  • Fracture is uncommon cause of lymphedema. The mechanism of lymphedema progression is still unknown, but disruption of the lymphatic system during and after fracture might be involved. In contrast, breast cancer surgery is a common cause of lymphedema and is usually caused by the removal of axillary lymph nodes. Sentinel node biopsy (SNB) has been adopted in early breast cancer to reduce the risk of lymphedema. Thus, the incidence of lymphedema in SNB was lowered. However, less than 10% of SNB patients are still diagnosed with lymphedema, but it is known that it took years to diagnose. Recently, we encountered atypical lymphedema occurred after breast cancer surgery with SNB. Symptoms started earlier than usual and were more severe. Interestingly, she has a history of a proximal radial fracture on the same side of SNB. We thought there could be a relationship between the acceleration of breast cancer-related lymphedema and fracture.

Comparison of the Changes in the Activation of the Quadriceps Muscle based on the Plantar Flexion Degree of the Ankle Joint in Healthy Young Females during the Stand-to-Sit movement

  • Purpose: The purpose of this study was to compare the changes in the muscle activation of the quadriceps muscle (rectus femoris, vastus lateralis, vastus medialis) during the stand-to-sit (StandTS) movement according to the plantar flexion angle of the ankle joint. Methods: A total of 22 healthy young females participated in this study. During the StandTS under the three conditions (plantarflexion angle 0°, 20°, and 45° of the ankle), electromyography (EMG) data (% maximum voluntary iso¬metric contraction) of the rectus femoris, vastus lateralis, and vastus medialis were recorded using a wireless surface EMG system. Results: There was a significant difference in the muscle activation of rectus femoris, vastus lateralis, and vastus medialis according to the plantar flexion angle (0°, 20°, and 45°) of the ankle. The muscle activation of the quadriceps was the highest at a 45° angle of plantarflexion and the lowest at 0°. One-way repeated ANOVA was used to analyze the muscle activation data of the lower extremity muscles according to the angle of the ankle joint. Conclusion: Based on the results of our study, it was confirmed that the muscle activity of the quadriceps can be increased even in the StandTS movement, which involves the eccentric contraction of the quadriceps muscle. This suggests that maintaining a plantar flexion posture for a long time, say by wearing high-heeled shoes, can quickly cause muscle fatigue in the lower-limb muscles, which can cause a decrease in balance ability leading to falls.

Septic arthritis of the hip joint caused by Klebsiella pneumoniae: a case report

  • Klebsiella pneumoniae is an uncommon cause of septic arthritis in adults. However, late detection can cause serious complications, including joint destruction and immobility. The purpose of this study was to report a case of successfully treated septic arthritis of the hip joint (SAHJ) caused by K. pneumoniae. A 49-year-old female patient presented to our hospital with fever and progressive severe pain in the right hip area. Although there was no abnormality on plain radiographs, ultrasonography revealed diffuse swelling of the right hip joint. Under ultrasonography guidance, the hip joint fluid was aspirated, and Gram staining and culturing were performed. The patient's pain was significantly reduced after the joint aspiration. The Gram staining and culturing revealed gram-negative bacilli, which were subsequently identified as K. pneumoniae. According to the results, systemic intravenous antibiotic (ceftriaxone) was administered without complications, and the patient was discharged on oral antibiotic (ciprofloxacin). Clinical cases of septic arthritis of the knee or sacroiliac joint have been occasionally reported in adults, but cases of SAHJ are rare. Moreover, K. pneumonia-induced SAHJ has not been reported to date. Therefore, we report this very rare case and its successful treatment.

  • A ski lifter for winter sports is one of the necessary and familiar facilities. But recently safety concern about ski lifter has been built. The ski lifter crash in Pocheon happened recently showed an example of safety concern and frigidity for safety awareness. In this report, the cause of the ski lifter crash in Pocheon was reviewed, and some suggestions were made to prevent a similar accident base on this review.

흉추 후만증에 관한 임상적 중요성과 측정법 (Clinical Importance and Measurement in Thoracic Kyphosis)

  • Thoracic kyphosis is occasionally used to describe someone with accentuated thoracic curvature, hyperkyphosis is preferred since kyphosis itself refers to the normal sagittal angle of thoracic curvature. The angle of thoracic kyphosis tends to increase with age resulting in hyperkyphosis in some individuals. The persons who suffer from hyperkyphosis are at increased risk for a variety of adverse health outcomes that include musculoskeletal alteration, physical functional limitations, poor quality of life, falls, and even earlier mortality. Hyperkyphosis may develop from vertebral fractures, degenerative disc disease, either muscle weakness, decreased mobility and sensory deficits. The gold-standard orthopaedic technique for assessment of thoracic kyphosis is standing lateral spine radiographs. Other clinical measures are Debrunner kyphometer, inclinometer, flexicurve ruler, arcometer, flexible electrogoniometer and spinal mouse.

레트 증후군의 이해 (Understanding of Rett Syndrome)

  • Purpose : The purpose of this study is understanding of Rett Syndrome. Rett Syndrome is a common developmental - neurologic disorder that has been reported almost exclusively in female. Recently mutations in the gene encoding X-linked methyl-CpG binding protein 2 (MECP2) have been identified as the cause of Rett syndrome. Consistent with the diagnostic criteria, hand skills, verbal or non - verbal communication skills and common motor skills were lost during regression. Regression most commonly occurred between 12 and 18 months of age. Methods : This is a literature study with books, articles, web site for Rett syndrome international association. Results : There is a continuing need to further elucidate the pre- and post - regression features of Rett syndrome. Rett syndrome need to physical therapy, musical therapy, special education and medical interventions. Conclusion : There has not been therapeutic method to the root of Rett syndrome but our goal is relaxation of symptom and physical therapist's study of Rett syndrome.

