치과에 내원하는 환자들의 치주질환 인식도에 관한 분석 (A analysis on the Patient's awareness of periodontal diseases)

  • The purpose of this study was to examine the awareness of adult patients visiting dental clinics about periodontal diseases. The subjects in this study were 204 patients who paid a visit to five dental clinics and a dental hospital in the region of Jeonju. After a survey was conducted, the results were obtained as follows: 1. With regard to toothbrushing method, the most of the male patients(43.8%) brushed their teeth by rolling the toothbrushes up and down, and the most of the female patients(46.1%) did that both up and down and right and left. As to the influence of smoking, the largest number of the smokers(50.0%) and nonsmokers(40.4%) brushed their teeth both up and down and right and left. statistically significant differences were shown by gender and smoking(p<0.05). 2. As for knowledge acquisition route on periodontal diseases by gender, there was no way for the men(38.2%) to get the information, and the largest number of the women (43.4%) acquired the information through the internet. With respect to smoking, the largest numbers of the smokers(38.2%) and nonsmokers(37.5%) got the information by the internet, and statistically significant differences were found (p<0.05). 3. Concerning treatment method of periodontal diseases provided by the dental clinic, the largest number of the men(33.7%) got their teeth scaled, and the greatest group of the women(29.6%) had their gums treated. The treatment method made statistically significant differences by gender (p<0.05). Regarding smoking, the largest group of the smokers(32.4%) had their teeth scaled, and the largest number of the nonsmokers(28.7%) got their gums treated. significant differences were made statistically by smoking (p<0.05). 4. As to instruction on the prevention of periodontal diseases, the largest group(63.2%) intended to take that. The second (23.0%) was eager to do it, and the third(13.8%) was unwilling to do it.

A randomized clinical trial to evaluate and compare the efficacy of triphala mouthwash with 0.2% chlorhexidine in hospitalized patients with periodontal diseases

  • Purpose: Triphala is a combination of three medicinal plants, extensively used in Ayurveda since ancient times. Triphala mouthwash is used in the treatment of periodontal diseases because of its antimicrobial and antioxidant properties. The aim of this study is to compare the efficacy of triphala mouthwash with 0.2% chlorhexidine in hospitalized periodontal disease patients. Methods: In this double-blind, randomized, multicenter clinical trial, 120 patients were equally divided into three groups. Patients in group A were advised to rinse their mouths with 10 mL of distilled water, group B with 0.2% chlorhexidine, and group C with triphala mouthwash for 1 minute twice daily for two weeks. The plaque index (PI) and the gingival index (GI) were recorded on the first and the fifteenth day. Results: There was no significant difference when the efficacy of triphala was compared with 0.2% chlorhexidine in hospitalized patients with periodontal disease. However, a statistically significant difference was observed in PI and GI when both group B and group C were compared with group A and also within groups B and C, after 15 days (P<0.05). Conclusions: The triphala mouthwash (herbal) is an effective antiplaque agent like 0.2% chlorhexidine. It is significantly useful in reducing plaque accumulation and gingival inflammation, thereby controlling periodontal diseases in every patient. It is also cost effective, easily available, and well tolerable with no reported side effects.

Gingival crevicular fluid CSF-1 and IL-34 levels in patients with stage III grade C periodontitis and uncontrolled type 2 diabetes mellitus

  • Purpose: Periodontal diseases are inflammatory conditions that alter the host's response to microbial pathogens. Type 2 diabetes mellitus (T2DM) is a complex disease that affects the incidence and severity of periodontal diseases. This study investigated the gingival crevicular fluid (GCF) levels of colony-stimulating factor-1 (CSF-1) and interleukin-34 (IL-34) in patients with stage III grade C periodontitis (SIII-GC-P) and stage III grade C periodontitis with uncontrolled type 2 diabetes (SIII-GC-PD). Methods: In total, 72 individuals, including 24 periodontally healthy (PH), 24 SIII-GC-P, and 24 SIII-GC-PD patients, were recruited for this study. Periodontitis patients (stage III) had interdental attachment loss (AL) of 5 mm or more, probing depth (PD) of 6 mm or more, radiographic bone loss advancing to the middle or apical part of the root, and tooth loss (<5) due to periodontal disease. Radiographic bone loss in the teeth was also evaluated; grade C periodontitis was defined as a ratio of the percentage of root bone loss to age greater than 1.0. The plaque index (PI), gingival index (GI), presence of bleeding on probing (BOP), PD, and clinical AL were used for clinical periodontal assessments. GCF samples were obtained and analyzed using an enzyme-linked immunosorbent assay. Results: All clinical parameters-PD, AL, GI, BOP, and PI-were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups for both the full mouth and each sampling site (P<0.05). The total IL-34 and CSF-1 levels were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups (P<0.05), and there were significant differences between the periodontitis groups (P<0.05). Conclusions: These findings suggest that IL-34 and CSF-1 expression increases in patients with SIII-GC-PD. CSF-1 was associated with the inflammatory status of periodontal tissues and T2DM, while IL-34 was associated only with T2DM.

