A polystyrene phantom was developed following the guidance of the International Atomic Energy Association (IAEA) for gamma knife (GK) quality assurance. Its performance was assessed by measuring the absorbed dose rate to water and dose distributions. The phantom was made of polystyrene, which has an electron density (1.0156) similar to that of water. The phantom included one outer phantom and four inner phantoms. Two inner phantoms held PTW T31010 and Exradin A16 ion chambers. One inner phantom held a film in the XY plane of the Leksell coordinate system, and another inner phantom held a film in the YZ or ZX planes. The absorbed dose rate to water and beam profiles of the machine-specific reference (msr) field, namely, the 16 mm collimator field of a GK PerfexionTM or IconTM, were measured at seven GK sites. The measured results were compared to those of an IAEA-recommended solid water (SW) phantom. The radius of the polystyrene phantom was determined to be 7.88 cm by converting the electron density of the plastic, considering a water depth of 8 g/cm2. The absorbed dose rates to water measured in both phantoms differed from the treatment planning program by less than 1.1%. Before msr correction, the PTW T31010 dose rates (PTW Freiberg GmbH, New York, NY, USA) in the polystyrene phantom were 0.70 (0.29)% higher on average than those in the SW phantom. The Exradin A16 (Standard Imaging, Middleton, WI, USA) dose rates were 0.76 (0.32)% higher in the polystyrene phantom. After msr correction factors were applied, there were no statistically significant differences in the A16 dose rates measured in the two phantoms; however, the T31010 dose rates were 0.72 (0.29)% higher in the polystyrene phantom. When the full widths at half maximum and penumbras of the msr field were compared, no significant differences between the two phantoms were observed, except for the penumbra in the Y-axis. However, the difference in the penumbra was smaller than variations among different sites. A polystyrene phantom developed for gamma knife dosimetry showed dosimetric performance comparable to that of a commercial SW phantom. In addition to its cost effectiveness, the polystyrene phantom removes air space around the detector. Additional simulations of the msr correction factors of the polystyrene phantom should be performed.
토모를 이용한 회전 방사선치료 시 2차원적인 선량분포 평가 대신 3차원적 선량분포 평가의 필요성에 관하여 연구하였다. 토모 치료 부위의 정확한 선량분포를 측정하기 위하여 RANDO phantom을 이용하였으며, 평가 대조군으로 gafchromic EBT2 필름의 선량분포와 3차원 체적팬텀인 ArcCHECK phantom을 이용하여 3차원적인 선량분포를 gamma correction(3%/3 mm, 2%/2 mm)으로 평가하였다. 팬텀에 대한 치료 영역은 각각 0.5, 1, 1.5, 2, 2.5, 3 cm로 설정하였으며, 처방선량을 1,200 cGy로 하여 5회씩 선량을 조사하였다. Gafchromic EBT2 필름을 이용한 절대선량 측정 시 평균오차는 $0.76{\pm}0.59%$이었으며, ArcCHECK phantom을 이용한 절대선량 측정 시 평균오차는 $1.37{\pm}0.76%$로 나타났다. 선량분포의 평가에서 gafchromic EBT2 필름인 경우 gamma correction(3%/3 mm)은 평균 $97.72{\pm}0.02%$, ArcCHECK phantom인 경우 평균 $99.26{\pm}0.01%$로 측정되었다. 또한 gafchro mic EBT2 필름에서 gamma correction(2%/2 mm)의 평균은 $94.21{\pm}0.02%$이며, ArcCHECK phantom에서는 평균은 $93.02{\pm}0.01%$로 측정되었다. 토모치료를 이용한 환자 DQA에서 3차원 체적팬텀인 ArcCHECK phantom을 이용한 선량분포 평가가 cheese phantom을 이용한 선량분포 평가에 비하여 치료영역 주변부에 대한 정확한 측정과 실시간 평가가 가능하므로 환자의 치료가 보다 더 정확하고 빨리 이루어질 수 있을 것으로 사료된다.
