Decommissioning process of nuclear facilities consist of a sequence of problem solving activities, because there may exist not only working environments contaminated by radiological exposure but also industrial hazards such as fire, explosions, toxic materials, and electrical and physical hazards. Therefore, not a few countries in the world have been trying to develop appropriate counter techniques in order to guarantee safety and efficiency of the process. In spite of that, there still exists neither domestic nor international standard. Unfortunately, however, there are few workers who experienced decommissioning operations a lot in the past. As a solution, it is quite necessary to utilize experts' opinions for risk assessment in decommissioning process. As for an individual hazard factor, risk assessment techniques are getting known to industrial workers with advance of safety technology, but the way how to integrate those results is not yet. This paper aimed to find out an appropriate technique to integrate individual risk assessment results from the viewpoint of experts. Thus, on one hand the whole risk assessment activity for decommissioning operations was modeled as a sequence of individual risk assessment steps which can be classified into two activities, decontamination and dismantling, and on the other, a risk assessment structure was introduced. The whole model was inferred with Fuzzy theory and techniques, and a numerical example was appended for comprehension.

PURPOSE: This review sought to confirm the correlation between dual-task gait and cognitive function in cognitively impaired and healthy older adults. METHOD: We used four databases (DBs), Pubmed, Cochrane library, Kmbase, and Koreamed. Searches were carried out according to the PICOS method, P (participants) were the elderly (above 65 years) with cognitive decline, I (intervention) was walking with dual tasks, C (control group) comprised the elderly without cognitive decline, O (outcome) was the correlation between gait and cognitive function and S (study) was the cross-sectional study. For the methodological quality assessment of each study, we used the Quality Assessment Tool for Observation Cohort and Cross-Sectional Studies provided by the National Institutes of Health (NIH). RESULTS: A total of 10 articles were included in this systematic review. For the components of gait, we used pace, rhythm, and variability and we observed that mild cognitive impairment mostly causes low gait performance while performing dual tasks. Among the 10 articles, 9 articles studied pace, of which 7 showed significant results. However, 2 were not significant. Also, 1 article that studied rhythm and 3 articles that studied variability showed significant results. The methodological quality of the 10 studies was fair. CONCLUSION: Gait pace was found to have a high correlation between memory, which is a cognitive ability, and overall cognitive function. It was observed that older adults with mild cognitive impairment have reduced gait pace in single-task walking, and further decrease in dual-task gait pace shows the correlation between memory and gait pace during walking.

PURPOSE: The purpose of this study was to investigate the effects of differences in exercise time arrangement on the functional recovery of the lower limbs and balance of stroke patients. METHODS: The subjects of this study were 100 patients who were hospitalized in two hospitals located in Gyeonggi-do. Before the experiment, 60 of these patients who met the inclusion criteria and did not meet the exclusion criteria were randomly divided into an experimental group (n = 30), whose exercise time was concentrated, and a control group (n = 30), whose exercise time was diffuse. The two groups underwent six weeks of physical and occupational therapy four times a day for five days a week. One session of therapy took 30 minutes, including three sets of physical therapy and one set of occupational therapy. The rest time between the sessions was different for each group. The experimental group had five minutes of rest between each therapy session, and the control group had two hours of rest time between each session. The Fugl-Meyer assessment (FMA), an assessment of each patient's limit of stability (LOS), and a timed up and go test (TUG) were used as test tools. RESULTS: Both groups showed statistically significant increases in their FMA results, LOS measurements, and TUG results. The FMA results of the experimental group were significantly higher than those of the control group. CONCLUSION: Thus, concentrated exercise time was more effective than diffuse exercise time for the recovery of motor function.

Purpose: This study examines the effects of elastic band of resistance exercise for balance control of the elderly. Methods: Thirty of eighty participants in experiment subjects who demonstrated balance-impairment through the use of primary screening tests including the one leg standing test (OLST), functional reach test (FRT) and timed up and go (TUG) were selected as subjects. Fifteen subjects that underwent muscle-strengthening exercise using an elastic band were selected as the exercise group and fifteen subjects were selected as a control group. Subjects undertook a home-based exercise program three times per week for 9 weeks. Muscle strength, functional assessment and a balance test were quantitatively measured before and after the exercise regimen. Results: After muscle strengthening exercises, changes in maximal voluntary isometric contraction (MVIC) showed a significant increase in all of the lower extremity muscles of the exercise group subjects. There were statistically significant differences between the exercise and control groups for changes in the OLST, FRT and TUG, which are functional assessments of balance ability, and changes of the unit path length and circumference area, measurement items of quantitative analysis. In addition, from examining correlations between MVIC, balance ability, it was found that an increase of muscle strength in the hip joint group of muscles among the lower extremity muscles had greater improvement in correlation with balance ability in this elderly population. Conclusion: Resistance exercise using elastic bands had significant effects on muscle strengthening in elderly subjects, with a resultant increase of lower extremity muscle strength with significant improvement of balance ability.
Background: Assessments of Sit-to-Stand (STS) and gait functions are essential procedures in evaluating level of independence for the patients after stroke. In a previous study, we developed the software to analyze center of pressure (COP) in standing position on Wii Balance Board (WBB). Objects: This purpose of this study is to measure test-retest reliability of ground reaction forces, COP and time using WBB on STS and gait in healthy adults. Methods: Fifteen healthy participants performed three trials of STS and gait on WBB. The time (s), vertical peak (%) and COP path-length (cm) were measured on both tasks. Additionally, counter (%), different peak (%), symmetry ratio, COP x-range and COP y-range were analyzed on STS, 1st peak (%), 2nd peak (%) of weight were analyzed on gait. Intra-class correlation coefficient (ICC), standard error measurement (SEM) and smallest real difference (SRD) were analyzed for test-retest reliability. Results: ICC of all variables except COP path-length appeared to .676~.946 on STS, and to .723~.901 on gait. SEM and SRD of all variables excepting COP path-length appeared .227~8.886, .033~24.575 on STS. SEM and SRD excepting COP path-length appeared about .019~3.933, .054~11.879 on gait. Conclusion: WBB is not only cheaper than force plate, but also easier to use clinically. WBB is considered as an adequate equipment for measuring changes of weight bearing during balance, STS and gait test which are normally used for functional assessment in patients with neurological problems and elderly. The further study is needed concurrent validity on neurological patients, elderly patients using force plate and WBB.

