의료기관 QI 담당자의 목표추구몰입에 관한 연구 (A Study on the Goal-Orientation of QI Performers in the Medical Centers)

  • The purpose of this research is to provide the data base for the activation of Quality Improvement operation through investigating the status of Quality Improvement operation, and finding out factors influencing on the goal-orientation of QI performers in the medical centers of more than one hundred beds where are practicing Quality Improvement operation. In order to reach the purpose, document study was carried out grounded on the proceeding researches and formulated statistical data in relation with the status of Quality Improvement performers, and proof study was carried out through questionnaire survey. The subjects of the survey were the Quality Improvement performers working in seventy three medical centers in Pusan-Gyeongnam, Daegu-Gyeongbuk, and Ulsan. Among eighty three Quality Improvement performers, fifty, five were questionnaire surveyed, on the result of which Reliability Analysis, Factor Analysis, and Multiple Regression Analysis were made, using statistical program. The the results of the proof analysis on this research are as follows. First, in the factors influencing the devoting to goal pursuit of QI performers, organization-goal contribution(0.44) had significant positive effects, while organization conflict(-0.25) had significant negative effects. In other words, the higher the organization-goal contribution was, the higher the devoting to goal pursuit was, while the less the organization conflict was, the higher the devoting to goal pursuit was, which was statistically significant.(p<0.05). Second, in the aspect of goal performance types of QI performers, the process-centered type showed high level of the devoting to goal pursuit, which was statistically significant.(p<0.05). Third, in the aspect of QI performance degree, the higher the devoting to goal pursuit was, the higher the QI performance degree was, which was statistically significant.(p<0.05). In addition, the performers who perceived their workplaces organic structure showed much higher QI performance degree, which statistically significant.(p<0.05). Generalizing the results of this research, it is possible to offer a few suggestions as follows. First, as the competition among the medical centers is more severe recently owing to medical center evaluation system, medical centers are practicing various Quality Improvement operation in all of medical services such as clinical performance and management performance, to reach the purpose of both cost-cutting and medical quality improvement. Thus in order to practice Quality Improvement operation more efficiently in medical centers, it is essential to nuke use of problem-solving methods and statistical members. This as the willingness of chief executives and positive attitude and recognition of organization members. This requires the installation of divisions in charge and disposition of persons in charge, not to speak of persistent training of Quality Improvement. Second, the divisions in charge of QI carry out Quality Improvement operation at the medical center level, and take the role of generalizing and adjusting QI performances of various departments. Owing to this role, the division in charge of QI is considered indispensable organization in the QI operation of medical centers along with medical QI committee, while it contributes to the government's goal of reducing quality level gaps among medical centers. Therefore it is necessary for government and QI organizations to give institutional support and resources for the sake of QI operation of medical centers, besides to supply systematic trainning and informations to the divisions and persons in charge of QI. Third, it is certain that disposition of persons in charge should be determined in view of the scale and the scope of QI operation in medical centers.

Prefetch R-tree: 디스크와 CPU 캐시에 최적화된 다차원 색인 구조 (Prefetch R-tree: A Disk and Cache Optimized Multidimensional Index Structure)

