왕대의 3방향에 따른 초음파적 특성 (Ultrasonic Properties of Phyllostachys bambusoides Sieb. et Zucc depending on Three directions)

  • This research was carried out to examine the ultrasonic properties of $Phyllostachys$ $bambusoids$ Sieb et Zucc depending on three directions for providing the fundamental properties of the Korea traditional flute, Daekeum. The ultrasonic properties of $Phyllostachys$ $bambusoids$ Sieb. et Zucc were found to be different from those of wood, because of the distinctive anatomical structure of bamboo tissue. The ultrasonic velocity of peripheral zone showed higher values of all three directions. The ratio of longitudinal velocities to perpendicular velocities showed 1.16 at peripheral zone, 1.70 at central zone, 1.38 at inner zone respectively. The ratio of radial velocities to tangential velocities were measured as 1.286 at peripheral zone, 1.325 at central zone, 0.829 at inner zone respectively.

Rat의 자궁벽에 발생된 stromal sarcoma의 관찰 (Observations of a endometrial stromal sarcoma in the rat uterus wall)

  • The present observation was focussed mainly on the histopathological and immunohistochemical finding of the tumor developing from the outside of rat uterus wall. In gross finding, this tumor were a oval form mass and 0.75cm and 1cm in greatest demension. The sectioned surface showed uniform, solid, gray-white. In histopathological finding, the tumor showed the form of lymphoid nodule-like structure and was composed of the cells with karryorrhexis, karyolysis and necrosis in the central zone, mature cells in the paracentral zone, and concentric layers of compressed, flattened and atrophied smooth muscle fibers by the proliferating of the cells closely resembling endometrial stromal ells in the peripheral zone and a few smaller nodule-like structure in certain areas of the peripheral zone. In immunohistochemical observations, the proliferating cells presents only among the concentric layers of the smooth muscle fibers in the peripheral zone. Labeling index of proliferating cells in the peripheral zone was $4.34{\pm}1.28%$. These tumor cells did not metastasized in another orgns and labeling index was low, So it was concluded that this tumor was benign stomal sarcoma.

Peripheral Blood Lymphocytosis without Bone Marrow Infiltration in a Dog with T-Zone Lymphoma

  • A 13-year-old neutered male mixed-breed dog presented with generalized lymphadenopathy and erythematous cutaneous lesions in the ear pinnae. Fine-needle aspiration cytology of the lymph nodes revealed small to intermediate lymphocytes with a "hand mirror" configuration as the predominant cell type. Histopathological analysis of the lymph node showed an infiltrate of CD3-positive small lymphocytes compressing the follicles against the capsule owing to neoplastic cell expansion. Flow cytometric analysis revealed a homogeneous population of CD3+/CD4-/CD5+/CD8-/CD21+/CD34-/CD45- cells in both the peripheral blood and aspirated lymph nodes, which supports the diagnosis of T-zone lymphoma. Laboratory tests revealed lymphocytosis (14,144 cells/µL) in the peripheral blood. However, contrary to expectations, the bone marrow examination revealed no evidence of lymphocytic infiltration. T-zone lymphoma is an indolent lymphoma with a long survival period, and knowledge of its characteristics may affect disease staging and prognosis evaluation. Therefore, peripheral blood count as a sole screening tool for bone marrow metastasis should be used with caution.

전립선 MRI에서 사용하는 1.5T 경직장 코일과 3.0T 위상 배열 코일의 성능 비교 평가 (Comparative assessment of a 1.5T endorectal coil and a 3.0T phased-array coil available for prostate MRI)

  • 1.5T에서 경직장 코일과 3.0T의 위상 배열 코일을 이용하여 신호대 잡음비를 비교하여 3.0T의 위상 배열 코일의 유용성을 제시한다. 전립선암으로 진단받은 환자 40명을 대상으로 1.5T MR scanner에서는 경직장 표면 코일과 3.0T MR scanner에서는 위상 배열 코일을 이용하여 T2 강조 영상, T1 강조영상, 확산 강조 영상을 각각 획득 후 전립선의 Central Zone(CZ)과 Peripheral Zone(PZ)에서 Right Peripheral Zone(PZ)과 Left Peripheral Zone(PZ) 세 부위에서 신호 강도(signal intensity)측정을 하였다. 정량적 분석방법으로 관심영역의 신호대 잡음비(signal to noise ratio)를 구하고 평균화 하여 1.5T MR scanner로 획득한 그룹과 3.0T MR scanner로 획득한 그룹에서의 신호대 잡음비를 비교하였다. 3.0T MR scanner에서 위상 배열 코일을 사용한 경우 1.5T MR scanner에서 경직장 코일을 사용한 경우보다 신호대 잡음비가 증가됨을 알 수 있었다.