When using a mobile X-ray unit, primary radiation creates medical images and secondary radiation scatters in many directions, which reduces image quality and causes exposure to patients, care givers and medical personnel. The purpose of this study was to develop a radiation shielding system for effectively shielding secondary radiation and evaluate its effectiveness. Using a mobile X-ray unit, spatial dose according to presence of human equivalent phantom and spatial dose using the developed shielding device were measured, and the phantom at 80 cm equidistance from center of X-ray was compared with spatial dose according to use of a shield. Measurements were taken at intervals of 10 cm every $30^{\circ}$ from the head direction($-90^{\circ}$) to the body direction($+90^{\circ}$). In the spatial dose measurement with and without the phantom, when the human equivalent Phantom was used, the spatial dose was increased by 40% in all directions from 40 cm to 100 cm from the central X-ray, and about 88% of the space dose was reduced when using the developed shields with the phantom. The equidistance dose at 80 cm from the central X-ray was increased by 39% from $5.1{\pm}0.26{\mu}Gy$ to $7.1{\pm}0.15{\mu}Gy$ when the human equivalent phantom was used, and when phantom was used and shielding was used, the spatial dose was reduced by about 90% from $7.1{\pm}0.15{\mu}Gy$ to $0.7{\pm}0.07{\mu}Gy$. The spatial dose of natural radiation was measured to be about $0.2{\pm}0.04{\mu}Gy$ when using the developed shielding with Phantom at a distance of 1 m or more. It is expected that by using the developed shielding system, it will be possible to effectively reduce secondary radiation dose received in all directions and to ensure safe imaging.
목 적: 불균질부를 포함하고 있는 치료부위의 치료계획 시 불균질 경계면에서의 TPS상 선량분포와 phantom을 이용하여 측정된 실제 선량분포를 비교하여 그 차이를 알아보고자 한다. 대상 및 방법: 4 cm 두께의 solid water phantom 사이에 폐와 유사한 밀도를 가진 8 cm 두께의 cork (density: 0.23 $g/cm^2$)를 위치시켜 phantom을 제작하여 CT 영상을 획득하였으며, 본원에서 사용하고 있는 Pinnacle 치료계획 시스템의 Collapsed-cone(CC) convolution 선량계산 알고리즘을 이용하여 6/15 MV 광자선으로 치료 계획된 선량분포와 실제 phantom에 EBT2 필름을 삽입해 측정한 선량을 비교 평가하였다. 또한 실제 폐암 환자와 유사한 치료계획을 비교하기 위해 Phantom 내부에 치료하고자 하는 종양부위(target volume)로 가정한 파라핀($3{\times}3{\times}3$ cm)을 Location "A" (일반조직과 떨어져있는 가상의 종양: 섬모델)와 Location "B" (일반조직과 붙어있는 가상의 종양: 반도모델)에 삽입하여 CT scan 후 치료계획을 시행하였다. 선량계획과 동일한 조건으로 Phantom을 set-up 후 Phantom의 paraffin target volume 경계면 A (Ant방향), B (Rt방향), C (Post 방향) point에 필름을 삽입하고 방사선을 조사하여 측정된 선량을 TPS선량과 비교평가 하였다. 결 과: 불균질 phantom을 이용한 계획선량과 측정선량과의 차이는 solid water와 cork 경계면을 제외한 부분에서 선량차이가 크지 않았지만 밀도가 급격히 변화하는 첫 번째 구간과 두 번째 구간에서 -5.4%~-12.6%의 선량감소를 보였다. 또한 paraffin target을 삽입한 실험에서는 Location "A"의 경우 실제 측정선량이 A, B, C point에서 각각 -2.5~-4.7%, -2.3~-2.8%, -4.5~-8.8%의 낮은 선량을 나타냈으며, Location "B"의 경우에도 A, B, C point에서 각각 0.08~5.27%, -3.17~-4.74%, -7.86~-11.56%의 선량 차이를 나타내었다. 결 론: 이번 연구의 결과 불균질부 내에서의 치료계획 시스템의 계획된 선량과 실제 측정된 선량에 오차의 가능성이 확인되었다. 급속도로 발전하고 있는 방사선 치료기술과 그만큼 정밀함을 요하는 치료계획 시 이러한 가능성에 대해 인지하고 선량검증에 대한 여러 방법들을 연구하고 개발하는 것이 치료의 발전과 필수적으로 동반되어야 할 것이며 본원에서도 이번 연구를 통해 치료계획 시 발생할 수 있는 변수에 대해서 더 주의 깊게 판단하고 적용할 수 있을 것으로 사료된다.
