The emergence of mucosal healing as a treatment goal that could modify the natural course of Crohn's disease and the accumulating evidence showing that biologics are most effective in achieving mucosal healing, along with the success of early treatment regimens for rheumatoid arthritis, have led to the identification of early Crohn's disease and development of the concept of catching the therapeutic window during the early disease course. Thus, an increasing number of pediatric gastroenterologists are adopting an early biologic treatment strategy with or without an immunomodulator. Although early biologic treatment is effective, cost and overtreatment are issues that limit its early use. Currently, there are insufficient data on who will benefit most from early biologics, as well as on who will not need early or even any biologics. For now, top-down biologics should be considered for patients with currently known high-risk factors of poor outcomes. For other patients, close, objective monitoring and accelerating the step-up process by means of a treat-to-target approach seems the best way to catch the therapeutic window in early pediatric Crohn's disease. The individual benefits of immunomodulator addition during early biologic treatment should be weighed against its risks and decision on early combination treatment should be made after comprehensive discussion with each patient and guardian.
Benzalkonium chloride (BAC) is a caustic agent which is used in farms, homes and hospitals for cleaning skin and wounds as an antiseptic solution. It may lead to digestive system injuries in case of ingestion. We present a two-days-old newborn case which was carried to the emergency unit with complaints of poor breastfeeding, uneasiness and crying for 4-6 hours. Her mom confessed that she had given a spoon of 10% BAC solution for her cough. Initial laboratory tests were in normal ranges. A gastroscopy performed in the second hour of her admission revealed an hyperemic and edematous mucosa in the middle third of esophagus and a circumferential ulceration followed in the distal portion. Hereupon, a conservative treatment for 10 days was administered and the control gastroscopy demonstrated that the damage was almost totally improved. She was the youngest case with this etiology and successfully treated with conservative approach.
Sialolithiasis is the formation of calcareous concretions within ductal system of a major or minor salivary gland. They are formed by deposition of calcium salts around a central nidus which may consist of desquamated epithelial cells, bacteria, foreign bodies, or products of bacterial decomposition. An 11-year-old boy complained of pain during meals and intermittent mild swelling in the right submandibular region. Although it was not detected in true occlusal radiograph, panoramic radiograph showed a round radiopaque mass 3mm in diameter. Computed tomography(CT) was taken for locating the stone and 3-dimensional reconstruction was performed. Under general anesthesia, sialoadenectomy was done through extraoral approach. Diagnosis of submandibular sialolithiasis using high-resolution CT with reconstructions was helpful for surgical decisions, namely radical removal of the submandibular gland and its duct.
Purpose: Pediatric facial laceration takes a huge part of patients visiting emergency room and generates social attention for its proper emergency care. So much more attention should be paid to the proper treatment at emergency care unit, and furthermore, thorough survey of background information of the pediatric facial laceration may offer more proper prevention. Methods: According to annual reports of 2009 and 2010, out of 5149 facial laceration patients who were given primary medical care at our clinic, 1452 patients were aged under 15 years old. Retrospective analysis of each pediatric facial lacerations were evaluated according to gender, age, periodic table, cause of injury, place of injury, sites of injury and so on. Results: Pediatric facial laceration was found to occur mostly at 1 year old as they learn to walk and explore their environment. Evaluated analysis revealed that pediatric facial accidents occurred mostly on forehead region (75%), on Sundays, from 5 p.m. to 8 p.m., at home (61.5%). Most common cause of injury was collision (54.5%). Conclusion: In large group of pediatric facial laceration cases provided us with an surprising fact that accidents most commonly occur under parental supervision. This fact gives an actual understanding regarding pediatric facial laceration and more realistic approach in its prevention strategy.
상악골과 치조골 전돌을 가진 II급 부정교합 증례 중 상악 치아 발치의 적응증이 아닌 경우와 하악 구치의 전방 이동을 허용할 수 없는 경우 상악 대구치의 원심이동 기법을 사용하게 된다. Pendulum 장치는 환자의 협조도 없이 상악 대구치를 원심 이동시킬 수 있는 효과적인 장치로 소구치를 안정화시키며, 구개측 고정원을 이용할 수 있다. 하지만 구치부 정출로 하안면 고경이 증가할 수 있고, 전치부 전방 이동이 동반되며, 추벽부 조직에 손상을 줄 수 있으므로 장치의 사용과 환자 선택에 신중을 기해야 한다. 이에 pendulum 장치를 사용하여 상악 대구치를 원심 이동시킴으로써 공간 문제를 해결하고, 대구치 관계를 개선한 증례들을 보고하는 바이다.
The Committee on Pediatric Obesity of the Korean Society of Pediatric Gastroenterology, Hepatology and Nutrition newly developed the first Korean Guideline on the Diagnosis and Treatment of Obesity in Children and Adolescents to deliver an evidence-based systematic approach to childhood obesity in South Korea. The following areas were systematically reviewed, especially on the basis of all available references published in South Korea and worldwide, and new guidelines were established in each area with the strength of recommendations based on the levels of evidence: 1) definition and diagnosis of overweight and obesity in children and adolescents; 2) principles of treatment of pediatric obesity; 3) behavioral interventions for children and adolescents with obesity, including diet, exercise, lifestyle, and mental health; 4) pharmacotherapy; and 5) bariatric surgery.