  • R-tree는 일반적으로 트리 노드의 크기를 디스크 페이지의 크기와 같게 함으로써 I/O 성능이 최적화 되도록 구현한다. 최근에는 주메모리 환경에서 CPU 캐시 성능을 최적화하는 R-tree의 변형이 개발되었다. 이는 노드의 크기를 캐시 라인 크기의 수 배로 하고 MBR에 저장되는 키를 압축하여 노드 하나에 더 많은 엔트리를 저장함으로써 성능을 높였다. 그러나, 디스크 최적 R-tree와 캐시 최적 R-tree의 노드 크기 사이에는 수십-수백 바이트와 수-수십 킬로바이트라는 큰 차이가 있으므로, I/O 최적 R-tree는 캐시 성능이 나쁘고 캐시 최적 R-tree는 디스크 I/O 성능이 나쁜 문제점을 가지고 있다. 이 논문에서는 CPU 캐시와 디스크 I/O에 모두 최적인 R-tree, PR-tree를 제안한다. 캐시 성능을 위해 PR-tree 노드의 크기를 캐시 라인 크기보다 크게 만든 다음 CPU의 선반입(prefetch) 명령어를 이용하여 캐시 실패 횟수를 줄이고, 트리 노드를 디스크 페이지에 낭비가 적도록 배치함으로써 디스크 I/O 성능도 향상시킨다. 또한, 이 논문에서는 PR-tree에서 검색 연산을 수행하는데 드는 캐시 실패 비용을 계산하는 분석 방법을 제시하고, 최적의 캐시와 I/O 성능을 보이는 PR-tree를 구성하기 위해, 가능한 크기의 내부 단말 노드, 중간 노드를 갖는 PR-tree 생성하여 성능을 비교하였다. PR-tree는 디스크 최적 R-tree보다 삽입 연산은 3.5에서 15.1배, 삭제 연산은 6.5에서 15.1배, 범위 질의는 1.3에서 1.9배, k-최근접 질의는 2.7에서 9.7배의 캐시 성능 향상이 있었다. 모든 실험에서 매우 작은 I/O 성능 저하만을 보였다.

  • Emergency operating procedures(EOPs) of nuclear power plants should be described considering the cognitive capability and limitation of operators and provide appropriate information in the aspect of human factors. Procedures which doesn't consider cognitive characteristics of operators can become causes of human errors. In previous researches, in order to reduce these problems related to the description of EOP, an improvement suggestion for EOP writer's guide has been proposed, which is reflected human factors aspects that should be considered when describing EOPs. The proposed items, however, have a necessity to be validated because it was listed from various documents such as standards and guidelines without any special validation process. For that reason, in this study, a validation process were performed to show that procedures, which are described in compliance with the requirement items proposed in the improved EOP writer's guide, have positive effects in the aspects of human errors and performance comparing with previous procedures. Experiments were performed to compare the performances of two tasks which are described in compliance with each writer's guide of before and after the improvement During each task was performed in experiments, changes of physiological responses such as EEG and ECG were measured to evaluate the cognitive workload and the stress of operators in each task. And also, as the performance, frequencies of errors and cognition speeds of each task were evaluated. In the results of the experiment, the portions of the ${\beta}$ wave decreased in the tasks overall after the improvement. In the case of ECG, change rates of the mean of R-R interval were decreased in the tasks after improvement. In the results of the performance, the cognition and the response time of the tasks after the improvement were predominant with statistical significancies. Error times in the tasks after improvement were decreased or same to the tasks before improvement. Conclusively, it was validated that the procedures were described in compliance with the improved EOP writer's guide had effects on the reduction of human errors and improvement of performance.

  • In this paper, a novel photonic bandgap(PBG) structure is proposed for increasing stropband of lowpass filter without the size increment of circuit for application in microstrip circuits. The proposed structure is connected in parallel two periodic structures which have different center frequency of the stopband. The wide stopband is achieved by two periodic structures of two different stopbands. We also show the performance improvement of microstrip patch antenna by etching of the proposed structure in ground plane.

  • The driver model during drift cornering was examined, and a technique to improve vehicle movement performance during drift cornering was investigated. Based on the results obtained, the driver was found to steer using feedback of the body slip angle and the body slip angle velocity during drift cornering. Moreover, improvement of the cornering force characteristic, at which exceeded the maximum cornering force calm as much as possible is important.

  • In this paper, GERT network in modeled to improve the network of manufacturing process with feedback loop. A lot of Information on the GERT network can be derived from the equivalent W-function and MGF(moment generating function) using Mason's rule. These equations are used in calculating the variations of the performance measure and in improving the system performance. System improvement in manufacturing process is achieved by increasing the equivalent probabilities of each branches and by decreasing the expected equivalent time.

  • This paper presents the results of materials and mechanical analyses and performance improvement of pull-off fitting used for catenary. The pull-off fitting is supporting the trolley wire which supplies the electricity. However, the pull-off fitting is repeatedly suffered by the mechanical stresses which produce cracks in the swivel clip, resulting in interruption of an electric car. The characteristic analyses of the swivel clip were performed and the improved swivel clip was suggested.
