경북지방(慶北地方)의 농약중독(農藥中毒)에 대한 역학적(疫學的) 조사(調査) (Epidemiology of Pesticide Poisoning in Kyungpook)

  • 농약사용량(農藥便用量)의 증가(增加)에 따라 농약중독자(農藥中毒者)가 전 세계적으로 증가(增加)하는 추세인데 우리나라에서는 아직까지 농약중독자(農藥中毒者)에 대한 역학조사(疫學調査) 보고(報告)가 없는 실정이다. 1981년(年)부터 1982년(年)까지 2년간(年間) 경상북도(慶尙北道) 전지역(全地域)을 대상으로 의원(醫院), 병원(病院) 및 보건소(保健所)의 의무기록(醫務記錄)을 조사(調査)하여 농약중독자(農藥中毒者)를 파악하여 그 실태를 분석(分析)하였다. 1981년(年)에 765명, 1982년에 853명의 중독자(中毒者)가 발생(發生)하였다. 연령별(年令別)로는 20대(代)가 제일 많았고 성별(性別)로는 남자가 70% 여자가 30%였다. 중독경위별로는 직업적인 중독이 27.8%, 우연에 의한 사고(事故)가 5.6%였고 자살목적(自殺目的)이 66.6% 였다. 월별(月別)로는 6, 7, 8, 9월(月)에 발생(發生)이 많고 7월(月)이 제일 많고, 월평균(月平均) 67명이 발생(發生)하였다. 의료기관이용(醫療機關利用)은 개인의원이 49%, 병원이 43%였다. 치명율(致命率)은 직업적인 중독이 0.9%, 사고(事故)에 의한 중독(中毒)이 5.6% 자살목적(自殺目的)의 중독(中毒)에서 20.3%로 평균(平均) 14.1%였다. 인구 100,000 당 농약중독(農藥中毒) 발생(發生)은 년간(年間) 25.4였다.

농약 노출과 건강 (Pesticide Exposure and Health)

  • Although pesticides have increased crop production and controlled disease, they have produced a number of adverse health effects. Pesticides have potential human toxicity and a variety of groups, such as farmers or industrial workers, as well as the general population, are exposed to pesticides. The purpose of this article, therefore, is to provide an overview of pesticide exposure and health through a literature review, focusing on exposure assessment, acute poisoning, chronic health effects, and future research needs. The exposure types and levels of pesticides vary by study subjects and need to be assessed by integrating several methods focused on the epidemiological purpose. Acute pesticide poisoning is a major public health problem in the world. Paraquat is the main causative pesticide for acute poisoning in Korea and should be banned in order to save several thousands of lives every year. Occupational pesticide exposure also causes numerous chronic diseases among farmers and industrial workers, including cancers, respiratory diseases, depression, retinal degeneration, diabetes, and abnormal menstrual cycle. However, controversy exists regarding the long-term effects of low-dose environmental pesticide exposure. The area of pesticide study is relatively new with broad study populations and it has received little academic and policy attention, particularly in Korea. More detailed studies investigating the risk of pesticide-related health effects and intervention efforts to reduce the problems are needed in Korea.

우리나라 폐사 야생조류에서의 농약 분석 (Determination of pesticides in dead wild birds in Korea)

  • Pesticides are extensively used for the control of crop pests in agriculture and forestry. Organophosphate (OP) and carbamate pesticides are especially effective for the control of a variety of harmful insects. However, these cholinesterase inhibitors are also dangerous to non-target organisms (wildlife and other animals) because of their high acute toxicity. Most poisonings by pesticides occur as a result of misuse or accidental exposure, but intentional killing of unwanted animals also occurs. At the request of a local autonomous entity, we investigated wild bird poisonings by pesticides from 2003 to 2007. The 207 suspicious samples of pesticide poisoning based on the necropsy were analyzed by GC/NPD, GC/FPD, or GC/MSD. We looked for trends in the identification of pesticides in wild birds thought to have died from poisoning. Pesticides were determined in 59% of the total samples analyzed. Phosphamidon and monochrotophos were the most common pesticides identified, which amounted to 77% of the subtotal. Other OP and carbamate pesticides were also found in various concentrations from dead wild birds. The determined rates of pesticides were as high as 86% and 76% in 2003 and 2006, respectively, during an outbreak of avian influenza in Korea.

