Comparison and Evaluation of Hematological Indices for Assessment of Iron Nutritional Status in Korean Pregnant Women(III) (우리나라 임신부의 혈액학적 철분 영양상태 평가 지표의 비교 분석 및 판정 (III))

  • The purpose of this research is to assess th iron nutritional status of pregnant women and to evaluate the appropriateness of the present cut off levels of hemoglobin(Hgb), hematocrit(Hct) and total iron binding capacity(TIBC) for assessing iron deficiency status. Pregnant women who were visiting public helath centers in Ulsan were interviewed and agreed to attend the study. Blood sample was taken and biochemical analysis of blood was performed. The collected data were classified into 3 trimesters by gestational age and then statistical analysis was performed. The prevalence of anemia in all subjects was 32.3% by WHO criteria(Hgb < 11.0g/dl) and 17.8% of all subjects was iron deficient anemia by CDC criteria(Hgb < 11.0/dl and serum ferritin < 12.0ug/l). Since the iron deficient anemia generally occures at the last stage of iron deficiency, it is not efficient to diagnose and prevent the iron deficient anemia in pregnant women by using the present cut off level of Hgb. Therefore, the new cut off level of iron status indices is necessary for assessing iron deficiency in early pregnancy before manifestation of anemia and for reducing the prevalence of anemia in later pregnancy. For this reason, the present cut off levels of iron status indices were estimated and compared by assessing the iron deficiency judged by serum ferritin level (<12.0ug/l)as true iron deficiency. It follows from the results of this research that present cut off levels of Hgb, Hct and TIBC were very insensitive in identifying the subjection with iron deficiency. The appropriate cut off levels of Hgb were 11.5g/dl for total period of pregnancy, 12.0g/dl for 1st and 3rd trimester, and 11.5g/dl for 2nd trimester. The cut off level of Hct was 34.0% for total period for pregnancy, 35.0% for 1st trimester, and 34.0% for 2nd and 3rd trimester. The cut off level of TIBC was 400ug/dl for total period, 360ug/dl for 1st 2nd trimester, and 450ug/dl for 3rd trimester.

Noise-Induced Hearing Loss in the Police Force

  • Background: Noise-induced hearing loss (NIHL) is a major preventable occupational health problem with 250 million people worldwide known to have disabling impairment of moderate to greater severity. The aims of the study are to estimate the prevalence of NIHL in the police force; and study its association with age, sex, duration of service (years), smoking and alcohol habits, use of hearing protective devices, as well as preexisting chronic diseases. Methods: A cross-sectional study was conducted on 543 police personnel who had undergone periodic medical examination over a 12-month period. The diagnostic criteria for NIHL were (1) history of occupational noise exposure, (2) bilateral hearing loss, (3) hearing loss of ${\geq}25dBA$ at 4,000 Hz in two consecutive audiograms, and (4) no significant medical history affecting hearing. Severity of NIHL was based on the World Health Organization grading. Results: Males (74.8%) made up the majority of the police force. The mean age for police personnel was $35.55{\pm}9.57years$, and the mean duration of service was $14.75{\pm}9.39years$. Compliance with the usage of hearing protective devices was seen in 64.4%. The prevalence of NIHL in this study population was 34.2%, with a higher prevalence in males (37.7%) than in females (23.9%). The study also showed strong associations between NIHL and male sex (odds ratio, 1.9; P < 0.05), and hypertension (odds ratio, 3.3; P < 0.001). Overall, 93% were found to have mild NIHL, 3.5% had moderate NIHL, and 3.5% had severe NIHL. No police personnel were found to have profound hearing loss. Conclusion: The prevalence of NIHL in this study is high compared to other similar studies among police personnel. This study shows that increasing age, male, presence of hypertension, diabetes, and longer duration of service are significant associated factors for NIHL. Preventative strategies include health surveillance, implementation of a hearing conservation program, and legislation.

