  • Objectives: The aim of this study was to examine the relationship between periodontitis, obesity, and health behavior according to sex and age in Korean adults. Methods: Data on 11,032 adults aged 19-79 years were obtained from the 7th Korea National Health and Nutrition Examination Survey (KNHNES). We used multivariate logistic regression analysis to test for associations. Results: The final model that was adjusted for demographic characteristics and health status, showed a higher risk of periodontitis with increasing body mass index (BMI), smoking, failure to use oral care products, and no dental check up. In the sub-group analysis, only smoking was significant in the 19 to 39-year-old age groups. In the 40 to 64-year-old age group, a BMI of 30 or higher, and smoking, use of oral care products, and dental check-up were significantly associated with periodontitis. In the female group, BMI, smoking, use of oral care products, and dental check-up were significantly related to periodontitis. However, in males, only smoking was significant. Conclusions: Obesity management can be helpful for periodontal health as periodontitis prevalence in adults increased as BMI increased. For periodontal health, an oral health program should include smoking cessation and the use of oral care products and dental check up as part of obesity management.

  • Objectives: This study aims to contribute to the prevention of dental diseases and health care in the elderly by investigating the relationship among the experiences of chronic diseases, dental health status, and the behaviors in the Korean elderly people. Methods: A total of 2,856 elderly people aged 65 or older were selected as the final analysis subjects using data from the 6th National Health and Nutrition Examination Survey (2013-2014). Chronic diseases were defined as 'cardiocerebrovascular diseases', 'diabetes', 'chronic respiratory diseases', and 'cancer' diagnosed by the doctors. For the statistical analysis, SPSS 21.0 for Windows was used. Descriptive analysis and a Chi-square test were conducted to investigate the relationship among the experiences of chronic diseases, general characteristics, dental health status, and the behaviors in the Korean elderly. Finally, logistic regression analysis was performed to investigate the relationship among the experiences of chronic diseases, dental health status, and the behaviors. Results:The prevalence of Cardiocerebrovascular diseases was significantly higher in the 3-4 group of community periodontal index with the score of 1.36 (95% CI 1.03-1.00) than in the 0-2 group. The prevalence of Cardiocerebrovascular diseases was significantly higher in the group without dental examination during the past one year with the score of 1.29 (95% CI 1.00-1.66). The prevalence of diabetes was significantly higher in the uncomfortable speaking state group with the score 1.46 (95% CI 1.12-1.91). The prevalence of cancer was significantly higher in the partial denture needs group with the score 1.67 (95% CI 0.98-2.83). Conclusions: Regular dental examinations and dental health care for the elderly with chronic diseases showed that periodontal health and residual teeth could be maintained and managed. Therefore, continuous customized dental health services should be implemented for the elderly with chronic diseases.

  • This study aimed to evaluate the correlation between metabolic syndrome and periodontal in the adult Korean population on the basis of the 2009 Korea National Health and Nutrition Examination Survey(NHANES) data. Of 19-year-old or older adult patients suffering from their disease diagnosed as metabolic syndrome, 1,315 subjects whose community periodontal index(CPI) information was fully available were finally analyzed, and the following conclusions were drawn. 1. It was found that of sociodemographic characteristics, age and educational background correlated to the prevalence of periodontal diseases. With increasing age and poorer educational background, the rates of periodontal diseases increased(p<0.001). 2. It was revealed that of general health behaviors, smoking, alcohol drinking and BMI correlated with the prevalence of periodontal diseases. Higher rates of periodontal diseases were shown in the group of patients who were current smokers(p<0.05), had the higher frequency of drinking per week(p<0.001), and had higher BMI(p<0.005). 3. It was ascertained that of oral health behaviors, the history of oral health examination in the previous year, the use of proxabrush or dental floss, and perceived(i.e., subjective) oral health status correlated to the prevalence of periodontal diseases. The higher rates of periodontal diseases were seen in those who had no history of oral health examination in the previous year(p<0.001), had used neither proxabrush nor dental floss(p<0.05), and perceived that their own oral health was poor(p<0.001).

