The purpose of this study was to provide experimental evidence and theoretical background for the applicability of bilateral transfer paradigm to the rehabilitation programs for children with hemiplegic cerebral palsy. Children with hemiplegia, which means unilateral motor disabilities, display abnormal motor and postural patterns of the affected side due to hemiparesis, spasticity, and sensory disorders, resulting in a decreased motor abilities of the affected side compared to unaffected side. Accordingly, they tend to rely on the unaffected limb for everyday activities, which further deteriorates the functions of the affected side by causing associated reaction, abnormal postural patterns, and hypertonus. Rehabilitation programs developed for children with hemiplegic cerebral palsy include neurodevelopmental treatment, application of cast or splint to unaffected limb, neuromuscular electrical stimulation, and task oriented model. These programs, however, have several drawbacks, such as discontinuity in treatment effect and psychological hatred to the force use of the affected side. In order to solve these problems and enhance the efficiency of the rehabilitation programs, it is required to maximize the use of the affected side without hatred. Characteristics of the control system, such as temporal coupling and spatial assimilation between limbs and neural crosstalk at different levels of central motor pathway, suggest that the bilateral transfer paradigm may enhance the efficiency of the rehabilitation programs for children with hemiplegic cerebral palsy.
When any damage or disease occurs, muscular strength and muscular endurance are lowered, and thus, if one is able to be restored from the damage or the disease, appropriate stimulus is required, since the muscles have to restore their proper functions. For such stimulus, the way of exercising and the way using electric stimulus are used in physical therapy. In order to examine the change in muscular strength, muscular endurance and girth of limbs for Biceps brachii, according to the lands of stimulus, in this article a total of 42 healthy male and female adults in their twenties were randomly sampled, and a series of tests were conducted for 6 weeks, for 15 minutes per once, 3 times per week, respectively, dividing them into the group for RUS(Russian Current Stimulation), PRE(Progressive Resistive Exercise) and P+R(RUS + PRE). The findings showed that the change in muscular strength was most significant as for the group for P+R, since it increased from $62.12{\pm}25.30$ before experiment to $95.78{\pm}34.07$ after 6 weeks: the change in muscular endurance was most significant as for the group for P+R, since it increased from $17.57{\pm}6.63$ to $42.86{\pm}10.24$; and the change in the girth of limbs was slightly significant only in the group for P+R, and the remaining two groups showed no significance.
Purpose: The purpose of this study was to investigate the effects of three-dimensional virtual reality horse riding simulator training using a head-mounted display on gait and balance in children with cerebral palsy. Methods: Ten children with cerebral palsy were randomly assigned to the horse riding simulator (HRS) group (n=5) or the horse riding simulator with virtual reality (HRSVR) group (n=5). To evaluate balance, center of gravity (COG) sway velocity and total sway distance of each group were assessed using the Wii balance board, and gait speed and stride length of each group were assessed using a gait analysis system. Results: Intra-group comparisons between pre- and post-intervention measures revealed that there were significant changes in all gait and balance variables such as stride length, gait velocity, COG sway velocity and COG sway distance in the HRSVR group (p<0.05). In the HRS group, there were significant changes in all variables except stride length (p<0.05). In addition, inter-group comparisons showed significant differences between the two groups in stride length, gait velocity and COG sway distance except COG sway velocity (p<0.05). Conclusion: The findings of this study suggest that horse riding simulator training combined with 3D virtual reality can be a new positive therapeutic approach for improving functional performance in children with cerebral palsy.
Purpose: This study examined the effects of backward walking training with task orientation on the functional walking ability of children with cerebral palsy. Methods: This study was a single-blinded, randomized controlled trial with a crossover design conducted at a single rehabilitation facility with cross-over to the other intervention arm following a two-week break. For a total of 12 children with spastic hemiplegia cerebral palsy, the forward walking training group (n=6) underwent training three times a week for three weeks, 40 minutes a day, and the backward walking training group (n=6) was also trained under the same conditions. To identify the functional walking ability, variables, such as the walking speed, stride length, and step length, were measured using a walk analyzer (OptoGait, Microgate S.r.l, Italy). Results: Both groups showed significant increases in walking speed, stride length, and step length (p<0.01). The backward walking group showed more significant improvement in the walking speed from pre- to post-test (p<0.05). The gait characteristics were similar in the two groups (stride length and step length) but the walking speed in the backward walking group showed a mean difference between the positive effects higher than the forward walking group. Conclusion: Task-oriented backward walking training, which was conducted on the ground, may be a more effective treatment approach for improving the walking functions of spastic hemiplegia children than forward walk training.

PURPOSE: To prevent secondary complications from decreased pulmonary functions and promote neurological recovery, identification of respiratory capacity change patterns depending on different postures of stroke patients and investigation of their properties are needed for active rehabilitation. Therefore, this study was conducted to investigate the changes in vital capacity in response to different positions and to implement the results as clinical data. METHODS: A respiratory function test was administered to 52 patients with stroke in the sitting, supine, paretic side lying, and non-paretic side lying positions. Pulmonary function indexes used for comparison were forced vital capacity (FVC), forced expiratory volume at 1 second (FEV1), forced expiratory flow 25-75% (FEF 25-75%), and maximum voluntary ventilation (MVV). One-way repeated ANOVA was used for analysis, and post hoc analysis was conducted using least significant difference (LSD). RESULTS: All pulmonary function indexes were measured in the order of sitting, paretic side lying, supine, and non-paretic side lying positions. Excluding the FEF25-75% and MVV of the supine compared with the paretic side lying position, all other pulmonary function indexes differed significantly (p<.05). CONCLUSION: There are differences in pulmonary function indexes depending on different postures of stroke patients, and the study showed that the non-paretic side lying position yielded the greatest effect on lung ventilation mechanisms. Based on these results, appropriate postures need to be considered during physical therapy interventions for stroke patients.