Survey on the Symptoms Related to Hair Dyeing among University Freshmen (일개 대학 신입생의 모발 염색 실태와 관련 증상)

  • Objectives : This study was carried out tn investigate the prevalence and symptoms related to hair dyeing among university freshmen. Methods : The authors conducted a questionnaire survey among 1,499 university freshmen from Mar 11 to 15, 2002. Results : The study group contained 710 tenses and 789 males. Up until 2002, 62.7% of the subjects had experienced hair dyeing, and this was significantly higher in females (66.2%, p<0.05), The period of first experience of hair dyeing was in high school for 361 cases (38.4%), after high school for 345 cases (36.7%t and before high school for 234 cases (24.9%), The major reasons of hair dyeing were 'to improve their appearance' in 465 cases (49.6%), and 'to follow the hair dyeing fashion' in 169 cases (18.0%). The prevalence of hair dyeing in 2002 was 47.8%, and again was significantly higher in females (53.7%, p<0.05). The major symptoms related to hair dyeing were 'cleaved and nonelastic hair' in 495 cases (69.0%), and 'thin and easily breakable hair' in 359 cases (49.3%). Of those, 361 eases (50.4%) appealed to three or more symptoms related to hair dyeing, Through multiple logistic regression, factors significantly associated with symptoms related to hair dyeing were found to be female (OR=2.14, 95% CI; 1.61-2.83), use of hair dryer (OR=1.36, 95% CI; 1.004-1.054), a frequency of hair dyeing of three or more (OR=1,40, 95% CI; 1.04-2.09), and a duration of processing hair dyeing of over 60 minutes (OR=2.18, 95% CI; 1.50-3.18). Conclusions : The prevalence and experience of hair dyeing were generally high among university freshmen. Therefore, more extensive epidemiological studies on the symptoms related to hair dyeing should be conducted.

Health Condition and Musculoskeletal Disorders (MSDs) in Fruit-growers (과수작목 농업인의 건강실태 및 근골격계 통증호소율)

  • The objective of this study was to analyze the factors regarding work-related musculoskeletal disorders (MSDs) in fruit-growers and offer the fundamental data for work improvement. The 587 fruit-growers (409 males and 178 females) working with 5 kinds of fruit: apple, pear, peach, grape, and mandarin participated in a questionnaire survey regarding MSDs and health condition in 2004 and 2005. The results are as follows: 1. Smoking rate was highest in apple growers. 2. Drinking rates were highest in male peach growers and in female grape growers. 3. Regular exercise rates were higher in pear and mandarin growers. 4. Physical and mental fatigue was higher in females than in males in most cases. 5. The prevalence rate of medically diagnosed diseases was highest for osteoarthritis (16.4%), herniated nucleus pulposus (HNP, 13.5%), and chronic gastritis/gastric ulcer (10.5%). As well as, the prevalence of dermatosis was higher in pear and mandarin growers. 6. The prevalence rate of musculoskeletal symptoms among the various pain areas was highest for lower back, shoulders, and knees. The prevalence rate was also high for lower back, shoulder, and knee pain in apple and grape growers. 7. The significant indexes used determining the musculoskeletal symptoms were BMI, working period, and regular exercise. These results can be practically used for work improvement for the fruit-growers to prevent MSDs.

Human Papillomavirus (HPV) Infection in Women Participating in Cervical Cancer Screening from 2006 to 2010 in Shenzhen City, South China

  • Purpose: Human papillomavirus (HPV) infection plays an important role in the development of cervical cancer, but the prevalence of HPV infection in women of Shenzhen city remains unclear. The present study was performed to describe the change of cervical HPV infection in females who participated in voluntary cervical cancer screening from 2006 to 2010 in Shenzhen city, China. Methods: A total of 4, 413 women were recruited. HPV infections were genotyped by polymerase chain reaction (PCR) and reversed dot blot hybridization in Shenzhen Maternity and Child Health Hospital. Results: The prevalence of HPV infection was 13.8%. The five most commonly found HPV types were HPV16 (3.47%), HPV58 (1.68%), HPV33 (1.38%), HPV43 (1.36%) and HPV18 (1.27%). The secular trends of major HPV type-specific were diverse. Among of them, the prevalence of HPV18 increased sharply while others increased slowly or even decreased in the period. The change of total HPV, single HPV and multiple HPV infection were similar during the five years. Conclusions: Our findings suggested that HPV infection is common with HPV16 and HPV 58 as the primary subtypes in women in Shenzhen city.The prevalence of HPV 18 infection is increasing faster than any others, which will lead it to be one of the main subtypes in this city in the future.