The absolute absorbed dose can be determined according to the measurement conditions ; measurement material, detector, energy and calibration protocols. The purpose of this study is to compare the absolute absorbed dose due to the differences of measurement condition and calibration protocols for photon beams. Dosimetric measurements were performed with a farmer type PTW and NEL ionization chambers in water, solid water, and polystyrene phantoms using 6MV photon beams from Siemens linear accelerator. Measurements were made along the central axis of $10{\times}10cm$ field size for constant target to surface distance of 100cm for water, solid water and polystyrene phantom. Theoretical absorbed dose intercomparisons between TG21 and IAEA protocol were performed for various measurement combinations on phantom, ion chamber, and electrometer. There were no significant differences of absorbed dose value between TG2l and IAEA protocol. The differences between two protocols are within $1\%\;while\;the\;average\;value\;of\;IAEA\;protocol\;was\;0.5\%$ smaller than TG2l protocol. For the purpose of comparison, all the relative absorbed dose were nomalized to NEL ion chamber with Keithley electrometer and water phantom, The average differences are within $1\%,\;but\;individual\;discrepancies\;are\;in\;the\;range\;of\;-2.5\%\;to\;1.2\%$ depending upon the choice of measurement combination. The largest discrepancy of $-25\%$ was observed when NEL ion chamber with Keithley electrometer is used in solid water phantom. The main cause for this discrepancy is due to the use of same parameters of stopping power, absorption coefficient, etc. as used in water phantom. It should be mentioned that the solid water phantom is not recommended for absolute dose calibration as the alternative of water, since absorbed dose show some dependency on phantom material other than water. In conclusion, the trend of variation was not much dependent on calibration protocol. However, It shows that absorbed dose could be affected by phantom material other than water.
High dose rate (HDR) brachytherapy in the treatment of cervix carcinoma has become popular, because it eliminated many of the problems with conventional brachytherapy. In order to improve clinical effectiveness with HDR brachytherapy, dose calculation algorithm, optimization procedures, and image registrations should be verified by comparing the dose distributions from a planning computer and those from a humanoid phantom irradiated. Therefore, the humanoid phantom should be designed such that the dose distributions could be quantitatively evaluated by utilizing the dosimeters with high spatial resolution. Therefore, the small size of thermoluminescent dosimeter (TLD) chips with the dimension of 1/8" and film dosimetry with spatial resolution of <1mm used to measure the radiation dosages in the phantom. The humanoid phantom called a pelvic phantom is made of water and tissue-equivalent acrylic plates. In order to firmly hold the HDR applicators in the water phantom, the applicators are inserted into the grooves of the applicator supporters. The dose distributions around the applicators, such as Point A and B, can be measured by placing a series of TLD chips (TLD-to- TLD distance: 5mm) in three TLD holders, and placing three verification films in orthogonal planes.
This study uses digital imaging and communications in medicine (DICOM) files acquired after CT scan to obtain the absorbed dose distribution inside the body by using the patient's actual anatomical data; uses geometry and tracking (Geant)4 as a way to obtain the accurate absorbed dose distribution inside the body. This method is easier to establish the radioprotection plan through estimating the absorbed dose distribution inside the body compared to the evaluation of absorbed dose using thermo-luminescence dosimeter (TLD) with inferior reliability and accuracy because many variables act on result values with respect to the evaluation of the patient's absorbed dose distribution in diagnostic imaging and the evaluation of absorbed dose using phantom; can contribute to improving reliability accuracy and reproducibility; it makes significance in that it can implement the actual patient's absorbed dose distribution, not just mere estimation using mathematical phantom or humanoid phantom. When comparing the absorbed dose in polymethly methacrylate (PMMA) phantom measured in metal oxide semiconductor field effect transistor (MOSFET) dosimeter for verification of Geant4 and the result of Geant4 simulation, there was $0.46{\pm}4.69%$ ($15{\times}15cm^2$), and $-0.75{\pm}5.19%$ ($20{\times}20cm^2$) difference according to the depth. This study, through the simulation by means of Geant4, suggests a new way to calculate the actual dose of radiation exposure of patients through DICOM interface.