의도성 중독의 변화 양상: 15년간의 후향적 단일병원 연구 (Trends of Intentional Poisoning: A Retrospective Single Center Study During 15 Years)

  • Purpose: Intentional poisoning is a major public health issue in many parts of the world. This study was conducted to provide details regarding the epidemiology of intentional poisoning in a metropolitan emergency department and to identify the changing patterns and epidemiology of poisoning. Methods: A retrospective study was conducted to evaluate intentional poisoning of patients who visited the emergency department in a tertiary teaching hospital between 2001 and 2015. All intentional poisoning-related emergency department visits over three five year periods (2001-2005 (P1), 2006-2010 (P2) and 2011-2015 (P3)) were reviewed to investigate trends in intentional poisoning patients. Information regarding patient sex, age, time from episode to admission, psychiatric history, type of intoxicants, alcohol co-ingestion, gastric lavage, charcoal administration, any previous suicide attempts, need for hospitalization and death before discharge was reviewed. Results: A total of 1269 patients were enrolled in this study. The number of patients admitted during each period was P1=515, P2=439 and P3=315. Comparison of the three groups revealed significant differences according to age (p<0.001), psychiatric history (p<0.001), alcohol co-ingestion (p=0.013), gastric lavage (p<0.001), charcoal administration (p<0.001), need for hospitalization (p=0.044), repeated attempt (p<0.001) and type of intoxicants (p<0.001). Conclusion: The average age of intentional poisoning patients has increased. While the use of sedatives and multiple drugs increased, the use of pesticides and the antihistamine decreased.

충남 일부 지역 병의원의 농약살포 중 중독 사례 분석 및 보건관리방안 연구 (A Study on the Case Analysis and Health Management of Patients with Pesticide Poisoning from Spraying Pesticide in Hospitals in the Chungnam, Korea)

  • Objectives: This study aims to examine patients who developed acute pesticide poisoning while spraying or using pesticide and presented to the emergency department in hospitals in Chungcheongnam-do Province. Based on the findings, this study will provide implications for safety and health management pertaining to the use of pesticides. Methods: Pesticide poisoning data collected by the Chungnam Center for Farmers' Safety and Health from 2014 to 2018 was cross-sectionally analyzed. A total of 331 patients with pesticide poisoning presented to one of hospitals and four of medical centers in the region(Dankuk University Hospital, Gongju and Hongsung, Cheongyang, Cheonan Medical Center). Seventeen of these patients (15 men and two women) developed poisoning while spraying pesticide. The patients' charts were reviewed to collect data on pesticide poisoning, namely currently working in farming-related occupation, means of transportation to the hospital, place of poisoning, symptoms of acute pesticide poisoning, treatment, pesticide used when poisoning occurred, and classification by technical ingredients. Results: Fifteen out of 17 patients who developed poisoning while spraying pesticide were men. Insecticide was used in 35.3% of the cases, and herbicide was used in 29.4% of the cases, which was different from cases of poisoning from ingestion poisoning. The major symptoms were vomiting (35.3%), nausea (29.4%), dizziness (29.4%), and headache (23.5%). A total 11 ingredients were identified in 12 patients, and the most common ingredient was glyphosate, which is an herbicide. Most patients showed a state of mild toxicity, but two patients showed a state of severe toxicity. These patients respectively used glyphosate and paraquat. Conclusions: Our findings can be useful for suggesting the need for a national healthcare system to manage occupational pesticide poisoning among farmers. Further, these findings can be used to increase the awareness of the risk of acute poisoning during pesticide spraying and suggest the need for a safety health education to increase farmers' awareness of pesticide poisoning.

Pesticide Poisoning Deaths Detected at the National Forensic Service Headquarters in Seoul of Korea: A Five-Year Survey (2005-2009)

  • Objectives : The records of 447 pesticide poisoning deaths from the National Forensic Service (NFS) headquarters located in Seoul of Korea from 2005 to 2009 were retrospectively reviewed. Methods : The data of each case were analyzed by using the SPSS program. Results : The mean age was $57.8{\pm}14.8$ years and the range was 16-92 years. The numbers of deaths of males and females were 301 and 134, respectively. The largest number of cases occurred in people aged 50-59 years (n=92, 20.6%) followed by the age groups 40-49 years (n=91, 20.4%), 60-69 years (n=88, 19.7%), and 70-79 years (n=75, 16.8%). The total number of deaths among other age groups (10-19, 20-29, 30-39, 80-89, and 90-99 years) was 73, representing only 16.3%. Of all pesticide poisoning deaths, 96.2% were due to suicide, and 28.4% of the total number who died received medical treatment. The mostfrequent site of ingestion was the person's own residence (n=279, 62.4%). The most common classes of pesticide were bipyridylium herbicide (paraquat, 31.1%), organophosphate insecticide (21.7%), and carbamate insecticide (15.4%). The major pesticides having a high proportion of fatalities were paraquat (31.1%), methomyl (11.4%), glyphosate (9.1%), dichlorvos (5.6%), phosphamidon (4.6%), and methidathion (4.0%). Conclusions : This study showed that poisoning deaths due to pesticides are one of the major public health problems in Korea. Enforcement of regulations and safety education to prevent pesticide poisoning should be carried out by the government.

