서울, 경기지역 종합병원의 흉부물리치료 실태조사 (An Investigation on chest physical therapy of hospital in seoul and Kyungido)

  • The purpose of this study was to investigate the chest physical therapy in the hospital of seoul, kyungido and to activate the chest physical therapy. Dept. of physical therapy in 15 hospitals, 65 physical therapists disabled and analysis of interview. The results are as follows: 1) $86.7\%$ of respondents were not chest physical therapist in the hospital. $79.4\%$ of respondents were a lack of 5 patients per month. 2) The chief diagnosis of chest physical therapy was respiretory muscles weakness by reason of spinal cord injury and muscle diseases. Many physical therapists was obtain the chest physical therapy information from university or collage. 3) Management of chest Physical therapy Patient was to be alike physical therapist and doctor A problem of chest physical therapy was a lack of chest physical therapy information, cognition of a physician. The results of this study suggested that chest physical therapy need to organization of the treatment and magnification of the treatment area.

종합병원 물리치료사의 임파워먼트가 직무만족에 미치는 영향 (The Effect of Empowerment on Job Satisfaction for Physical Therapists in General Hospitals)

  • PURPOSE: This study aimed to investigate the effect of empowerment on job satisfaction for physical therapists in general hospitals. METHODS: A total of 297 physical therapists working in general hospitals throughout the country participated in the study survey using self-administered questionnaires. The questionnaires comprised three major parts, i.e., questions about general characteristics, empowerment, and job satisfaction, which were rated using a 5-point Likert scale. Statistical analysis was performed to analyze the frequency and percentage of participants, and the independent t-test and analysis of variance were performed to determine the difference in job satisfaction according to the general characteristics. Stepwise multiple regression was used to evaluate the factors that affect job satisfaction. RESULTS: The average of total empowerment, including two subordinate concepts, was 3.29 points, that of psychological empowerment was 3.53 points, and that of structural empowerment was 3.05 points. The average job satisfaction of the participants was 3.43 points, and it showed a significant difference in age, overall work experience, current work experience, position, annual salary, and daily work hours. Moreover, job satisfaction increased as the subordinate factors for empowerment, i.e., support, resources, chance, and competence, increased. Other factors that affect job satisfaction were daily work hours, number of colleagues, type of work, annual salary, and therapeutic assistance. CONCLUSION: This study demonstrated that job satisfaction of physical therapists working in general hospitals was related to empowerment, thereby indicating that job satisfaction and work performance can be increased by improving their empowerment.

  • The process of physical therapy uses a problem-solving approach to enhance a patients's functioning status. The International Classification of Functioning, Disability and Health(ICF) is the common concept for the functioning in the world. Physical therapists require the ability to identify problems, formulate hypothesis, and plan intervention strategies through clinical reasoning. In the clinical process, physical therapists need to use standard and common languages in speech and in documentation. The purpose of this study was to suggest the process of making strategy for efficient intervention, examining and evaluating the functional problem of the person with stroke using ICF tools. For the first step in this process model, therapists could list the information relating to functional problems used by the ICF Core set and then could identify the interaction among the problems using the ICF assessment sheet. For the next step, therapist is needed to make the hypothesis and hypothesis testing, and then set a primary functional goals and therapeutic goals in detail after prioritizing the problems to be managed based on the problem list. Finally, after setting the identified problems as the purpose of intervention through the hypothesis testing, therapist could do some intervention after making a plan to solve these problems, and find out the outcomes using the ICF evaluation display. This report illustrates how to apply the process based on ICF concept into physical therapy practice. Making a decision for the most efficient intervention requires that therapists use the clinical reasoning process based on ICF concept.