뇌출혈 진단 방법 중 CT는 비침습적으로 피사체의 3차원 영상을 제공할 수 있다. 그래서 응급실에서 급성인 환자 상대로 많이 사용되고 중요한 역할을 담당하고 있다. 뇌혈관 CT는 다른 혈관 CT에 비해 비교적 촬영 빈도가 높으며 뇌혈관 CT 검사 시 적절한 SNR, 합리적인 유효선량으로 검사를 해야한다. 뇌혈관 CT 검사 시 이중에너지와 단일에너지를 이용하였을 때 실질적으로 어느 것이 유효선량이 적으며, SNR이 차이가 없는지 환자영상과 Phantom영상을 같이 비교하였다. SNR과 CNR의 P값이 0.05이상일 때 통계적으로 차이가 없다고 보았고, 유효선량은 0.05미만일 경우 통계적으로 차이가 있다고 보았다. 실험에서는 병원영상의 환자선량을 비교하였을 때 이중에너지의 유효선량이 53.53% 적게, Phantom의 OLSD 이중에너지 유효선량이 57.94% 적게, Phantom의 Dose Report의 이중에너지 유효선량이 56.04% 적었다. 그래서 뇌혈관조영 CT는 이중에너지를 권장한다.
방사선 치료분야에서 선량 보상체가 널리 이용되고 있으나, 그 보상효과에 대한 확실한 검증 방법은 알려진 바가 거의 없다. 본 연구에서는 Missing Tissue 뿐 아니라, Internal Tissue Inhomogeneity 까지 고려한 3차원 보상체를 제작하고, Exit Beam Dose Profile의 측정값과 본 연구에서 고안한 방법으로 기대값을 구해 비교함으로써 보상체의 성능을 평가하고자 하였다. 환자정보는 CT Simulator를 사용하여 얻었고, 보상체 정보는 Render Plan 3-D Planning System을 통해 얻었다. Computer Controlled Milling Machine으로 알루미늄 보상체를 제작해서 보상체가 있는 경우와 없는 경우의 선량 프로파일을 측정하여 비교하였다. 측정은 폴리스티렌 팬톰 사이에 필름을 삽입하여 팬톰 내에서의 실제 선량 분포를 구하고, 필름 카셋트를 이용해서 Exit Beam Dose Profile 을 동시에 얻었다. Oblique Beam, Parallel Opposing Beam, Inhomogeneus Human Phantom에 대해 제작된 보상체가 각각 선량보상 효과가 잘 나타남을 볼 수 있었고, 이 연구에서의 성능확인 방법을 통해 보상체의 성능을 확인할 수 있었다.
본 연구의 목적은 128 MDCT(Multi-detector computed tomography)의 CareDose 4D 선량감소 효과와 임상적 유용성을 평가하고자 시행되었다. Phantom과 임상 복부 검사 연구라는 두 가지 방법을 통해 128 MDCT CareDose 4D 시스템 적용 전후의 피사체 피폭선량과 영상 평가를 실시하였다. Phantom 연구에서는 CareDose 4D 적용 전후의 두 그룹에 대하여 중앙과 3, 6, 9, 12시의 방향으로 ROI(Region of interest)를 위치시켰고, 임상 연구에서는 간의 각 8구획에 대하여 CareDose 4D 적용 전 후 두 그룹에 대하여 ROI를 위치하여 CT Number, Noise, DLP(Dose length product)선량을 측정하였다. 측정결과 Phantom 및 임상연구에서 CT Number는 적용 전 후 값에 차이가 없었고(p>.05), 선량관련 CTDIvol(CT dose index volume) 측정값 및 유효선량은 CareDose 4D 적용 후가 낮았다 (p<.05). 결론적으로 CareDose 4D를 사용하면 영상의 화질을 저하시키지 않고 최적의 영상정보를 획득하면서 환자 선량이 감소되는 효과를 얻을 수 있다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
