smartphone applications with the good validity have been designed to assess human balance ability. Objects: The purpose of this study is to evaluate the feasibility of smartphone acceleration in the assessment of postural control ability for six different conditions. Methods: Twenty healthy college-aged individuals volunteered. Static balance ability was measured twice with one-day interval using smartphone application and 3D motion capture system under the six different conditions. Results: Dominant frequencies for each test condition did not show significant differences except for two conditions. The intra-rater correlation coefficient between the first and second tests showed high correlations in six conditions(r>.70, p<.05). Smartphone acceleration and the acceleration calculated from the 3D marker position data showed high correlation coefficient(r>.80, p<.001). Conclusion: Acceleration recorded from a smartphone could be useful assessment variables for balance test in the clinical field.
