Psychogenic nonepileptic seizures (PNES) is a neuropsychiatric condition that causes a transient alteration of consciousness and loss of self-control. PNES, which occur in vulnerable individuals who often have experienced trauma and are precipitated by overwhelming circumstances, are a body's expression of a distressed mind, a cry for help. PNES are misunderstood, mistreated, under-recognized, and underdiagnosed. The mind-body dichotomy, an artificial divide between physical and mental health and brain disorders into neurology and psychiatry, contributes to undue delays in the diagnosis and treatment of PNES. One of the major barriers in the effective diagnosis and treatment of PNES is the dissonance caused by different illness perceptions between patients and providers. While patients are bewildered by their experiences of disabling attacks beyond their control or comprehension, providers consider PNES trivial because they are not epileptic seizures and are caused by psychological stress. The belief that patients with PNES are feigning or controlling their symptoms leads to negative attitudes of healthcare providers, which in turn lead to a failure to provide the support and respect that patients with PNES so desperately need and deserve. A biopsychosocial perspective and better understanding of the neurobiology of PNES may help bridge this great divide between brain and behavior and improve our interaction with patients, thereby improving prognosis. Knowledge of dysregulated stress hormones, autonomic nervous system dysfunction, and altered brain connectivity in PNES will better prepare providers to communicate with patients how intangible emotional stressors could cause tangible involuntary movements and altered awareness.

The aim of this study was to review our experience in terms of etiology, surgical strategy, and hearing outcomes in pediatric patients who underwent CI removal and reimplantation. Subjects and Methods: All patients received implants of the same brand. Pre and postoperative Categories of Auditory Performance score and aided free-field pure tone audiometry thresholds were noted. In vivo integrity tests were performed for each patient and the results of ex vivo tests of each implant were obtained from manufacturer. Results: A total of 149 CIs were placed in 121 patients aged <18 years. The revision rate in children was 6.7% (10/121 children). Six patients had a history of head injury leading to a hard failure. The causes of reimplantation in others were soft failure (n=1), electrode migration (n=1), infection (n=1), and other (n=1). All patients showed better or similar postreimplantation audiological performance compared with pre-reimplantation results. Conclusions: It is very important to provide a safe school and home environment and educate the family for reducing reimplantation due to trauma. Especially for active children, psychiatric consultation should be continued postoperatively.

상악 유절치가 조기 상실되는 경우는 유년기 우식증이나 외상 등으로 인하여 종종 발생한다. 상악 유절치 조기 상실시 치아간의 접촉이 긴밀하거나 총생이 있는 유치열의 경우와 유견치 맹출 전에 유절치가 상실된 경우에는 공간의 상실이 일어날 수 있으나, 그 외에 치간 공간이 있는 유치열이나 유견치 맹출 후에 유절치가 상실된 경우에서는 공간의 상실이 거의 없다고 알려져 있다. 따라서 유절치 조기 상실시 수복의 목적은 공간 유지라기 보다는 심미성 회복과 발음문제 해결, 혀내밀기 등의 구강습관 방지에 있다고 하겠다. 본 증례에서는 4세 5개월 된 여아에서 ECC로 인하여 상악 우측 유중절치를 조기 발거한 후 심미적 문제 해결을 위하여 상악 우측 유중절치 부위를 납접한 개창 금속관으로 수복하여 양호한 결과를 얻었다.
Lesch-Nyhan 증후군은 드물게 나타나는 X 염색체 열성 유전 질환으로 purine의 대사 장애를 보이는 질환이다. Purine의 대사 장애는 대사에 관여하는 효소인 hypoxanthine guanine phosphoribosyl transferase의 결손 또는 활성 감소로 인하여 나타나며, 요산의 과다 생산에 의한 요산과다혈증, 요산과다뇨증 등의 신장계 증상과 정신지체, 무도증 등의 신경학적 장애를 보이고, 구강이나 손가락에 대한 자해를 나타낸다. 이 중 자해 행위는 입술과 혀, 손가락을 깨무는 경우가 흔하고, 심하면 혀와 손가락이 완전히 절단될 수도 있다. 이 경우 자해로 인한 통증뿐 아니라, 연조직 결손으로 인한 심미적인 문제와 상처 부위로의 2차 감염이 발생할 수 있다. 이러한 Lesch-Nyhan 증후군 환자에서 자해를 막기 위해 여러 방법이 사용되고 있는데, 이 중 치과적으로 접근하는 방법으로는 마우스 가드나 lip bumper 등의 장치를 이용하는 방법과 해당 치아들을 모두 발거하는 방법, 악교정 수술을 통해 개방 교합을 형성해 주는 방법 등이 있다. 본 증례는 13세 9개월의 Lesch-Nyhan 증후군 환아로서 자해에 의해 혀에 심각한 외상을 가한 경우로 마우스 가드를 이용한 보존적인 방법을 사용하였으나 계속적으로 혀에 외상을 가해 하악 전치 뿐 아니라 구치까지 발거하여 치료하였다.