간소엽내 동모양혈관내피창에 관한 주사전자현미경적 연구 (A Scanning Electron Microscopic Study on the Sinusoidal Fenestrations in the Hepatic Lobule of Normal Rat)

  • 간소엽에 있는 동모양혈관내피창은 혈관과 간세포사이의 물질교환에 중요한 역할을 하는 부위로 알려져 있다. 그러나 이들 내피창에 관해서는 아직도 검토되어야 할 문제가 많이 남아 있다. 본 연구에서는 동모양혈관내피창 중 특히 큰창의 실재 여부와 창의 소엽내 분포상을 관찰하고 창의 크기와 분포상을 기초로 하여 내피창의 유형과 분포상을 재검토하고자 하였다. 실험동물은 식이조건을 일정하게 하기 위하여 최종 식이 투여 3시간 후에 관류고정하였으며 식에 따라 제작된 모든 조직표본은 주사전자현미경으로 관찰하였다. 그 결과 동모양혈관의 내면에서 여러가지 크기의 내피창이 관찰되었는데 이들은 작은창, 중간창 및 큰창의 3가지 유형으로 구분되었다. 작은 창은 낱개로 나타나거나 여러 개가 모여서 나타났으며 중간창은 낱개로 또는 작은창에 섞여서 나타났다. 큰 창도 낱개로 나타나거나 작은창과 중간창에 둘러싸여 나타났다. 특히 중간창은 소엽의 중심대에 많이 분포하는 경향이 있어 새로운 유형으로 분류하였다. 큰창은 다른 유형의 창과 마찬가지로 그 윤곽이 평탄하였으며 세포질에 의해 분할된 상도 관찰되었으나 파열된 상은 찾아 볼 수 없었기 때문에 인공산물이 아닐 것으로 추정하였다. 소엽은 창의 분포에 따라 주변대, 중간대 및 중심대로 구분되었다. 주변대에서는 작은창만이 보이는 동모양혈관이 많이 관찰되었다. 중심대에서는 중간창과 작은창이 보이는 동모양혈관이 많이 관찰되었다. 중간대에서는 큰창과 중간창 및 작은창이 다 같이 보이는 동모양혈관이 많이 관찰되었다. 이상의 소견으로 미루어 동모양혈관내피창은 소엽의 주변대에서 중간대로 가면서 점차적으로 커지다가 중간대에서 중심대로 가면서 작아지는 경향을 보이지만 중심대의 창은 주변대의 것에 비하면 큰 편이다.

Immunohistochemical study on the insulin-immunoreactive cells in the developing pancreas of the Korean native goat (Capra hircus)

  • The distribution and relative frequency of insulin-immunoreactive cells in the pancreas was studied during developmental stages (fetus, neonate, 1-month-old, 6-month-old and adult) of the Korean native goat by immunohistochemical methods. The different distribution and relative frequency of glucagon-immunoreactive cells in the pancreas of the Korean native goat was observed during development. Insulin-immunoreactive cells were detected in the exocrine and endocrine portions (pancreatic islets) of the all ages, and in the duct of the 6-month-old. The relative frequencies of these cells were increased in the pancreatic islets with ages but decreased in the exocrine portions. Generally, they were distributed in the interacinar spaces or central zone of the pancreatic islets in all ages. However, the distributions and relative frequencies in the pancreatic islets of the neonate Korean native goat were divided into three patterns : 1) located in the inner zone with numerous frequencies, 2) the peripheral zone of the pancreatic islet with moderate frequencies and 3) the peripheral zone of the pancreatic islet with a few frequencies patterns.

Feasibility of Novel Three-Dimensional Magnetic Resonance Fingerprinting of the Prostate Gland: Phantom and Clinical Studies

  • Objective: To evaluate the feasibility of a new three-dimensional (3D) MR fingerprinting (MRF) technique for the prostate gland by conducting phantom and clinical studies. Materials and Methods: The new 3D MRF technique used in this study enables quick data acquisition and has a high resolution. For the phantom study, the MRF T1 and T2 values in an in-house phantom were compared with those of goldstandard mapping methods using linear regression analysis. For the clinical study, we evaluated 90 patients who underwent prostate imaging with MRF for suspected prostate cancer between September 2019 and February 2020. The mean T1 and T2 values were compared in the peripheral zone, transition zone, and focal lesions using paired t tests. The differences in the T1 and T2 values according to cancer aggressiveness were evaluated using one-way analysis of variance. Results: In the phantom study, the MRF T1 and T2 values showed a perfect correlation with the gold-standard T1 and T2 values (R > 0.99). In the clinical study, the T1 and T2 values in the peripheral zone were significantly higher than those in the transitional zone (p < 0.001, both). The T1 and T2 values in prostate cancer were significantly lower than those in the peripheral and transitional zones. The higher the grade of cancer, the lower the T2 values. Conclusion: The T1 and T2 values obtained from the 3D MRF showed a perfect correlation with the gold standard values in the phantom study. Differences in the T1 and T2 values among the different zones of the prostate gland were identified using 3D MRF in patients.

