A survey of the intravenous sedation status in one provincial dental clinic center for the disabled in Korea

  • Background: The objective of the present study was to examine the status of patients who had received dental treatment under intravenous (IV) sedation at Chungnam Dental Clinic for the Disabled in Korea from its inception to the present time, and to review the analysis results. Methods: Retrospective analysis was performed on 305 cases of patients who had received dental treatments under IV sedation between January 2011 and May 2016. The analysis examined the patient's sex, age, primary reason for IV sedation, duration of anesthesia and dental treatment, type of dental treatment performed, number of clinical departments involved in the dental treatment and level of multidisciplinary cooperation, and annual trends. Results: Most dental treatments using intravenous sedation were performed on medically disabled patients or dentally disabled patients with an extreme gag reflex or dental phobia. The mean duration of IV sedation was 72.5 min, while the mean duration of treatment was 58.0 min. The types of dental treatments included surgical treatment (n = 209), periodontal treatment (n = 28), prosthodontic treatment (n = 28), restorative treatment (n = 23), implant surgery (n = 22), endodontic treatment (n = 9), reduction of temporomandibular joint dislocation (n = 1), and treatment of traumatic injuries (n = 1), with treatments mostly performed on adult patients. Conclusions: With increasing demand for minimally painful treatment, cases using IV sedation are on an upward trend and are expected to continue to increase.

서울대학교 치과병원 장애인진료실의 외래환자마취 실태 분석 (ANALYSIS ON THE OUTPATIENT ANESTHESIA AT DENTAL CLINIC FOR DISABLED IN SEOUL NATIONAL UNIVERSITY DENTAL HOSPITAL)

  • 치과적 장애인이란 치과진료 시 치과의사에게 자발적인 협조가 힘든 사람을 의미한다. 본 연구는 서울대학 치과병원 장애인진료실에서 치료받은 치과적 장애인 환자들의 임상진료 실태를 마취과적으로 분석하고 향후 장애인 치과치료에 대한 지침을 마련하고자 하였다. 1999년 1월부터 2002년 10월까지 서울대학교 치과병원 장애인진료실에 내원한 장애인 환자들을 대상으로 조사한 결과, 총 89명의 환자들에게 93건의 치과진료가 시행되었다. 환자의 대다수는 정신지체였고 전신마취 하 보존치료를 받았다. 주로 마취유도제는 thiopental, 근이완제는 vecuronium을 이용하였으며 산소, 아산화질소, enflurane을 함께 흡입시켜 마취를 유지하였다. 총 마취지속시간과 회복실 체류시간은 각각 $164.4{\pm}57.2$ 분과 $106.2{\pm}50.5$ 분이었고 회복 과정에서 심각한 합병증은 관찰되지 않았다. 본 연구는 외래환자마취에 기반한 서울대학교 치과병원 장애인진료실의 치료방침에 따라 특별한 문제없이 성공적인 마취가 가능하였음을 보여 주었다. 또한 증가하는 장애인 치과치료에 대한 수요를 충족시키기 위해서 적절한 시설과 인력을 갖춘 보다 맡은 외래환자마취 센터의 필요성도 제시한다고 할 수 있다.

  • Background: The number of patients with disabilities who require general anesthesia for dental treatment is increasing. Methods: We reviewed the cases of 538 patients with disabilities who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul national university dental hospital according to the types of disabilities and comprehensive treatments. Results: The patients mean age was 17.4 years and fifty-eight percent of the patients had epilepsy, autism, mental retardation and cerebral palsy. About twenty-five percent of the patients were medically compromised such as brain disease, heart disease and genetic problem. In 142 cases, more than two dentists from the clinical departments systemically co-worked to provide the patients with comprehensive treatment as accurately and quickly as possible under general anesthesia. Conclusions: The number of medically compromised patients was high and the system of comprehensive dental treatment was well run.

소아 치과치료시 심박동과 동맥혈 산소포화도의 변화 (PULSE RATE AND OXYGEN SATURATION IN CHILDREN DURING ROUTINE RESTORATIVE DENTISTRY)

  • 소아환자의 진정법시 사용되는 환자감시 기구 중 하나인 pulse oximeter는 산소포화도를 감시하여 조기에 저산소혈증을 발견하는데 유용하게 이용된다. 이 연구의 목적은 진정법을 시행하지 않은 소아환자에서 pulse oximeter(pulse oximeter7845, Kontron Instrument Ltd., England)를 이용하여 일상적인 수복치료시 치료단계에 따른 심박동과 동맥혈 산소포화도의 양상이 어떻게 나타나는지 알아보고자 한다. 전북대학교병원 소아치과에 내원한 53명의 어린이를 대상으로 치료전 3분과 치료후 3분을 포함하여 수복치료 단계별로 심박동과 동맥혈 산소포화도가 기록되었고, 기록된 수치를 바탕으로 다음의 결과를 얻었다. 1. 비마취군에서 상, 하악 치료군 간 심박동 변화를 비교시 상악의 경우 러버댐 장착시 심박동이 급격히 증가한데 비해 하악치료군에서는 와동 형성시 심박동이 크게 증가하였다. 2. 마취군에서 상, 하악 심박동 변화를 비교시 상악치료군에서는 러버댐 장착을 기점으로 심박동이 감소되는 양상을 보인 반면, 하악 치료군은 와동형성시점부터 심박동이 감소되었다. 3. 비마취군에서 상, 하악 치료군 간 동맥혈 산소포화도를 비교하였을 때 상악 치료군은 치료 전부터 치료 후까지 비교적 일정한 값을 나타내며 모든 단계에서 하악보다 약간 높은 값을 보인 반면, 하악 치료군은 러버댐 장착시 증가하여 유지되다가 치료 후 다시 치료 전 수준과 유사하게 감소하는 양상을 보였다. 4. 마취군에서는 모든 단계에서 동맥혈 산소포화도 값이 99% 수준에 머물고 있으며, 상악과 하악 치료군 사이에 유의한 차이는 존재하지 않았다(p>0.05).

