The purpose of this study was to investigate the anti-inflammatory effects of extract from GuBoEum(GBE) on the peritoneal macrophage. To evaluate anti-inflammatory effects of GBE. I measured cytokines (interleukin-6; IL-6, interleukin-12; IL-12, tumor necrosis factor-$\alpha$; TNF-$\alpha$) and nitric oxide (NO) production in lipopolysacchride (LPS)-induced macrophages. Furthermore, I examined molecular mechanism using western blot and also LPS-induced endotoxin shock. Extract from GBE does not have any cytotoxic effect in the peritoneal macrophages. Extract from GBE reduced LPS-induced IL-6, TNF-$\alpha$, IL-12 and NO production in peritoneal macrophages. GBE inhibited the activation of extracelluar signal-regulated kinase (ERK), C-Jun $NH_2$-terminal kinase (JNK) but not of p38, degradation of $I{\kappa}B-{\alpha}$ in the LPS-stimulated peritoneal macrophages. GBE inhibited the production of TNF-$\alpha$, IL-6 and IL-12 in serum after LPS injection. These results suggest that GBE may inhibit the production of TNF-$\alpha$, IL-6, and IL-12 through inhibition of ERK and JNK activation, and that GBE may be beneficial oriental medicine for inflammatory diseases.

Non-infectious complications of peritoneal dialysis (PD) are relatively less common than infectious complications but are a potentially serious problem in patients on chronic PD. Here, we present a case of a non-infectious complication of PD in a 13-year- old boy on chronic PD who presented with symptoms such as hypertension, edema, dyspnea, and decreased ultrafiltration. Chest and abdominal radiography showed pleural effusion and migration of the PD catheter tip. Laparoscopic PD catheter reposition was performed because PD catheter malfunction was suspected. However, pleural effusion relapsed whenever the dialysate volume increased. To identify peritoneal leakage, computed tomography (CT) peritoneography was performed, and a defect of the peritoneum in the left lower abdomen with contrast leakage to the left rectus and abdominis muscles was observed. He was treated conservatively by transiently decreasing the volume of night intermittent PD and gradually increasing the volume. At the 2-year follow-up visit, the patient had not experienced similar symptoms. Patients on PD who present with refractory or recurrent pleural effusion that does not respond to therapy should be assessed for the presence of infection, catheter malfunction, and pleuroperitoneal communication. Thoracentesis and CT peritoneography are useful for evaluating pleural effusion, and timely examination is important for identifying the defect or fistula.
Although primary tumors in the mesentery and omentum are relatively rare, it is often necessary to distinguish them from other non-tumorous diseases. Since the omentum and mesentery are major routes for the spread of various abdominal diseases, the anatomy, type, and pattern of the diseases affecting these organs should be known in detail for accurate differential diagnosis. In addition, it is important to detect and promptly treat hidden lesions in the mesentery and omentum. Therefore, careful observation of the area where the lesion occur should be emphasized when assessing mesentery and omentum in abdominal CT.
