목적: 뇌종양에서 자기공명 뇌혈류량지도(MR cerebral blood volume map)의 유용성을 평가하고자 하였다. 대상 및 방법: 15예의 두개강내 종괴(다형성 교모세포종 2예, 저등급 교종 3예, 뇌농양 2예, 뇌수막종 3예, 신경세포종 1예, 배아종 1예, 방사선괴사 1예, 전이암 1예) 에서 관류 MR영상을 수술전에 시행하였다. 환자의 평균연령은 42세였고(22-68), 남자 10명, 여자 5명이었다. MR영상기기는 1.5T unit(Signa, GE Medical Systems, Milwaukee, Wisconsin)를 사용하였다. 조영제 주입후 조영제의 일차 통과시 나타나는 자화율을 얻었다. (조영제는 최오의 MR 촬영시작후 10초부터 시작하여, 총량 15cc 의 Gadopentate dimeglumine(Magnevist)를 약 2ml/sec의 속도로 손으로 주입하였다). 각 환자마다 160초 동안 6 slice에서 slice당 80 image씩 총 480 image를 얻었으며 interleaved single shot gradient EPI기법을 사용하였다. 영상변수는 TR 2000ms, TE 50ms, FOV $240{\times}240mm,{\;}matrix{\;}size{\;}128{\times}128$, slice thickness/gap 5/ 2.5mm, flip angle $90^{\circ}$로 하였다. 얻은 영상데이터는 GE workstation으로 전송한 후, 자체적으로 개발한 software에 의해 각 kvoxel마다 시간경과에 따른 신호크기의 로그변화곡선(${\Delta}R2^{*}=-In(S/S_0$)의 적분값을 구하여 국소뇌혈류량(rCBV)영상을 구성하였다. 이의 시각적 해석을 용이하게 하기 위해 정상 뇌백질의 관류정도를 기준으로 하여 상대적인 RGB 색수치로 변환하여, color rCBV map을 얻었다. 관류의 정도와 조영증강정도를 중심으로 관류 MR 영상소견과 조직학적 소견을 관련지어 분석하였다. 결과: 조영증강 T1강조MR영상에서 환상조영증강을 보이는 다형성 교보세포종 2예에서는 변연부 외륜이 고관류를, 중심부의 괴사부위는 저관류로 나타났다. 저등급 교종은 경계가 불분명한 저관류부위로 보였다. 뇌농양 2예는 변연부 외륜이 경도의 고관류를, 중심부는 저관류로 나타났다. 뇌수막종은 미만성의 균일한 중등도 혹은 고도의 고관류로 보였으며, 임파종과 배아종은 경계가 명확한 저관류부위로 나타났다. 신경세포종은 종괴\ulcorner 일부에 중등도 혹은 고도의 고관류부위가 관찰되었고, 전이암은 다수병변중 일부에서 중등도의 고관류를 보였다. 방사선괴사는 저관류부위내에 국소적 고관류부위를 보였다. 결론: 관류 MR영상은 뇌종양의 관류상태를 비교적 잘 반영하며, 조직학적 특성을 예측하는데에 도움을 주 수 있을 것으로 기대된다. 뇌종야에서의 관류MR영상의 분명한 역할을 규명하기 위해서는 앞으로 더 많은 임상적 연구가 필요할 것으로 생각된다.