Background: Although there are interventions available for the improvement of arm functions of patients with stroke, measuring changes in global synkinesis (GS) according to changes in the muscle activity of paretic and nonparetic side muscles is an important factor and studies to investigate such potential factors are evaluated necessary. Objective: To examine the correlation between the affected arm muscle activity and GS changes in patients with stroke. Design: Randomized controlled trial Methods: In order to measure muscle activities of 30 stroke patients, anterior deltoid, posterior deltoid, biceps brachii and triceps brachii of the affected arms were measured using surface electromyography (EMG) and for analyzing GS, biceps brachii and triceps brachi of the affected arms were measured using the same instrument. Results: When the correlations between the muscle activity and GS of the affected arm were analyzed, the results showed significant correlations between the posterior deltoid and the triceps brachii(p < .01) and between the triceps brachii and GS(p < .05). Conclusion: : The results of this study suggest that the efficient movements of the affected arms of stroke patients, it is possible to increase extensor activity by improving smooth antagonism of the arm.
Purpose : This purpose of this study was to investigate the effects of elastic band resistance exercise program in the falls prevention of elderly women. Methods : The subjects of this study, 46 people elderly women who participated in the exercise program of Jeollanamdo Y elderly welfare center were chosen as subjects and use the elastic band resistance exercise program for 12 weeks. Results : There was a significant increase on the physical function and fall efficacy of all the subjects in the experimental group and the control one after the experiment. Furthermore, the experimental group showed more meaningful improvement than the control one. Conclusion : Falls Efficacy of the experimental group before the experiment, after the changes in difference was noted. More results from the 12 weeks of using the elastic band resistance exercise program to increase the efficacy of body functions (power of lower extremity, muscle endurance, flexibility, balance, and agility), and falls efficacy of elderly women is considered as an effective fall prevention exercise program.
The purpose of this study was to determine the effect of exercise designed on physiological variables, physical functions in the elderly. The research design used was the one group pre-post test quasi-experimental design. The subjects for this study were 23 elderly, who were conveniently assigned to experimental group. The experimental group were selected from G-elderly church club in G-city. The data were collected from March 5, 2001 to June 11, 2002, and analyzed by frequency, t-test using SPSS/PC program. The results of this study were summarized as follows ; 1) The heart rate in the experimental group show statistically significant improvement (t=2.316, p=0.030). 2) The systolic blood pressure & diastolic blood pressure in the experimental group show statistically significant improvement(t=2.277, p=0.033 & t=-2.989, p=0.007). 3) The peripheral vessel $O_2$ Saturation in the experimental group don't show statistically significant improvement(t=-1.096, p=0.285). 4) The physical function in the experimental group don't show statistically significant improvement(t=-0.746, p=0.464). After 15 weeks of exercise designed, it was shown that the program was effective to improve the physiological variables.
The purpose of the present research is to investigate the effects of acute soccer game on serum levels of neurotrophins and neurocognitive function. The subjects of the research were 15 healthy male adolescents. The subjects underwent two experiments: one experiment in the soccer game treatment (SOC) condition, and the other in the self-study treatment (CON) condition. Blood samples were collected at three times: before treatment (Pre), after treatment (Post), and 2 hours post treatment (Post-2 h) for the analyses of serum brain-derived neurotrophic factor (BDNF), nerve growth factor (NGF), and insulin-like growth factor-1 (IGF-1). For the measurements of neurocognitive functions, the Stroop Color-Word test was performed at blood collecting times. The results of the research showed that the serum levels of BDNF, NGF, and IGF-1 were significantly increased after the soccer game (p<0.05), and significantly higher in SOC than CON at Post (p<0.05). In the Stroop Color-Word test, significantly increased scores were observed in SOC at Post (p<0.05), and significantly higher in SOC than in CON at Post and Post-2 h (p<0.05). These results suggest that acute soccer game has positive effects on neurocognitive functions by increasing the neurotrophins.
This study was to determine the effect of 12-week balance training with visual feedback on balance and walking functions in patients with post-stroke hemiparesis. Twenty individuals with chronic stroke volunteered to participate in this study. They were randomly allocated to either experimental group (EG) or control group (CG), with 10 subjects in each group. Subjects from the group 1 underwent 15-min balance training with visual feedback and 15-min routine-scheduled physical therapy, and subjects from the group 2 performed 30-min routine-scheduled physical therapy only, which comprised mat exercise, strengthening, postural correction, and functional practice. Assessment tools were made with the Functional Reach Test (FRT), Timed Up and Go Test (TUGT), and 6 Min Walk Test (6MWT). In within-group comparison, the EG showed significant differences in all parameters (p<0.05), while significant differences were not found for the CG (p>0.05). In between-group comparison, the change values of the FRT and 6MWT appeared to be significantly greater for the EG than those of the CG (p<0.05). These findings suggest that balance training with visual feedback may be favorably used to improve balance and walking functions in patients with chronic post-stroke hemiparesis. Further studies with larger sample and long-term follow-up period need to generalize the results of this study.
