전신 마취를 이용한 자폐아동의 치험예 -증례보고- (Dental Treatment in a Autistic Child under General Anesthesia)

  • Autism is characterized with an abnormality of reciprocal social relatedness and of communication development. Also, autistic children show repetitive and stereotypical behavior pattern and deficiency of endurance against change. Dental problems related to autism are self-injury and trauma from accidents. Sometimes self-injury is due to discomfort resulting from restoration and local anesthesia. Also, gingivitis and multiple caries are common because of poor oral hygiene. Autistic children are anxious about visual and auditory stimuli because they don't understand the dental procedure. If the face of any changes in their daily life, their behavior becomes uncontrollable. In a setting such as the outpatient office, an autistic child's uncontrollable behavior makes it difficult to carry out proper treatment. If they need treatment under general anesthesia, it is very difficult to manage them in a ward as they are severly anxious about being admitted to a hospital. Therefore it is necessary to treat these children according to a specific management program under general anesthesia in an outpatient office. This case study reports of a 14 year old autistic teenager who although became uncontrollable because of the pain from multiple caries and from anxiety of the dental treatment eventually received treatment under general anesthesia and under conscious sedation in an outpatient office.

The combined effect of extraoral vibratory stimulus and external cooling on pain perception during intra-oral local anesthesia administration in children: a systematic review and meta-analysis

  • This study aimed to assess the combined use of extraoral vibratory stimulation and extraoral cooling in reducing the pain (subjective and objective) of dental local anesthesia administration in children. PubMed, Cochrane Central Register of Controlled Trials, and Ovid SP databases were searched up to July 2021. Article titles were screened and full-text evaluations of the selected articles were performed. Finally, seven studies (391 children, aged 4 - 12 years) were included in this qualitative and quantitative analysis. The pooled data determined the combined effect of extraoral vibration and extraoral cooling as a single measure. Extraoral vibration or cooling alone were not compared. The measured primary and secondary outcomes were pain perception and subjective and objective pain, respectively. When compared with the control, extraoral vibration and cooling resulted in significant differences in the mean combined data for the variables, pain perception, and pain reaction. Children's subjective pain as measured by pain scores were reduced when extraoral vibration and cooling was used during local anesthesia administration (mean difference -3.52; 95% confidence interval [-5.06 - 1.98]) and objective pain (mean difference -1.46; 95% confidence interval [-2.95 - 0.02] ; mean difference -1.93; 95% confidence interval [-3.72 - 0.14]). Within the confines of this systematic review, there is low-quality evidence to support the use of combined extraoral vibration and cooling for reducing pain (subjective and objective) during intraoral local anesthesia administration in children.

이탈된 위루관에 대한 투시경하 변형 Seldinger씨 방법을 이용한 경피적 위루관 재삽입술 (Percutaneous Gastrostomy Tube Reinsertion after Accidental Dislodgement Using Modified Seldinger's Technique)

  • This case report describes a baby who received a laparoscopic gastrostomy tube insertion, which was dislodged accidentally at $16^{th}$ postoperative day. After the dislodgement, cutaneous tract rapidly closed, and reinsertion seemed to be impossible. However, gastrostomy tube was reinserted safely with fluoroscopy-guided Seldinger's technique under local anesthesia with sedation. This is the unique method of modified Seldinger's technique for reinsertion of gastrostomy tube under local anesthesia and sedation for accidentally dislodged gastrostomy tube. This method was thought to be safe, easy and useful technique for gastrostomy reinsertion after dislodgement of gastrostomy tube.

Intraosseous anesthesia using a computer-controlled system during non-surgical periodontal therapy (root planing): Two case reports

  • Local anesthesia is administered to control pain, but it may induce fear and anxiety. Root planing is a non-surgical periodontal therapy; however, when it is performed in an extensive manner, some tissue removal is inevitable. Notably, this removal may be so painful that local anesthesia is required to be administered to the area scheduled for the treatment. Although patients tend to accept root planing easily, they frequently express a fear of local anesthesia. Intraosseous anesthesia (IA) is an intraosseous injection technique, whereby local anesthetic is injected into the cancellous bone supporting the teeth. A computer-controlled IA system (CIAS) exhibits multiple benefits, such as less painful anesthesia, reduced soft tissue numbness, and the provision of palatal or lingual, as well as buccal, anesthesia via single needle penetration. In this report, we present two cases of root planing that were performed under local anesthesia, using a CIAS.