국내 농약중독 사망사고 현황분석과 사고 예방대책 (Examination of Pesticide Poisoning Deaths Statistics in Korea and Precautionary Measures Against Pesticide-Poisoning)

  • 매년 증가하고 있는 농약중독사고 예방대책 마련을 위하여 통계청 마이크로데이터시스템에서 2000년${\sim}$2005년 까지 최근 6년간의 자료를 제공받아 국내 농약중독사망사고 현황을 분석하였다. 농약중독 사망자수는 제초제 및 살균제에 의한 사망수가 가장 많았고, 약제별로는 특히, 패러�R디클로라이드 액제, 유기 인계 농약, 글라이포세이트 순으로 조사되었다. 전체약물중독사고 사망자중 농약으로 인한 사망자가 매년 증가하여 2005년에 68.5% 를 차지하였다. 농약중독사고 원인은 자살 비율이 $80.5{\sim}91.9%$로 농약에 의한 자살을 예방할 수 있는 대책 마련이 시급한 실정이다. 지역별 농약중독사망자수를 분석한 결과 주요 7대 도시지역이 전체 사망자수의 평균 21.6%를 차지하였고, 전국 9개 도지역에서 평균 78.4%를 차지하였으며, 직업별 분포는 농림, 어업인이 27.4%, 주부와 학생을 포함한 무직 등 비농업인이 72.6%로 조사되었다. 이상의 자료분석결과, 농약중독사망자수가 매년 증가하는 경향을 보이고 있으므로, 사망자수를 줄이고 농약중독 사고를 예방하기 위해서 농업 인에게는 농약안전사용을 위한 교육을 강화하고, 비농업 인에게는 독성이 강한 농약의 구입을 제한하는 등 철저한 농약관리가 필요한 것으로 판단된다.

일부 농촌지역의 農藥中毒 態度에 관한 연구 (A Study on Pesticide Poisoning in Rural Area)

  • In order to offer an basic data for the prevention of pesticide poisoning this study was carried out at Chun Song County, Kang Won Province for the period from 1983 to 1985. The major results are as follows: 1. The rates of those who read an explanation of pesticidesbefore scattering are 76.5% in 1983, 82.6% in 1984, 85.8 % in 1985 but among these people the rates of those who practice as explanation are 78.2%, 72.2%, 41.4% every year. 2. More than 50% of those who scatter pesticides feel pesticide poisoning. 3. The difference in average time of pesticide scattering and pesticide poisoning reveals statistical significance (P < 0.05). 4. Among those who feel pesticide poisoning, 85.5% in 1984, 87.3% in 1985 are due to insecticides.

농업인의 농작업 위험 수용도 및 관련 요인 (Risk-Accepting Personality and Associated Factors among Korean Farmers)

  • Objectives: This study aimed to examine risk-accepting personality traits and associated factors among farmers. Methods: Data were collected from farmers aged over 40 living in Goryeong, North Gyeongsang-do Province, Korea. Study participants were enrolled in the Korean Rural Cohort study from 2011 to 2014. Surveys for the risk perception of farming were made through face-to-face interviews. Chi-square tests and logistic regression analysis were conducted adjusting for gender, age, marital status, and education. Results: Several factors such as gender, age, marital status, age first worked as a farmer, duration of farming, and history of pesticide poisoning were associated with risk-accepting personality traits among farmers. Farmers with risk-accepting personalities tend to be older, less educated, and have self-employed health insurance. Farmers who started farming after the age of 50 are likely to be risk averse compared to farmers who started farming before the age of 20 (OR=0.41, 95% CI; 0.21, 0.82). Frequent spraying of pesticides was marginally associated with higher risk acceptance among female farmers (OR=1.61, 95% CI: 0.99, 2.64). Male farmers who had experienced pesticide poisoning in their lifetime showed higher odds of risk-accepting personality traits (OR=2.20, 95% CI: 1.03, 4.72) Conclusion: Risk-acceptors were more likely to spray pesticides frequently and to experience more pesticide poisoning compared to risk-averse individuals. This result suggests that farming hazards are to some degree driven by risk-accepting personality traits. Further investigation of how risk-accepting behaviors influence farming practices and pesticide poisoning is needed.

급성 농약 중독환자에서 호흡 부전 발생의 위험 인자 (Risk Factors to Predict Acute Respiratory Failure in Patients with Acute Pesticide Poisoning)

  • Acute respiratory failure is an important risk factor for mortality in patients with acute pesticide poisoning. Therefore, it is necessary to investigate the risk factors to predict respiratory failure in these patients. This study retrospectively investigated the clinical features of respiratory failure among patients with acute pesticide poisoning requiring mechanical ventilation. This study included patients who were admitted with intentional poisoning by pesticide ingestion from January 2017 to December 2019. Paraquat intoxication was excluded. Among 469 patients with acute pesticide poisoning, 398 patients were enrolled in this study. The respiratory failure rate was 30.4%. The rate of respiratory failure according to the type of pesticide was carbamate (75.0%), organophosphate (52.6%), glufosinate (52.1%), glyphosate (23%), pyrethroid (8.9%), and others (17%). The mortality was 25.6% in the respiratory failure group. The risk factors for respiratory failure were old age, low body mass index, and ingestion of more than 300 mL. In conclusion, respiratory failure is a risk factor for mortality in pesticide poisoning. Old age, low body mass index, and ingestion of more than 300 mL are the risk factors for predicting respiratory failure.