Sabaiksan has been prescribed to treat various allergic diseases in herbal medicine which were induced by various vasoactive amine released from the mast cells. The constituents of Sabaiksan are Mori Cortex Radices(MCR), Lycii Cortex Radicis(LCR) and Glycyrrhizae Radix(GR). Recently, simple models of compound 48/80 induced anaphylactic shock and cutaneous reaction in vivo were developed to test various agents employed in the field of allergy and toxicology research. The purpose of this study is to evaluate the effects of Sabaiksan on compound 48/80 induced anaphylactic stock, cutaneous reaction and mesenteric mast cell degranulation rate in ICR mice, and on compound 48/80 induced peritoneal mast cell degranulation and histamine release in vitro. Groups of ICR mice were intraperitoneally pretreated with $100{\mu}{\ell}$ of saline, $MCR(2g/m{\ell}),\;LCR(2g/m{\ell}),\;GR(g/m{\ell})$ or Sabaiksan itself(MCR+LCR+GR) at 24, 12 and 1 hour before compound 48/80 solution ($10{\mu}{\ell}/gm$ B. W) were peritoneally given into them, and then mortality within 72 hours after the compound 48/80 injection, and mesenteric mast cell degranulation rate at 15 minutes after compound 48/80 injection were calculated. In vitro experiment, $400{\mu}{\ell}$ of rat peritoneal mast cell suspension$(10^6cell/m{\ell})$ were pretreated with $50{\mu}{\ell}$ of saline, $MCR(2g/m{\ell}),\;LCR(2g/m{\ell}),\;GR(g/m{\ell})$ or Sabaiksan itself at room temperature for 30 minutes, and then $50{\mu}{\ell}$ of compound 48/80 solution $(100{\mu}g/m{\ell})$ were added into it. 30 minutes after the addition of compound 48/80 solution, histamine release assay in the supernatant of peritoneal mast cell suspension were performed employing radioisotope enzymatic assay and morphologic changes of mast cells in each regular time point were photographed. Compared with controls, compound 48/80 induced anaphylactic shock was significantly inhibited by MCR and GR pretreatment into the ICR mice. Significant inhibition of compound 48/80 induced cutaneous reaction, mesenteric mast cell degranulation rate in vivo and histamine release from the rat peritoneal mast cells in vitro was observed only in MCR pretreated group. From the above results, it is suggested that MCR component of Sabaiksan may playa key role to suppress mast cell function since it has been applied to various allergic diseases.
Purpose : The pathophysiologic mechanism of nephrotic syndrome is not yet known clearly. At least in some cases, certain 'circulating factors' are thought to increase the glomerular protein permeability. Considering the systemic effect of the circulating factor on peritoneal membrane, we evaluated the loss of protein through peritoneal membrane in patients on peritoneal dialysis due to the end stage renal disease (ESRD) caused by steroid resistant nephrotic syndrome (SRNS). Methods : We retrospectively reviewed the medical records of 26 pediatric patients on peritoneal dialysis ensued during the period from 2001 to 2007 at our clinic. Twelve patients had SRNS, while 14 patients had ESRD caused by the congenital anomalies of urinary system. Results : While the other parameters including nPNA indicating the adequacy of protein intake were similar between the two groups, serum albumin was lower in SRNS patients than the non-SRNS patients ($3.7{\pm}0.3$ g/dL vs. $4.0{\pm}0.4$ g/dL, P=0.021). Peritoneal protein loss was higher in SRNS patients than in non-SRNS patients ($3,044.4{\pm}837.6\;mg/m^2$/day vs. $1,791.6{\pm}1,244.0\;mg/m^2$/day, P=0.007). The protein permeability of the peritoneal membrane measured by the ratio of total protein concentration in dialysate to plasma was twice as high in SRNS patients as the non-SRNS ($1.06{\pm}0.46%$ vs. $0.58{\pm}0.43%$, P=0.010). After 1 year, peritoneal protein loss increased in both patient groups, but to a significantly greater degree in non-SRNS patient (P=0.023). Conclusion : The results of our study support the notion that in nephrotic syndrome there are some 'circulating factors' with the systemic effect. Since the greater protein loss through peritoneal membrane in SRNS was confirmed in this study, more meticulous nutritional support and close monitoring on the nutrition are required in these patients.
Corylopsis gotoana (Hamamelidaceae) has been used as a traditional medicine for the treatment various diseases including cold, edema and vomiting. However, previous studies regarding component analysis and pharmacological actions of C. gotoana are extremely limited until now. In this study, we investigated anti-inflammatory activities of the methanolic extract of the twigs of Corylopsis gotoana (MCG) both in vitro and in vivo. MCG effectively inhibited excessive NO production in IFN-${\gamma}$ and LPS-stimulated mouse peritoneal macrophages without notable cytotoxicity. In addition, we also found that MCG could attenuate the expression of inflammatory mediators such as inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2). We further tested in vivo anti-inflammatory activity of MCG using paw edema mouse model. Herein, MCG demonstrated significant suppression on the paw edema induced by both of trypsin and carrageenan. These results indicated that MCG has potent anti-inflammatory potential and may be useful for prevention and treatments of inflammatory diseases.