  • Attention deficit hyperactivity disorder (ADHD) is characterized by inattention, impulsivity, and hyperactivity. Given high incidence of ADHD, many children with ADHD is likely to present for anesthesia. This case report suggests intramuscular premedication as an alternative method for anesthetic induction. A 9-year-old male patient with ADHD was transferred for dental treatment under general anesthesia. The patient refused to go into dental clinic office. Oral midazolam was given to the patient, however, he was resistant to take midazolam via oral route. Instead, we administer midazolam and ketamine via intramuscular route. After less than 10 miniutes, the patient became drowsy and was transferred to dental chair. Intravenous access and mask inhalation was possible. The patient received dental treatment under general anesthesia and recovered in a non-complicated way. In this case, intramuscular sedation with midazolam and ketamine was used as a premedication in highly uncoopearive patient refused to take oral sedative medication.

  • Numerous neoplastic lesions can arise in the orofacial region in the pediatric populations. Odontogenic tumors typically affect the mandible more than the maxilla. Airway management can be challenging in pediatric oral tumors because of the distorted anatomy and physiological variations. Conventional awake fiberoptic intubation is not always possible owing to limited cooperation from the pediatric populations. Herein, we report the case of a 1-year-old child with odontogenic myxoma of the mandible and an anticipated difficult airway. Given the expected difficulties in the airway, video laryngoscope-assisted orotracheal intubation under general anesthesia with maintenance of spontaneous breathing was scheduled. Proper planning and thorough examinations are vital for successful airway management in pediatric patients.

행동유도법에 대한 보호자의 인식도 조사 (A Survey of Guardian's Attitudes toward Behavior Guidance Techniques)

  • 본 연구의 목적은 국내 실정에 맞는 행동유도법에 대한 보호자의 인식도를 조사하여 어린이 환자와 보호자, 그리고 치과 의사간의 긍정적인 관계를 설정하는데 기초 자료를 제공하는 것이다. 117명의 보호자에게 소아치과에서 사용되는 행동유도법에 대해 구두로 설명한 후 설문지를 작성하게 하였다. 설문지에 포함된 행동유도법은 다음과 같다: 말-시범-시행법, 음성조절법, 기구에 의한 보호안정법, 의료진에 의한 보호안정법, 아산화질소-산소 흡입진정법, 의식하 진정법, 전신마취법. 106명의 보호자의 설문지가 분석을 위해 채택되었다. 말-시범-행동법에 대한 인식도가 가장 높았다. 그 뒤로 음성조절법, 의료진에 의한 보호안정법, 아산화질소-산소 흡입진정법, 기구에 의한 보호안정법, 의식하 진정법, 전신마취법 순으로 낮아졌다. 보호자의 연령, 성별, 환자의 나이, 환자의 성별, 환자의 치과 경험 유무, 치과 방문의 종류, 의료진의 직위는 보호자의 행동유도법에 대한 인식에 영향을 미치지 않았다. 보호자들은 의사소통을 통한 행동유도법(말-시범-시행법, 음성조절법)을 가장 긍정적으로 생각하며, 전신마취법에 의한 행동유도법을 가장 부정적으로 생각하는 것으로 나타났다.

  • General anesthesia is commonly used to facilitate dental treatment in patients with anxiety or challenging behavior, many of whom are children or patients with special needs. When performing procedures under general anesthesia, dental surgeons must perform a thorough pre-operative assessment, as well as ensure that the patients are aware of the potential risks and that informed consent has been obtained. Such precautions ensure optimal patient management and reduce the frequency of morbidities associated with this form of sedation. Most guidelines address the management of pediatric patients under general anesthesia. However, little has been published regarding this method in patients with special needs. This article constitutes a review of the current literature regarding management of patients with special needs under general anesthesia.

통원수술센터를 이용한 장애아동의 치과치료에 대한 증례보고 (DENTAL CARE FOR HANDICAPPED PATIENTS AT DAY SURGERY CENTER:A CASE REPORT)

  • 심신장애자, 심한 불안과 공포가 있는 경우, 너무 어려서 의사소통이 되지 않는 환자 등, 일반적인 행동조절방법이 가능하지 않은 경우에 전신마취를 고려하게 된다. 이런 환자들은 대부분 소아 치과에서 치료하게 되므로 소아치과의사의 역할이 중요하다. 전신마취하에 시행되는 치과치료를 고려할 때, 환자의 전신적 건강상태, 환자의 현재 구강 형태, 필요한 치과치료의 종류, 책임감 있는 보호자의 유무에 따라서 입원없이 행해지는 외래환자수술을 고려할 수 있다. 전신마취하에 치과치료를 시행하고자 할 때, 치과의사나 보호자의 편의를 위해서 사용되어져서는 안되며, 적절한 환자 선택하에 시행된다면, 보통의 방법으로 치과치료를 받기 어려운 장애아동에게 유리한 치료가 될 수 있다.