이 논문은 저한성 외배엽 이형성증(Hypohidrotic Ectodermal Dysplasia, HED)으로 인한 부분무치증을 가진 소아 환자에게 의치의 유지력 증가 방법에 관한 두 가지 증례를 보고한다. 두 환자 모두 다수의 치아 결손 및 원추형의 견치와 치조골 위축, 골격적 III급 경향이 나타났다. 첫 번째 증례에서는, 변형된 형태의 Conical-Crown Retained Denture (CCRD)를 이용해 견치를 피개하였다. 이 방법은 치아를 삭제하거나 코핑 없이 진행되었다. 두 번째 증례에서는, 심한 치조제 흡수와 함께 하악 좌측 견치가 원심으로 경사지어 맹출한 상황에서, 하이브리드 세라믹 크라운을 사용하여 견치를 수복한 후, 흡착성 원리를 이용한 클라스프 유지 가철식 의치로 변경하였다. 두 환자는 현재 보철물 유지 관리 및 성장양상을 평가하기 위해 주기적으로 치료를 받고 있다. 이 증례는 HED로 인하여 의치의 유지력이 부족한 소아 환자에게 적용된 창의적인 보철 치료 방법을 제시하였다.
Background: The purpose of this study was to present a practical method of medical treatment to improve the balance and motor ability of the mentally retarded child with a single mentally retarded child-subject. Methods: The subject of the study was a 39-month-old mentally retarded female. This study included a 2-week basic period and a 13-week treatment period. The treatment method was based on the Bobath Approach. Gross motor function measurement (GMFM) was used to examine changes in motor ability, and the Pediatric Balance Scale (PBS) was used to measure changes in balance ability. The curative program was composed of normalization of muscle tone, strengthening of leg endurance and muscular strength, the improvement of trunk alignment, and the increase of balance. Visual rate of change was used to examine the results. Results: As a result of this study, balance ability increased on the Pediatric Balance Scale (PBS) by 24 points, and motor function increased in terms of Gross Motor Function Measurement (GMFM) by 6.9% (18 points). Standing increased by 41% (16 points), and walking, running, and jumping increased by 31.9% (23 points) compared to thebasic period. Therefore, the Bobath Approach appears to be an appropriate method to improve balance and motor ability in mentally retarded children. Conclusion: It is surmised that aggressive intervention by physical therapists and occupational therapists, and a follow-up study, are required for the growth of motor ability in mentally retarded children.
Foreign body ingestion is a common problem among paediatric populations. Most of the ingested foreign bodies spontaneously pass through the gastrointestinal tract, but approximately less then 10% of them remain without being discharged, and trigger complications. Therefore, proper evaluation and treatment according to the situation is required. In this study, clinical progress and complications were analyzed according to the clinical features and treatment in children who ingested foreign bodies. Among pediatric patients under 18 who were admitted to Chonnam National University Hospital after ingesting foreign bodies between January 2008 to June 2012, only the patients who had their foreign body in the gastrointestinal tract were included in this study. Based on medical records, age, type of foreign body, time spent till admission, and whether the endoscopy was done or not, complication were researched retrospectively. According to symptoms and plain abdomen X-ray findings, treatment was chosen and conducted among endoscopy, observation and emergency operation. Among 273 patients, 9 (3.3%) of them had surgical removal. Seven (2.6%) of them had an emergency operation on the day of admission, and the rest 2 (0.7%) had operation during observation. Removal through initial endoscopic approach was tried in 157 (57.5%) patients. Eleven (70.8%) of them had their foreign body removed at the initial trial, and 5 (4.9%) of them at the second trial. Among 109, who were on observation status, 9 (8.3%) of them needed endoscopic removal, and 2 (1.8%) of them suffered from surgical removal. It is thought to be better to approach slowly considering the type, size and symptoms in foreign body ingestion of pediatric patients, rather than immediate and invasive removal.
이소맹출은 치아가 비정상적인 위치나 방향으로 맹출하는 것으로 정의되며 구치부에서는 근심측으로 맹출하여 인접치아의 치근 흡수나 조기 탈락을 야기하여 공간 상실을 일으킬 수 있고, 부분 맹출된 경우 치은 하방의 치아우식 위험성이 증가하고 농양을 형성할 수도 있다. 구치부의 이소맹출 가운데 66% 정도는 가역적인 경우로 특별한 치료 없이 자연적으로 정상적인 맹출이 일어나며, 비가역적인 이소맹출의 경우에는 치아의 맹출 상태, 인접치에 의해 매복된 양, 인접치의 동요도, 통증이나 감염 여부 등의 여러 요인을 고려하여 본격적인 치료가 필요하다. 본 논문에서는 비가역적으로 이소맹출된 상악 제1대구치와 하악 제2대구치를 치간이개 고무줄, modified Halterman appliance 그리고 외과적 노출 후 modified Halterman appliance를 이용한 방법 등으로 정상적인 맹출을 유도한 증례를 보고 하는 바이다.
