Objective : The purpose of this study was to get the mean of duration of post-rotatory nystagmus test in normal children and to differentiate the duration of post-rotatory nystagmus test between normal children and children with pervasive developmental disorder. Method : 84 subjects were between 3 and 5 years of age and consisted of 64 normal children and 20 children with the pervasive developmental disorder. Analysis of the data was done by using t-test and ANOVA. Results : The results were as follows: 1. Range of duration of post-rotatory nyatagmus test in normal children was $5{\sim}22$second on left and $7{\sim}21$ second on right and the mean was 12.63 second on left and 12.59 second on right. 2. Range of duration of post-rotatory nystagmus test in children with the pervasive developmental disorder was $3{\sim}11$ second on both and the mean was 5.65 second on left and 5.60 second on right. 3. There was no significant difference between males and females with normal children in duration of post-rotatory nystagmus test. However, there was a significant difference of the mean duration between 3 and 5 years old normal group. 4. Children with pervasive developmental disorder significantly have relatively lower duration than the duration of post-rotatory nystagmus test of normal children. Conclusions : The results of the study showed significant difference between normal children and children with pervasive developmental disorder in duration of post-rotatory nystagmus test and suggest that they could be applied to the baseline of clinical therapy.

전반적 발달장애는 가장 심각한 소아정신과 장애중 하나로 발달의 여러가지 영역에 있어 이상 소견을 보인다. 약물치료로 전반적 발달장애를 완치 할 수 없으며 치료효과 역시 비특이적이다. 그러나 일부의 전반적 발달장애 아동에게 약물치료는 매우 중요하기도 하고, 행동치료나 교육에 임하는 데에 있어 휠씬 도움을 줄 수 있다. 지금까지 가장 많이 연구된 항정신병 약물인 halopcridol은 임상적으로나 통계적으로 확실히 위약보다 훌륭한 효과를 보이고 있고, 약물의 부작용을 초래하지 않으면서 변별학습이나 모방적 언어사용등 긍정적인 기능을 촉진하는 것으로 보고되고 있다. 그러나 이러한 halopcridol의 투여는 약물과 관련된 운동장애를 초래할 수 있기 때문에 보다 안전한 다른 약물을 찾고자 하는 이유가 된다. 지금까지의 몇몇 생화학적인 연구들은 전반적 발달장애 아동의 일부에서 대조군에 비하석 혈중내 세로토닌치가 높거나, 내인성 opioid 측정치가 높은 군이 있을 것이라는 보고를 하고 있다. 이러한 소견을 근거로 하여 약물치료가 시도 되기도 하였는데, 예를들면, fenfluramine이나 naltrexone등이 그러하다. 그러나 아직 까지의 결과는 결론적이지 못하다. 이 약물들과 이 밖에도 지금까지 전반적 발달장애 아동에게 사용되어진 약물들과 그들의 효과에 대하여 고찰하고자 한다.

전반적발달장애 환자와 발달성언어장애 환자의 뇌혈류량의 분포를 조사하여 각 질환의 국소적 뇌기능의 특성을 알아보고자 한다. 14명의 전반적발달장애 환자와 13명의 발달성언어장애 환자를 대상으로 뇌 technetium-99m-EDC SPECT를 촬영하여 2명의 핵의학 전문의가 판독하고, 대뇌의 측두엽, 두정엽, 기저핵, 시상, 소뇌를 관심 영역(region of interest)으로 하여 뇌혈류량을 양적으로 측정하여 양군을 비교한다. 뇌 SPECT 소견상 전반적발달장애와 발달성언어장애에서 모두 측두엽, 두정엽, 기저핵, 시상, 소뇌의 혈류량의 감소를 보였고, 혈류량을 양적으로 측정한 결과, 상기부위의 혈류량이 전반적발달장애군과 발달성언어장애군에서 유의한 차이를 보이지 않았다. 이는 전반적발달장애와 발달성언어장애는 뇌기능의 특정 국소부위의 이상이 아니고, 대뇌피질과 피질하부위의 신경발달-신경연결(interneural connection)-상의 이상에 의한 질환이며, 두 질환이 스펙트럼장애일 가능성을 시사한다.

