A Comparison of Three Low Back Disability Questionnaires With Rasch Analysis (라쉬분석을 이용한 세 가지 요통 장애 설문지의 비교)

  • The purpose of this study was to review existing assessment tools for patients with low back pain and improve them through combination. A total of 314 patients with low back pain participated. Their condition was assessed using the Oswestry Disability Questionnaire (ODQ), the Quebec Back Pain Disability Scale (QBPD), and the Back Pain Functional Scale (BPFS). Rasch analysis was applied to identify inappropriate items, item difficulties, and the separation index. In this study, the 'sex life' item of the ODQ (10 items) and the 'sleeping' item of the BPFS (12 items) showed misfit statistics, whereas all items of the QBPD (20 items) were appropriate. After combining the ODQ, QBPD and BPFS, Rasch analysis was applied. The 'pain intensity', and the 'sex life' item of the ODQ and the 'throw a ball' item of QBPD showed misfit statistics. These 3 items were retained for further analysis. The remaining 42 combined ODQ-QBPD-BPFS items were arranged according to difficulty. For all subjects, the most difficult item was 'pain intensity', whereas the easiest was 'take food out of the refrigerator'. As the separation index of 42 combined ODQ-QBPD-BPFS was higher than that of the three questionnaires separately, difficulty of items varied with some need for rearrangement. The results of this study confirmed the possibility and need for a new back pain disability assessment tool, and produced one. Further study is needed to refine the questionnaire in consideration of psychosocial and occupational factors.

The Relationship Between Resting Scapular Position and Pain Level in Unilateral Shoulder Pain (편측 견부 통증 환자의 안정시 견갑골 자세와 통증수준과의 상관성 연구)

  • The purpose of this study was to investigate the most effective and comprehensible method for the assessment of resting scapular position (RSP) and pain level (PL) in unilateral shoulder pain (USP). Fifty volunteers with USP were involved in the study. Resting scapular assessments of the patients' pain sides (PS) and non-pain sides (NPS) were evaluated. The assessment tools for RSP are: 1) sternal notch (SN) to coracoid process (CP) distance 2) 3rd thoracic spinous process (T3S) to posterolateral angle of acromion (PLA) distance 3) scapular index 4) 8th thoracic spinous process (T8S) to inferior angle of scapular (IAS) distance 5) supine measurement of pectoralis minor (PM) distance 6) standing PM distance 7) PM index (PMI) and 8) PM pain. The paired t-test was used to compare PS and NPS in RSP. Pearson correlation analysis was used to confer a relationship between the PL and RSP. The results of this study indicated that: 1) all the variables between the PS and NPS for RSP were statistically significant(p<.05) and 2) the PMI showed the strongest relationship in the correlation analysis between RSP and PL(p<.05, r=.37). Therefore, it can be concluded that there is a relationship between PMI and PL and it is suggested that an assessment tool using PMI to diagnose shoulder pain would be clinically effective.

The Assessment of Survey on the Indoor Air Quality at Schools in Korea (국내 일부학교 건축물의 실내공기질 평가)

  • Recently, indoor air quality (IAQ) in workplace, residential environments and schools has been concerned of people, scientists and related the public, and has recognized the health effects related to indoor air pollution. Therefore, this study was performed to investigate the characteristics of IAQ in 55 kindergartens, elementary school, middle schools, and high schools from June, 2004 to May, 2005 in Korea. We measured indoor air pollutants($PM_{10},\;CO_2$, HCHO, total bacteria colony(TBC), CO, radon, TVOCs, asbestos, and $O_3$), and physical factors(noise, temperature, relative humidity, and illumination) with necessary of management for IAQ in school. We classified into 5 kinds of the school by period since building completion, <1 year, 1-3 years, 3-5 years, and 5-10 years. The concentration of pollutants and the level of physical factors compared with standards and guidelines of IAQ on the Ministry of Environment, the Ministry of Health and Welfare, and the Ministry of Education and Human Resources Development. The major results obtained from this study were as follows. Temperature, relative humidity and illumination among the physical factors did not exceed the standards, but noise exceeded it. Asbestos and $O_3$ did not detect in surveyed classrooms. CO, TBC, TVOCs, and HCHO in kindergartens, TBC in elementary schools, TBC, TVOCs dnd HCHO in middle schools, and HCHO in high schools detected the standards. This study is conducted as a part of efforts to provide a foundational data for further relative researches on management of IAQ of school. Therefore, we suggest that country plan for management of IAQ in school should be established through long-term and continuous investigation for assessment on IAQ in school and health risk assessment for students.