Comparative evaluation of efficacy of Physics Forceps versus conventional forceps in pediatric dental extractions: a prospective randomized study

  • Background: This study aimed to determine the efficacy of Physics Forceps in pediatric dental extractions. Methods: This was a double-blind, randomized controlled trial with a parallel-arm design and identical allocation ratio (1:1). Children (n=104) were randomly divided into two groups for extraction of mandibular primary teeth (group I: Physics Forceps; group II: conventional forceps). The outcome variables assessed in the study were the time taken for extraction, pre- and postoperative anxiety (using RMS pictorial scale), incidence of fractured teeth, and postoperative pain on the first and third days (using the Wong-Baker faces pain scale). Results: A significant reduction (P < 0.001) in intraoperative time, anxiety, and incidence of tooth fracture was confined to group I. The pain significantly reduced from the first to the third postoperative day in both groups, but the mean reduction in RMS scores in the physics forceps group was far better than that in the conventional forceps group. Conclusion: Physics Forceps aid in extraction of primary teeth with minimal trauma to supporting structures, as well as reducing anxiety in the pediatric population.

Computer-assisted Virtual Surgery and Splint Fabrication for Paediatric Mandible Fracture

  • Closed reduction using acrylic splints with circummandibular fixation has been known to be useful techniques in pediatric mandibular fractures. However, this technique has some shortcomings, including needs for impression taking or additional laboratory process, which can increase the exposure time of general anesthesia or make an additional sedation visit. Recently, the advancement of computer-aided maxillofacial surgery offers to clinicians to expansion of its application. This case report represents a technique of computer-assisted virtual reconstruction and computer-aided designed splint fabrication in a 2-year-old boy with mandibular body fracture.

Pediatric advanced life support and sedation of pediatric dental patients

  • Programs provided by the Korea Association of Cardiopulmonary Resuscitation include Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Korean Advanced Life Support (KALS). However, programs pertinent to dental care are lacking. Since 2015, related organizations have been attempting to develop a Dental Advanced Life Support (DALS) program, which can meet the needs of the dental environment. Generally, for initial management of emergency situations, basic life support is most important. However, emergencies in young children mostly involve breathing. Therefore, physicians who treat pediatric dental patients should learn PALS. It is necessary for the physician to regularly renew training every two years to be able to immediately implement professional skills in emergency situations. In order to manage emergency situations in the pediatric dental clinic, respiratory support is most important. Therefore, mastering professional PALS, which includes respiratory care and core cases, particularly upper airway obstruction and respiratory depression caused by a respiratory control problem, would be highly desirable for a physician who treats pediatric dental patients. Regular training and renewal training every two years is absolutely necessary to be able to immediately implement professional skills in emergency situations.

소아청소년의 약물적 행동유도 (Pharmacological Behavioral Management for children and adolescence)

  • Managing uncooperative behaviors related to dental treatment is necessary for guiding children and adolescence to more favorable behaviors. The first approach should be controlling their behaviors using non-phamarcologic behavior management techniques. However, if this approach fails, it is helpful to control negative behaviors pharmacologically. Accordingly, sedation is frequently used to relieve anxiety related to dental treatment. Also, general anesthesia has been applied to the situations in which sedation is either ineffective or impossible to gain cooperation during treatment. This article discusses the pharmacology of widely used sedatives for children and adolescence and clinical considerations of managing uncooperative children and adolescence with the use of sedation. Furthermore, we recommend clinical indication of selecting general anesthesia rather sedation for the purpose of behavior management.

자폐증 환자의 행동조절을 위한 Midazolam Intranasal Spray 사용사례 (Use of Midazolam Intranasal Spray for Dental Treatment of Autism Patients)

  • Autism patients in general have bad oral hygiene. It is hard for autism patients to get dental treatment as poor communication. Therefore, they may have to be treated by deep sedation or general anesthesia in numerous cases. However, this process requires induction with mask, so it is not easy to do for disobliging autism patient. Midazolam is a water-soluble benzodiazepine, has been used in pediatric dentistry or dentistry for the handicapped because of rapid onset. Midazolam can be administered through oral, rectal, intramuscular,intravenous, and intranasal (IN) routes. IN route of midazolam may be considered as effective way to allay for uncooperative autism patients before general anesthesia. In this case report, two autism patients required dental treatment. Intranasal spray of midazolam before general anesthesia was safe and effective procedure of behavioral management.

  • Topical anesthetics act on the peripheral nerves and reduce the sensation of pain at the site of application. In dentistry, they are used to control local pain caused by needling, placement of orthodontic bands, the vomiting reflex, oral mucositis, and rubber-dam clamp placement. Traditional topical anesthetics contain lidocaine or benzocaine as active ingredients and are used in the form of solutions, creams, gels, and sprays. Eutectic mixtures of local anesthesia cream, a mixture of various topical anesthetics, has been reported to be more potent than other anesthetics. Recently, new products with modified ingredients and application methods have been introduced into the market. These products may be used for mild pain during periodontal treatment, such as scaling. Dentists should be aware that topical anesthetics, although rare, might induce allergic reactions or side effects as a result of an overdose. Topical anesthetics are useful aids during dental treatment, as they reduce dental phobia, especially in children, by mitigating discomfort and pain.