  • Purpose: The present study aimed to evaluate the clinical benefit of additional toothbrushing accompanying non-surgical periodontal treatment on oral and general health in patients with type 2 diabetes mellitus (T2DM). Methods: We conducted a doubled-blind randomized controlled trial in 60 T2DM patients between June 2013 and June 2014. The patients were randomly assigned to the scaling and root planing (SRP) group; the scaling and root planing with additional toothbrushing (SRPAT) group, in which additional toothbrushing was performed by toothpick methods; or the control group. Microbiological and oral examinations were performed for up to 12 weeks following treatment. Non-surgical treatment was conducted in the experimental groups. The SRP group received scaling and root planing and the SRPAT group received additional toothbrushing with the Watanabe method once a week from the first visit through the fifth visit. The primary outcomes were changes in haemoglobin A1c (or glycated haemoglobin; HbA1c) levels, serum endotoxin levels, and interleukin-1 beta levels. Periodontal health status was measured by periodontal pocket depth, the calculus index, and bleeding on probing (BOP). Results: Both the SRP and SRPAT groups showed improvements in periodontal health and HbA1c, but the SRPAT group showed significantly less BOP than the SRP group. Furthermore, only the SRPAT group showed a statistically significant decrease in serum endotoxin levels. Conclusions: Non-surgical periodontal treatment was effective in improving HbA1c and serum endotoxin levels in T2DM patients. Furthermore, non-surgical treatment with additional tooth brushing had a more favourable effect on gingival bleeding management. • Background: The purpose of this study was to evaluate the association between metabolic syndrome and periodontal disease in Korean adults. Methods: This study analyzed the screening data of 12,686 adults aged ≥19 years, including demographic characteristics, lifestyle habits, and each component of metabolic syndrome, obtained from the sixth Korea National Health and Nutrition Examination Survey database. Periodontal health status was measured by the community periodontal index. Subjects with three or more risk factors were considered as having metabolic syndrome. The Rao-Scott chi-square test was performed to assess the relationships between demographic characteristics, lifestyle habits, components of metabolic syndrome, and periodontal disease. Logistic regression analysis was performed based on the complex sample to evaluate the relationship between metabolic syndrome and periodontal disease. Results: The prevalence of periodontal disease was higher among the subjects with advancing age, lower average household income and education level, those working in production, residents of eup-myeon areas, in past or current smoker, those with excessive alcohol consumption habit in a week, and reduced brushing frequency and the use of oral care products (p<0.001). Each component of metabolic syndrome was associated with higher prevalence of periodontitis in the subjects with abnormal than in those with normal levels (p<0.001). The prevalence of periodontal disease in subjects with metabolic syndrome was approximately 1.443 times higher than that in normal subjects (odds ratio,1.443; p<0.001). Conclusion: This study confirmed the association between metabolic syndrome and periodontitis in Korean adults, and further studies will be needed to determine the causal relationship between the two conditions.

  • Since 2013, adults aged over 20 can receive national health insurance scaling once a year in South Korea. In this study, we analyzed the usage status of national health insurance care service for periodontal disease in 2010-2018 by using Healthcare big data of the Health Insurance Review and Assessment Service. The increase rate of the dental care users was very high at 7.8 and 11.2% in 2013 and 2014, respectively. These are higher than the increase rate of all medical institution users, which is between -1.7 and 3.7%. In 2017, the rate of dental use was 44.4%, which has increased more than 10% compared to 2012. Percent receiver of national health insurance scaling was 19.5% in 2017. The 20s had the highest rate of 23.2%. The rate decreased with age. Based on these results, it can be evaluated that the expansion of national health insurance coverage for scaling improves accessibility to dental care. A more long-term assessment of the effect of periodic dental examination and scaling on reducing the prevalence of periodontal disease is needed. National health insurance coverage should be extended to oral hygiene education and supportive periodontal therapy in order to prevent periodontal disease.

  • Objectives: This study aimed to examine the association between age at menarche and two major oral diseases. Methods: Data from the 7th Korean National Health and Nutrition Examination Survey (2016-2018) were used. The participants included girls and women aged 12 years and above. Thedataofa total of 7,420 participants were analyzed. Statistical analyses were performed using complex samples cross-tabulation analysis and complex samples general linear model. Results: The average age at menarche of those aged 12 to 80 years was 13.81 years. The age at menarche was found to be associated with decayed-missing-filled teeth (DMFT) (p<0.05). When the age at menarche was nine years, DMFT was 4.20 when it was 19 years, DMFT was 10.80. The age at menarche was found to be associated with periodontal tissue state (p<0.05). The group with early menarche age had healthier periodontal tissues (17.1%), whereas the group with late menarche age had more unhealthy periodontal tissues (17.9%) (p<0.001). Conclusions: The age at menarche was associated with two major oral diseases. Therefore, it is necessary to provide women with oral hygiene care and oral health education to prevent major oral diseases before their secondary sex characteristics appear.

  • Objectives: The aim of this analysis was to investigate factors associated with dental visits in terms of getting diagnosis and receiving care for diagnosed dental diseases among Korean adults. Methods: Data used in this analysis were from the 2005 Korean National Health and Nutrition Examination Survey which involved a representative sample of 25,487 adults nineteen years of age and older who resided in Korea. This analysis used data of adults who had completed health interview survey (n = 25,215). Uni- and bi-variate analysis, Chi-square test, and logistic regression analyses. were conducted using SAS. Results: The percentages of people diagnosed (treated) dental canes, periodontal disease. and temporomandibular joint disorder were 70.4(77.1), 16.1 (55.7), and 0.6(37.4), respectively. The reception of treatment for diagnosed dental caries and periodontal disease was significantly associated with older age, high monthly household income, and high education attainment. Logistic regression model indicated that age, monthly household income, education attainment, type of health insurance, and chewing problem were significantly associated with getting diagnosis and treatments of dental diseases. Most frequently answered reason for non - or delayed treatments of diagnosed dental diseases during the last one year was cost of treatments. Conclusions: The findings indicated that socio-economic-status still significantly affected reception of needed dental treatments among Korean adults despite the national health insurance system. Oral health policy and programs should be augmented to provide further support to adults of low socio-economic-status who are more prone to dental diseases yet lack resources for needed dental treatments.
