Efficacy and Safety of Incontinence Surgery According to the Surgeon's Specialty and Performance of a Preoperative Urodynamic Study

  • Purpose: The aim of this study was to analyze the efficacy and to estimate the complication rate of incontinence surgery according to the surgeon's specialty and whether a preoperative urodynamic study (UDS) was performed, using a nationally representative dataset. Methods: We enrolled 356,155 women over 20 years old who had undergone surgery for stress urinary incontinence between 2006 and 2015. Patients were followed for up to 3 years to analyze the reoperation and complication rates. Data were obtained from the National Health Claims Database of the National Health Insurance Service (NHIS) of Korea. Multiple Cox regression analysis was conducted to examine the efficacy and safety of incontinence surgery according to the surgeon's specialty and whether a preoperative UDS was performed. Results: The hazard ratio (HR) for reoperation was significantly higher for procedures performed by nonurologists than for procedures performed by urologists (HR, 1.174; 95% confidence interval [CI], 1.103-1.249). Acute urinary retention, postoperative infections, procedure-associated pain, and other complications were also more common in procedures performed by nonurologists than in those performed by urologists. When stratified by whether a preoperative UDS was performed, the HR for reoperation according to the surgeon's specialty varied by performance of a preoperative UDS. While the reoperation rate was significantly higher in procedures performed by non-urologists when a preoperative UDS was performed (HR, 1.208; 95% CI, 1.122-1.3), there was no significant difference in the HRs for reoperation according to specialty when a preoperative UDS was not performed. Conclusions: This population-based study showed that the postoperative outcomes of incontinence surgery were dependent upon the surgeon's specialty and that the reoperation rate according to the surgeon's specialty varied based on whether a preoperative UDS was performed.

Observational Arc-Length Effect on Orbit Determination for KPLO Using a Sequential Estimation Technique

  • In this study, orbit determination (OD) simulation for the Korea Pathfinder Lunar Orbiter (KPLO) was accomplished for investigation of the observational arc-length effect using a sequential estimation algorithm. A lunar polar orbit located at 100 km altitude and $90^{\circ}$ inclination was mainly considered for the KPLO mission operation phase. For measurement simulation and OD for KPLO, the Analytical Graphics Inc. Systems Tool Kit 11 and Orbit Determination Tool Kit 6 software were utilized. Three deep-space ground stations, including two deep space network (DSN) antennas and the Korea Deep Space Antenna, were configured for the OD simulation. To investigate the arc-length effect on OD, 60-hr, 48-hr, 24-hr, and 12-hr tracking data were prepared. Position uncertainty by error covariance and orbit overlap precision were used for OD performance evaluation. Additionally, orbit prediction (OP) accuracy was also assessed by the position difference between the estimated and true orbits. Finally, we concluded that the 48-hr-based OD strategy is suitable for effective flight dynamics operation of KPLO. This work suggests a useful guideline for the OD strategy of KPLO mission planning and operation during the nominal lunar orbits phase.

Clinical Characteristics of Patients with Bronchioloalveolar Carcinoma: A Retrospective Study of 44 Cases

  • Background: Bronchioloalveolar carcinoma (BAC) is considered a subtype of adenocarcinoma of the lung. Recently BAC has been variously termed adenocarcinoma in situ, minimally invasive adenocarcinoma, lepidic predominant invasive adenocarcinoma, and invasive mucinous adenocarcinoma. The aim of the study was to analyze and detect prognostic factors of patients with BAC over a 7-year period. Materials and Methods: This retrospective single-center study included 44 patients with BAC. The impact on survival of fifteen variables (gender, age, smoking status, cough, dyspnea, hemoptysis, fever, chest pain, sputum, metastasis number, Karnofsky performance status, pT, pN, TNM stage, cytotoxic chemoterapy) were assessed. Results: Median age was 55 years (38-83). Most patients were male (63.6%) and stage IV (59.1%). Twenty-one patients (47.7%) received cytotoxic chemotherapy (platinum-based regimens) for metastatic disease. Objective response rate was 33.3% (4 partial, 3 complete responses). Stable disease was observed in nine in patients (42.8%). Disease progression was noted in 5 (23.8%). The median OS for all patients was 12 months (95%CI, 2.08-22.9 months). Independent predictors for overall survival were: Karnofsky performance status (HR:3.30, p 0.009), pN (HR:3.81, p 0.018), TNM stage (HR:6.49, p 0.012) and hemoptysis (HR:2.31, p 0.046). Conclusions: Karnofsky performance status, pN, TNM stage and hemoptysis appear to have significant impact on predicting patient survival in cases of BAC.