Equisetum hyemale L. has been prescribed widely as a traditional medicine for the treatment of inflammatory diseases such as rheumatoid arthritis, conjunctivitis, pyelonephritis. In order to identification the mechanism, we examined an antioxidant and anti-inflammatory activity of 85% methanol extract of E. hyemale. In this study E. hyemale exhibited strong scavenging effect on 1,1-diphenyl-2-picrylhydrazyl (DPPH) free radical, superoxide radical, and nitric oxide. To elucidate the anti-inflammatory properties of E. hyemale, we investigated the inhibition effects of nitric oxide and IL-6 by E. hyemale in IFN-gamma and LPS-stimulated mouse peritoneal macrophages. E. hyemale suppressed nitric oxide, IL-6 production and iNOS expression dose-dependently without notable cytotoxic activity. These data suggest that E. hyemale might be useful in inflammatory diseases by inhibiting the free radicals and inflammatory mediators.

Nardostachys chinensis has been used widely as a traditional medicine for the treatment of diverse diseases. The dried plant was extracted with 85% methanol extract (NC). We investigated the antioxidant properties of NC on the 1,1-diphenyl-2-picryhydrazyl (DPPH) radical, superoxide anion and nitric oxide radical scavenging capacity under in vitro assays. NC showed potent antioxidant activity, compared to ascorbic acid. In macrophages, nitric oxide is released as an inflammatory mediator and has been proposed to be an important modulator of many pathophysiological conditions in inflammation. In the present study, it was also investigated that the inhibition effects on NO and the mechanism of down-regulation of immune response by NC in IFN-IFN-$\gamma$/LPS-stimulated mouse (C57BL/6) peritoneal macrophages. Extracts of NC suppressed NO production and the expression of iNOS and COX-2. The present results indicate that NC has an antioxidant and anti-inflammatory properties and therefore may be beneficial in diseases which related to oxidative stress-mediated chronic inflammatory disorders.
The immediate-type allergic reaction (anaphylaxis) is involved in many allergic diseases such as asthma, allergic rhinitis, and sinusitis. The discovery of drugs for the treatment of immediate-type allergic diseases is a very important subject in human health. In this study, we investigated the effect of Rubus coreanus Miq.(Rosaceae) unripe fruits (RCF) on mast cell-mediated allergic reaction and inflammatory cytokine secretion. RCF inhibited compound 48/80-induced systemic reactions in mice. RCF attenuated immunoglobulin (Ig) E-mediated local allergic reactions. In addition, RCF dependently reduced histamine release from rat peritoneal mast cells local allergic reactions. In addition, RCF dependently reduced histamine release from rat peritoneal mast cells activated by compound 48/80 or IgE. Furthermore, RCF decreased the phorbol 12-myristate 13-acetate plus calcium ionophore A23187-stimulated tumor necrosis factor $(TNF)-{\alpha}$ and interleukin (IL)-6 secretion in human mast cells. Our findings provide evidence that RCF inhibits mast cell-derived immediate-type allergic reactions.

Mast cells play an important role in the pathophysiologlcal changes observed in acute cutaneous and inflammatory diseases In order to see whether Kum-Hwang-San has an influence on mast cell- mediated immediate cutaneous reactions, the author has undertaken an animal study. Ears of mice were treated with a compound 48/80 solution topically at 30 min after the cutaneous application of Kum-Hwang-San. At each point, an ear swelling response was measured with a digimatic thickness micrometer. Ear swelling response induced by compound 48/80 was significantly suppressed dose-dependently by Kum-Hwang-San 30 min before topical application as compared with that in nonapplicated control mice, and the value returned to normal levels by 120 min. Compound 48/80- induced mast cell degranulation by Kum-Hwang-San was also remarkably decreased in accordance with the suppression in ear swelling response. Kum-Hwang-San dose-dependently inhibited histamine release from the rat peritoneal mast cells activated by compound 48/80. Another way to test acute cutaneous reaction is to induce passive cutaneous anaphylactic reaction. Kum-Hwang-San significantly inhibited passive cutaneous anaphylactic reaction activated by anti-dinitrophenyl IgE on both topical application and intradermal injection. Kum-Hwang-San also inhibited histamine release from the rat peritoneal mast cells induced by anti-DNP IgE. This study provides evidence that Kum-Hwang-San will be beneficial in the treatment of acute cutaneous diseases.
