비후된 비골 결절에 의해 발생한 비복신경 포착 및 장비골건의 건막염: 증례 보고 (Sural Nerve Entrapment and Tenosynovitis of Peroneus Longus by Hypertrophied Peroneal Tubercle: A Case Report)

  • A hypertrophied peroneal tubercle can present as a bony prominence at the lateral aspect of the foot and a peroneal tenosynovitis or tear. We report a case of a 52-year-old man complaining of lateral foot tingling pain and numbness. The sural nerve entrapment and peroneus longus tenosynovitis by hypertrophied peroneal tubercle were confirmed. Good results were obtained after excision of the hypertrophied peroneal tubercle and sural nerve release.

건강한 젊은 성인의 가자미근 신장이 앞정강근과 긴종아리근의 근두께와 근 긴장도에 미치는 영향 (The Effect of Soleus Muscle Stretching on Thickness and Muscle Tone of The Tibialis Anterior and Peroneus Longus Muscles in Healthy Young Adults)

  • Purpose: The purpose of this study was to investigate the effect of soleus muscle stretching on the muscle thickness and muscle tone of the tibialis anterior and peroneus longus muscles in healthy young adults. Methods: This study was an observational, cross-sectional study design in healthy young adults. Thirty healthy young adults participated in the study. To investigate the effect of agonist elongation on the muscles' antagonist and synergist characteristics, this study conducted the dynamic stretching of the soleus and plantarflexor muscles for 20 seconds. This study measured the muscle thickness and muscle tone of the soleus, tibialis anterior and peroneus longus muscles before stretching, immediately after stretching, and five minutes after stretching. Results: After analysis, the muscle tone of the soleus muscle was significantly decreased immediately after stretching (20.91±2.61Hz) compared to before stretching (21.83±2.78Hz). The muscle tone of the tibialis anterior was significantly decreased both immediately after stretching (21.76±2.73Hz) and five minutes after stretching (21.72±3.25Hz) compared to before stretching (22.61±3.29Hz). The muscle thickness of the soleus muscle was significantly decreased immediately after stretching (2.04±0.52mm) compared to before stretching (2.21±0.51mm) and was significantly increased five minutes after stretching (2.14±0.49mm) compared to immediately after stretching. Conclusion: The results of this study showed the static stretching of the soleus muscle changed the muscle tone of the tibialis anterior, but not of the peroneus longus muscle. This study suggests that the dynamic stretching of the agonist muscle would show meaningful muscle tone change in the antagonist.

Comparison of Subjects with and without Pes Planus during Short Foot Exercises by Measuring Muscular Activities of Ankle and Navicular Drop Height

  • PURPOSE: Despite the abundant literature available regarding the activity of intrinsic muscles, few studies have investigated the muscle activity of extrinsic muscles. Therefore, the present study compared the muscle activity of the peroneus longus, tibialis anterior, and abductor hallucis during short foot exercise in subjects with and without flat feet. METHODS: Twelve subjects with and without pes planus participated in this study. During the short foot exercises, muscular activity of the tibialis anterior, fibularis longus, and abductor hallucis longus were measured in both groups. To identify the effects of short foot exercises, navicular drop height was also investigated in pre and post short foot exercises. RESULTS: In a symptomatic group, the navicular drop height was significantly reduced at post measurement compared with pre-measurement. During the short foot exercise, the pes planus group showed significantly lower activities of the fibularis longus than the control group (p<.05). CONCLUSION: Similar to previous studies and clinical literature, short foot exercise was effective for alleviating navicular drop for a population with pes planus. In addition, subjects with pes planus showed decreased muscular activities of the fibularis longus, which suggests that considering extrinsic muscles such as fibularis longus is also important for rehabilitation of pes planus patients.

  • Purpose : The study was to evaluate the effect of joint therapy and active exercise on balance and lower foot and ankle muscle MVIC in supination foot. Methods : The subjects of this study were 20. Subjects were 20 to 25($22.20{\pm}1.54$) completed the study and participated three times a week for 4 weeks. Subjects were assessed by utilizing two different EMG MVIC. Results : The change in peroneus longus MVIC significant on pre-test and post test (p<.05). The not change in tibialis anterior, tibialis posterior, peroneus brevis MVIC on pre-test and post-test(p<.05). Conclusion : The study suggest that subtalar joint therapy and active exercise have a increase peroneus longus MVIC for supination foot. Therefore, the subtalar joint therapy and active exercise recommended for supination foot.

