Effectiveness of CO2 Fractional Laser on Linear Scars in Children

  • Background and Objectives A facial laceration is a commonly occurring trauma of children. In addition, appropriate treatment and scar management are very important because the psychosocial impact of facial scars is much greater in children than in adults. Materials and Methods In the authors' hospital, primary closure was conducted targeting a total of 8,924 patients from January 2015 to December 2017. Of these, scar management was conducted on 808 people using a CO2 fractional laser and Q-Switch Nd:YAG laser. To prove the effectiveness of the laser treatment on the linear scar, the patients were divided retrospectively into children and adults and sub-divided into those who received the treatment and those who did not. The subjective satisfaction of patients was evaluated using the Numeric Rating System (NRS). The objective scar scoring was evaluated using the image panel assessment based on the Vancouver Scar Scale (VSS), and the scoring was made by averaging the evaluations of three plastic surgeons. Results Evaluated NRS showed 8.50 ± 1.83 for adults without a laser, 8.51 ± 1.47 for adults with a laser, 7.93 ± 2.10 for children without a laser, and 8.9 ± 1.24 for children with a laser. The image panel assessment revealed 7.06 ± 1.31 for adults without a laser, 6.40 ± 2.37 for adults with a laser, 6.86 ± 1.45 for children without a laser, and 5.93 ± 1.21 for children with a laser. Conclusion When the linear scar is managed using a CO2 Fractional laser, it can be managed in children better than adults, which is believed to be useful because it leads to better satisfaction and prognosis.

가정 내 낙상으로 인한 아동 손상의 유발 환경 인자에 관한 예비연구 (A Pilot Study on Environmental Factors Contributing to Childhood Home Slip-Down Injuries)

  • Purpose: The purpose of this study was to investigate environmental factors contributing to childhood home slip-down injuries. Methods: Among a total of 2,812 injured children in our Customer Injury Surveillance System (CISS), we performed a prospective study on 262 children with home slip-down injuries who visited the pediatric emergency department of Asan Medical Center between March 2008 and February 2009. We made a frequency analysis on parameters such as activities just before the accident, the presence of any obstacles or lubricant materials, specific home place in the home where the injuries occurred, flooring materials on which the slip-down happened, additional objects hit after slip down, the site and kind of injury, the duration of therapy, and the disposition. Results: Walking was the most common activity just before the injury. Because rooms and bathrooms were most common places in the home for slip down injuries, laminated papers/ vinyl floor coverings and tiles were the most common flooring materials used in the places where the injuries occured. Most commonly, no obstacles caused the children to slip down, but the furniture, stairs, doorsills, wetness, or soapy fluid followed after that. Over half of the children who slipped (58%) also collided with other than the floor itself after the slip-down, most common objects hit were the edges of the furniture, and doorsills, followed by stairways. The head and neck were the most commonly injured sites, and a laceration was the most common kind of injury. Most children needed less than 1 week of therapy, only 4 children (1.53%) admitted. There were no mortalities. Conclusion: The environmental factors contributing to slip-down injuries were the bathroom, laminated papers/vinyl floors, the furniture, stairs, doorsills, and wetness or soapy fluid. Especially, the furniture, stairs, and doorsills can be both primary obstacles and secondary collision objects. For the safety of our children, we must consider these factors on housing, when decorating or remodeling our house.

소아 장중첩증에서 공기 주입 정복술 시행 도중 발생한 장천공 (Colon Perforation during Air Enema Reduction of Intussusception)

  • 목 적 : 장중첩증의 진단 및 치료에 효과적인 방법인 공기 주입 정복술을 시행하는 도중 발생한 장천공에 대한 국내의 연구는 미미한 실정이다. 따라서 저자들은 이에 대한 위험인자 및 수술 합병증, 예후 등에 대하여 분석하고자 하였다. 방 법 : 1990년 1월부터 2001년 12월까지 가천의대 길병원 소아과에 장중첩증으로 입원하였던 환아 중 공기 주입 정복술을 시행하는 도중 장천공이 발생하였던 12명의 환아를 대상으로 병록지를 통하여 성별 및 연령 분포, 주요 임상 증상, 증상 발현후 내원까지의 경과 시간, 장중첩증의 유형, 장중첩증의 천공 부위와 수술소견, 병리소견 등을 후향적으로 조사하여 분석하였다. 결 과 : 1) 공기 주입 정복술을 시행하였던 657명의 환아 중에서 596명이 성공적으로 치료되어 정복율은 90.7%였고, 장천공은 모두 12례에서 발생하여 천공율은 1.83%였다. 공기 주입 정복술 도중 천공된 환아들 중 남아는 11명(91.7%), 여아는 1명(8.3%)이었으며, 평균연령은 $5.3{\pm}2.7$개월이었다. 2) 천공된 장중첩증의 내원 당시 임상 증상을 살펴볼 때 구토 는 11명(91.7%), 주기적 보챔이 9명(75.0%), 혈변은 9명(75.0%)에서 발생하였고, 복부종괴는 5명(41.7%)에서 촉진되었다. 천공된 장중첩증 환아의 내원까지의 평균 경과 시간은 $44.7{\pm}23.7$시간이었다. 3) 천공된 장중첩증의 유형은 상대적으로 회장맹장(ileocecal) 형과 회장회장결장(ileoileocolic)형이 많았다. 수술로 확인된 장천공 부위는 상행 결장이 6명(50%), 횡횡 결장이 6명(50%)으로 모두 중첩된 장의 근위부에 발생하였고, 단발성 천공이 10명(83.3%)으로 대다수를 차지하였다. 4) 장천공이 발생한 부위의 병리 조직 소견은 대부분에서 장천공 주변부위의 점막이 출혈성 괴사와 장막열상 소견을 보였다. 천공된 장중첩증의 합병증으로는 긴장성 기복이 7례(58.3%)에서 발생하여 즉각적인 복부 감압이 필요하였고, 1례에서는 수술 18개월 후 유착성 장 폐쇄(adhesive band)가 발생하였다. 결 론 : 공기 주입 정복술 시행도중 장천공의 발생은 6개월이하에서 방사선 소견상 소장 폐색 증상이 있는 경우에 자주 발생하며, 증상 발현 시간이 오래될수록, 공기 주입 압력이 높을수록 잘 생기므로 이런 경우 정복시 환아에 대한 세심한 관찰과 주의가 필요할 것으로 사료된다.