당뇨병성 족부병변의 관리 (Management of Diabetic Foot Problems)

  • The purposes of this article are to review the pathogenesis, prevention, and management of amputation due to diabetes mel1itus complications, and to report one case who had lost his toes due to diabetes mellitus. A primary cause for hospital admission of the patient was foot ulcer. Since many amputations in diabetic patients are precipitated by such ulcers, a program for active prevention and optimal treatment of diabetic foot lesions might decrease the risk of amputation. Diabetic foot ulcers and, ultimately, amputation can stem from a variety of pathways. The combination of peripheral neuropathy, peripheral vascular disease and infections is the harbinger of the final cataclysmic events of gangrene and amputation. As the physical therapist is often involved in the treatment of diabetic patients, the therapist should be aware of the followings: the patient's type of diabetes and the severity of the diabetes, the complications of the disease, the effects of exercise, the importance of wearing proper shoes and education to patients about appropriate diabetic foot care.

불유합 치료에 관한 고찰 (A study on nonunion treatment of fracture)

  • The nonunion means a state that the healing of fracture has stopped, and the part of fracture remains to not heal up in several months. The medical treatment of nonunion was tried several way to add stability to fracture. The treatment of The electrical stimulation and medicine which is make from calcium, phosphorus, and Vitamin A, D have used, but recently try to use Ultrasound and Laser in the physical therapy. In the text, we look into definition of nonunion, a cause, fracture of treatment process and approach of nonunion treatment. So, using Ultrasound and Laser in the physical therapy can be great help to the fracture and nonunion

Comparison of Clinical Characteristics Between Patients With Different Causes of Vocal Cord Immobility

  • Objective To analyze the clinical characteristics between neurogenic and non-neurogenic cause of vocal cord immobility (VCI). Methods The researchers retrospectively reviewed clinical data of patients who underwent laryngeal electromyography (LEMG). LEMG was performed in the bilateral cricothyroid and thyroarytenoid muscles. A total of 137 patients were enrolled from 2011 to 2016, and they were assigned to either the neurogenic or non-neurogenic VCI group, according to the LEMG results. The clinical characteristics were compared between the two groups and a subgroup analysis was done in the neurogenic group. Results Among the 137 subjects, 94 patients had nerve injury. There were no differences between the neurogenic and non-neurogenic group in terms of demographic data, underlying disease except cancer, and premorbid events. In general characteristics, cancer was significantly higher in the neurogenic group than non-neurogenic group (p=0.001). In the clinical findings, the impaired high pitched 'e' sound and aspiration symptoms were significantly higher in neurogenic group (p=0.039 for impaired high pitched 'e' sound; p=0.021 for aspiration symptoms), and sore throat was more common in the non-neurogenic group (p=0.014). In the subgroup analysis of neurogenic group, hoarseness was more common in recurrent laryngeal neuropathy group than superior laryngeal neuropathy group (p=0.018). Conclusion In patients with suspected vocal cord palsy, impaired high pitched 'e' sound and aspiration symptoms were more common in group with neurogenic cause of VCI. Hoarseness was more frequent in subjects with recurrent laryngeal neuropathy. Thorough clinical evaluation and LEMG are important to differentiate underlying cause of VCI.

근육 근막 장애가 자세균형에 미치는 시각적 분석 (The Visual Analysis of Myofascial Syndrome on Balance Posture)

  • 인체가 균형상태에 있지않게 되면 중력으로부터 보상작용은 과도한 에너지소비로 구축을 야기시켜 이차적질병과 외부로부터 가해지는 충격을 흡수하지 못하고 약한 충격에서도 손상을 초래하게 된다. 또한 물리적 외상, 염증이나 감염의 진행으로부터 오는 구조적 불균형은 부적절한 근막의 긴장으로 초래하게 되고 이 긴장은 근육근막의 제한으로 신체에 기묘한 비특이적 반응으로 나타나 피부분절과 일치하지 않고 무관한 것처럼 보이나 통증이나 기능이상을 초래하여 그 인접관절과 근육에 기능적으로 방해가 되어 중력으로부터 정상적인 자세를 유지하지 못하고 신체의 불균형을 초래하게된다. 신체의 어느 한부분의 근육근막 제한은 인체전체의 균형을 파괴하여 골반 왜곡 또는 다리길이가 불일치한 경우도 나타나며 요천추부위에 긴장을 초래하게 되어 기능장애나 유연성이 감소되어 운동제한과 통증이 생기고 외상이 발생하기 쉽다. 1. 인체는 강력한 근막망(powerfull fascial network)의 상호관련된 통합된 전체로 보아야 한다. 2. 근막은 신체구조들의 응집을 방지, 지지해줄 뿐만아니라 외부로부터 충격을 적절히 분산하여 신체적 균형을 유지시킨다. 3. 근막의 장애는 근육긴장과 경축이 생기고 이완되지 않으면 근육결절로 진행하여 동통을 유발한다. 4. 근막장애는 탄력과 단축을 잃게하고 유연성이 없어져 중력으로부터 지지하는 능력이 떨어져 자세의 변화를 가져온다. 5. 국소부위의 근막장애는 중력으로부터 지탱하려는 인접관절과 근육에 영향을 미쳐 인체전체의 자세에 뷸균형을 초래한다. 6. 자세분석은 근육근막장애에 따른 중력의 역학적 변화를 고려하여 평가하여 분석하여야 것이다.