Dynamic Performance Evaluation of New Type PSC Railroad Bridges (신형식 PSC 철도교량의 동적성능 평가)

  • After the commercial opening of the KTX in 2005, the high speed railroad has been rapidly emerged as the major transportation means due to its high energy efficiency. Recently, the government has announced its plan to build the future transportation system around the high speed railroad. Based on this policy, the existing lines as well as the lines under construction or design are planning to increase design speed. In this paper, the suitability of the mid-span PSC girder bridges for the high speed railroad is evaluated via dynamic analysis. IT, Precom, and WPC girder bridges are considered for the purpose of this study and, for comparison, the identical modeling method and the analysis technique are utilized. The performance indices used for dynamic performance evaluation are the natural frequency, the vertical displacement, the end axial displacement, track irregularity, etc. The KTX train is utilized as a dynamic load, and the dynamic analysis is performed up to the train speed of 420km/hr with the increment of 10km/hr.

Cutting Performance of Si$_3$N$_4$ Based SiC Ceramic Cutting Tools

  • Composites of Si$_3$N$_4$-SiC containing up to 30 wt% of dispersed SiC particles were fabricated via hot-pressing with an oxynitride glass. To determine the effect of sintering time and SiC content on the mechanical properties and the cutting performance, the composites with fixed 8hr-sintering time and 20 wt% SiC content were fabricated and tested. Fracture toughness of the composites increased with increasing sintering time, while the hardness increased as the SiC content increased up to 20 wt%. The hardness of the composites was relatively independent of the grain size and the sintered density. For machining heat-treated AISI4140, the insert with 20 wt% SiC sintered for 8hr showed the longest tool life while the insert with 20 wt% SiC sintered for 12hr showed the longest tool life for machining gray cast iron. An effort was made to relate the mechanical properties, such as hardness, fracture toughness and wear resistance coefficient with the tool life. However, no apparent relationship was found between them. It may be stated that tool life is affected by not only the mechanical properties but also other properties such as surface roughness, density, grian size and the number of the inherent defects in the inserts.

Very short-term rainfall prediction based on radar image learning using deep neural network (심층신경망을 이용한 레이더 영상 학습 기반 초단시간 강우예측)

  • This study applied deep convolution neural network based on U-Net and SegNet using long period weather radar data to very short-term rainfall prediction. And the results were compared and evaluated with the translation model. For training and validation of deep neural network, Mt. Gwanak and Mt. Gwangdeoksan radar data were collected from 2010 to 2016 and converted to a gray-scale image file in an HDF5 format with a 1km spatial resolution. The deep neural network model was trained to predict precipitation after 10 minutes by using the four consecutive radar image data, and the recursive method of repeating forecasts was applied to carry out lead time 60 minutes with the pretrained deep neural network model. To evaluate the performance of deep neural network prediction model, 24 rain cases in 2017 were forecast for rainfall up to 60 minutes in advance. As a result of evaluating the predicted performance by calculating the mean absolute error (MAE) and critical success index (CSI) at the threshold of 0.1, 1, and 5 mm/hr, the deep neural network model showed better performance in the case of rainfall threshold of 0.1, 1 mm/hr in terms of MAE, and showed better performance than the translation model for lead time 50 minutes in terms of CSI. In particular, although the deep neural network prediction model performed generally better than the translation model for weak rainfall of 5 mm/hr or less, the deep neural network prediction model had limitations in predicting distinct precipitation characteristics of high intensity as a result of the evaluation of threshold of 5 mm/hr. The longer lead time, the spatial smoothness increase with lead time thereby reducing the accuracy of rainfall prediction The translation model turned out to be superior in predicting the exceedance of higher intensity thresholds (> 5 mm/hr) because it preserves distinct precipitation characteristics, but the rainfall position tends to shift incorrectly. This study are expected to be helpful for the improvement of radar rainfall prediction model using deep neural networks in the future. In addition, the massive weather radar data established in this study will be provided through open repositories for future use in subsequent studies.

  • Although previous studies show that organizational culture and organizational communication could be influential factors in knowledge management and organizational performance, the more integrated research to indicate relationships among all of the factors is still needed. By indicating the inter-relationships among the factors and the direction of the influence, this paper suggests various ways to develop an integrated approach to the improvement of organizational performance through knowledge management and other related factors. Thus, the purpose of this study is to investigate the relationships among national culture, organizational culture, organizational communication, knowledge management, and organizational performance. Based on a comprehensive review of extant literature on the relationships among these factors, the relationships are summarized in the conceptual model. According to the model, organizational performance is influenced by knowledge management, organizational communication, and organizational culture, and knowledge management is influenced by organizational communication and organizational culture. Based on the conceptual model, implications for human resource (HR) researchers and practitioners seeking to optimally improve organizational performance are presented.