  • Background: The purpose of this study is to investigate the effect of the intensive functional electrical stimulation(FES) on the improvement of the gait pattern of the chronic hemiplegic patients. Method: Six hemiplegic patients, who could walk independently but have equinovarus deformity during the gait cycle, participated in this study. The affected peroneus longus and tibialis anterior muscles of all subjects were stimulated for 2 weeks period (20 minutes duration, 6 times/day). We measured the activities (mean voltage) of those muscles during the walking, using dynamic EMG. Results: After treatment, there were significant improvements in the strength of peroneus longus and tibialis anterior muscles and the gait speed, but there was no interval change of the spasticity of plantar flexor. The mean voltages of two muscles are significantly increased in all the patients (p<0.05). Conclusion: The results showed that the intensive FES on affected peroneus longus and tibialis anterior muscles in chronic hemiplegic patients could be useful for the improvement of functional gait.

스포츠 손상 후 비골건 봉합술 (Peroneal Tendon Repair in Sports Injury)

  • Purpose: This study examined the clinical outcomes and assessed the average time to return to play following a peroneal tendon repair in Korean athletes. Materials and Methods: Between March 2004 and February 2017, a total of 30 athletes underwent peroneal tendon repair for a peroneal tendon tear. The indications of surgical treatment were chronic pain or intractable symptoms after a previous ankle sprain affecting sports activity refractory to conservative treatment for at least six months. The patient underwent tubulization for a longitudinal tendon rupture. Peroneus longus to peroneus brevis tenodesis was performed when tendon repair was impossible due to total rupture or multiple longitudinal rupture. Results: Twenty patients not included in this study were as follows: insufficient follow-up, previous surgery, and additional bone surgery. All 10 patients had a previous ankle sprain history, tenderness and swelling on the retromalleolar area. In the 10 patient population, there were five peroneus brevis tendon tears, three peroneus longus tendon tears, one peroneus longus and brevis tendon tear, and one peroneus brevis and superior peroneal retinaculum tear. In the 10 patients, six cases of peroneal brevis tendon repair and four cases of peroneal longus to brevis tenodesis were performed. The preoperative American Orthopaedic Foot and Ankle Society score was improved from a mean of 60.6 (standard deviation [SD], 8.64) to a mean of 90.2, postoperatively (SD, 4.4; p<0.012). The preoperative visual analogue scale was improved from a mean of 5.43 (SD, 1.2) to 0.5 (SD, 0.16), postoperatively (p<0.023). The mean length of time to return to play was 12.2 weeks (range, 8~16 weeks). Conclusion: Peroneal tendon tear can occur due to sports injuries. If there is tenderness at the retromalleolar area, the surgeon should consider a peroneal tendon lesion. Surgical repair of the peroneal tendon can be an effective treatment to help athletes to return to play.

시각적 피드백을 이용한 닫힌 사슬 운동이 만성 발목 불안정성 성인남성의 하지 근활성도 및 발목 불안정성 지수에 미치는 영향 (Effects of Visual Feedback Closed Kinetic Chain Exercise on the Lower Limb Muscles Activity and ankle Instability in Adult men with Chronic Ankle Instability)

  • PURPOSE: This study examined the effects on muscle activation and ankle instability following sudden ankle inversion of the closed kinetic chain exercise using visual feedback to evaluate chronic ankle instability in adult males. METHODS: In this study, 28 subjects with chronic ankle instability were recruited and randomly divided into a closed kinetic chain exercise using a visual feedback group(n=13) and an open kinetic chain exercise group(n=15). Each group underwent 30 minutes of exercise three times a week for 8 weeks. The electromyographic(EMG) values of the rectus femoris, biceps femoris, tibialis anterior, peroneus longus, peroneus brevis, and lateral gastrocnemius muscles were measured to analyze muscle activity. The Cumberland ankle instability tool(CAIT) was used to measure ankle instability. RESULTS: Both groups showed improvements in tibialis anterior, peroneus longus and CAIT score(p<.05). In the experimental group, there were significant improvements in the rectus femoris, biceps femoris, and lateral gastrocnemius muscle activation(p<.05). CONCLUSION: The results suggest that closed kinetic chain exercise using visual feedback can be a useful treatment method for patients with chronic ankle instability.