상악 측절치에서 발견되는 구개치은구로 인한 치주 병소의 치료 : 증례 보고 (Treatment of periodontal lesion caused by palatogingival groove in maxillary lateral incisor: case reports)

  • Purpose: The palatogingival groove is a developmental anomaly of the incisor teeth, which often presents severe localized periodontal disease. The purpose of this study was to evaluate the clinical outcome of palatogingival groove-associated periodontal lesion following flap operation with glass ionomer filling. Methods: Four patients with periodontal lesion associated with the palatogingival groove were chosen for this case study. Clinical indices were taken and radiographic exam was performed at the baseline of the study and four patients were treated by flap operation with GI filling. Post-surgical visits were scheduled at regular intervals to check clinical and radiographic changes. Results: Symptoms and signs of periodontal lesion were almost completely resolved with improvement of periodontal indices. Conclusions: Flap operation with direct glass ionomer restoration is thought to be an acceptable method which can produce favorable results in the treatment of periodontal lesion caused by palatogingival groove on the maxillary lateral incisor.

일부 당뇨병 환자의 구강관리 실태와 그 연관요인 (Oral care status and its related factor in diabetic patients)

  • Objectives : This study conducted to investigate oral care abilities in diabetic patients to find a good dental method which may efficiently control diabetes. Methods : The diabetic group consisted of 40 patients with type 2 diabetics, while the non-diabetes were 34 persons with no signs of diabetics, who had visited a Public Health Center and dental clinic of a general hospital in Gwangju city from Dec. 2008 to Sep. 2009. Periodontal disease, gingivitis, dental caries, tongue plaque and halitosis between diabetic group and non-diabetic group were examined and at the same time a questionnaire survey was conducted. The data were analyzed with chi-square, t-test, pearson correlation coefficients using the SPSS WIN 15.0 program. Results : The mean values of both the community periodontal index and the gingival index of diabetic group were 2.18 and 1.75, respectively, and those were higher than those (1.79 and 1.50) of non-diabetic group with no statistical significance. The mean saliva secretion of non-diabetics was 16.74 mL, which was higher than 13.90 mL of diabetic patients(p<0.05). The oral care ability(mean 3.10) acquired from plague index in diabetic group was worse than in non-diabetic(mean 2.33). Conclusions : The high blood sugar concentration in diabetic patients causes low saliva secretion and high saliva viscosity, with the results halitosis and periodontal disease take place. This study suggested that good oral care ability of diabetic patients was very helpful to prevent periodontal disease and halitosis. In addition to this, it may reduce complications of diabetic patients.

치주임상에서의 정주진정법의 적용 (Intravenous Sedation in Periodontal Practice)

  • Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.

치주치료가 약물성 치은비대 환자의 치은두께에 미치는 효과에 관한 연구 (The effect of periodontal therapy on the gingival thickness in patients with drug-induced gingival enlargement)