Infection Patterns of Porcine Epidemic Diarrhea Virus (PEDV) by Sera-epidemiological Analysis in Korean Pig Farms (혈청역학적 분석을 통한 한국의 돼지 유행성 설사병 바이러스 장염양상)

  • To investigate the infection patterns of porcine epidemic diarrhea virus (PEDV) in Korean pig farms, a total of 4,768 swine sera samples from 159 pig farms were taken twice, in June (n=82) and October (n=77) in 2007. In each farm selected for the survey, 10 samples from breeding pigs and 4 from each of the 5 age groups (30, 60, 90, 120, and 150 days) were taken, and all serum samples were tested for PEDV by the serum neutralization test. The overall seroprevalence was 62.6% (2,983/4,768), with the highest prevalence in breeding pigs (93.5%, 1,485/1,589). The prevalence showed an increasing trend with increasing age (30.8, 27.2, 44.7, 61.6, and 71.2% respectively in the 30, 60, 90, 120, and 150 days age groups) (p<0.0001 for $x^2$ trend test). The association between age and PEDV prevalence was similar in both surveys, indicating that the infection of PEDV seemed to be occurring repeatedly in the farms surveyed. This inference could also be explained by the fact that prevalence in sows was very high despite low vaccination coverage, as they are continuously exposed to PEDV in potentially infected farms for a longer period. Based on the neutralizing antibody levels in sows and growing pigs, the majority of farms (91.8%, n=146 farms) were endemically infected with PEDV, and most of pigs seemed to be intensively infected with PEDV at around early growth (41.8%) and weaning (31.5%). On the other hand, serum neutralizing antibodies were not detected in pigs older than 30 days of age in farms classified as having no PEDV infection (n=13 farms), indicating the level of maternal antibody against PEDV is decreased on a non-detectable level before the piglet is 60 days old in the field situation. The results indicated that most farms surveyed in 2007 were affected with endemic PEDV infection. Therefore, a national monitoring and control program for the endemic type PEDV infection needs further attention.

Use and Effects of Malaria Control Measures in Pregnancy in Lagos, Nigeria

  • In Nigeria, malaria causes up to 11% of maternal mortality. Our main aim was to find out the most common mosquito control measures employed by the pregnant women in Lagos and their effects on malaria infection. The study was carried out over a period of 6 months during which trained interviewers administered questionnaires to 400 pregnant women. The prevalence of malaria was 8.4%. There was no significant association between the prevalence of malaria and age, level of education, or occupation of the participants. Pregnant women in the age range 26-30 had the mean parasite density ($409.9{\pm}196.80$). Insecticide spray (32.8%), mosquito coil (27.5%), and insecticide-treated nets (ITN) (15.5%) were the major mosquito control measures employed by the participants while the prevalence of infection among them were 2.3%, 6.2%, and 3.2%, respectively (P<0.05). Only 18.3% of the women had taken more than one dose of intermittent preventive treatment (IPT), while another 11.8% had taken a single dose. The infection rate among them was 4.1% and 6.4%, respectively. Malaria prevalence was highest among those who had not received any dose of IPT (10%). This study showed that the use of ITN and IPT among the pregnant women were still unacceptably low. It also showed that the use of insecticide spray which was the most common malaria control measure adopted by the participants was effective despite the fact that it is not a National Malaria Control Policy. We recommend that a sustained integrated mosquito man-agement and public education should be strengthened in Nigeria.

A Clinical Study of According to Prevalence of Dermatoses in a Oriental Medicine Hospital of Gang-Dong Area in Seoul (서울 강동지역 소재 한방병원 피부과 내원환자의 질환 빈도에 따른 임상적 관찰)