Objectives: The purpose of this study was to analyze the discriminant validity and the clinical cut off scores of the Child Behavior Checklist 1.5-5 (CBCL 1.5-5) in the diagnosis of autism spectrum disorder (ASD) and non-ASD. Methods: In total, 104 ASD and 441 non-ASD infants were included in the study. T-test, discriminant analysis, receiver operating characteristic (ROC) curve analysis, and odds ratio analysis were performed on the data. Results: The discriminant validity was confirmed by mean differences and discriminant analysis on the subscales of Emotionally reactive, Somatic complaints, Withdrawn, Sleep problems, Attention problems, Aggressive behavior, Internalizing problems, Externalizing problems, and Total problems, along with the Diagnostic and Statistical Manual of Mental Disorders (DSM)-oriented scales between the two groups. ROC analysis showed that the following subscales significantly separated ASD from normal infants: Emotionally reactive, Somatic complaints, Withdrawn, Sleep problems, Attention problems, Aggressive behavior, Internalizing problems, Externalizing problems, Total problems, and DSM pervasive developmental problems. Moreover, the clinical cut off score criteria adopted in the Korean-CBCL 1.5-5 were shown to be valid for the subscales Withdrawn, Internalizing problems, Externalizing problems, Total problems, and DSM pervasive developmental problems. Conclusion: The subscales of Withdrawn, Internalizing problems, Externalizing problems, Total problems, and DSM pervasive developmental problems significantly discriminated infants with ASD.
This study was conducted to evaluate the effects of the program for the autistic children which was designed to promote mother-child attachment. The subjects of this study were consisted of 11 pairs of mother and child( 7 for participant group:PG, 4 for non-participant group: NPG), who were diagnosed as Pervasive Developmental Disorder. The intervention was consisted with play activity centered mother-child interaction, educational activity for mothers, lecture, video-feedback, and supports. The main data were collected by video-taping and analyzed by Wilcoxon Rank Sign Test, and Content Analysis. The results obtained were as follows, 1. Before the program, there were no significant group differences on the children's and the mother's characteristics. After the program, total score on the attachment of PG was higher than that of NPG, but not significant. Only proximity-seeking behaviors and contact-maintaining behaviors were higher significantly(p<.05). Then the characteristics on contact- maintaining behaviors of PG were lasting longer and reciprocal than those of NPG. 2. After the program, the score on mother's nurturing behavior of PG was significantly more increased. The mother's behaviors to her child of PG became more child- centered, positively responsive, expressive supportively. But there were pretty big individual difference. It can be concluded that Mother-child Attachment Promotion Program is effective. Thus it can be recommended to be a early intervention model for autistic children.

Objectives : The purpose of this study was to investigate the effectiveness of a day-hospital treatment program designed to help development of children with pervasive developmental disorder (PDD) and mental retardation (MR). Methods : Retrospective review of the charts of 32 children (28 with PDD, 4 with MR), who participated in a day-hospital treatment program of Seoul Metropolitan Eunpyeong Hospital, from October 2008 to February 2012, was conducted. Development level of each patient was evaluated according to the Psycho-Educational Profile-Revised (PEP-R), Social Maturity Scale (SMS), and Childhood Autism Rating Scale (CARS). Evaluation was done at two points, before participation and after 1 year participation. Results : Children who participated in the day-hospital treatment program showed significant improvement in all categories of PEP-R, SMS, and CARS. Conclusion : Day-hospital treatment program is effective for helping development of children with PDD and MR.