Objective: To determine the usefulness of perfusion MR imaging in assessing the histologic grade of cerebral gliomas. Materials and Methods: In order to determine relative cerebral blood volume (rCBV), 22 patients with pathologically proven gliomas (9 glioblastomas, 9 anaplastic gliomas and 4 low-grade gliomas) underwent dynamic contrast-enhanced T2*-weighted and conventional T1- and T2-weighted imaging. rCBV maps were obtained by fitting a gamma-variate function to the contrast material concentration versus time curve. rCBV ratios between tumor and normal white matter (maximum rCBV of tumor / rCBV of contralateral white matter) were calculated and compared between glioblastomas, anaplastic gliomas and low-grade gliomas. Results: Mean rCBV ratios were 4.90°±1.01 for glioblastomas, 3.97°±0.56 for anaplastic gliomas and 1.75°±1.51 for low-grade gliomas, and were thus significantly different; p < .05 between glioblastomas and anaplastic gliomas, p < .05 between anaplastic gliomas and low-grade gliomas, p < .01 between glioblastomas and low-grade gliomas. The rCBV ratio cutoff value which permitted discrimination between high-grade (glioblastomas and anaplastic gliomas) and low-grade gliomas was 2.60, and the sensitivity and specificity of this value were 100% and 75%, respectively. Conclusion: Perfusion MR imaging is a useful and reliable technique for estimating the histologic grade of gliomas.
Therapeutic hypothermia in cardiac arrest patients is associated with favorable outcomes mediated via neuroprotective mechanisms. We report a rare case of a 32-year-old male who demonstrated complete recovery of signal changes on perfusion-weighted imaging after therapeutic hypothermia due to cardiac arrest. Brain MRI with perfusion-weighted imaging, performed three days after ending the hypothermia therapy, showed a marked decrease in relative cerebral blood flow (rCBF) and delay in mean transit time (MTT) in the bilateral basal ganglia, thalami, brain stem, cerebellum, occipitoparietal cortex, and frontotemporal cortex. However, no cerebral ischemia was not noted on diffusion-weighted imaging (DWI) or fluid-attenuated inversion recovery (FLAIR) sequences. A follow-up brain MRI after one week showed complete resolution of the perfusion deficit and the patient was discharged without any neurologic sequelae. The mechanism and interpretation of the perfusion changes in cardiac arrest patients treated with therapeutic hypothermia are discussed.

Purpose: Cerebral perfusion, as measured by interictal SPECT and PET, is known to be decreased in the affected hippocampus of the patients with hippocampal sclerosis. The purpose of th study is to evaluate the capability of perfusion MR imaging to demonstrate ipsilate hypopefusion in patients with hippocampal sclerosis.
To measure regional cerebral blood volume (rCBV) with perfusion MR imaging of cerebral fat embolism by neutral fat and free fatty acids in cats. Triolein (group 1, n=15), oleic acid (group 2, n=9) and linoleic acid (group 3, n=11) were infused into unilateral internal carotid artery using microcatheter through the transfemoral approach. PVA particle was used as a non-fat embolic material in a control group (group 4, n=9). Perfusion-weighted MR image was obtained at 30 minutes and 2 hours postembolization, based on T2-and diffusion-weighted images. The data of lesion and contralateral normal area were transferred to personal computer, time-to-signal intensity curve was drawn and trans for used to △R2/sup */ curve in regular order. The process in the personal computer was done by using the author's developmental image processing program and interactive data language (IDL) softwares. Statistical significance was approved by paired t-test and ANOVA. rCBV of the lesion was decreased comparing to the normal area in all groups. The ratios of rCBV were as follows (group No, at 30 minutes, at 2 hours); group 1,32%, 51%; group 2, 30%, 44%; group 3, 39%, 61%; group 4, 21%, 36%. rCBVs of 2 hours was significantly increased compared to those of 30 minutes in all groups (P<0.005). rCBV was decreased at 30 minutes in cerebral fat embolism and recovered a little, but significantly at 2 hours. Perfusion-weighted images was useful method in offering hemodynamic information in cerebral fat embolism.