치아의 맹출은 일반적으로 치근의 1/2~2/3이 완성(Nolla stage 7)되었을 때 임상적으로 출은(emergence)하여 치근 형성이 완성되기 전에 교합평면에 도달한다. 조기 맹출이란 맹출이 가속화되어 정상 맹출 시기 보다 이른 시기에 맹출하는 것으로, 일반적으로 정상 범주를 벗어나는 영구치의 조기 맹출은 매우 드물며 유치의 만성 치근단 주위염 등의 국소적 원인으로 인한 계승 영구치의 상방 치조골이 광범위하게 파괴되었을 때 치아의 치관이나 치근의 형성이 거의 이루어지지 않은 채 계승 영구치가 조기 맹출하는 것을 관찰할 수 있다. 또한 영구치의 조기 맹출에 영향을 미칠 수 있는 여러 전신 질환이 있는 경우에도 영구치의 조기 맹출이 가능하다. 조기맹출 치아의 임상 소견으로는 심한 동요도와 저작 시 동통, 법랑질의 저석회화, 치아의 편향과 변위, 회전 등이 있으며, 방사선학적 소견으로는 미발육된 치근과 골지지의 부족 등이 있다. 조기 맹출 영구치의 발생 가능한 합병증으로는 만성적인 외상과 동통 및 부종이 있을 수 있고, 맹출 순서의 변경과 인접치의 이동으로 인한 소구치 매복율의 증가, 치아의 변위와 회전이 야기될 수 있으며, 영구치의 탈락도 있을 수 있다. 따라서 증상이 있는 조기 맹출 영구치의 경우 공간 유지 및 영구치 고정과 주위 조직의 회복, 치근 발육의 유도를 위해 적절한 처치가 필요하다. 전신 질환이 없는 만7세 여환이 상악 양쪽 제 1소구치의 심한 동요도와 동통을 주소로 치과 의원에서 의뢰되었다. 임상 및 방사선 검사상 치근 형성이 거의 없이 양측 상악 제 1소구치들이 조기 맹출되어 심한 동요도와 부종 및 저작 시동통을 나타내어 상악 제 2유구치에 교정용 band와 spur를 이용한 구내 고정식 장치를 제작하여 14개월 간 장착하여 조기 맹출한 치아들을 안정화시키고 성공적으로 치근 형성을 도모하였기에 이를 보고하는 바이다.
치아유착은 치근의 상아질 또는 백악질이 치조골과 융합된 것으로 치아의 맹출이상을 야기하여 치조골 성장의 장애를 일으킨다. 본 6세 여아는 유구치의 유착과 영구 소구치의 결손을 주소로 본과에 의뢰되었으며 특이할 만한 의과적, 치과적 병력은 없었다. 구강 검진 소견상 상, 하악의 좌, 우측 유구치에서 저위교합이 관찰되었으며, 다수의 치아우식증이 존재하였다. 방사선 사진 검사 소견상 상악의 좌, 우측 영구 견치와 계승 소구치의 결손, 상악 유구치의 치근흡수와 하악 유구치의 치주인대공간의 소실 및 미약한 치근흡수가 관찰되었다. 계승 영구치의 결손을 동반한 유구치 유착시 치료목표는 적절한 치조골 성장과 유치의 유지이며, 유착의 발현 시기와 치근의 흡수 정도에 따라 관찰, 수복 또는 발치 등의 치료를 고려할 수 있다. 유착치아의 발치는 인접 치조골 성장에 문제가 없고 인접치의 경사가 일어나지 않도록 최대성장점(growth spurt)을 고려하여 그 시기를 결정해야 한다.
