  • A case of bronchogenic cyst associated with a partial pericardial defect is reported. Bronchogenic cysts are not so rare in incidence, but they are more rare when associated with a pericardial defect, the first case being reported by Rusby and Sellors in 1945. Recently, we experienced such a rare case of a bronchogenic cyst with a partial pericardial defect. The patient is a 39-year-old female and she was found to have a left anterior mediastinal mass during routine chest X-ray. During the operation, we detected partial pericardial defect after removal of the mediastinal mass. The pericardial defect was repaired with a Gore-Tex Membrane. The pathological examination of the mass showed a bronchogenic cyst. The patient had an uneventful hospital course.

선천성 좌측 심낭결손증 [1례 보고] (Congenital Left Pericardial Defect: A Case Report)

  • Congenital pericardial defect is a rare anomaly, which was first described by M. Columbus in 1559. Four hundred years later the first clinical diagnosis was reported by Ellis et al. The congenital pericardial defect Is usually asymptomatic and Is found Incidentally at thoracotomy and autopsy, but it appears that partial absence of pericardium Is not Innocuous because of sudden death due to herniation of a portion of heart. We experienced congenital left pericardial defect in 20 year old female who was diagnosed as left ventricular aneurysm before operation. This patient complained of dyspnea on exertion and anterior chest discomfortness. Physical examination revealed Grade II pansystolic murmur on the 3rd and 4th intercostal space left sternal border. There were specific abnormal findings on the chest plain film, EKG, ultrasonography, and left ventriculography. On 9th July 1981, an operation was performed and found the left partial pericardial defect through which a large portion of left ventricle was herniated Into left pleural space. The method of operation was removal of adhesion and widening of the pericardial defect to avoid Incarceration. After operation, we observed marked Improvement of symptoms and disappearance of cardiac murmur.

  • We recently experienced a case of bronchogenic cyst associated with situs inversus and partial pericardial defect. The patient was 26-day-old-male who showed severe respiratory difficulty. Left upper lobectomy and direct suture of partial pericardial defect were undergone successfully, but his postoperative course was unfortunate because of respiratory insufficiency.

Does Early Drain Removal Affect Postoperative Pericardial Effusion after Congenital Cardiac Surgery?

  • Background: Patients undergoing cardiac surgery require postoperative chest drainage. However, the drain is difficult to keep in place in children with congenital heart disease. Since 2015, at Kyungpook National University Hospital, the chest tube is removed on postoperative day 1 in patients who have undergone simple congenital cardiac surgery (i.e., closure of an atrial or ventricular septal defect). In this study, we evaluated the relationship between the duration of drain placement and the likelihood of pericardial effusion after congenital cardiac surgery. Methods: The medical records of patients who underwent closure of an atrial or ventricular septal defect at our hospital between January 2014 and December 2016 were reviewed. In total, 162 patients who received follow-up echocardiography and had information available on postoperative pericardial effusion after the repair procedure were enrolled. Results: Echocardiography was performed at a median of 5 days (range, 4 to 6 days) postoperatively before discharge from the hospital. Pericardial effusion occurred in 21 patients (13.0%), of whom only 3 (1.9%) had moderate or greater pericardial effusion, regardless of the drain duration. All patients improved during outpatient follow-up without invasive management. No patient had severe complications because of pericardial effusion. The duration of drain placement did not affect the incidence of postoperative pericardial effusion (p=0.069). Operative survival was 100%. Conclusion: Based on our study, we recommend removing the drain as soon as its role is complete, generally on postoperative day 1, because early removal does not increase the incidence of pericardial effusion in patients undergoing simple congenital cardiac surgery.

  • 본 저자들은 활로씨 4징증을 동반한 완전방실중격결손 환자에서 완전교정수술을 시행하였다. 술전 검사로 심초음파, 심도자검사, 심조영술을 시행하였다. 수술은 체외순환을 시행하여 두 개의 첩제를 이용하여 방실중격결손을 폐쇄하였고, 우심실절개와 폐동맥판교련술을 통해 우심실 누두부 절제술과 경판륜 첩제를 사용하여 우심실유출로 확장술을 시행하였다. 술후 시행한 초음파 검사상 경도의 승모판과 삼첨판의 폐쇄부전이 나타났으나 혈역학적 이상소견은 없었다.