The purpose of this study was to examine the effect of the upper limb nerve mobilization (ULNM) on functional recovery of upper extremity in hemiplegic patients following stroke. Twenty patients who had functional impairment on upper extremity were participated. Subjects were randomly divided into two groups: Control group (n=10) received traditional physical therapy only for 4 weeks; Experimental group (n=10) received ULNM treatment along with traditional physical therapy for the same period. Upper extremity functions were assessed by manual muscle test (MMT), modified Ashworth scale (MAS), and Fugl-Meyer assessment (FMA) before and after the treatment. In both experimental and control group, upper extremity functions were significantly improved in MMT (p<.01) and FMA (p<.01), however only experimental group showed significant improvement in FMA after the treatment (p<.05). Moreover, experimental group showed significantly greater improvement than control group in MMT (p<.05), MAS (p<.05), and FMA (p<.05). We conclude that the upper extremity functions is a useful additional therapeutic technique for the effective treatment of upper extremity deficits in hemiplegic patients.

Background: The effect of mobilization on lumbar back pain has been fully described in several clinical aspects, but evidence for muscle strength would be still less clear. Objective: To assess the effect of lumbar mobilization on lower limb strength in healthy individuals. Methods and Analysis: Healthy people aged 18-65 will be included regardless of race or sex. Original peer-reviewed primary reporting randomized controlled trials (RCTs) will be included. Electronic databases, such as MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science, Pedro, CINAHL, ClinicalTrials.gov will be searched from inception until July 30. Only studies published in English will be included in this review. Two reviewers will complete the screening for eligibility independently, and the other two reviewers will also complete the risks of data extraction and bias assessment independently. Lower Limb strength will be assessed as primary outcome, and particular intervention or participant characteristics will be assessed as the secondary outcomes. Meta-analysis will be conducted using Review Manager 5.3.3, and evidence level will be assessed using the method for Grading of Recommendations Assessment, Development and Evaluation. Outcomes will be presented as the weighted mean difference or standardized mean difference with 95% CI. If I2 ≤ 50%, P>.1, the fixed effect model will be used, otherwise, random-effects model will be used. Ethics and dissemination: This review might not be necessary ethical approval because it does not require individual patient's data; these findings will be published in conference presentations or peer-reviewed journal articles. River restoration and environmental improvement projects have been peformed by social needs, therefore methodology for evaluating such projects must be provided. The PHysical HABitat SIMulation system (PHABSIM) is proposed as a tool for the assessment of hydraulic habitat suitability for aquatic species related to flow regime in river. This study evaluates the change of physical habitat for Zacco platypus according to small dam removal and the model suitability by applying PHABSIM to the reach where small dam was removed. It is shown that the physical habitat is generally increased and improved where the small dam was removed. However, physical habitat in the spawning stage that has a weak swimming speed is decreased by increased flow velocity in the upstream area of small dam, so proper countermeasure for that condition should be needed. Consequently, PHABSIM can be effectively used to provide methodology for assessment and necessity of various river projects including a removal of out-aged hydraulic structures.
Understanding sciences behind fall-related hip fractures in older adults is important to develop effective interventions for prevention. The aim of this review is to provide biomechanical understanding and prevention strategies of falls and related hip fractures in older adults, in order to guide future research directions from biomechanical perspectives. While most hip fractures are due to a fall, a few of falls are injurious causing hip fractures, and most falls are non-injurious. Fall mechanics are important in determining injurious versus non-injurious falls. Many different biomechanical factors contribute to the risk of hip fracture, and effects of each individual factors are known well. However, combining effects, and correlation and causation among the factors are poorly understood. While fall prevention interventions include exercise, vision correction, vitamin D intake and environment modification, injury prevention strategies include use of hip protectors, compliant flooring and safe landing strategies, vitamin D intake and exercise. While fall risk assessments have well been established, limited efforts have been made for injury risk assessments. Better understanding is necessary on the correlation and causation among factors affecting the risk of falls and related hip fractures in older adults. Development of the hip fracture risk assessment technique is required to establish more efficient intervention models for fall-related hip fractures in older adults.
Purpose : Many researchers have attempted to identity the reliability used in clinical examination of balance and gait performance for individuals of hemiparetic stroke. The study aims to evaluate whether the reliabilities of three popular clinical measures of balance and gait performance was consistency regardless of applicate experience of those clinical measures compared with previous studies for persons with hemiparetic stroke. Methods : A total of three hemiparetic stroke populations and twenty-six physical therapists were recruited from Glory hospital, Inchen, Korea in this study. The three clinical measures, involving Berg balance test (BBT), dynamic gait index (DGI), and Tinetti performance-oriented mobility assessment (POMA), were assessed in two sessions that were seven days apart. Results : The POMA was showed a good intrarater and interrater reliabilities in people with hemiparetic stroke regardless of measure's experience in clinical field. However BBT and DGI were showed below moderate intrarater and interrater reliabilities. Conclusion : The POMA could be a reliable measure to evaluate functional postural stability and gait performance in hemiparetic stroke patients compared with other two clinical measures regardless of measure's experience of physical therapists.
