치매의 FDG PET 영상 (FDG PET Imaging For Dementia)

  • Dementia is a major burden for many countries including South Korea, where life expectancy is continuously growing and the proportion of aged people is rapidly growing. Neurodegenerative disorders, such as, Alzheimer disease, dementia with Lewy bodies, frontotemporal dementia, Parkinson disease, progressive supranuclear palsy, corticobasal degeneration, Huntington disease, can cause dementia, and cerebrovascular disease also can cause dementia. Depression or hypothyroidism also can cause cognitive deficits, but they are reversible by management of underlying cause unlike the forementioned dementias. Therefore these are called pseudodementia. We are entering an era of dementia care that will be based upon the identification of potentially modifiable risk factors and early disease markers, and the application of new drugs postpone progression of dementias or target specific proteins that cause dementia. Efficient pharmacologic treatment of dementia needs not only to distinguish underlying causes of dementia but also to be installed as soon as possible. Therefore, differential diagnosis and early diagnosis of dementia are utmost importance. F-18 FDG PET is useful for clarifying dementing diseases and is also useful for early detection of the diseases. Purpose of this article is to review the current value of FDG PET for dementing diseases including differential diagnosis of dementia and prediction of evolving dementia.

PET/CT 영상 움직임 보정 (Motion Correction in PET/CT Images)

  • PET/CT fused image with anatomical and functional information have improved medical diagnosis and interpretation. This fusion has resulted in more precise localization and characterization of sites of radio-tracer uptake. However, a motion during whole-body imaging has been recognized as a source of image quality degradation and reduced the quantitative accuracy of PET/CT study. The respiratory motion problem is more challenging in combined PET/CT imaging. In combined PET/CT, CT is used to localize tumors and to correct for attenuation in the PET images. An accurate spatial registration of PET and CT image sets is a prerequisite for accurate diagnosis and SUV measurement. Correcting for the spatial mismatch caused by motion represents a particular challenge for the requisite registration accuracy as a result of differences in PET/CT image. This paper provides a brief summary of the materials and methods involved in multiple investigations of the correction for respiratory motion in PET/CT imaging, with the goal of improving image quality and quantitative accuracy.

판정행렬분석을 통한 PET-MRI의 간암 진단성능 평가 (Evaluation of the Liver Cancer Diagnosis Function of PET-MRI Based on Decision Matrix Analysis)

  • 최근 임상에서 활용하고 있는 일체형 PET-MRI의 간암 진단능력을 평가하기 위해 $2{\times}2$ 판정행렬을 이용하여 유용성을 평가하였다. 실험대상의 PET-MRI 검사 결과를 통해 간암 판정 여부 즉 비정상과 정상 판정을 받은 경우는 각각 98건, 51건 이었으며, 세포병리학적 결과가 양성과 음성 판정을 받은 경우는 각각 103건, 62건으로 나타났다. 이 중 두가지 검사에서 진양성의 경우는 95건, 위양성은 3건으로 나타났으며, 진음성은 62건, 위음성의 경우는 5건으로 분석되었다. 실험결과 PET-MRI 검사의 예민도는 95.00%, 특이도는 95.38%, 위음성률은 0.05%, 위양성률은 0.05%, 정확도는 95.15%로 분석되었다. 따라서 간암의 진단에 있어 수술 전 병기 결정이나 치료 후 재발 및 원격전이의 발견, 불분명한 원발 림프절 전이 등의 평가 등에 활용 가능성이 높을 것으로 판단되며, 특히 병리학적 검사와의 복합적 진단 및 추적검사를 통해 간암 진단을 위한 PET-MRI 임상적 유용성은 충분할 것으로 사료된다.

Hybrid Fuzzy Association Structure for Robust Pet Dog Disease Information System

  • As the number of pet dog-related businesses is rising rapidly, there is an increasing need for reliable pet dog health information systems for casual pet owners, especially those caring for older dogs. Our goal is to implement a mobile pre-diagnosis system that can provide a first-hand pre-diagnosis and an appropriate coping strategy when the pet owner observes abnormal symptoms. Our previous attempt, which is based on the fuzzy C-means family in inference, performs well when only relevant symptoms are provided for the query, but this assumption is not realistic. Thus, in this paper, we propose a hybrid inference structure that combines fuzzy association memory and a double-layered fuzzy C-means algorithm to infer the probable disease with robustness, even when noisy symptoms are present in the query provided by the user. In the experiment, it is verified that our proposed system is more robust when noisy (irrelevant) input symptoms are provided and the inferred results (probable diseases) are more cohesive than those generated by the single-phase fuzzy C-means inference engine.

Recent Update of Advanced Imaging for Diagnosis of Cardiac Sarcoidosis: Based on the Findings of Cardiac Magnetic Resonance Imaging and Positron Emission Tomography

  • Sarcoidosis is a multisystem disease characterized by noncaseating granulomas. Cardiac involvement is known to have poor prognosis because it can manifest as a serious condition such as the conduction abnormality, heart failure, ventricular arrhythmia, or sudden cardiac death. Although early diagnosis and early treatment is critical to improve patient prognosis, the diagnosis of CS is challenging in most cases. Diagnosis usually relies on endomyocardial biopsy (EMB), but its diagnostic yield is low due to the incidence of patchy myocardial involvement. Guidelines for the diagnosis of CS recommend a combination of clinical, electrocardiographic, and imaging findings from various modalities, if EMB cannot confirm the diagnosis. Especially, the role of advanced imaging such as cardiac magnetic resonance (CMR) imaging and positron emission tomography (PET), has shown to be important not only for the diagnosis, but also for monitoring treatment response and prognostication. CMR can evaluate cardiac function and fibrotic scar with good specificity. Late gadolinium enhancement (LGE) in CMR shows a distinctive enhancement pattern for each disease, which may be useful for differential diagnosis of CS from other similar diseases. Effectively, T1 or T2 mapping techniques can be also used for early recognition of CS. In the meantime, PET can detect and quantify metabolic activity and can be used to monitor treatment response. Recently, the use of a hybrid CMR-PET has introduced to allow identify patients with active CS with excellent co-localization and better diagnostic accuracy than CMR or PET alone. However, CS may show various findings with a wide spectrum, therefore, radiologists should consider the possible differential diagnosis of CS including myocarditis, dilated cardiomyopathy (DCM), hypertrophic cardiomyopathy, amyloidosis, and arrhythmogenic right ventricular cardiomyopathy. Radiologists should recognize the differences in various diseases that show the characteristics of mimicking CS, and try to get an accurate diagnosis of CS.