이 연구의 목적은 소아 및 청소년의 영구치 치관 파절시 파절편 재부착술의 추적 관찰 결과를 평가하고자 함에 있다. 21명의 27개의 영구치 파절편 재부착술을 시행한 환자를 대상으로 임상 사진, 방사선 사진 및 의무기록을 활용하여 평가하였다. 파절편 탈락은 총 17개 치아에서 관찰되었으며 반복적인 외상이 가장 빈번한 실패 원인이었다. 파절편 재부착술과 치수치료 시행 유무는 통계적으로 유의미하지 않았다(p > 0.05). 평균 파절편 비율은 0.482 ± 0.147이며 성공률은 파절편 비율과 유의미한 상관관계를 나타냈다(p = 0.018). 평균 파절편 유지 기간은 72개월이며 파절편 유지 기간과 파절편의 비율은 통계적으로 유의미한 상관관계를 나타냈다(p = 0.003). 소아 및 청소년의 영구치 치관 파절시 파절편 비율이 50% 미만인 환자에서 파절편 재부착술은 예측 가능한 치료 방법이 될 수 있다고 판단된다.

Purpose: The COVID-19 pandemic led to significant changes in the lifestyle patterns of children and affected the patterns of pediatric injuries. This study analyzed the changing patterns of pediatric injury overall and by age groups, based on the datasets before and during the COVID-19 pandemic. Methods: This study is based on the data of patients who presented with injuries at 23 hospital emergency departments participating in the Emergency Department-based Injury In-depth Surveillance (EDIIS) conducted by the Korea Disease Control and Prevention Agency. The surveillance data was categorized by injury mechanism, location, activity, and severity. We analyzed the injury patterns of pediatric patients aged 0 to 15 years. Subgroup analysis was conducted by age group in children aged 7 to 15 years, 1 to 6 years, and <1 year. Results: When comparing the COVID-19 pandemic period to the pre-COVID-19 period, the total number of pediatric patients with injuries decreased by 38.7%, while the proportions of in-home injuries (57.9% vs. 67.9%), and minor injuries (38.9% vs. 39.7%) increased. In the 7 to 15 years group, bicycle riding injuries (50.9% vs. 65.6%) and personal mobility device injuries (2.4% vs. 4.6%) increased. The 1 to 6 years group also showed an increase in bicycle accident injuries (15.8% vs. 22.4%). In the <1 year group, injuries from falls increased (44.5% vs. 49.9%). Self-harm injuries in the 7 to 15 years group also increased (1.6% vs. 2.8%). Conclusions: During the COVID-19 pandemic period, the overall number of pediatric injuries decreased, while injuries occurring at home and during indoor activities increased. Traffic accidents involving bicycles and personal mobility devices and self-harm injuries increased in the 7 to 15 years group. In the <1 year group, the incidence of falls increased. Medical and societal preparedness is needed so that we might anticipate these changes in the patterns of pediatric injuries during future infectious disease pandemics.
치아종은 치성 종양 중 가장 흔한 형태로 대체로 병소의 크기나 위치에 따라 증상이 없는 경우가 대부분이며 성장이 제한적이다. 보통 10대 이전에 주로 진단되며 종종 영구치의 매복이나 맹출 지연을 유발한다. 형태에 따라 복합 치아종과 복잡 치아종으로 구별된다. 복잡 치아종은 치성 조직이 조직화되지 않은 덩어리로 형태학적으로 정상 치아 형태를 닳지 않으며 전체 치아종의 약 25%, 악골내 발생하는 치성 종양의 약 22%를 차지하고 남성에서 하악 구치부에 호발한다. 치아종의 원인은 아직 밝혀져 있지 않으나, 아마도 해당 부위의 국소적인 외상, 감염, 유전적 원인이 관여 할 것이라 추측된다. 치아종의 치료는 대부분 보존적인 외과적 적출술이 추천되며 재발은 거의 없다. 본 증례는 상악 제 1대구치의 매복을 일으킨 복잡 치아종으로 진단된 두 어린이에게서 병소의 외과적 적출술 및 개창술을 시행하고 관찰한 결과 약 2년후 제 1 대구치가 자발적으로 맹출함을 보여 이에 보고하는 바이다.