Usability Test for Motion Tracking Gait Assistive Walker

  • Background: This study evaluates the usability of the Motion-Tracking Gait Assistive Walker (MTGAW) designed for elderly individuals and those with disabilities, identifying areas for improvement through interviews with physical and occupational therapists. Design: A survey study involves the usability test for MTGAW. Methods: Usability evaluations were conducted with 37 physical therapists and occupational therapists. The process included explanation, product usage, satisfaction surveys, and interviews. A satisfaction survey covering 19 items across safety, maneuverability, usability, and management areas was administered. Individual interviews identified areas for improvement. Results: Overall, high satisfaction was reported across the four areas, but interviews highlighted the need for improvements, such as addressing discomfort due to slow speed and enhancing safety measures to prevent rear-end falls. Adjusting the walker's height and width to suit the user's physique was also suggested. Conclusion: MTGAW enhances walking support and hand movement freedom but needs refinement in speed control, fall prevention, and customization based on the user physique. Future efforts should focus on developing an improved MTGAW, considering recommendations from physical therapy experts, and conducting studies to analyze its clinical effectiveness for commercialization.

물리치료사와 작업치료사의 필라테스에 대한 인식도 조사 (Recognizable Investigation of Physical and Occupational Therapist on the Pilates)

  • Background: This study is intended as an investigation of the recognition for physical and occupational therapist on the Pilates. Methods: Thirty three therapist were participated in instruction of C-college, completed questionnaire form and then analyzed data. Results: Subjects who don't instruct or practice on the Pilates was 90.9% and physical therapist(PT) was 83.3%, occupational therapist(OT) 100%. 57.6% subjects was well aware of the Pilates by the media and was a most higher respondents. Therapists knowing the exact effect of the Pilates was 54.5%. PT was 33.3%, OT 80.0% and between PT and OT groups were significant difference(p<.05). Therapists knowing the exact practice with Pilates method of applicable a patient was 54.5%. In relation to Pilates and rehabilitation, a lot was 18.2%, a yes 69.7% and a little 12.1%. Therapists have applied the Pilates in rehabilitation was 12.1%. In minded therapists applying the Pilates after instruction, a lot was 24.2%, a yes 69.7%, a little 6.1% and a none 0%. Conclusion: Judging from the above, the knowledge and recognition of the Pilates for PT and OT was low and the will applying for therapy was high. Therefore, therapist is in need of instruction of the Pilates and further research on the Pilates and therapist would clarify the development of PT or OT.

우리나라 의료재활 전문인력 수요${\cdot}$공급 및 추계에 관한 연구 (An Analysis on Demand and Supply for Medical Rehabilitation Professionals in Korea)

  • Korea will soon experience a high demand for medical rehabilitation specialists, if it tries to deliver advanced health welfare service. In order to medical rehabilitation manpower policies, this study attempts to analyse, estimate and plan a long-term supply for physiatrists, physical therapists, and occupational therapists. The study analysed both national and foreign statistical data of manpower supply for medical rehabilitation specialists. Based on the above data, the demand of and supply for each specialists were estimated for long term up to the year 2030. Based on the comparative analysis results of the future demand and supply. the author intended to develop a new supply plan for the three specialist categories. The major finding of the supply plan are as follows : First, as for the supply plan for physiatrists, the author recommends to adopt the demand estimation 1 as the most suitable. In order to prevent an oversupply of physiatrists, the supply plan 1 is recommended which annual enrollment of specialists will maintain with the quota of fixed number of 63 from the year of 1999. Second, it is estimated that there was already an oversupply of physical therapists in Korea. This oversupply is expected to continue even though there would be an increase in rehabilitation hospital beds, rehabilitation facilities for the elderly, and nursing homes, Thus, it would be desirable to cut down the number of students admitted to physical therapy schools each year. Third, there will be a high demand for occupational therapists in the near future as people become more aware of the usefulness of this therapy. Thus, it is urgent to establish a supply plan to meet the demand. Given the close relationship between physical therapy and occupational therapy, the study recommends that the universities already having the the department of physical therapy open the department of occupational therapy as well.