How Can the Gender Pay Gap be Overcome?: The Effect of Rational HR System based on Management Philosophy of CEO (성별임금격차는 어떻게 완화되는가?: 최고경영자의 경영철학에 기반한 합리적 인사제도의 효과)

  • It is important to realize employment equality to fulfill corporate social responsibility. The most suitable indicator for assessing its performance is the gender wage gap. Korea is considered the country with the most severe gender wage gap among OECD member countries, however, studies on the gender wage gap have been mainly attempted to explain in terms of the structure of the labor market, government policies, etc. This study focus on the characteristics of CEO and HR systems among the characteristics of organizations affecting the gender wage gap. The management philosophy sets the direction of organizational decision-making and activates the system. In addition, the HR system enables fair and objective organizational management for members through rules and procedures. However, even in organizations seeking rationalization, minority people may experience discrimination. Moreover, the rational HR system may act as a mechanism to justify discrimination, contrary to existing intentions. This study proposes that in order for the rational HR system to work positively, it must be based on the management philosophy. In other words, it is intended to derive a mechanism that can alleviate the gender wage gap from the integrated perspective of the characteristics of the CEO and the rational HR system. In particular, it aims to provide specific implications for how the organization should operate the HR system by examining the gender wage gap based on internal factors of companies that utilize manpower.

Effects of Supplementation of Fat Sources, Ca and Mg on In Vitro Fermentation and the Performance of Finishing Hanwoo Bulls (지방의 공급형태와 Ca 및 Mg의 첨가가 In Vitro 발효 및 비육후기 한우의 성장성적에 미치는 영향)

  • This study was aimed at investigating the effect of fat supplementation with divalent ions such as MgO and $CaCl_2$ on 1) in vitro ruminal fermentation characteristics and insoluble fatty acid formation, and on 2) animal performance in finishing Hanwoo bulls. In in vitro trial, five different types of diets based on supplementation sources of fat and divalent ions, i.e. T=basal diet+4% tallow, T-Ca=T+0.5% $CaCl_2$, T-Mg=TA+0.5% MgO, T-MgCa = T +0.5% $CaCl_2$+0.5% MgO, T-caS =4% Ca salt tallow, were tested. Higher pH values were observed at 6 hr incubation(P<0.01) while higher amount of VFA were produced in diets 4 and 5 at 12 hr incubation(P<0.05). Nutrients(DM, OM, Crude protein and NDF) degradation tended to increase in divalent ions or Ca-salts treated tallow treatments compared with tallow treatment after 12 h. The amount of insoluble fatty acid increased by adding MgO or $CaCl_2$ to tallow or Ca soap tallow during incubation(P<0.05). In in vivo trial, thirty finishing Hanwoo(average BW 460kg) were divided into three groups based on fat sources and divalent ions, i.e. Control(EE 2.40), T-MgCa = control + tallow + $CaCl_2$ + MgO, T-CaS = control + Ca soap tallow (EE 5.30%). After feeding each diet for 80 days, average daily weight gain showed 0.89, 1.02, 1.17kg in diets 1, 2 and 3, respectively. The highest feed efficiency was observed(0.12) in diet 2 group, followed by diet 3 (0.10) and 1 groups(0.08; P < 0.05). In conclusion, the present results could be sununarized that the performance of Hanwoo bulls was improved by tallow with divalent ions without any negative effect on rumen fermentation.

Performance Evaluation of Rainfall Disaggregation according to Temporal Scale of Rainfall Data (강우자료의 시간해상도에 따른 강우 분해 성능 평가)

  • In this study, rainfall data with various temporal scales (3-, 6-, 12-, 24-hr) are disaggregated into 1-hourly rainfall data to evaluate the performance of rainfall disaggregation technique. The rainfall disaggregation technique is based on a database generated by the stochastic point rainfall model, the Neyman-Scott Rectangular Pulse Model (NSRPM). Performance evaluation is carried out using July rainfall data of Ulsan, Changwon, Busan and Milyang weather stations in Korea. As a result, the rainfall disaggregation technique showed excellent performance that can consider not only the major statistics of rainfall but also the spatial correlation. It also indirectly shows the uncertainty of future climate change scenarios with daily temporal scale. The rainfall disaggregation technique is expected to disaggregate the future climate change scenarios, and to be effective in the future watershed management.
