Retropharyngeal abscess coinfected with Staphylococcus aureus and Mycobacterium tuberculosis after rhinoviral infection in a 1-month-old infant

  • A retropharyngeal abscess is a rare disease entity in young infants but can develop after nasopharyngeal viral infection. Group B Streptococcus and Staphylococcus aureus are the most common pathogens in young infants, however, Mycobacterium tuberculosis is very rare. We report the case of retropharyngeal abscess and coinfection with S. aureus and M. tuberculosis in a very young infant presenting with respiratory symptoms due to upper airway obstruction. Usually tuberculous retropharyngeal abscesses are caused by the direct invasion of the bacteria from the spine via anterior longitudinal ligament of the spine. However, in this case, no associated spinal disease was observed.

  • Tuberculosis is primarily a pulmonary disease, but extra-pulmonary manifestations are not uncommon, especially in children and adolescents. Ten percent of extra pulmonary tuberculosis localizes to the bones and joints, and 56% of such cases affect the spine. We treated a childhood case of spinal tuberculosis misdiagnosed as muscular dystrophy in a patient without specific constitutional symptoms. We report this case because the patient had an unusual presentation of spinal tuberculosis.

  • Pulmonary cryptococcosis is rare in immunocompetent subjects. Here, we present the case of a 16-year-old boy who was referred to our pediatric department for the management of multiple consolidations detected on chest radiography, which was routinely performed when the patient was being evaluated for an ankle fracture. Fine needle aspiration biopsy was performed, and the definitive diagnosis was established as cryptococcal pneumonia. After 8 weeks of antifungal treatment, the pulmonary nodules on the chest radiographs disappeared.

  • Objectives Oriental pediatrics during the Late-Joseon dynasty had constantly developed positively, independently, originally, but had declined since Japanese invasion because the western medicine flowed into Korea and Japanese imperialism carried our the oriental medical obliterating policy. As a effort of the oriental medical group that coped with the policy of Japanese imperialism, there were publication of oriental medical academic journal and there were the most important data. Those were the bases of our study which was about the tendencies of oriental pediatrics in modern Korea. Methods We studied academic journals on oriental medicine in Knowledge of Oriental Medicine Web Service and selected 31 volumes of nine academic journals our of 80 volumes of twelve academic journals, which were about pediatrics. Results and Conclusions Research into these journals has derived the conclusion that the oriental pediatrics academic trends at the time were transformed into new state which were brought the interaction and were balanced with the merit of oriental-western medicine.

동의보감(東醫寶鑑) 소아문(小兒門)에 기재된 단방(單方)에 대한 고찰 (A Study of Classifying Single-medicine Prescriptions Mentioned in Pediatric Chapter in Dong-eui-bo-gam)

  • Objectives: The purpose of this study is to find a way to apply current oriental pediatrics treatment guidelines based on a pediatric herb chapter in Dong-eui-bo-gam. Methods: We classified herbs based on the four properties, five flavors, attributive channel, a dosage form of the medicine, diagnosis, toxicity and character. Results & Conclusions: The result of this study is that there were lots of things that we have to learn - characteristics of common herbs in pediatrics and pediatric diseases and pathology. From this study, we also learn about changes of pediatric physiology and pathology. More related studies can be helpful for updating the current oriental pediatric guidelines.

Serum Periostin Is Negatively Correlated With Exposure to Formaldehyde and Volatile Organic Compounds in Children

  • Epidemiological studies have shown that exposure to tobacco smoke causing irritation and inflammation in the airways tends to reduce serum periostin concentrations in adults. We now investigate prospective cross-sectional study on 135 Korean students aged 7 years in the first grade who were participating in the Seongnam Atopy Project for Children's Happiness 2016 ($SAP_{2016}$) cohort. To the best of our knowledge, this is the first study to show significant inverse correlations between serum periostin concentration and exposure to xylene and formaldehyde in children. Our findings suggested the need for caution in using the serum periostin level as a marker for allergic diseases, since exposure to volatile organic compounds and formaldehyde may confound the interpretation of these results.

식물성에스트로겐 작용이 보고된 한약재에 관한 고찰 (The Review of the Herbal Medicines with Phytoestrogenic Effect)

  • Objectives Medical plant has a big role in Herbal Medicines, and most medical plant has phytoestrogens. Therefore some estrogenic effects have been reported in a lot of Korean Medicine literature. Phytoestrogens should be used carefully in children. The objective of this study is to identify reported phytoestrogenic effects in Herbal Medicines and to evaluate the characteristics of the phytoestrogens in Herbal Medicines. Methods A literature search was done with using 8 databases with a limit of reports until 12/31/2013. The estrogenic effects were summarized by each individual Korean Medicine. The frequency of the phytoestrogens was also investigated depending on the Korean medical categorization by the treatment effect. Results and Conclusions Phytoestrogenic effects were reported in 89 Herbal Medicines. Phytoestrogens were act bidirectionally, and the effect was fairly weak compared to estrogen. Phytoestrogenic effect was variable on different cells and tissues. The most frequent phytoestrogenic effect was in tonifying and replenishing medicinal, the following orders were heat-clearing, exterior-releasing, and blood-activating and stasis-dispelling medicinal. Phytoestrogens were not reported in dampness-resolving, digestant, orifice-opening, emetic medicinal.

한약을 복용한 소아에게 발생한 부작용에 대한 연구 (A Study of Side Effects from Korean Herb Medicine on Children)

  • Objectives: The purpose of this study is to investigate the frequency of side effects, and the range of the side effects from Korean herb medicine on children. Methods: The study has been carried out from 212 children who took Korean herb medicine in Department of Pediatrics, $\bigcirc\;\bigcirc$ Oriental Medical Hospital from September 2009 to February 2010. The study was completed through patients chart review and telephone survey. Results: 1. There were 6 side effect cases on male, and 3 side effect cases on female out of 212 children. The incidence of side effect was 4.2%. 2. The most common side effect was abdominal pain. The most common related organ was digestive organ. 3. The side effect occurred within one to three days after taking Korean herb medicine, and the symptoms were mostly disappeared after taking the Korean herb medicine into divided dose or less dose. Conclusions: The symptom of side effects on children was not as severe as other adverse drug reactions often seen in Western medication. The most common symptom was abdominal pain. Further studies are needed.