소아물리치료사와 소아작업치료사의 보조공학 활용 수준 및 자신감 (A Study on Assistive Technology for Pediatric Therapists)

  • Purpose: This study sought to identify the utilization and confidence in assistive technology (AT) for pediatric physical therapists (PPTs) and pediatric occupational therapists (POTs) for the purpose of improving the utilization of AT in clinics. Methods: The subjects of this study were 167 therapists (98 PPTs and 69 POTs) who work at general hospitals, welfare centers, facilities for the disabled, and special education schools in Seoul and Gyeonggi province. The frequency analysis and chi-squared test were used. Results: This study found that both PPTs and POTs recognize that AT benefits infants with disabilities; however, they show low confidence in using AT. Applications of AT are concentrated on wheelchairs for mobility and assistance with seating position, with orthotics/prosthetics for both purposes. POTs were found to use a wider variety of AT devices. While both PPTs and POTs frequently select/recommend devices, both groups are less involved with delivery methods, manufacture of devices, and education on AT use. Conclusion: These findings show the actual conditions of utilization and confidence in AT of PPTs and POTs deeply involved with AT, and emphasize that AT training is essential for pediatric treatment by comparing domestic and foreign studies.

임상 물리치료사의 경험에 따른 시각적 보행 분석의 신뢰도 연구 (Reliability of Visual Gait Analysis according to Clinical Experience Level of Physical Therapists)

  • Purpose: Visual gait analysis plays a pivotal role in determining the important gait problem of patients. A few studies have been published and have received little attention regarding visual gait analysis on patients with orthopedic problems. The purposes of this study were to investigate the difference of reliability levels according to experience of clinical physical therapists. Methods: Thirty-five clinical physical therapists, 5 high experienced, 15 experienced, and 15 inexperienced, were recruited and individually evaluated these videotaped gait patterns of the participants, and filled up the structured gait analysis form. The gait of nine participants was videotaped. Reliability levels were calculated by the Intraclass Correlation Coefficients (ICC). Results: The inter-rater reliability of high experienced group (ICC=0.56; 95% CI: 0.50-0.62) was comparable to that of the experienced raters (ICC=0.48; 95% CI: 0.43-0.53) and inexperienced group (ICC=0.42; 95% CI: 0.38-0.46). High experienced group reached a higher inter-rater reliability level. The average intra-rater reliability of the high experienced group was 0.70 (ICCs ranging from 0.54 to 0.82). The experienced group reached an average intra-rater reliability of 0.61 (ICCs ranging from 0.47 to 0.81). The inexperienced group attained average ICC values of 0.53 (ICCs ranging from 0.30 to 0.74). Conclusion: Use of a structured gait analysis form as described in this study was found to be moderately reliable. Clinical experience appears to increase the reliability of visual gait analysis.

대구지역 물리치료사들의 직업과 관련된 근골격계장애 조사 (A survey on the Work-Related Musculoskeletal Disorders in Physical Therapist in Daegu)

  • Disorders of the musculoskeletal system are common in physical therapist. Physical therapist are exposed to occupational risk factors leading to work-related musculoskeletal disorders(WMD). Back pain is an especially frequent complaint. The purpose of this study was to determine the prevalence and anatomical areas of WMD and the job factors that may be associated with these disorders in physical therapists. An 2-page questionnaire was used. 105 physical therapist responded. Despcriptive statistics, Chi-square analysis and odds ratio were used. The results were as follows: 1. Ninty-nine respondents(94.2%) reported experiencing WMD. 2. The highest prevalences of WMD in physical therapist were in the following areas;lowback(74.7%), shoulder(66.6%), neck and wrist(41.4%). 3. The prevalence of WMD was affected by age, years as physical therapist. and number of patients. 4. The job factor rated most problematic for physical therapists was 'performing th same task over and over' (41.3%), 'treatingan excessive number of patients in 1 day'(39.5%)