  • Objective and Methods : To study the prevalence and patterns of common dermatoses and compare these to previous reports, we reviewed 128 new outpatients who visited Allergy/Derma-cosmetic Clinic at Kyung Hee Neo Medical Center in Gang-Dong area of Seoul from January to June, 2007. Results : The results of this study are summarized as follows : 1. Among the 128 outpatients, the total number of male patients were 40(31.2%) and female patients were 88(68.8%). 2. In the age distribution, the most frequently visited age groups were 10-19 years old and 20-29 years old(each 21.1%). 3. The most common dermatoses were atopic dermatitis(27.3%), acne(14.1%), allergic contact dermatitis(11.7%), other eczemas(7.8%) and seborrheic dermatitis(7.0%). 4. Among atopic dermatitis patients, the most populated age groups were 0-9 years old and 10-19 years old(each 37.1%), these results are different from past studies revealing that 0-9 years old patients are dominant. 5. Among acne patients, the most frequent age groups were 20-29 years old(50%) and 30-39 years old(27.8%), these results are different from past studies revealing that 20-29 years old and 10-19 years old patients are dominant. 6. Among Allergic contact dermatitis patients, each age group showed no difference of population. And Allergic contact dermatitis patients visited outpatients department at early stage. Conclusion : This study shows that Oriental medicine is popular especially in atopic dermatitis, acne and allergic contact dermatitis. We should investigate long-period and multi-centered study in order to know prevalence tendency of dermatoses in Oriental Medicine. And it is nessessary to research using more efficient evaluation method and treatment for these special diseases.

Association between smoking cessation and metabolic syndrome in Korean Men: Korean National Health and Nutrition Examination Survey 2013-2014 (한국 남성의 금연과 대사증후군과의 관련성: 2013-2014년 국민건강영양조사 활용)

  • Objectives: This study aimed to investigate the association between smoking cessation and metabolic syndrome components in Korean male and to clarify how long metabolic syndrome and its components remains after smoking cessation. Methods: A total of 4,408 participants from the Korean National Health and Nutrition Examination Surveys were included in this cross-sectional study. Smoking status was categorized into three groups, current-smoker, ex-smoker and non-smoker. Also smoking cessation period was categorized into three groups, less than 5 years, 5 years to 15 years and more than 15 years. Results: Smoking cessation were associated with prevalence of metabolic syndrome and its components. The multi-variate adjusted odds ratio for the prevalence of metabolic syndrome was 1.68(1.22-2.31) for smoker, 1.04(0.74-1.46) for ex-smoker, as compare with non-smoker; adjusted odds ratio for the prevalence of hypertriglyceride 2.17(1.77-2.66) for smoker, 1.27(1.02-1.59) for ex-smoker and high waist circumference 1.36(0.98-1.88) for smoker, 1.07(0.78-1.48) for ex-smoker, as compare with non-smoker. Conclusions: Smoking cessation in Korean male reduced odds ratio for the prevalence of metabolic syndrome and its 4 components except for hypertension.

The changes of prevalence and etiology of pediatric pneumonia from National Emergency Department Information System in Korea, between 2007 and 2014

  • Purpose: Understanding changes in pathogen and pneumonia prevalence among pediatric pneumonia patients is important for the prevention of infectious diseases. Methods: We retrospectively analyzed data of children younger than 18 years diagnosed with pneumonia at 117 Emergency Departments in Korea between 2007 and 2014. Results: Over the study period, 329,380 pediatric cases of pneumonia were identified. The most frequent age group was 1-3 years old (48.6%) and the next was less than 12 months of age (17.4%). Based on International Classification of Diseases, 10th revision diagnostic codes, confirmed cases of viral pneumonia comprised 8.4% of all cases, pneumonia due to Mycoplasma pneumoniae comprised 3.8% and confirmed cases of bacterial pneumonia 1.3%. The prevalence of confirmed bacterial pneumonia decreased from 3.07% in 2007 and 4.01% in 2008 to 0.65% in 2014. The yearly rate of pneumococcal pneumonia also decreased from 0.47% in 2007 to 0.08% in 2014. A periodic prevalence of M. pneumoniae pneumonia (MP) was identified. Conclusion: The increased number of patients with pneumonia, bacterial pneumonia, pleural effusion, and empyema in 2011 and 2013-2014 resulted from an MP epidemic. We provide evidence that the frequency of confirmed cases of bacterial pneumonia and pneumococcal pneumonia has declined from 2007 to 2014, which can simultaneously reflect the effectiveness of the pneumococcal conjugate vaccine.