A Study of relationship between Housewives' Health KAP level and Family Health in Buan and Chunan Area. (가정주부의 건강에 대한 지식, 태도 및 실천수준과 가족건강간의 관련성 연구 - 천안 및 부안지역을 중심으로 -)

  • This study aims to explore the relationship between housewives' health KAP level and the physical health of families. The data used in this study are obtained from the Last Evaluation Program of the National Note for Health surveyed in July, 1989. The respondents for this study are 770 housewives residing in Chunan and Buan. The reason why this study focus on KAP level of housewives is to find out whether a housewife as a emotional supporter contributes to the physical health of her families. A housewife gives her families emotional satisfaction insteade of economic satisfaction. So she has the most interest in family health among the members of her family. Therefore, housewife's KAP level will influence her family health. The independent variables chosen for the analysis are the general characteristic variables and KAP level. And the dependent variable is the physical health of families which excluded psychosocial one. This level of family health includes weight for family health and seriouseness of disease. The result of this study was summarized as follows. (1) KAP level was significantly correlated with variables which have mainly the socioeconomic characteristics. The variables were: area of residence, education level occupation, self assessment on wealth, and exposure to mass communication. (2) In the analysis of relationship between the general characteristic variables and family health, family health was significantly correlated with almost all variables. The variables were: Presence of the aged, Health status, Experience in disease, Self assessment on health, No. of families, Occupation, Education level, Self assessment on wealth, Concern on health, and Exposure to mass communication. (3) In the analysis of relationship between family health and KAP level, family health was not significantly correlated with KAP level but. (4) Also in the stepwise regression analysis, the general variables account for about 32.1 percent of the variance in the dependent variable, family health. The variable with the greatest explanatory power was presence of the aged. On the contrary, KAP level explain about 0.4 percent of the variance in the dependent variable. In sum, the study shows that housewives' health KAP level has relatively weak relationship with the physical health of families

The Relationship Between Postural Control, ADL Function, Muscle Tone, and Functional Improvement in Chronic Stroke Patients (만성 뇌졸중 환자의 자세 조절과 일상생활동작, 근긴장도, 그리고 기능증진과의 관계)

  • The purpose of this study was to find any correlations among Postural Assessment Scale for Stroke (PASS), Modified Barthel Index (MBI), Tone Assessment Scale (TAS), Motor Assessment Scale-Gait (MAS-G), Fugl Meyer-Balance (FM-B), and to predict MBI from subscales of the PASS. The subjects were 41 stroke patients of the Korea National Rehabilitation Center in Seoul. The main outcome measures were postural control (PASS), gait (MAS-G), Balance (FM-B), Tone (TAS), ADL (MBI). The data was analyzed using the Pearson product correlation. PASS scale was used between other clinical and instrumental indexes, multiple stepwise regression analyses were performed to identify prognostic factors for ADL incline, and Cronbach's alpha coefficient was used to identify internal consistency on PASS scale. The results of this study areas follows: 1. The highest level was sitting without support, the lowest level was standing on paretic leg on PASS scale. The highest level was chair/bed transfer, the lowest level was bathing on MBI. 2. All items of the PASS, except postural tone were significantly correlated with Gait, Balance, MBI (p<.01), 3. The Internal Consistency (Cronbach's alpha coefficient=.85) was very high, indicating that the PASS is homogeneous and is likely to produce consistent response. Furthermore, the sums of maintaining position items and of changing-position items were strongly correlated (r=.64, p<.05) and there were significant correlations between sums of PASS, sums of maintaining position items (r=.87, p<.01), and changing-position items (r=.93, p<.01). 4. The standing without support of the PASS items was the strongest variance ($R^2$=.85) of the predicting ADL function. These findings provide strong evidence of the predictive value of the postural control on gait, Balance, ADL function in stroke patients and to can provide a reference for the successful therapeutic program and more improved functional recovery.

Evaluation of the Relationships Between Kellgren-Lawrence Radiographic Score and Knee Osteoarthritis-related Pain, Function, and Muscle Strength

  • Background: Knee osteoarthritis (OA) diagnosis using Kellgren-Lawrence scores is commonly used to help decision-making during assessment of the severity of OA with assessment of pain, function and muscle strength. The association between Kellgren-Lawrence scores and functional/clinical outcomes remains controversial in patients with knee OA. Objects: The purpose of this study was to examine the relationships between Kellgren-Lawrence scores and knee pain associated with OA, function during daily living and sports activities, quality of life, and knee muscle strength in patients with knee OA. Methods: We recruited 66 patients with tibiofemoral knee OA and determined knee joint Kellgren-Lawrence scores using standing anteroposterior radiographs. Self-reported knee pain, daily living function, sports/recreation function, and quality of life were measured using the knee injury and OA outcome score (KOOS). Knee extensors and flexors were assessed using a handheld dynamometer. We performed Spearman's rank correlation analyses to evaluate the relationships between Kellgren-Lawrence and KOOS scores or muscle strength. Results: Kellgren-Lawrence scores were significantly negatively correlated with KOOS scores for knee pain, daily living function, sports/recreation function, and quality of life. Statistically significant negative correlations were found between Kellgren-Lawrence scores and knee extensor strength but not flexor strength. Conclusion: Higher Kellgren-Lawrence scores were associated with more severe knee pain and lower levels of function in daily living and sports/recreation, quality of life, and knee extensor strength in patients with knee OA. Therefore, we conclude that knee OA assessment via self-reported KOOS and knee extensor strength may be a cost-effective alternative to radiological exams.