  • A 75-year-old man was admitted to the hospital because of a pericardial effusion.After 3 L of blood-stained pericardial fluid was drained, clinical examination together with echocardiography and chest computed tomography showed a tumor in the right atrium. At operation a pedunculated vascular tumor was found with a broad base which was embedded in the atrial wall and extended into the pericardium.A wide resection was performed resulting in a large defect of the right atrial wall. The defect was reconstructed with a pericardial patch. The patient did well postoperatively, but bloody pleural effusion developed later, presumably because of pulmonary metastasis. The patient died 2 months after surgery as a result of respiratory failure.

Left Ventricular Pseudo-pseudoaneurysm with Hemopericardium

  • We report a case of pseudo-pseudoaneurysm, which is a very rare complication of myocardial infarction. A 69-year-old man was admitted to our clinic with chest tightness and dyspnea. He had undergone aortic valve replacement with a pericardial bioprosthetic valve, ring mitral annuloplasty, and reconstruction of an aortic annular defect due to infective endocarditis with bovine pericardium 4 years prior. Echocardiography and computed tomography showed pericardial effusion and a 16-mm cavity at the anterolateral wall of the left ventricle. Magnetic resonance imaging suggested either pseudo-pseudoaneurysm or myocardial abscess. We successfully repaired the myocardial defect using a patch made from a vascular graft with pledgeted horizontal mattress sutures under cardiopulmonary bypass.

  • After a penetrating thoracic injury early detection of intracardiac injury and early surgical repair when indicated are essential. A case presenting severe respiratory distress two weeks after a penetrating thoracic injury is reported. Transesophageal echocardiography showed massive pericardial effusion ventricular septal defect and mirtal regurgitation, The infundibular ventricular septal perforation was repaired using a Dacron patch the anterior mitral leaflet by interrupted sutures and the ruptured chordae of the posterior leaflet by a new chordae formation.

원발성 우심방 횡문 근육종 -1례 보고- (Primary Rhabdomyosarcoma of the Right Atrium -A Case Report-)

  • Primary cardiac tumors are extremely rare and about 20-25% of primary cardiac tumors are malignant. Recently we have experienced a case of primary cardiac rhabdomyosarcoma originated on the right atrial wall. Patient was 41 years-old women and chief complaints were pitting edema and exertional dyspnea. Emergency operation was performed to relieve symptoms by pericardial effusion that hac been seen on the echocardiogram and large soft mass in right atrial cavity on the MRI. About 700ml amounts of bloody pericardial effusion was collected into the pericardial cavity. 4X5X5 cm sized ingrowing soft mass in the right atrial cavity and two small sized outgrowing masses were inspected. A tumor was invaded into the SA node and superior limbic portion of fossa ovalis. And then mass & right atrial free wall was resected but areas of SA node and fossa ovalis were not resected. Defect of right atrial wall was reconstruced with bovine patch. She was discharged after 2 weeks without any problems and she has been treated with chemotherapy.

방실중격결손증의 외과적 치료 (Surgical Treatment of Atrioventricular Septal Defect)

  • Twelve patients had undergone repair of atrioventricular septal defects. Age at operation ranged from 2.4 years to 17 years[mean, 8.25 years]. Five patients were male and seven were female. Three patients had complete atrioventricular septal defect[Rastelli type A] associated with Down`s syndrome. One of the three patient with complete atrioventricular septal defect had tetralogy of Fallot. Three patients had the intermediate form and seven patients had the partial form. The primum atrial septal defect was closed with pericardial patch. The atrioventricular valve septal commissure[mitral cleft] was closed with pledgeted sutures. Three complete atrioventricular septal defect were undergone by two-patch technique. A crescent-shaped Dacron patch was used to occlude the ventricular septal defect. One patient of partial form was sudden death 5 days postoperatively. There were no another complications after surgery. One patient underwent reoperation for opened mitral cleft 2.5 years postoperatively. New York Heart Association functional class of patients was improved postoperatively.