  • The purpose of this study was to observe the effects of periodontal therapy, including nonsurgical periodontal therapy with azithromycin, surgical therapy, and maintenace therapy on the drug-induced gingival enlargement, by means of measuring gingival thickness. The test group of 18 patients with drug-induced gingival enlargement received scaling, root planing with azithromycin for 5 days, with or without surgical periodontal treatment. The control group of 18 patients who had not taken any medication, received scaling and root planing, with or without surgical periodontal treatment. Both groups received supportive periodontal therapy every 3 months for 2 years. The mean period of total treatment is 32 months in the test group and 31 months in the control group. The thickness of the buccal gingiva was measured using an ultrasonic device of $SDM^{(R)}$(Krupp Corp., Essen, Germany). The results revealed that the test $group(1.21{\pm}0.51mm)$ showed statistically thicker buccal gingiva than the control $group(1.01{\pm}0.3mm)$. In the test group, the buccal gingiva was thickest on 2nd molars and was thinnest on canines of both dental arches. In the control group, the buccal gingiva was thickest on central incisors in the maxilla and 2nd molars in the mandible, while the thinnest areas were on canines in the maxilla and 1st premolars in the mandible. It would be concluded that the periodontal treatment with azithromycin aids in decreasing the degree of the gingival enlargement but cannot prevent the recurrence completely.

고정성 교정장치 장착환자의 치주질환관련 지식 및 구강건강관련 행태 (Periodontal disease-related recognition and oral health-related behavior in orthodontic patients with fixed appliance)

  • Objectives: The purpose of study is to investigate periodontal disease-related recognition and oral health-related behavior in orthodontic patients with fixed appliance. Methods: A self-reported questionnaire was completed by 286 orthodontic patients with fixed appliance in Gwangju, Jeonnam from September 1 to September 27, 2016. The questionnaire consisted of general characteristics (3 items), orthodontic related characteristics (3 items), knowledge of periodontal disease (3 items), and oral health-related behavior (4 items). The data were analyzed by frequency analysis, percentage and chi-square analysis using SPSS 21.0 program. Results: 62.8% had experiences of dental treatment and 67.5% had intention of involvement on incremental care program in orthodontic treatment periods. Accuracy rate of cause about periodontal disease was high in female and case of acquiring information experiences on periodontal disease (p<0.05). 67.2% performed correct toothbrushing for the management of periodontal disease in the experiences of acquiring information on periodontal disease in orthodontic treatment periods (p<0.05). The proportions of using interdental toothbrush and mouth rinsing solutions were high among those over 20 years old and students in the subjects (p<0.05). Conclusions:The accuracy rate were high in the answers about cause and management of periodontal disease in case of acquiring information experiences on periodontal disease in orthodontic treatment periods. Therefore, there is a need to further development and implementation of dental hygiene intervention program for periodontal disease care with fixed orthodontic appliances in that regard.

  • Cigarette smoking is a significant risk factor for periodontaldiseases and implant treatments. Smoking control is regarded as a key to the success of dental treatments as well as the well-being of the patients. The aim of this study was to reviewthe effects of smoking on periodontal health, and the results of periodontal therapy and implant treatments. Also, in vitro, microbiological, immunological and epidemiological relationships were studied. In vitro studies show that smoking interferes with normal healing process and increased tissue destruction. There is still controversy on the population of microbes of smokers. Smokers showed significantly less gingival inflammation and bleeding on probing compared with non-smokers. After periodontal treatments, a compromised clinical outcome was noted for smokers in terms of pocket depth reduction and gain in attachment levels. In conclusion, data from in vitro, epidemiological, cross-sectional and case-control studies strongly suggest that quitting smoking is beneficial to patients before periodontal and implant treatments.

  • During the past decade, the use of osseointegrated implants as a foundation for prosthetic replacement of missing teeth has become highly predictable and successful. SPT(Supportive Periodontal Therapy) identified as regular visits to the therapist for periodontal control and maintenance in a well-organized scheme, the number of appointments per year following a pre-designed subject-tooth/implant-site risk assessment method. Peri-implant disease was a frequent finding in subjects having natural healthy dentition and in subjects without periodontitis. Supportive periodontal program were found to be strongly related to implant survival. This study demonstrates that regular maintenance reduces the risk for peri-implant inflammation significantly as compared with irregular maintenance. This underlines the value of the SPT in enhancing the long-term outcomes of implant therapy, particularly in subjects affected by periodontitis, in order to control reinfection and limit biological complications. It is highly recommended to maintain implant patients under a strict supportive periodontal treatment protocol that might contribute to implant survival, and regular maintenance reduces the risk for periimplant inflammation significantly as compared with irregular maintenance. Ideally, patients may be informed on the beneficial effect of a regular patient-related post-therapy care before implant insertion.