진단검사의학과의 검사기능과 공간구성에 관한 연구 - 1000병상급 종합병원을 중심으로 (A Study on the Laboratory Function and Spatial Organization for Laboratory Medicine - Focused on the Tertiary Level General Hospitals having more than 1000 Beds)

  • Purpose: As the structure of the disease has been changed and the infectious disease has been increased, the demand for diagnostic examination has been increasing. So, the department of laboratory medicine in hospital has playing the important role accounting for about nineteen percent of total medical expenses in korea. This study is to investigate the laboratory function and spatial composition of the department and figure out the space area and space organization. Methods: Explore the literature review to identify the laboratory function. Limit to five cases of tertiary level general hospital having about a thousand bed and analyze the space layout and floor area to confirm the spatial composition. Classify the exam function and check the spatial composition and spatial organization. Results: This study allows 5 conclusions to be summarized. Laboratory medicine divided into nine part in regulation, but in space allocated into core lab, emergency lab and six part lab. Total laboratory area is $2,036m^2$ in average, and is composited with 60% for lab, 17% for office and 20% for public. Lab area per a bed presents $1.88m^2$. Microbiolgy and molecular lab area are getting large. Laboratory space organized into the four zone, like an entrance zone, core zone, rear zone and peripheral zone. Emergency and transfusion lab are allocated in entrance zone, hematology chemistry and immunology in core lab, microbiology and molecular lab in rear, support offices in peripheral zone. The most important point was to check the spatial composition of the Laboratory Medicine according to the inspection function. Implications: This study can be used as a useful data in planning and designing a Laboratory Medicine Department.

Elemental characteristics of sialoliths extracted from a patient with recurrent sialolithiasis

  • The exact mechanism of sialolith formation has yet to be determined. Recurrence of sialolithiasis is rare, affecting only 1%-10% of patients. The current study presents a case of recurrent stones that occurred twice on the right submandibular gland 6 months postoperative and 7 months after reoperation in a 48-year-old female patient. The stones were analyzed using histology, scanning electron microscopy, energy dispersive spectroscopy, and transmission electron microscopy (TEM). The first stone showed a three-layered structure with a poorly mineralized peripheral multilayered zone, highly mineralized middle layer, and the central nidus. The stones were composed of Ca, C, O, Cu, F, N, P, Si, Zn, and Zr. In TEM, compact bi-layered bacterial cell membrane was found on the peripheral layer and the central nidus of the stone as well as exosomes in the central nidus. The results demonstrated the essential components of sialolith formation, including bacteria, inflammatory exosomes, and exfoliated salivary epithelial cells that cooperatively underwent the pathogenetic progresses of central nidus formation, induction of compact zone calcification of the middle layer, and repeated subsequent deposition in the peripheral multilayer zone. The rapid recurrence could have resulted from residual pieces of a sialolith acting as the nidus of bacterial infection.

Layered structure of sialoliths compared with tonsilloliths and antroliths

  • Objectives: The aim of this study was to perform a comparative analysis of the ultrastructural and chemical composition of sialoliths, tonsilloliths, and antroliths and to describe their growth pattern. Materials and Methods: We obtained 19 specimens from 18 patients and classified the specimens into three groups: sialolith (A), tonsillolith (B), and antrolith (C). The peripheral, middle, and core regions of the specimens were examined in detail by histology, micro-computed tomography (micro-CT), scanning electron microscopy (SEM), energy dispersive X-ray spectroscopy, and transmission electron microscopy (TEM). Results: In the micro-CT, group A showed alternating radiodense and radiolucent layers, while group B had a homogeneous structure. Group C specimens revealed a compact homogeneous structure. Histopathologically, group A showed a laminated, teardrop-shaped, globular structure. Group B demonstrated degrees of immature calcification of organic and inorganic materials. In group C, the lesion was not encapsulated and showed a homogeneous lamellar bone structure. SEM revealed that group A showed distinct three layers: a peripheral multilayer zone, intermediate compact zone, and the central nidus area; groups B and C did not show these layers. The main elemental components of sialoliths were O, C, Ca, N, Cu, P, Zn, Si, Zr, F, Na, and Mg. In group B, a small amount of Fe was found in the peripheral region. Group C had a shorter component list: Ca, C, O, P, F, N, Si, Na, and Mg. TEM analysis of group A showed globular structures undergoing intra-vesicular calcification. In group B, bacteria were present in the middle layer. In the outer layer of the group C antrolith, an osteoblastic rimming was observed. Conclusion: Sialoliths had distinct three layers: a peripheral multilayer zone, an intermediate compact zone and the central nidus area, while the tonsillolith and antrolith specimens lacked distinct layers and a core.