Assessment of the Location of the Peroneus Longus Tendon in the Cuboid Groove Using 3D Isotropic Fast Spin-Echo MRI

  • Purpose: To investigate normal location of the peroneus longus tendon (PL) in the cuboid groove by evaluating it between ankles with no significant abnormality (asymptomatic group) and those with retromalleolar PL dislocation (dislocation group) using three-dimensional isotropic fast spin-echo (3D-FSE) magnetic resonance imaging (MRI) of the ankle. Materials and Methods: Thirty-six and 32 3D-FSE ankle MRI were assigned to the asymptomatic group and the dislocation group, respectively. Using multiplanar reformatted 3D-FSE, qualitative PL location (i.e., outside, overlying, and inside in relation to the cuboid groove), quantitative PL location (i.e., distance between the proximal margins of PL and cuboid groove), and cuboid groove size were measured in lateral, middle, and medial levels of the cuboid groove. Results: In the asymptomatic group, 64%, 42%, and 11%, respectively, had the outside or overlying-located PL in lateral, middle, and medial levels of the cuboid groove and the quantitative location gradually decreased from lateral to medial level. Qualitative and quantitative PL locations were not significantly different between the asymptomatic group and dislocation group. Cuboid groove size showed significant negative correlation with quantitative PL location in both groups. Conclusion: Outside- or overlying-located PL in lateral and middle levels of the cuboid groove would be a normal finding, regardless of PL status at the retromalleolar level.

Comparisons of Chicken Muscles between Layer and Broiler Breeds Using Proteomics

  • The present study was carried out to investigate differentially expressed chicken muscle proteins using proteomics approach. More than 300 protein spots were investigated for the muscle samples in 2DE gels and the differentially expressed protein spots between pectoralis and peroneus longus muscles from Cornish and White Leghorn breeds were characterized by MALDI-TOF. In pectoralis muscles, PGAM1 protein was detected as differentially expressed between White Leghorn and Cornish breeds. On the other hand, 4 protein spots (SP22, nxf-2, SOD1, TNNI2) were differentially expressed between White Leghorn and Cornish breeds in peroneus longus muscles. These proteins assumed to be related with muscle development, growth, stress, and movements in chicken. In this experimental process, 2D reference map of the chicken muscle proteins was needed and 25 proteins, which were commonly expressed in both pectoralis and peroneus longus muscles in both breeds, were selected and characterized. Upon finishing the exact roles of the differentially expressed proteins, the identified 5 proteins will be used as valuable information for the fundamental mechanisms of muscle biology and underline genetics.

요추 추간원판에 의한 요천추 선경근 병변의 근전도 연구 (Electromyographic Study of Lumbosacral Radiculopathy by Lumbar Disc)

  • The aim of this study is to examine the sensitivity and characteristics of electromyography abnormalities detected by using various paramenters in patients with lumbosacral radiculopathies. EMG is widely used for disgnosing and localizing the level of radiculopathy. The results of the study were as follow : 1. In electromyography, L5 radiculopathy usa 95 cases(51.690). S1 radiculopathy was $45m(24.5\%)$ L4 radiculopathy was 18cases $(9.8\%)$, and L2, 3 radiculopathy was 8cases$(4.3\%)$. Remains 18cases$(9.8\%)$ had no definite radiculopathy. 2. Peroneal and tibial motor nerve conduction velocity studies were not significant as compared to the side to side. 3. Latency of H-reflex in L5 radiculopathy was $30.55\pm2.47$ in affected side, $29.47\pm2.29$ in unaffected side, in S1 radiculopathy was $33.00\pm2.03$ in affected side, R30.18\pm2.21$ in unaffected side. It was statistically significant(p<0.01). H-reflex mean difference of S1 radiculopathy group was significantly prolonged as compared to the L5 and S1 radiculopathies(p<0.001). 4. In L2, 3 radiculopathy, abnormal spontaneous activities and motor unit action potentials were showed high sensitivity in upper lumber paraspinal, hip adductors, quadriceps and iliopsoas muscles. 5. In L4 radiculopathy, lower lumbar paraspinal, tibialis anterior, quadriceps muscles were showed high sensitivity. 6. In L5 radiculopathy, lower lumbar paraspinal, extensor hallucis longus, extensor digitorum longus, peroneus longus, extensor digitorum brevis, gluteus maximus, tensor fasciae latae muscles were showed high sensitivity. 7. In S1 radiculopathy, lower lumbar paraspinal, gluteus maximus, peroneus longus, soleus, abductor hallucis, hamstrings, extensor digitorum brevis, extensor hallucis lognus, gastrocnemius muscles were showed high sensitivity.