급성 뇌졸중의 경우 빠른 시간 내의 진단과 치료가 예후에 큰 영향을 미친다. 본 연구에서는 초급성기 뇌경색 환자에서 관류CT와 확산강조MRI의 영상을 비교하여 허혈 부위와 경색부위에 나타나는 차이점을 알아보고자 하였다. 뇌관류 CT와 확산강조 자기공명영상(diffusion weighted MR imaging, DW-MRI)을 시행한 12명의 급성뇌경색 환자를 대상으로 병변부위와 정상부위에서 각각의 CBF, CBV, MTT, TTP지도와 DW-MRI의 신호강도 값을 비교하고, 관류CT와 DW-MRI에서 병변의 크기 비교를 해보았다. CBF, CBV, MTT, TTP는 모두 관류결손을 보였고, 관류 결손이 인지되는 부위에서 MTT와 TTP시간의 현저한 지연이 있었다. 뿐만 아니라, MTT와 TTP 지도의 결손부위 면적은 DW-MRI 보다 크게 나타나 허혈성 반음영을 추측할 수 있었고, 일부 DW-MRI에서 경색부위를 나타내지 못하는 경우도 있었다. 결론적으로 관류 CT의 지도를 이용하면 뇌경색의 조기 진단뿐만 아니라 허혈 중심부, 그리고 허혈성 반음영을 예측하여 관류결손 부위의 혈류 역학적 상태를 평가 할 수 있어 보다 효과적인 치료계획을 세울 수 있다.

Background: Normalized cerebral blood volume (nCBV) can be measured using manual or semiautomatic segmentation method. However, the difference in diagnostic performance on brain tumor differentiation between differently measured nCBV has not been evaluated. Purpose: To compare the diagnostic performance of manually obtained nCBV to that of semiautomatically obtained nCBV on glioblastoma (GBM) and primary central nervous system lymphoma (PCNSL) differentiation. Materials and Methods: Histopathologically confirmed forty GBM and eleven PCNSL patients underwent 3T MR imaging with dynamic susceptibility contrast-enhanced perfusion MR imaging before any treatment or biopsy. Based on the contrast-enhanced T1-weighted imaging, the mean nCBV (mCBV) was measured using the manual method (manual mCBV), random regions of interest (ROIs) placement by the observer, or the semiautomatic segmentation method (semiautomatic mCBV). The volume of enhancing portion of the tumor was also measured during semiautomatic segmentation process. T-test, ROC curve analysis, Fisher's exact test and multivariate regression analysis were performed to compare the value and evaluate the diagnostic performance of each parameter. Results: GBM showed a higher enhancing volume (P = 0.0307), a higher manual mCBV (P = 0.018) and a higher semiautomatic mCBV (P = 0.0111) than that of the PCNSL. Semiautomatic mCBV had the highest value (0.815) for the area under the curve (AUC), however, the AUCs of the three parameters were not significantly different from each other. The semiautomatic mCBV was the best independent predictor for the GBM and PCNSL differential diagnosis according to the stepwise multiple regression analysis. Conclusion: We found that the semiautomatic mCBV could be a better predictor than the manual mCBV for the GBM and PCNSL differentiation. We believe that the semiautomatic segmentation method can contribute to the advancement of perfusion based brain tumor evaluation.

Susceptibility-weighted imaging (SWI) is well known for detecting the presence of hemorrhagic transformation, microbleeds and the susceptibility of vessel signs in acute ischemic stroke. But in some cases, it can provide the tissue perfusion state as well. We describe a case of a patient with hyperacute ischemic infarction that had a slightly hypodense, patchy lesion at the left thalamus on the initial SWI, with a left proximal posterior cerebral artery occlusion on a magnetic resonance (MR) angiography and delayed time-to-peak on an MR perfusion performed two hours after symptom onset. No obvious abnormal signals at any intensity were found on the initial diffusion-weighted imaging (DWI). On a follow-up MR image (MRI), an acute ischemic infarction was seen on DWI, which is the same location as the lesion on SWI. The hypointensity on the initial SWI reflects the susceptibility artifact caused by an increased deoxyhemoglobin in the affected tissue and vessels, which reflects the hypoperfusion state due to decreasing arterial flow. It precedes the signal change on DWI that reflects a cytotoxic edema. This case highlights that, in some hyperacute stages of ischemic stroke, hypointensity on an SWI may be a finding before the hyperintensity is seen on a DWI.