Relationship Between a New Functional Evaluation Model and the Fugle-Meyer Assessment Scale for Evaluating the Upper Extremities of Stroke Patients

  • Purpose: The aim of this study was to investigate the relationship between a functional evaluation model and the Fugl-Meyer assessment (FMA) scale in evaluating the upper extremities of stroke patients Methods: Thirty-eight stroke patients were evaluated using the FMA and performed reaching and grasping motions using a three-dimensional motion analysis (Qquas 1 series, Qualisys AB, Sweden). The participants sat on a chair with a backrest. The position of the cup was located at a distance of 80% to the front arm length. The markers were attached to the sternum, acromion, elbow lateral epicondyle, ulnar styloid process, three metacarpal heads, and the distal phalanges of the thumb and index finger. The variables of the correlation between the functional evaluation model and the FMA scale were analyzed. Multiple regression (stepwise) was used to investigate the effect of the kinematic variables. Results: A significant negative correlation was found between the movement time (p < 0.05), movement unit (p < 0.05), and trunk displacement values (p < 0.05) in the FMA total scores, while a positive correlation was found between the peak velocity (p < 0.05) and maximum grip aperture values (p < 0.05). As a result of the multiple regression analysis, the most significant factor was the movement unit, followed by the general movement assessment and trunk displacement. The explained FMA total score value was 62%. Conclusion: This study presents a new functional evaluation model for assessing the reaching and grasping ability of stroke patients. The factors of the proposed functional evaluation model showed significant correlations with the FMA scale scores and confirmed that the new functional evaluation model explained the FMA by 67%. This suggests a new functional evaluation model for reaching and grasping stroke patients.

Body mass index and relative handgrip strength are associated with the prevalence of hypertension in Korean elderly: Korean national fitness assessment in 2019

  • [Purpose] This study aimed to analyze the prevalence of hypertension according to the body mass index (BMI) and relative handgrip strength (RHGS) among elderly individuals in Korea. [Methods] We analyzed the data of 44,183 Korean elderly individuals over 65 years old (men: n = 15,798, age = 73.31 ± 5.04 years, women: n = 28,385, age = 72.14 ± 5.04 years) obtained from the Korean National Fitness Assessment in 2019. All the participants were categorized into three groups according to the BMI and RHGS; additionally, one-way ANOVA and logistic regression analysis were performed. [Results] Overweight (men: 1.16 odds ratio [OR] 1.06-1.26, 95% confidence interval [CI]; women: 1.15 OR, 1.07-1.23 95% CI) and obese (men: 1.54 OR, 1.42-1.66 95% CI; women: 1.44 OR, 1.36-1.53 95% CI) elderly individuals showed a higher prevalence of hypertension than elderly individuals with normal weight, after controlling for age. In men, a lower RHGS was associated with a higher prevalence of hypertension after controlling for age (weak RHGS: 1.09 OR, 1.00-1.17 95% CI; middle RHGS: 1.21 OR, 1.12-1.31 95% CI vs. strong RHGS). [Conclusion] A higher BMI was associated with the prevalence of hypertension in the elderly Korean population. In addition, a lower RHGS was associated with the prevalence of hypertension in elderly Korean men.

Development of Patient Classification System in Long-term Care Hospitals (요양병원 환자분류체계 개발)

  • Purpose: To develop the patient classification system based on the resource utilization for reimbursement of long-term care hospitals in Korea. Method: Health Insurance Review & Assessment Service (HIRA) conducted a survey in July 2006 that included 2,899 patients from 35 long-term care hospitals. To calculate resource utilization, we measured care time of direct care staff (physicians, nursing personnel, physical and occupational therapists, social workers). The survey of patient characteristics included ADL, cognitive and behavioral status, diseases and treatments. Major category criteria was developed by modified delphi method from 9 experts. Each category was divided into 2-3 groups by ADL using tree regression. Relative resource use was expressed as a case mix index (CMI) calculated as a proportion of mean resource use. Result: This patient classification system composed of 6 major categories (ultra high medical care, high medical care, medium medical care, behavioral problem, impaired cognition and reduced physical function) and 11 subgroups by ADL score. The differences of CMI between groups were statistically significant (p<.0001). Homogeneity of groups was examined by total coefficient of variation (CV) of CMI. The range of CV was 29.68-40.77%. Conclusions: This patient classification system is feasible for reimbursement of long-term care hospitals.

  • This article considers the trend of Korea physical therapy reaearch. Because physical therapy research is important for the continued growth of the physical therapy profesion. Such research results in improved patient care and heightens the professional standing of the physical therapy field. The purpose of this article is to discuss a current research trend and to suggest a direction for developing a physical therapy research. We discussed a research design, purposes, and methods. Measurement are fundamental to the practice of physical therapy. The physical therapy evaluation is the foundation for the measurement of the outcome of our therapertic intervention. And we must measure thess outcomes. Achieving a high quality of physical therapy practice requires us to evaluate the client, selecting and adminstering a variety of tests and measurent. For the future study, physical therapist has a good treatment model and research program for evaluating and assessment.