유방암에서 $^{18}F$-FDG PET/CT의 pSUV와 병리학적 진단과의 연관성 (Relationship Between pSUV of $^{18}F$-FDG PET/CT and Pathological Diagnosis in Breast Cancer)

  • 이 연구는 $^{18}F$-FDG PET/CT의 SUV에 영향을 미치는 병리학적 요인들을 알아보는 것이다. 2012년 09월부터 2013년 07월까지 한 대학병원에서 유방암으로 진단되어 $^{18}F$-FDG PET/CT를 시행하고 외과적 수술을 받은 39명을 대상으로 하여, pSUV와 종양크기, 조직학적 등급, 핵등급, 호르몬 수용체와 C-erbB-2의 발현 상태 간의 관계를 분석하였다. 연구결과 종양의 크기가 클수록 pSUV가 높은 것으로 나타났으나 크기에 따른 통계적 유의성은 없는 것으로 나타났다. 조직학적 등급은 고등급의 pSUV가 더 높은 것으로 나타났으며 유의한 양의 상관관계가 있었다.ER( estrogen receptor), PR( progesteron receptor )은 양성에 비해 음성의 pSUV가 더 높았으며 유의한 음의 상관관계가 있었다. 유방암에서 $^{18}F$-FDG PET/CT의 pSUV는 조직학적 등급, ER과 PR과 상관관계가 있었다. 따라서 유방암 초기 진단에 있어 $^{18}F$-FDG PET/CT를 이용한다면 조직검사나 수술을 통해 얻을 수 있는 조직학적 정보를 수술 전에 예측하여 치료방향에 도움을 줄 수 있을 것으로 생각된다.

  • This review focuses on the clinical use of $^{18}F$-FDG PET to evaluate multiple myeloma. $^{18}F$-FDG PET is useful for diagnosis, staging of multiple myeloma and differential diagnosis of myeloma related disease such as monoclonal gammopathy of undetermined significance or plasmacytoma. For therapy response, $^{18}F$-FDG PET may be effective after chemotherapy for multiple myeloma and radiotherapy for plasmacytoma.

살모넬라 감염성 복부 대동맥류 환자에서 F-18 FDG PET/CT의 역할 (Role of F-18 FDG PET/CT in the Management of Infected Abdominal Aortic Aneurysm due to Salmonella)

  • We present a case of infected abdominal aortic aneurysm due to salmonella enteritidis. F-18 FDG PET/CT was performed to diagnosis and during follow-up after antibiotic treatment. Computed tomography (CT) is considered to be the best diagnostic imaging modality in infected aortic lesions. In this case, a combination of CT and FDG PET/CT provided accurate information for the diagnosis of infected abdominal aortic aneurysm. Moreover, FDG PET/CT made an important contribution to monitoring disease activity during antibiotic treatment.

  • With the development of systemic diagnostic technique in cancer, the diagnostic methods of head and neck region are developing, also. Now, it is usually used computed tomography(CT), magnetic resornance image(MRI) in head and neck cancer and positron emission tomography(PET) is being increased in diagnostic use because of tumor specificity and accuracy. However, CT and MRI show the advantage of showing precise anatomical landmarks, but the disadvantage of these methods is much affecting by anatomical variations and changes. Otherwise, PET presents the imaging of physiologic and biochemical phenomenon and the disadvantage is the difficult differentiation of normal physiologic uptake, the lack of normal anatomical landmarks. PET/CT, the combination of clinical PET and CT imaging in a single unit is introduced recently, and it helps to get more accurate diagnostic interpretation and to improve in evaluating response to therapy, in management of patients with malignant tumors. So, we report the advantages of PET/CT in the diagnosis of oral cancer with review of literatures.

이중시간 PET/CT 검사에서 췌장암 판별을 위한 최적의 Scan time 분석 (Optimal Scan time Analysis for Pancreatic Cancer Distinction in Dual time PET-CT Exam)

  • 본 연구에서는 이중 시간 PET/CT 검사에서 지연검사 시간이 다른 두 그룹의 40명의 췌장암 환자에 대한 SUV 데이터의 통계 분석을 통해 최적의 지연검사 시간을 제시하였다. 췌장암의 유무를 확인해야 될 이중 시간 PET/CT 검사에서 새로운 수학적 예측 모델을 만들어 정확도 높은 최적의 지연검사 시간을 제시하였다. 시간에 따른 포도당 대사를 적용한 방사능 붕괴식을 예측 모델로 제시하였다. 지연검사 시간이 다른 두 그룹의 데이터를 가지고 시뮬레이션한 결과 PET/CT에서 췌장암 판별을 위한 최적의 지연검사 시간이 131.5분으로 제시하였다. 제안된 지연검사 시간은 췌장암 결정에 적용될 수 있는 통계적 유의 수준을 보였다(p<0.05). 제안된 지연 검사 시간 131.5분을 결정하여 검사를 진행할 때, 췌장암 조기 판별에 도움을 줄 수 있을 것으로 사료된다.