In addition, the clinical cutoff score criteria adopted in the Korean CBCL 1.5-5 for subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems were shown to be valid. Conclusion: The subscales of Withdrawn, Attention problems, Internalizing problems, Externalizing problems, Total problems, DSM pervasive developmental problems, DSM attention deficit/hyperactivity problems, and DSM oppositional defiant problems significantly discriminated for the diagnosis of ASD.
목적 : 본 연구의 목적은 정신장애 영유아의 K-CBCL 1.5-5(Korean-Child Behavior Checklist 1.5-5) DP(Dysregulation Profile)의 요인구조를 확인하고, 정신장애 진단 집단 간 차이를 검증하였다. 연구방법 : 서울 소재의 종합병원 소아정신과, 발달장애 클리닉 등에서 2010년에서 2015년까지 수집된 자료를 활용하였다. DSM-IV에 근거하여 정신장애로 진단된 영유아 265명의 어머니에게 실시된 K-CBCL 1.5-5 자료를 최종분석에 사용하였다. 결과 : 정신장애 영유아의 K-CBCL 1.5-5 DP의 요인구조는 이중요인 모형(Bifactor)이 상대적으로 가장 적합하였다(χ2 = 893.722, TLI = .782, CFI = .822, RMSEA = .064). 또한 의사소통장애, 전반적 발달장애, 정서장애, 발달지체 등 영유아의 정신장애 진단 집단 간 차이가 유의하였다(F(3, 259) = 9.780, p < .001). 그 중 전반적 발달장애 집단과 정서장애 집단이 의사소통장애 집단보다 높은 조절곤란을 보이는 것으로 확인되었다. 결론 : 본 연구에서는 정신장애 영유아의 조절곤란 문제의 존재를 확인하였다. 이중요인 모형을 통해 K-CBCL 1.5-5 DP가 불안/우울, 공격행동, 주의집중문제의 특정요인과 특정요인 이상의 일반요인으로 구성된다는 것을 확인하였고, K-CBCL 1.5-5 DP의 다차원성을 규명하였다. 또한 정신장애 집단 간 유의한 차이가 있었고 조절곤란 문제는 영유아기부터 전반적 발달장애와 정서장애의 증상 발현과 심화에 기여하는 것으로 사료된다.
목적 : 본 연구는 국외 학술지를 대상으로 체계적인 문헌고찰을 통해 자폐스펙트럼장애(autism spectrum disorder, ASD)아동들을 대상으로 사용되는 조기 중재 방법의 효과를 확인하는 것이었다. 연구방법 : 2017년 12월 이전까지의 국외 학술지에 게재된 논문을 Pubmed를 통하여 검색하였다. 주요 검색어로는 'autism', 'autism spectrum disorder', 'ASD', 'high function autism', 'high function ASD', 'Asperger syndrome', 'pervasive developmental disorder', 'PDDNOS', 'intervention'과 'early intervention'를 사용하였다. 최초 검색된 논문은 724편이었으나 포함 및 배제기준을 적용하여 최종 10편의 연구가 선정되었다. 결과 : 자폐스펙트럼장애 아동의 조기 중재는 10편의 선정된 연구 중에서 8편의 연구가 응용행동분석에 기초한 중재 프로그램을 적용하였고, 다른 2편의 연구들은 감각 통합적 접근과 테크놀로지에 기반을 둔 중재접근을 사용하였다. 대부분의 연구가 조기 중재로 의사소통기술과 사회적 상호작용기술, 지능, 적응 행동의 증가, 동시집중력의 향상과 같은 긍정적인 효과를 보고하였다. 결론 : 본 연구는 자폐스펙트럼장애 아동의 조기 중재에 대한 근거를 제시하며, 향후 국내 연구에서 효과적인 조기 중재에 대한 효과를 확인하는 연구가 필요할 것으로 사료된다.
Objectives This study reviewed the effects of a combined treatment with Kampo and Western medicine for Developmental disability in Japan, and the provision of education programs in clinical care. Methods The search database includes J-STAGE. To narrow the search, the following key words were used: 'pervasive developmental disorders, Attention-Deficit/Hyperactivity Disorder, Learning Disorders or Learning Disabilities, Intellectual Disability, and Kampo'. The search was limited to the publication date from 2001 to 2019. Results 1. Japan analyzed five sections: The usage of the Kampo medicine ranges from 25.2% to 71.6%, and the Kampo medicine was highly used in large cities. 2. In Japan, the educational programs were provided for the caregiver and special educational programs were available for children with disabilities. 3. In Japan, there were 9 studies regarding developmental disability treating with herbal remedies. There were seven clinical trial reports, and two were published in a review or report form. 4. The results showed benefits of using Kampo for patients with lack of Yin in blood in treatment of developmental disorder. It is also important to control the liver qi and Yin in blood. 5. Seven papers reported no side effects or abnormal findings. They have reduced the use of antipsychotics. Conclusions These review studies in regards to the combined treatment of Kampo and Western medicines can be helpful to improve long term side effects of the antipsychotics used in developmental disorders.
